Innovative Dietary Intervention Answers to Baby Colic

  • Purpose: The purpose of this paper is to evaluate the efficacy of a lactose- reduced synbiotic partial whey hydrolysate in formula fed infants presenting with colic and the impact of this dietary intervention in mean crying time and quality of life. Methods: Forty infants with infantile colic were treated during one month with parental reassurance and the intervention formula (partial whey hydrolysate, reduced lactose, Bifidobacterium lactis BB12 and galacto-oligosaccharides) and were compared to a control group of 20 infants with infantile colic treated with parental reassurance and a standard infant formula. Parents completed a quality of life (QoL) questionnaire assessing the burden of infantile colic. Wilcoxon test, t-test and Mann-Whitney test were used to compare QoL scores before and after intervention as well as between the intervention and control group. Results: At inclusion, duration of crying did not differ between both groups. Crying duration decreased with 2.7 hours (from 3.2 to 0.5 hours) in the intervention group while duration of crying decreased only with 1.2 hours in the control group (p<0.001). Stool composition became looser in the intervention group, but defecation frequency did not change. The median scores of the QoL questionnaire improved significantly in the intervention group for all parameters. In the control group, parameters improved significantly also but not for the parent-child and social interaction. The score changes were significantly greater in the intervention than in the control group. Conclusion: The intervention formula (partial whey hydrolysate, synbiotic, reduced lactose) significantly reduced the duration of crying and improved QoL of the parents and infants.

A Synbiotic Infant Formula with High Magnesium Content Improves Constipation and Quality of Life

  • Purpose: To evaluate the efficacy of synbiotic formula with partial whey hydrolysate and high magnesium content in infants presenting with functional constipation. Methods: Sixty-five infants with functional constipation were included. Forty infants were treated during one month with parental reassurance and the intervention formula and were compared to a control group of 25 infants treated with parental reassurance only. Parents completed a quality of life (QoL) questionnaire at baseline and during the last week of the study. Results: At inclusion, stool characteristics and QoL were similar in both groups. The control group was slightly older than the intervention group ($7.5{\pm}3.9$ vs. $6.2{\pm}3.6$ weeks). At onset, stool composition was "hard and tight" (Bristol stool scale 1 and 2) in all infants. After one month, stool composition remained unchanged in the control group except in two infants that developed "creamy" stools (Bristol stool scale type 3 and 4). In the intervention group, stools remained "hard and tight" in 27.5%, and became "creamy" in 47.5%, "loose" (Bristol stool scale type 5) in 22.5% and "watery" (Bristol stool type 6 and 7) in 2.5%. The benefit of the intervention formula was estimated to be "very important" in 70%. The median scores for QoL improved significantly in the intervention group for all parameters and for one in the control group. Conclusion: The intervention formula significantly improved functional constipation resulting in a better QoL of the parents and infants.

  • Purpose: The purpose of this study was to examine the effects of structured nursing intervention for caregivers on maintenance of intravenous (IV) infusions in infants. Methods: The structured nursing intervention was developed through the processes of interviews with nurses from pediatric wards. To identify the effects of the developed nursing intervention, a non-synchronized non-equivalent control group pretest-posttest design study was conducted with caregivers from the pediatric ward from a hospital in Seoul. Of 100 admitted infants, 50 caregivers were assigned to the intervention group and received the structured nursing intervention for maintenance of intravenous infusions in infants. The others were assigned control group and received routine care. Data were collected on patient factors, IV insertion factors, treatments, and IV related complications. The form developed for the structured nursing intervention was used by staff nurses. Results: Compared to the control group, IV insertion frequency in experimental group infants was significantly lower and IV related complications decreased (p<.05). Conclusion: These results suggest that the structured nursing intervention for caregivers on maintenance of intravenous infusions may have effects on maintenance of intravenous infusions in infants, and decreasing IV related complications. This nursing intervention can be used to improve IV related problems of admitted infants.

The impacts of exercise on pediatric obesity

  • Over the last few decades, the rates of pediatric obesity have more than doubled regardless of sociodemographic categorization, and despite these rates plateauing in recent years there continues to be an increase in the severity of obesity in children and adolescents. This review will discuss the pediatric obesity mediated cardiovascular disease (CVD) risk factors such as attenuated levels of satiety and energy metabolism hormones, insulin resistance, vascular endothelial dysfunction, and arterial stiffness. Additionally, early intervention to combat pediatric obesity is critical as obesity has been suggested to track into adulthood, and these obese children and adolescents are at an increased risk of early mortality. Current suggested strategies to combat pediatric obesity are modifying diet, limiting sedentary behavior, and increasing physical activity. The effects of exercise intervention on metabolic hormones such as leptin and adiponectin, insulin sensitivity/resistance, and body fat in obese children and adolescents will be discussed along with the exercise modality, intensity, and duration. Specifically, this review will focus on the differential effects of aerobic exercise, resistance training, and combined exercise on the cardiovascular risks in pediatric obesity. This review outlines the evidence that exercise intervention is a beneficial therapeutic strategy to reduce the risk factors for CVD and the ideal exercise prescription to combat pediatric obesity should contain both muscle strengthening and aerobic components with an emphasis on fat mass reduction and long-term adherence.

국내 임상에서 실시된 아동간호중재 연구논문(2000-2011년) 동향 분석 (The Analysis of Research Trend on Pediatric Nursing Intervention Studies in Korea, 2000-2011)

  • Purpose: The purpose of this study was to analysis the research trends of pediatric nursing intervention studies in Korea form 2000 to 2011. Methods: A literature search was conducted from the databases. RISS and 'Infant', 'Toddler', 'Preschooler', 'Schooler', 'Adolescent', 'Child', 'Effect', 'Program', 'Intervention' and 'Nursing' were used for keyword searches to find relevant studies. A total of 37 published articles and dissertations in Korea from 2000 to 2011 were reviewed using a structured analytical frame. Results: A quasi-experimental design was more common at 91.9% with a true experimental design at 2.7%. The portion of studies in which theoretical framework was suggested was 27.0%. Only 8.1% mentioned International Review Board (IRB) approval and 78.4% received only the participant's agreement. The participants' developmental stage was mostly preschool stage, and 37.0% of the child's health problem was a chronic disease. Sensory interventions made up 45.9% of the types of intervention and most of the sensory interventions were distraction interventions. The contents of intervention outcomes were physiological (54.0%) and psychological (28.0%) categories mostly. Conclusion: This study will help in identifying current research trends of pediatric nursing intervention studies in Korea and provide basic data for the direction of developing pediatric nursing interventions.

국내 아동간호단위 간호중재 분석 (A Survey of the Nursing Interventions Performed by Pediatric Care Unit Nurses)

  • The purpose of this study was to identify nursing interventions performed by pediatric care unit nurses. For data collection this study used the taxonomy of Nursing Intervention Classification (NIC : 486 nursing intervention) which was modified by McCloskey & Bulecheck(2000). The 419 nursing interventions were selected by panel group, which consist of pediatric clinical experts and nursing scholars. The data were collected 104 nurses of pediatric care unit. There were 158 nursing interventions identified as being used at least monthly 50% or more of the nurses. The 32 nursing interventions were used at least daily, indication a set of core interventions unique to pediatric care unit practice. The most frequently used nursing interventions were 'Medication administration: intravenous' & 'Medication administration: oral'. The 27 nursing interventions were rarely performed by 90% or more of the nurses. The rarely used interventions were 'Ostomy care' & 'Rectal prolaps management'. The domain received the highest mean score was Health System, followed by Physiolocal: complex, Physiolocal: Basic, Safty, Community, Family, Behavior domain. These findings will help in building of a standardized language for the pediatric care units and enhance the quality of nursing care.

지압이 암환아의 오심 구토 정도와 체중변화에 미치는 효과 (Effect of Acupressure on Nausea-vomiting and Weight Change among Pediatric Cancer Patients Receiving Anti-cancer Chemotherapy)

  • Purpose: This study was to confirm the effect of acupressure on the emesis control and the weight change among pediatric cancer patients receiving anti-cancer chemotherapy. Method: Forty pediatric cancer patients, receiving the induction stage of chemotherapy with MTX and vincristine, were divided into control(n=20) and the intervention group(n=20). Both groups received regular anti-emesis medication, but the intervention group was added acupressure maneuver for 5 minutes on P6 point for 3 times a day for 5days: before chemotherapy, lunch and dinner by investigator during the hospitalization and by mother at home. The instruments for this study were Rhode's(1986) Index of nausea, vomiting and retching(INVR), Cas electric scale and pamphlet developed by researcher. Result: Significant differences in the degree of nausea and vomiting were observed between the control and the intervention group as measured by INVR(t=4.73; p=.01). Repeated measures ANOVA also shows that the group effect was significant(F=22.39, P=.01) as was the time effect(F=380.35, P=.01). The group by time interaction was also significant(F=5.27, P=.01). Acupressure maneuver was apparently effective in reducing the degree of chemotherapy-induced nausea and vomiting. There were also statistically significant weight loss noted in the control group than the intervention group(t=5.42, p=.01). Conclusion: Acupressure on P6 point shows an effective adjunct maneuver in reducing the degree of nausea and vomiting and conserving the weight in pediatric cancer patients. Therefore, it is proposed that acupressure should be applied as supportive nursing intervention strategies to relieve chemotherapy induced nausea and vomiting and to prevent weight loss in pediatric cancer patients.

아동작업치료사의 직무분석: 감각통합치료 중심으로 (Job Analysis of Pediatric Occupational Therapists in Korea: Focused on Sensory Integration Intervention)

  • 목적 : 본 연구의 목적은 감각통합치료를 하는 아동작업치료사의 직무를 정의하고, 직무기술서를 도출하며 아동작업치료사를 대상으로 각 책무별 과제와 과제요소의 중요도, 빈도, 난이도 인식도를 조사하는 것이었다. 연구방법 : 2017년 5월부터 9월까지 직무분석을 하였다. 1단계에서 직무분석을 위해 10명의 전문가로 구성된 DACUM 위원회를 조직하였고, 워크숍 및 메일을 통해 감각통합치료를 하는 아동작업치료사의 직무를 정의하고 직무기술서를 완성하였다. 2단계에서 141명의 아동작업치료사를 대상으로 완성된 직무기술서로 설문조사하여 과제와 과제요소의 중요도, 빈도, 난이도를 알아보았다. 연구결과 : 직무의 정의는 '아동 청소년에게 작업치료서비스를 제공하여 일상생활 및 놀이를 포함한 작업수행과 사회참여를 지원한다'이었다. 직무기술서를 통해 9개의 책무(상담, 평가, 치료계획, 치료, 기록, 교육, 자기개발, 관리, 행정), 28개의 과제, 169개의 과제요소가 도출되었다. 중요지수가 가장 높은 과제는 '안전유지하기'와 '치료실행하기'이었고, 과제요소는 '아동과 도구, 주변의 안전 확보하기', '안전한 환경 준비하기'이었다. 결론 : 본 연구의 결과는 감각통합치료를 하는 아동작업치료사의 직무범위를 정의하고, 관련 교육과정을 개발 및 개선하는데 활용할 수 있을 것으로 보인다.

The prevalence and prevention strategies of pediatric obesity: a narrative review

  • Pediatric obesity has rapidly increased globally over the past few decades, including in Korea. We aimed to discuss trends in the prevalence of pediatric obesity and effective prevention strategies. Its prevalence has markedly increased in most high-income nations. According to recent reports, this increase has slowed in developed countries, but the levels remain alarmingly high. In Korea, the rate of pediatric obesity has surged notably since the 1990s; however, since the 2000s, this increase has become more gradual. According to recently published 2017 growth charts, the prevalence of pediatric obesity in Korea varies slightly depending on the data source. The National School Health Examination data showed that pediatric obesity gradually increase from 11.5% in 2014 to 15.1% in 2019, and after the coronavirus disease 2019 pandemic, it sharply increased to 19% in 2021. Based on data from the Korea National Health and Nutrition Examination Survey, the prevalence of pediatric obesity gradually increased from 10.8% in 2017 to 13.6% in 2019. This trend, which accelerated sharply to 15.9% in 2020 and 19.3% in 2021, was especially severe in boys and older children. Pediatric obesity not only affects health during childhood but also increases the risk of developing obesity and associated health conditions in adulthood. Despite ongoing research on treatment options, obesity prevention and control remain challenging. Hence, prioritizing early intervention and prevention of pediatric obesity through healthy eating habits and lifestyles is crucial. This requires intervention at the individual, family, school, and community levels.

Chemoport 제거 중 발생한 카테터 색전증 (Catheter Embolism during Chemoport Removal)

  • Chemoport is widely used in pediatric surgery field. But various complications can occur during the process of insertion or removal of chemoport. Surgeons must be familiar with the treatment of these complications. We had one catheter cuts off during chemoport removal, become a catheter embolism. Interventional radiologic removal was successful. Verifying the length of removed catheter and careful observation of the catheter tip during removal procedure is important to prevent the possibility of catheter embolus. Radiologic intervention was accessible to remove the retained catheter.
