A STUDY OF NEW-PATIENT DISTRIBUTION AND THE MOTIVES FOR VISITING IN THE DEPARTMENT OF PEDIATRIC DENTISTRY AT SEOUL NATIONAL UNIVERSITY DENTAL HOSPITAL (서울대학교 치과병원 소아치과에 내원한 신환 분포 및 내원 동기에 관한 연구)

  • The number of patients requesting services of pediatric dental clinics has been steadily increasing in South Korea. The pediatric dental clinics are comprised of hospital-based clinics and local clinics. The purpose of this study was to analyze new-patient distribution and to survey the motives for visiting the department of pediatric dentistry at Seoul National University Dental Hospital(SNUDH), utilizing questionnaires for parents from May to July 2010. The results are as follows: 1. Age distribution showed that 0-3 year-olds formed the largest group of 29.3%. The average age of patients was 6.2 years old. 2. In the surveys for the parents, the major reason for visiting SNUDH was the referral from other clinics(30.6%). 3. The 75% of patients previously visited other clinics before visiting SNUDH. 1) The greater number of patients(52%) visited general practitioner's clinic than pediatric specialists(48%). 2) More than 90% of the patients visited SNUDH after visiting other clinics for reassuring diagnosis and referrals. 3) The crucial deterring factor of being treated at SNUDH was difficult accessibility.

Sacrococcygeal Abscess in Neonates (신생아 천미추부 농양)

  • The sacrococcygeal region is the frequent site for meningocele, congenital dermal sinus and pilonidal cyst. From May 1995 to July 1998, we have treated 8 neonatal patients with an abscess in the sacrococcygeal area. The mean age at onset was 8.3 days with a range from 6 to 11 days. The sex ratio was 5:3 with male preponderance. Mild fever was the only systemic symptom. Ultrasonogram revealed a slightly hypo echoic lesion in the subcutaneous tissue which became more hypoechoic with time. Pus cultures showed $Staph.$ $aureus$ in 7 patients, two of them had mixed infection with $E.$ $coli$ and other 2 had methicillin-resistant $Staph.$ $aureus.$ The remaining one patient had a mixed infection with $Klebsiella$ and $Proteus.$ Histopathological examination revealed non-specific granuloma and fibrinoid necrotic debris. All patients were easily treated by incision, drainage and proper antibiotics.

Current issues of pediatric inflammatory bowel disease in Korea

  • Inflammatory bowel disease (IBD) is a chronic relapsing disorder of unknown etiology, which is believed to be multifactorial. Recently, the incidence of pediatric IBD has steeply increased in Korea since 2000. Poorly controlled disease activity can result in complications such as intestinal fistulae, abscess, and stricture, as well as growth retardation and delayed puberty in children. Because of a lack of confirmative tests, various diagnostic modalities must be used to diagnose IBD. Onset age, location, behavior, and activity are important in selecting treatments. Monogenic IBD must be excluded among infantile and refractory very-early-onset IBD. Early aggressive therapy using biologics has recently been proposed for peripubertal children to prevent growth failure and malnutrition.

A Clinical Study on Characteristics of Pediatric Patients with Halitosis (소아.청소년 구취 환자의 임상적 특징에 대한 연구)

  • Objectives Halitosis affects a large proportion of the population, and it may cause a significant social or psychological problems but pathophysiological mechanism of halitosis is unclear. The purpose of this study is to investigate pediatric halitosis patients' characteristics and their related symptoms. Methods 133 patients were participated at the Pediatric Clinic of Kyanhee Oriental Medical Center from May 29th, 2010 to July 10th, 2010. We measured sulfide level and reviewed medical charts and asked some questions. Patients of sex, age, respiratory symptoms, digestive symptoms, systemic symptoms and sulfide level were analyzed. Results Stuffy nose, cough after sleep, sputum, snoring, indigestion, stomachache, motion sickness, and Bi-We-Soo diagnosis grades were significantly associated with halitosis Conclusions Based on data, we found that halitosis is related to the respiratory and digestive symptoms. Also, we can refer this study when diagnosing patients or giving treatments to patients.

Rapidly Enlarging Pediatric Cortical Ependymoma

  • We report a 10-year-old boy with supratentorial cortical ependymoma that rapidly grew in the course of 3 years. He suffered generalized seizures when he was 5 years old; MRI showed a small cortical lesion in the right postcentral gyrus. MRI performed 2 years later revealed no changes. For the next 3 years he was free of seizures. However, at the age of 10 he again suffered generalized seizures and MRI disclosed a large parietal tumor. It was resected totally and he remains free of neurological deficits. The histopathological diagnosis was ependymoma. Pediatric supratentorial cortical ependymomas are extremely rare. We recommend including cortical ependymoma as a differential diagnosis in pediatric patients with cortical mass lesions presenting with seizures and careful follow-up even in the absence of symptoms because these tumors may progress.

  • Congenital segmental dilatation of the colon is a very rare entity of unknown etiology, characterized by a localized dilatation of a bowel segment of the colon of variable length and an abrupt transition between the normal and dilated intestine. It can affect any part of the colon, with the rectosigmoid colon being the most commonly affected site. The clinical and radiological features may resemble that of Hirschsprung disease, but differ in that the normal ganglion cells are found in the dilated and normal segment of the colon. We performed laparoscopic-assisted transanal endorectal pull-through for segmental dilatation of rectosigmoid colon in an 8-year-old boy with chronic constipation since the age of 5 months.

Outcome of Extended Porta Hepatis Dissection and Hepatic Portojejunostomy for Biliary Atresia (담도 폐쇄증에서 광범위 간문부 절제 및 간문부-공장 문합술의 치료성적)

  • Extended porta hepatis dissection and hepatic portojejunostomy was performed on 14 biliary atresia patients during last 13 years by a single surgeon. The average age at operation was 68 days(range from 37 days to 98 days). The patients were admitted for 8 weeks postoperatively for administration of parenteral antibiotics. There was one operative mortality due to acute hepatic necrosis. Among 13 patients remaining, 12(92.5 %) became chemically jaundice-free within 36 weeks postoperatively(average 16.8 weeks). the earliest 8 weeks, and in one patients jaundice persisted. Five(38.5 %) patients developed cholangitis after operation. Among jaundice-free patients, one patient died of unrelated disease 2 years after hepatic portojejunostomy, who underwent left lateral segmentectomy because of a biloma. Eleven survivors(78.6 %) are jaundice-free. The oldest one is 13 years old, enjoying a normal life. The mean period of follow-up is 7 years and 3 months.

  • Rectal bleeding is a common and worrisome problem in pediatric practice. The condition is usually alarming to patient and children. Although most cases are benign and self-limited, sometimes serious pathology also present in this manner. And severe rectal bleeding necessitates prompt diagnostic notice and appropriate treatment. The etiologies of rectal bleeding are somewhat specific to certain age group in children. Accurate history and through physical examination are paramount importance to help differentiate of the diverse etiologies causing rectal bleeding in children. This review describes patterns of rectal bleeding, historical and physical aspect, differentiation of clinical characteristics of rectal bleeding according to etiology, and diagnostic investigations.

Clinical Study of Neonatal Gastric Perforation (신생아 위 천공의 임상적 고찰)

  • Gastric perforation of the newborn is a rare and life threatening problem. The pathogenesis of gastric perforation is not clear. Since ischemia is responsible for intestinal perforation, a similar mechanism may result in gastric perforation. Twelve patients with neonatal gastric perforation who were treated at the Department of Pediatric Surgery, Hanyang University Hospital from 1987 to 2002 were reviewed. Eight patients were male and four female. The age of perforation was 1 day to 8 days of life. Ten patients were operated upon and 2 patients were treated nonoperatively. The perforation site was located on the anterior wall along the greater curvature of the stomach in 8 patients and along the lessor curvature of the stomach in 2. The precipitating factors were prematurity, gastroschisis, mechanical ventilation, intestinal obstruction, cyanotic heart disease and indomethacine medication. In 5 cases the cause of perforation was not identified. The mortality rate was 25 % (3 of 12). Earlier recognition and treatment were thought to be crucial prognostic factors.

Fluroscopic Removal of the Foreign Bodies from Gastroesophagus Using the Magnet (자석을 이용한 식도 위 이물 제거술)

  • Ingested foreign bodies are common occurrences in the pediatric population. From October 2002 to April 2006, eight patients (6 male, mean age: $30.9{\pm}14.4$ months, range: 7~45 months) who had ingested metallic foreign bodies, such as bar magnets, coin-type magnets, screws, metal beads, and disk batteries, were selected for foreign body removal using a magnetic device under floroscopic control. A 1-cm-long cylindrical magnet (6 mm in diameter) was placed at the end of a 150-cm-long plastic tube from an IV set. The magnet was passed through the mouth into the stomach. Under fluoroscopic control, the magnet was maneuvered so that it attached to the metallic foreign bodies. The forgeign body was then easily removed by retracting the magnet with the metallic object attached. This procedure was successful in six patients of 8 patients. This procedure is a minimally-invasive and may avoid the use of anesthesics, endoscopy or surgery.
