Should Adjuvant Radiotherapy Be Recommended for Pediatric Craniopharyngiomas?

  • Intracranial tumors secondary to radiotherapy are rare. In this group gliomas are the rarest. Only 6 cases of glioblastoma multiforme (GBM) have been reported in patients undergoing radiotherapy (RT) for craniopharyngiomas of which only 4 have been in children less than 18 years of age. In recent years RT has become a mainstay of adjuvant therapy for recurrent or partially excised craniopharyngiomas. We report a child of 12 years who had previously undergone RT for a suprasellar craniopharyngioma and presented 10 years later with a GBM. This is the 5th pediatric case in literature demonstrating a GBM after RT for a craniopharyngioma. The implications of subjecting the pediatric population to RT for a benign lesion versus the outcome of gross total removal and management of RT induced tumors is discussed and the need to avail of safer alternatives such as stereotactic radiosurgery is stressed.

  • Sialolithiasis is the formation of calcific concretions within the ductal system of major or minor salivary glands. The exact nature of sialolith evolution is not known. It may occur at any age but, it is most common in middle-aged adult and rare in childhood. In this paper, a case of sialolithiasis just beneath the mucosa in the anterior portion of the Wharton's duct observed in a 5-year old boy. It was approximately $2.5{\times}5$ mm in size and has no clinical symptoms. Under local anesthesia, it was removed by dilatation of orifice of the duct and pressure on the floor of oral cavity by finger.

  • Nitrous oxide has acquired widespread use as a tool for management of anxious and comprehensive pediatric dental patients. Clinical impressions have suggested that inhalation sedation with $N_2O/O_2$ reinforces the effect of other sedatives. Fifteen uncooperative children, mean age of 37months, who were regarded as fail in the first treatment using chloral hydrate-hydroxyzine orally, midazolam orally or intranasally, were sedated with the same sedatives in conjunction with $N_2O/O_2$. All the children were restrained in a pediwrap and were monitored with pulse oximeter for their heart rate and oxygen saturation. Degree of sleep, crying and movement were evaluated. The clinical results indicated that a satisfactory level of sedation was achieved in approximately two thirds of the cases (about 66 %) under those sedation manner. Complications were rare and those of treatments were successfully completed without difficulty.

Pediatric Nurses' Knowledge and Attitude towards Management of Children's Postoperative Pain (아동병동 간호사의 수술 후 통증 관리에 대한 지식과 태도)

  • Purpose: This study aimed to describe pediatric nurses' knowledge and attitude towards management of children's postoperative pain. Methods: In this cross-sectional descriptive study, the participants were 220 pediatric nurses who worked at a general hospital in Seoul. The survey questionnaires used to assess the nurses' knowledge and attitude regarding children's postoperative pain were developed for this study. Results: The average score for knowledge was 67.7 out of 100. The mean score for attitude was 72.5 out of 100. The factor related to the knowledge level was education for pain management. Moreover, age, working department, position, working experience, education level, and number of children were associated with the attitude. Conclusion: The findings of this study suggested the need for a systematic education program for pain management of children in the postoperative condition.

  • Fewer patients undergo pediatric lung transplantation (PLT) than adult lung transplantation. Size mismatch is the key factor that limits the availability of potential donors. Every candidate for PLT is in a different scenario in terms of age, height and weight, size of structures, indications for PLT, the concomitant presence of a cardiac anomaly, and other individual-specific factors; thus, a thorough understanding of pediatric patients' medical problems is essential. Living-donor lobar lung transplantation (LDLLT) has only been performed once in Korea to date. However, since each step in the LDLLT is a well-established procedure, including intrapericardial lobectomy, lung procurement, and lobar lung transplantation, qualified surgeons and lung transplantation teams are competent to perform LDLLT in clinically necessary situations.

  • Ameloblastic fibro-odontoma (AFO) is a rare odontogenic tumor, which occurs in young children before the age of 20 years. Radiologically, it is a well-defined radiolucent lesion containing radiopaque masses. Histopathologically, AFO is composed of odontogenic epithelium in a primitive-appearing connective tissue and hard tissue consisting of enamel and dentin. It is commonly found in the mandibular posterior region. AFO may be asymptomatic and is often associated with delayed tooth eruption. As it shows similar characteristics clinically and radiologically to odontoma, differential diagnosis through histopathological examination is important. Treatment of AFO is conservative enucleation, and teeth enclosed or associated with the lesion may require extraction. In this report, 2 young patients who visited our clinic with a chief complaint of delayed eruption were diagnosed as AFO with radiological and histopathological examination. After the surgery, the healing status was found to be favorable, and no evidence of recurrence was observed.

AGE ESTIMATION USING PANORAMIC RADIOGRAPHS OF DEVELOPING PERMANENT TEETH (발육중인 영구치의 파노라마 방사선 사진 계측을 이용한 연령 추정)

  • The aim of this study was to test whether metric measurements of crown length, root length and apex width during tooth development could be a better basis for correlation with age than the classical methods based on subjective estimations of various stages of tooth development. Panoramic radiographs of 120 children, aged 7 to 9 years, were collected from the department of the pediatric dentistry of Chonnam National University Hospital, Korea. The methods of Mornstad was used to estimate age. The structures measured were crown length root length and apex width in panoramic radiographic. The results were as follows : 1. In the boys, it showed higher correlation between lower 2nd molar crown length, lower 1st molar root length or lower 1st permolar apical width and age. In the girls, it showed higher correlation between lower 2nd premolar crown length, lower 2nd molar root length or lower 1st molar apical width and age. 2. With the aid of a multiple regression model, a linear relationship between some of these distances and age was shown. Boy(months) = 43.958 + lower 2nd molar crown length ${\times}$ 4.392 + lower 1st molar root length ${\times}$ 2.255 - lower 1st permolar apical width ${\times}$ 2.046, Girl(months) = 75.213 + lower 2nd premolar crown length ${\times}$ 3.910 lower 2nd molar root length ${\times}$ 2.280 - lower 1st molar apical width ${\times}$ 6.217 Age was estimated in boys and girls using the mathematic model ; the mean difference between chronological and estimated ages was $-2.1{\pm}6.8$ months for boys and $6.1{\pm}6.2$ months for girls. Therefore, it seems to be more accurate and easier than the earlier methods.

Comparison of the Laparoscopic and Open Peritoneal Dialysis Catheter Insertion in Children (소아 환자에서 복강경 복막 투석관 삽입술과 개복 삽입술의 비교)

  • The aim of this study was to compare peritoneal dialysis catheter insertion by the open method to the laparoscopic method. Twenty four laparoscopic and 10 open peritoneal dialysis catheter placements were performed in children between 2001 and 2008. Patient characteristics, operation related data, procedural complications and clinical outcome were compared. Although there were no cases of catheter obstruction, exit site infection or bleeding in the laparoscopic group, compared to the open method, there was no statistically significant difference between the two groups. Catheter removal rate due to complication was high in the open group and catheter survival rate was high in the laparoscopic group. We concluded that laparoscopic peritoneal dialysis catheter placement is feasible in children of all age groups with at least equivalent functional results compared to the open method. An advantage of laparoscopic catheter insertion is removal of the great omentum and easy fixation of the catheter to the abdominal wall.

Pediatric Laparoscopic Splenectomy (소아의 복강경하 비장절제술)

  • Pediatric laparoscopic splenectomy has been gradually accepted as the surgical management of a various splenic disorders, particularly in hematologic diseases. We report our experience with 16 patients who underwent this procedure because of hematologic disorders during the past 3 and a half years at the Department of Surgery, St. Mary's Hospital, the Catholic University Medical College. The mean age was 10 years(range 6-16 years) and the mean spleen weight was 210 gm(range 85-500 g). The indication for splenectomy were hereditary spherocytosis(6 cases), idiopathic thrombocytopenic purpura(8 cases), autoimmune hemolytic anemia(1 case), and idiopathic splenomegaly(1 case). All splenectomies were performed safely with mean estimated blood loss of 233 ml. Mean operative time and mean postoperative hospital stay were 157 min and 4.5 days, respectively. Postoperative pain, medication was needed in 3 cases, just one injection in immediate postoperative period. Diet was started on the second or third postoperative day. In conclusion, laparoscopic splenectomy in pediatric patients is a safe procedure, offering a small of abdominal scar, much less pain, a shorter hospital stay and car the lower postoperative morbidity.

Treatment of Anogenital Condyloma Acuminata in Children (소아에서항문성기주위에 발생한 Condylomy acuminata의 치료)

  • Condyloma acuminata, an uncommon disease m the pediatric patients, is being reported with increasing frequency in both adults and children. During the last 10 years, we managed 11 children with condyloma acuminata of the anogenital area. The patients' age at presentation varied from 9 months to 11 years, and boys outnumbered girls by a ratio of 2 : 1. The lesions were found in the perianal area in 9 patients and the genital area in 2. No history of sexual contact or abuse could be elicited. Although the exact mode of transmission of human papillomavirus(HPV) couldn't be defined, maternal-infant transmission at birth and close nonsexual family contact were the main possible causes of HPV infection in our patients. Our experience suggest that fulguration of the warts and concomitant excision with scissors of the larger lesions are the most satisfactory method of treatment in children. However, condyloma acuminata have high recurrence rate despite these treatments. Viral typing and careful epidemiological investigation on a larger series of patients may clarify the causative factors and effective modalities of treatment in childhood anogenital condyloma acuminata.
