Current status of vivax malaria among civilians in Korea (한국에서의 민간인 삼일열말라리아 발생현황)

  • A result of national malaria surveillance in Korean civilians was described. Since a case of indigenous vivax malaria was detected in 1993, a total of 2,198 cases was confirmed by blood smear up to 1997. Of them, 1,548 cases were soldiers serving in the demilitarized zone (DMZ), while 650 cases were civilians. Number of civilian cases was 3 in 1994, 19 in 1995, 71 in 1996, and 557 in 1997. Of them, 239 were ex-soldiers who discharged after military service in the prevalent areas such as Paju, Yonchon, Kimpo, Kangwha, Tongduchon in Kyonggi-do and Chorwon in Kangwon-do while 308 patients were civilian residents in the prevalent areas. Seventy-two patients, living nationwide, had a history of visiting the prevalent areas during transmission season. Only 32 civilian patients denied any relation with the prevalent areas. As a whole, a half of the civilian cases was diagnosed when living in non-prevalent areas. Male patients in their twenties was the highest in number. Annual parasite index is steadily elevated in residents living in the prevalent areas. Monthly incidence showed an unimodal distribution, forming a peak in August. Ex-soldiers exhibited a delayed incubation ranging from 153 to 452 days ($279{\pm}41$ days). The time required for diagnosis was shortened from 23.6 days in 1995 to 13.7 days in 1997. Although the current epidemic of vivax malaria started as a border malaria, it seems highly probable that vivax malaria is established in the local areas and responsible for at least a part of transmission.

A Comparison of Clinical Manifestations of Patients with Tsutsugamushi Disease between Children and Adults (쯔쯔가무시병의 임상 양상에 대한 소아와 성인의 비교)

  • Purpose: We compared the clinical manifestations of patients with tsutsugamushi disease between children and adults. Methods: From January 2003 to December 2012, 768 patients diagnosed with tsutsugamushi disease were retrospectively reviewed, and the clinical characteristics, laboratory findings, and complications were compared between children and adults. Results: No patterns of annual increases in the number of patients were noted in both children and adults. The higher incidences occurred in October and November respectively. By gender, male outnumbered female in children, but the opposite trend was seen in adults. By residential area, the urban distribution of children was higher than that of adults. Rashes (P =0.001) and eschar (P =0.004) were more common in children, while myalgia was more common in adults. Children had a high prevalence of anemia (P =0.041), and low incidence rates of thrombocytopenia, abnormal liver and renal function. Children yielded better results in the duration of their hospital stay and the incidence of complications (P <0.001). A comparison of the therapeutic effects of doxycycline and macrolide antibiotics, which was performed only on the children, did not reveal any significant differences. Conclusion: Compared to adults, children had higher incidence rates of male patients and more often suffered from rashes and eschar. Children yielded better results in the laboratory findings and duration of the hospital stay and complications. Therefore, when children are suspected to have tsutsugamushi disease, especially during its peak occurrence period, detailed physical examination and serological test should be performed to ensure a prompt diagnosis, and the use of macrolide antibiotics, which have fewer side effects, is expected to yield the same therapeutic effects.

Skin Radioprotector (Diethone) Modifying Dermal Response of Radiation on Rats (방사선 보호제(Diethone)의 랫드 피부반응에 대한 수식작용)

  • Investigations were carried out into the time-and dose-related changes in acute skin reaction following graded single dose (20,30 and 40 Gy) of x-ray irradiation in Wistar rats, in order to evaluate the radioprotective effect of Diethon on skin. For the duration of skin response over 1. 5 score in dose of 40 Gy, the Diethone group of 24.7 days was significantly different (p<0.02) from that of control (29.8 days) and vaseline (29.2 days) groups, it was $17.1\%$ diminution of skin response period compared with that of control group. By the averaging daily scores for 10 days during peak skin reaction the mean scores were obtained. Mean score of Diethone group $(2.43\pm0.22)$ was significantly different (p<0.01) from that of control $(2.91\pm0.23)$ and vaseline $(2.81\pm0.18)$ groups of 40Gy dose. By iso-effect dose obtained at level of 2.5 score the dose reduction factor (DRF) was 1.41 which reduced radiation dose of $41\%$ by radioprotective effect of Diethone. From this experimental data, it may be possible to give higer radiation dose to large and/or radioresistant tumor mass rather than conventional treatment doses for improving therapeutic ratio by using topical application of skin radioprotector.

Inelastic Time History Analysis of an Unbraced 5-Story Steel Framed Structure for Arrangement of Semi-Rigid Connection (반강접 접합부 배치에 따른 비가새 5층 철골골조구조물의 비탄성 시간이력해석)

  • In this study, an unbraced five-story steel-framed structure was designed in accordance with KBC2005 to understand the features of structural behavior for the arrangement of semi-rigid connections. An inelastic time history analysis of structural models was performed, wherein all the connections were idealized as fully rigid and semi-rigid. Additionally, horizontal and vertical arrangements of semi-rigid connections were used for the models. A fiber model was utilized for the moment-curvature relationship of a steel beam and a column, a three-parameter power model for the moment-rotation angle of the semi-rigid connection, and a three-parameter model for the hysteretic behavior of a steel beam, column, and connection. The base-shear force, top displacement, story drift, required ductility for the connection, maximum bending moment of the column, beam, and connection, and distribution of the plastic hinge were investigated using four earthquake excitations with peak ground acceleration for a mean return period of 2,400 years and for the maximum base-shear force in the pushover analysis of a 5% story drift. The maximum base-shear force and story drift decreased with the outer vertical distribution of the semi-rigid connection, and the required ductility for the connection decreased with the higher horizontal distribution of the semi-rigid connection. The location of the maximum story drift differed in the pushover analysis and the time history analysis, and the magnitude was overestimated in the pushover analysis. The outer vertical distribution of the semi-rigid connection was recommended for the base-shear force, story drift, and required ductility for the connection.

Quality Evaluation of Extracted Citron Juice by Long Term Storage (장기저장에 따른 착즙 유자 과즙의 품질 평가)

  • This study was performed to compare the changes of quality in citron(Citrus junos Sieb) juice between sampleII stored at 5$^{\circ}C$ for 1 year after extraction and sampleI made from raw citrons by the belt-pressing extraction method. Compared with sampleI, the soluble solid of sampleII was decreased more than 1$^{\circ}$brix, and the moisture increased 3%. The acidity reduced from 5.83 to 5.23 at the pH rose from 2.68 to 2.84. Although it decreased more than 50% in vitamin C and over 20~30% in amino acid, the changes of the other proximate components, amino nitrogen and free sugar content were very little at the range 0.1~1.0%. Volatile compounds in citron juices between sampleI and II were analyzed by GC and GC-MS. Sample I and II showed about 70 of volatile compounds. But only 13 compounds were identified by mass spectrometer. Major volatile compounds were aromatic compounds of limonene, terpinene, terpineol and terpinolene. Amounts of volatile compounds in citron juices depended on the storage period. The recovery of volatile compounds of citron juices, reduced 30~50% after storage for 1 year and the trace component disappeared during storage. The sensory characteristics including color, aroma, taste and overall acceptability and sugar recipe were not significantly.

Osteosarcoma in an 8 Month-Old Infant treated with Limb Sparing Operation (8개월된 유아 골육종 환자에서의 사지 보존술)

  • Osteosarcoma is the most common tumor in malignant bone tumors. The peak age incidence in osteosarcoma is between 10 to 14 years of age. This tumor rarely develops under 6 years of age and the youngest patient in the previous literature was a 13 months old girl who had an osteosarcoma involving the second metacarpal bone. We report a case of an 8 month old male infant, who had an osteosarcoma involving the right proximal tibia. This patient was treated by wide excision with transepiphyseal resection and reconstruction with allograft. At 20 months after operation, the varus deformity was developed at the proximal junctional site of allograft. Thereafter, the revision was performed with correction of deformity and augmentation with the proximal fibula transfer. At 51 months after operation, he has been remained as free of disease, and he has recovered the knee motion ranged from 15 degree to 75 degree. The osteosarcoma in infant is very rare but it should be considered the osteosarcoma in the differential diagnosis of any bone lesion. Instead of amputation, the limb sparing operation and the solutions for limb length inequility in growing period should be carefully considered in the infantile osteosarcoma.

An Analysis of Primary Causes for Waiting for Inpatient Admission and Length of stay at Emergency Medical Center(EMC) (응급의료 센터의 체류 및 입원대기 시간 지연 요인 - 일개 의료기관을 중심으로 -)

  • This research identifies the ingress to egress primary factors that causes a patient to receive delayed emergency medical care. This material was collected between February 1st to 28th, 1998. Research envolved 4,118 people who visited the college emergency medical center in Kyeongido Province, South Korea. Medical records were examined, using the retrospective method. to determine the length of stay and the main cause for waiting. Results are as follows : 1. The age group with the highest admission rate was 10 and under, approximately 1,394 (33.9%). Followed by an even distribution for ages between 11-50 at 10-15% for their respective ranges. The lowest admission rate was 50 years and above. 2. From the 4,118 records examined, 3,489 received outpatient treatment (84.7%); 601 were admitted for inpatient care (14.6%); 25 arrived dead on arrival (0.6%); and 4 people died at the hospital. 3. Between 7PM to 12AM, 42.9% were admitted to the EMC. The hours from 9PM to 11PM recorded the highest admission rate and 5AM to 8AM was the lowest From 8PM to 12AM, the most beds were occupied. 4. For most patients. the average length of stay was approximately 2.2 hours. By medical department, external medicine was the longest for 2.8 hours. Pediatrics was the shortest for 1.6 hours. The average waiting period for inpatient admission was 2.6 hours. Inpatient admission for pediatrics and external medicine was 3.4 hours and 2.2 hours respectively. 5. Theses are primary factors for delay at EMC: 1) pronged medical consultations to decide between inpatient versus outpatient treatment, and delaying to be inpatient, 2) when you call physicians they are delayed to come 3) Understaffing during peak or critical hours, 4) Excessive consulting with different medical departments, 5) some patients require longer monitoring periods, 6) medical records are delayed in transit between departments, 7) repeated laboratory tests make delay the result, 8) overcrowded emergency x-ray place causes delay taking x-ray and portable x-ray, 9) the distance between EMC and registration and cashier offices is too far. 10) hard to control patient's family members. The best way to reduce EMC waiting and staying time is by cooperation between departments, both medical and administrative. Each department must work beyond their job description or duty and help each other to provide the best medical service and satisfy the patient needs. The most important answer to shortened the EMC point from ingress to egress is to see things from a patient point of view and begin from there to find the solution.

Soil CO2 Efflux and Leaf-Litter Decomposition of Quercus variabilis and Pinus densiflora Stands in the Southern Region of Korean Peninsular

  • It is necessary to determine the amount of carbon dioxide ($CO_2$) absorbed by plants and released from forest floor into atmosphere, to gain a better understanding how forests participate in the global carbon cycle. Soil $CO_2$ efflux, litter production, and decomposition were investigated in Q. variabilis and P. densiflora stands in the vicinity of Gwangju, Chonnam province. Soil $CO_2$ efflux was measured using Infrared Gas Analyzer (IRGA) at midday of the 10th day at every month over 12-month period, to quantify seasonal and annual budgets of soil $CO_2$ efflux. Soil temperature and soil moisture were measured at the same time. Seasonal soil $CO_2$ efflux in Q. variabilis and P. densiflora were the highest in summer season. In August, maximum soil $CO_2$ efflux in Q. variabilis and P. densiflora was 7.49, $4.61CO_2{\mu}mol{\cdot}m^{-2}{\cdot}s^{-1}$, respectively. Annual $CO_2$ efflux in each stand was 1.77, $1.67CO_2kg{\cdot}m^{-2}$ respectively. Soil $CO_2$ efflux increased exponentially with soil temperature and related strongly in Q. variabilis ($r^2$=0.96), and in P. densiflora ($r^2$=0.91). Litter production continued throughout the year, but showed a peak on November and December. Annual litter production in the Q. variabilis and P. densiflora stands were $613.7gdw{\cdot}m^{-2}{\cdot}yr^{-1}$ and $550.5gdw{\cdot}m^{-2}{\cdot}yr^{-1}$.$yr^{-1}$, respectively. After 1 year, % remaining mass of Q. variabilis and P. densiflora litter was 48.2, 57.1%, respectively. The soil $CO_2$ efflux rates in this study showed clear seasonal variations. In addition, the temporal variation in the $CO_2$ efflux rates was closely related to the soil temperature fluctuation rather than to variations in the soil moisture content. The range of fluctuation of soil $CO_2$ efflux and litter decomposition rate showed similar seasonal changes. The range of fluctuation of soil $CO_2$ efflux and litter decomposition rate was higher during summer and autumn than spring and winter.

Effects of n-3 Polyunsaturated Fatty Acids-enriched Diet Supplemented with Different Levels of α-Tocopherol on Lipid Metabolism in Laying Tsaiya Ducks

  • The objective of this experiment was to determine the effects of n-3 polyunsaturated fatty acids (n-3 PUFAs)-enriched diet supplemented with different levels of $\alpha$-tocopherol on the activities of hepatic lipogenic-related enzymes and the contents of liver and plasma lipid fractions in laying Tsaiya ducks. A total of 180 30-wk-old laying Tsaiya ducks, at the beginning of peak production, were allotted into 6 treatments with 3 replicates each. Ducks were fed one of the 6 experimental diets, containing 4% tallow (control), and 4% fish oil supplemented with graded levels of $\alpha$-tocopheryl acetate ($\alpha$-tocopherol) at 0, 100, 200, 300 and 400 mg/kg, respectively, for 6 wks. Feed and water were supplied ad libitum throughout the experimental period. The results indicated that the n-3 PUFAsenriched diet supplemented with different levels of $\alpha$-tocopherol did not affect (p>0.05) egg weight, feed intake, body weight change or liver and abdominal fat weights. Egg production, egg mass and feed efficiency significantly (p<0.05) improved as dietary $\alpha$-tocopherol levels increased. The activities of hepatic lipogenic-related enzymes including acetyl-CoA carboxylase (EC 6. 2. 1. 3; ACC), glucose-6-phosphate dehydrogenase (EC 1. 1. 1. 49; G-6-PDH), ATP-citrate cleavage enzyme (EC 4. 1. 3. 8; CCE), NADP-malate dehydrogenase (EC 1.1.1.40; NADP-MDH) and fatty acid synthetase (FAS) were higher (p<0.05) in birds fed with the tallow diet than in those fed with fish oil diets and increased with increasing dietary $\alpha$-tocopherol levels. None of the dietary treatments significantly affected the contents of triglyceride and total cholesterol in the liver, or total cholesterol, phospholipid and total lipid in the plasma. However, the contents of phospholipid and total lipid in the liver, and triglyceride in the plasma increased as dietary $\alpha$-tocopherol levels increased. Increasing dietary $\alpha$-tocopherol levels decreased the non-esterified fatty acid (NEFA) content in the plasma and trended to decrease the cholesterol contents in the egg yolk. The lipid metabolism of laying Tsaiya ducks was influenced not only by the dietary fat but also by the supplementation levels of $\alpha$-tocopherol.

Reaction of Mast Cells and Goblet Cells in the Small Intestine of C57BL/6 and C3H/HeN Mice Infected with Echinostoma hortense

  • Mast cells and goblet cells have been known to protect the host against parasites. In this study, we examined the response of the mast cells and goblet cells over a period of 6 weeks in the duodenum, jejunum and ileum of C3H/HeN and C57BL/6 mice infected with Echinostoma hortense (E. hortense). In addition, we investigated whether the worm recovery rate of uninfected mice (the control group) or E. hortense-infected mice (the experimental group) was associated with the number of mast cells and goblet cells. The worm recovery rate was higher in the C3H/HeN mice than in the C57BL/6 mice. The number of goblet cells significantly increased in the experimental group of the C3H/HeN and C57BL/6 mice compared with the control group of both strains (P<0.005). Worm recovery peaked 3 weeks after the infection of the C57BL/6 mice and at 2 weeks after the infection of the C3H/HeN mice, and it was higher in the duodenum than in the jejunum and ileum. However, the infected site in the intestine had no relation with worm expulsion. In the C3H/HeN and C57BL/6 mice, the number of goblet cells in the experimental group was significantly higher than that in the control group (P<0.005). The number reached a peak 2 weeks after the infection and it even increased in duodenum, jejunum and ileum. The increased number of goblet cells was retained 6 weeks after infection. The number of goblet cells was higher in the C3H/HeN mice than in the C57BL/6 mice (P<0.01). These results indicate that goblet cells are related with the worm expulsion. Furthermore, immunohistostaining of the antral intestinal walls for lectin showed the significant increase of the number of goblet cells in the experimental group (P<0.001). The high infection rate in the duodenum was found during the early infection. An increased infection rate in the jejunum and ileum was found 3 weeks after infection and the infection rate was higher in the C3H/HeN mice than in the C57BL/6 mice. Taken together, the present study indicates that goblet cells, rather than mast cells, may play critical roles in parasite expulsion.
