The Clinical Results of Anterior Cruciate Ligament Reconstruction in over 50 year (50세 이상의 연령에서 전방십자인대 재건술의 임상적 결과)

  • Purpose: To compare the clinical results after reconstruction of the anterior cruciate ligament (ACL) between above the 50 years old patients and below the 50 years old patients retrospectively. Materials and Methods: The results obtained 18 patients above 50 years old (mean age 55.1) and 198 patients below 50 years old (mean age 31.2) who underwent reconstruction of the ACL between July 2004 and April 2008. Clinical evaluation was performed with the International Knee Documentation Committee (IKDC) and Lysholm knee score scale. Anterior displacement of knee was measured by KT-2000 arthrometer. Results: Lysholm scores improved from 69.5 to 93.6 in patients above 50 years old, from 69.0 to 92.6 in patients below 50 years old (p<0.05). According to IKDC score, results more than nearly normal was 83% (15 cases) in patients above 50 years old, 86% (170 cases) in patients below 50 years (p<0.05). The mean difference of anterior displacement measured by KT-2000 was decreased from 7.4 mm to 2.2 mm in patients above 50 years old, from 7.1 mm to 2.0 mm in patients below 50 years old (p<0.05). Conclusion: There was no significant difference between the patients above 50 years old and below 50 years old. In patients above 50 years old, the reconstruction of ACL obtained satisfactory results.

Surgical Treatment of Patent Ductus Aretriosus: Report of 40 cases (개방성 동맥관 수술 보고 (40 예))

  • Since 1973, 40 patients with Patent Ductus Arteriosus were operated in The Department of Thoracic and Cardiovascular Surgery, Korea University Medical College. Clinical analysis of these cases: 1. Age ranged from 8 months old to 28 years old Sex ratio was 22 females to 18 males. 2. Pulse pressure widening above 50 mmHg was appeared in 27 patients. In 31 patients, continuous machinery murmur was heard on left second and third intercostal space, but 9 patients has holosystolic murmur on left sternal border. 3. Retrograde Aortography was performed in 11 patients and right heart catheterization, 17 patients, In 8 patients, pulmonary hypertension [above 30 mmHg in` systole] was noted. 3 out of 8 patients was combined with Ventricular Septal Defect. Severe pulmonary hypertension [above 80mmHg in systole] was presented in 3 patients. 4. In 38 patients, operative method was performed with multiple suture ligation of PDA, and in 2 patients, suture closure through pulmonary arteriotomy under cardiopulmonary bypass. 5. One operative death occurred in a patient in this group. Cause of death was right heart failure after multiple suture ligation of PDA and pulmonary artery bandings.

Clinical Investigation of Herpes Zoster and Postherpetic Neuralgia above T4 Dermatome (상흉부 이상 부위의 대상포진 및 대상포진후 신경통 환자에 대한 임상적 고찰)

  • Postherpetic neuralgia is frequently painful, incapacitating, mood depressing, and sometimes lifelong. We investigated the influence of duration from eruption to nerve blocks in conjunction with patients age on analgesic and preventive effect for postherpetic neuralgia. We retrospectively evaluated 50 outpatient medical records for the above T4 dermatome. Patients had been referred to pain clinic and were treated over 2weeks from Jan. 1988 to Dec. 1993. Fifty patients were divided into 4 groups as follows: Group I (a): less than 4weeks from eruption to nerve block and younger than 65 years old. Group I (b): less than 4weeks from eruption to nerve block and older than 65 years old. Group II (a): more than 4weeks from eruption to nerve block and younger than 65 years old. Group II(b): more than 4weeks from eruption to nerve block and older than 65 years old. Mean number of stellate ganglion blocks were 29.7. Tramadol, amitriptyline, nicardipine were most commonly prescribed. Group I (a): had the most improvement rate(77.8%) as compared with other group(46.6, 52.7, 56.0%). Number of patients who complained of severe pain (VAS > 5) were 0, 3(39%), 2(15.4%), 5(30%) in I (a), I (b), II (a), II (b) group respectively. In conclusion, analgesic effect was best in cases of patients younger than 65 years old whose treatment were started within 4 weeks of eruption. Patients older than 65 yrs, analgesic effect did not vary on the timing of treatment.

  • The author reviewed 50 cases of positive sputum AFB patients following pulmonary resection of pulmonary tuberculosis [total 617 cases] operated on at the National Kongju Hospital during 6 years period, from January, 1985 to December, 1990. 1. There were 36 male and 14 female patients ranging from 20 to 50 years old[mean age, 33.8 years] 2. An average duration of pulmonary tuberculosis history was 7.5 years. 35 cases[76%] had many drug resistance of tuberculosis [above 5 drugs]. 3. The majority indication for pulmonary resection were persistent positive sputum AFB with cavity or destroyed lung or hemoptysis. 23 cases[46%] underwent pneumonectoy and 13 cases[23%] lobectomy. The postoperative complications occured in 19 cases [38%]. 4. 34 cases[68%] occured sputum AFB positive following operation unitil 6 months, and 6 cases[12%] occured 2 years later 5. 21 cases[42%] got conversion to negative sputum, and then 29 cases[58%] remained persistent positive sputum.

Laparoscopic Total Gastrectomy in Elderly Patients (≥70 Years) with Gastric Carcinoma: A Retrospective Study

  • Purpose: To compare the surgical outcomes of laparoscopic total gastrectomy between elderly and non-elderly patients. Materials and Methods: Between 2008 and 2015, a total of 273 patients undergoing laparoscopic total gastrectomy for gastric carcinoma were divided into two age groups: elderly (${\geq}70$ years, n=71) vs. non-elderly (<70 years, n=172). Postoperative outcomes, including length of hospital stay, morbidity, and mortality were compared between the groups. Results: The elderly group showed a significantly higher rate of comorbidities and American Society of Anesthesiologists scores than those in the non-elderly group. No significant differences were found with respect to lymphadenectomy or combined organ resection between the groups. After surgery, the elderly group showed a significantly higher incidence of grade III and above complications than the non-elderly group (15.5% vs. 4.1%, P=0.003). Among the complications, anastomosis leakage was significantly more common in the elderly group (9.9% vs. 2.9%, P=0.044). Univariate and multivariate analyses showed that old age (${\geq}70$ years) was an independent risk factor (odds ratio=4.42, 95% confidence interval=1.50~13.01) for postoperative complications of grade III and above. Conclusions: Elderly patients are more vulnerable to grade III and above complications after laparoscopic total gastrectomy than non-elderly patients. Great care should be taken to prevent and monitor the development of anastomosis leakage in elderly patients after laparoscopic total gastrectomy.

Pattern of Lymph Node Pathology in Western Saudi Arabia

  • Background: This study aimed to characterize the histopathological pattern of lymph node pathology among Saudi patients and to highlight the age and gender variations of these lesions as base line data. Materials and Methods: We retrospectively analyzed the data from lymph node biopsy specimens received at the Department of Pathology, King Fahad Hospital, Madinah, Saudi Arabia from January 2006 to December 2013. Results: Of the 289 lymph node biopsy specimens received, 154 (53.3%) were from males and 135 (46.7%) from females giving a male: female ratio of 1.14:1. Age of the patients ranged from 2.5 to 96 years with a mean age 33.9 years. The commonest lymph node group affected was the cervical (30.4%) followed by axillary (9.7%) and inguinal (8.7%). Malignant lymphoma [71 Hodgkin's disease (HD), 57 non Hodgkin's lymphoma (NHL)] 128 (44.3%), reactive hyperplasia 68 (23.5%), and tuberculosis 41 (14.2%) were the common causes of lymph node enlargement. While HD, reactive hyperplasia and tuberculosis were commonest in young adult patients (10-29 years old) and rare above the age of 50 years; NHL was the predominant cause of lymph node enlargement above 50 years. Conclusions: Lymph node biopsy plays an important role in establishing the cause of lymphadenopathy. Among the biopsied nodes, lymphomas were the most common (44.3%) followed by non-specific reactive hyperplasia (23.5%) and tuberculous lymphadenitis (14.2%).

Assessment and Clinical Significance of Haematuria in Malaysian Patients - Relevance to Early Cancer Diagnosis

  • Aim: To study the causes and significance of both microscopic and macroscopic haematuria in adult patients and assess possible relevance to early detection of urological cancers. Methods: 417 patients presenting with haematuria were assessed in our Urology Unit. Following confirmation of haematuria, these patients were subjected to imaging techniques and flexible cystoscopy. Parameters analysed included clinical characteristics, imaging results, flexible cystoscopy findings, time delay to diagnoses and eventual treatment and final diagnoses of all cases. Results: 390 haematuria cases were analysed from 417 consecutive patients with haematuria. After 27 cases were excluded as they had previous history, 245 microscopic and 145 macroscopic. Age range was 17 to 95 years old with predominance of 152 females to 239 males. The racial distribution included 180 Chinese, 100 Indians,95 Malays and 15 other races. The final diagnoses were benign prostatic hyperplasia (22.6%), no cause found (22.3%), other causes (18.7%), urolithiasis (11.5%), urinary tract infection UTI (10.8%), non specific cystitis (10.3%), bladder tumours (2.8%) and other genitourinary tumours (1%). 11 new cases (2.8%) of bladder cancers were diagnosed, with a mean age of 59 years. Only 3 of 245 (1.2%) patients with microscopic haematuria had newly diagnosed bladder tumour compared with 8 of 145 (5.5%) patients with frank haematuria (p=0.016). Mean time taken from onset of symptoms to diagnosis of bladder cancer was 53.3 days with definitive treatment (TURBT) in 20.1 days from diagnosis. Conclusion:- This study has highlighted the common causes of haematuria in our local setting. We recommend that full and appropriate investigations be carried out on patients with frank haematuria especially those above 50 years old in order to provide earlier detection and prompt management of bladder diseases especially tumours.

A Study on Knowledge and Disease Management of Tuberculosis by Themselves of Tuberculosis Patients Among the Middle and High School Students in Seoul. (서울시내 중고등학교 결핵이환학생에 대한 결핵관리실태 및 지식에 관한 조사연구)

  • The purpose of this survey was to find out the knowledge and the disease management of the Tuberculosis patients by themselves among the first grade middle and high school students in Seoul city during the period of June 15-July 19, 1986. Questionaires were used to collect the data and were analysed from answers of 188 students at the 113 schools. The results were as follows: 1. The ages of the students were distributed as follows: in middle school, 13 years old was $70.0\%$. 14 years old, $20.0\%$, and 15 years old, $6.7\%$. In high school, 16 years old was $66.5\%$, 17 years old, $18.4\%$, and 15 years old, $10.1\%$. 2. In X-ray mass examination by school, the rate of execution was $50.4\%$ in middle school and $96.7\%$ in high school, and in X-ray mass examination by student, it was $50.1\%$ in middle school and $97.3\%$ in high school. 3. The prevalence of Tuberculosis among the middle school students was $00.3\%$and high school students, $0.15\%$. 4. Of the total, $77.1\%$ of the respondents did not realized Tuberculosis bdore X-ray mass examination 5. The perfect cure rate of the respondents was $52.7\%$. 6. General characteristics of the respondents: a. The educational background was varied as follows: of the total $47.9\%$ of the fathers had the level of high school education and $37.2\%$ of the mothers had the level of middle school education. The educational background of the parents had no statistical significant to the medical cure rate of Tuberculosis. (P>0.05). b. The average monthly income of the family was as follows : above five hundred thousand won was $21.8\%$, three or four hundred thousand won was $22.9\%$, and below two hundred thousand won was $10.6\%$. The most frequent family size was 5-6 persons. $(59.6)\%$. 7. The actual situation of Tuberculosis control and the variables related to the treatment: a. $69.1\%$ of the respondents wanted mental support from their surroundings. $48.7\%$ of the respondents answered that their parents or the other family helped treatment as mental supporter, b. As a medical service, $53.2\%$ of the respondents were treated at Health Center, $38.8\%$ were treated at a hospital. A medical service was statistically significant to the medical cure (P<0.01). c. Family members of $61.7\%$ of the respondents had checked chest X-ray. A X-ray examination of family was statistically significant to the medical cure (P<0.005). d. $73.9\%$ of the respondents had taken the Anti-Tuberculosis-drugs regularly. Regular taking of Anti-Tuberculosis drugs was statistically significant to the medical cure (P<0.005). e. $89.4\%$ of the respondents had received a regular examination during the treatment. A regular examination was statistically significant to the medical cure (P<0.05). f. The period of perfect cure was that $50.0\%$ of the respondents took from half a year to one year, $25.2\%$ took below half a year and $16.2\%$ took from one year to one year and a half. g. The rate of the respondents who abhored to let anyone know their disease was $93.1\%$. 8. Knowledge related with Tuberculosis: a .$63.3\%$ of the respondents answered that Tuberculosis is a communiable disease. b. $89.9\%$ of the respondents answered that there is a preventive method of Tuberculosis. Among them, $28.4\%$ answered that it is B.C.G. vacination. c. $96.8\%$ of the respondents belived they can be cured perfectly. d. $42.4\%$ of the perfect curer answered that they had have permanent immunity of Tuberculosis. According to the results of above study, it is desired to be practiced X-ray mass examination to the total middle school students. Nurse teachers and the responsible persons who participated to the helping of disease management to the Tuberculosis patients must make an offer knowledge of Tuberculosis to the Tuberculosis patients. And also, it will be very helpful to the cure of Tuberculosis patients if they do their best and to have a mental supporter.

Clinical Characteristics of Acute Appendicitis in Children and Elderly Patients (소아와 고령자에서 급성 충수염의 임상적 특성)

  • Acute appendicitis is one of the most common diseases of young people that requires emergency operation. This is especially true for those age 10 years old and older. However, the numbers of cases of appendicitis are increasing in both the young and the elderly. The main purpose of this study is to understand the clinical features of acute appendicitis in children and elderly patients. We retrospectively compared clinical characteristics in 4 groups of patients who underwent appendectomy in our hospital. There were 16 patients in the pre-school age group, 9 were males and 7 females. The total number of patients in the juvenile group were 287, 156 were males and 131 females. The total numbers of patients in the adult group were 794, 436 were males and 358 females. The numbers of elderly patients were 189, 91 were males and 98 females. Complicated appendicitis was found in 15 patients (93.7 %) in the pre-school age group, 79 patients (27.5 %) in the juvenile group, 332 patients (41.8 %) in the adult group, and 96 patients (50.7 %) in the elderly group. Four patients (40 %) had generalized panperitonitis in the pre-school group. The occurrence of perforated appendicitis was the highest in the pre-school age group and the lowest in the juvenile group. Since generalized panperitonitis has a higher incidence in the pre-school age group, prudent and careful diagnosis and treatment are required for the pre-school age group.

Prostate Cancer in Younger and Older Patients: Do We Treat Them Differently?

  • Diagnostic and therapeutic strategies of prostate cancer may largely influenced by patients' age at presentation. This study is aimed to evaluate the characteristics, diagnostic and treatment strategies in prostate cancer patients in our centres. A cross-sectional analytic study of prostate cancer data in two main referral cancer centres, Cipto Mangunkusumo General Hospital and Dharmais National Cancer Centre from 1995-2010, was therefore performed. Patients were divided into 2 sub-populations; below 60 years (younger patients) and 75 years old and above (older patients). PSA levels, diagnostic modalities, Gleason score and therapeutic options were analysed for both and compared using bivariate analysis. 152 patients were <60 years and 210 were ${\geq}75$ years. There was no statistical difference in mean PSA level (797.9ng/mL vs 345.3 ng/mL, respectively; p>0.05) and diagnosis was made by biopsy in majority of patients in both groups (68.2% and 71.6% in younger and older groups respectively). Most presented with an advanced disease stage (65.1% and 66.0%, respectively) and there was no statistically significant difference in mean Gleason scores f (8.1 vs 7.7; p>0.05). Primary androgen deprivation therapy (PADT) was the main treatment for overall patients (48.0% and 50.7%, respectively). Radiotherapy and radical prostatectomy are the main therapeutic modalities for younger patients with local and locally advanced disease (39.6% and 35.4% respectively), while the majority of older patients with the same disease stage were treated with radiotherapy and PADT (45.8% and 39.0% respectively). Differences observed in treatment modalities were statistically significant (p<0.0003). We conclude that there is no difference in disease clinical aggressiveness of the two groups but significant differences were observed in therapeutic strategies utilised with younger and older patients.
