Changes of the liver volume and the Child-Pugh score after high dose hypofractionated radiotherapy in patients with small hepatocellular carcinoma

  • Purpose: To investigate the safety of high dose hypofractionated radiotherapy (RT) in patients with small hepatocellular carcinoma (HCC) in terms of liver volumetric changes and clinical liver function. Materials and Methods: We retrospectively reviewed 16 patients with small HCC who were treated with high dose hypofractionated RT between 2006 and 2009. The serial changes of the liver volumetric parameter were analyzed from pre-RT and follow-up (FU) computed tomography (CT) scans. We estimated linear time trends of whole liver volume using a linear mixed model. The serial changes of the Child-Pugh (CP) scores were also analyzed in relation to the volumetric changes. Results: Mean pre-RT volume of entire liver was 1,192.2 mL (range, 502.6 to 1,310.2 mL) and mean clinical target volume was 14.7 mL (range, 1.56 to 70.07 mL). Fourteen (87.5%) patients had 4 FU CT sets and 2 (12.5%) patients had 3 FU CT sets. Mean interval between FU CT acquisition was 2.5 months. After considering age, gender and the irradiated liver volume as a fixed effects, the mixed model analysis confirmed that the change in liver volume is not significant throughout the time course of FU periods. Majority of patients had a CP score change less than 2 except in 1 patient who had CP score change more than 3. Conclusion: The high dose hypofractionated RT for small HCC is relatively safe and feasible in terms of liver volumetric changes and clinical liver function.

Correction of Dose Distribution at Total Body Irradiation using Compensator

  • The using of compensator is required to adjust the irregular dose distribution due to irregular thickness of the body in Total Body Irradiation. Aluminuim, copper or lead is generally used as compensator. In our study, we would like to introduce a result of the attenuation and compensation effect of radiation use compensator made by duralumin and its clinical use. The thickness of compensator was calculated by the attenustion of radiation, which was measured by polystyrene phantom and ionization chamber(farmer). The compensation effect of radiation was measured by diode detector. All of conditions were set as in real treatment, and the distanc from source to detector was 446 cm. We also made fixation of device to easily attach the compensator to LINAC. Beam spoiler was menufactured and placed on the patient to irradiate sufficient dose to the skin. diode detector were placed on head, neck, chest, umbilicus. pelvis and knee with each their entranced exit points, and datas of dose distribution were evaluated and compared in each points for eleven patients(Feb. 96-Feb. 97). The attenuation rate of irradiation by duralumin compensator was measured as $1.4\%$ in 2mm thickness. The mean attenuation rate was $1.3\%$ per 2mm as increasing the thickness gradually to 50 mm. By using duralunim compensator, dose distribution in each points of body was measured with ${\pm}2.8\%$ by diode detectior. We could easily calculate the thickness of compensator by measuring the attenuation rate of radiation, remarkably reduce the irragularity of dose distribution duo to the thickness of body and magnify the effect of radiation therapy.

Is it possible to treat COVID-19 pneumonia by low-dose radiation?

  • Low-dose radiotherapy has been known to have anti-inflammatory activity and been used for treatment of pneumonia together with anti-serum and sulfanilamide. However, it rapidly discontinued after the development of various antibiotics showing outstanding effect. Recently, it was re-considered to treat COVID-19 which has very limited treatment such as remdesivir and dexamethasone. So, several studies of COVID-19 therapy using low-dose radiation were reported very recently. They showed that low-dose radiation of 0.5~1.5 Gy were useful for decreasing the oxygen consumption and hospitalization period of COVID-19 patients without adverse reaction. Radiopharmaceuticals such as [99mTc]Tc-macroaggregated albumin (MAA) also might be used for low-dose radiotherapy. Administration of vitamin D having anti-inflammatory effect would also be helpful for therapy with synergistic effect.

Moderately Hypofractionated Conformal Radiation Treatment of Thoracic Esophageal Carcinoma

  • Aims: To prospectively assess the efficacy and safety of moderately hypofractionated conformal radiotherapy in patients with thoracic esophageal cancer. Methods and Materials: From Sept. 2002 to Oct, 2005, 150 eligible patients with T2-4N0-1M0 stage thoracic esophageal squamous cell cancers were enrolled to receive either conventional fractionated radiation (CFR) or moderately hypofractionated radiation (MHR) with a three-dimensional conformal radiation technique. Of the total, 74 received moderately hypofractionated radiation with total dose of 54-60Gy/18-20fractions for 3.5-4 weeks in the MHR arm, and 76 received conventional radiation with total dose of 60Gy/30 fractions for 6 weeks in the CFR arm. Concurrent chemotherapy comprised of paclitaxel and cisplatin. Safety was evaluated, and local control and overall survival rates were calculated. Results: Statistically significant differences between the CFR versus MHR arms were observed in local/regional failure rate (47.3% v 27.0%, P=0.034) and the percentage of patients with persistent local disease (26.3% v 10.8%, P=0.012). But 3 and 5-year overall survival rates (43.2%, 38.8% v 38.2%, 28.0%, respectively) were not different between the two arms (P=0.268). There were no significant differences in the incidences of grade 3 or higher acute toxicities (66.3% v 50.0%) and late complications rates (27.0% v 22.4%) between the MHR and CFR arms. Conclusions: Moderately hypofractionated, three-dimensional radiation treatment could improve the local control rate of esophageal cancer and potentially increase patients' survival.

편평상피 세포성 폐암의 방사선치료 실패 양상 (The Pattern of Failure after Definitive Radiotherapy for Squamous Cell Carcinoma of the Lung)

  • From Feb.1985 to Feb.1988,76 patients with squamous cell carcinoma of the lung treated at the Department of Therapeutic Radiology in Kyungpook National University Hospital were available for the analysis of this study. All patients received radiation of 4000cGy-6600cGy with curative aim. The overall rate of complete response was 25.0% and partial response was 452.6% The complete and partial regression of tumor was 14.3% in patients treated with dose below 5000cGy and 84.1% in the group treated with dose above 5000cGy (p<0.01). The complete response was seen only in the group of patients received radiation at least 6000cGy. The patterns of failure were as follows. The rate of initial intrathoracic recurrence was 52.6% in patients with complete response. The overall rate of failure was 68.8%. Distant metastasis was found in 47.4% of patients. Bone, contralateral lung, and brain were common metastatic sites in decreasing order All of the distant metastases and 80% of local recurrences were found within the first year after treatment.

Is higher dose always the right answer in stereotactic body radiation therapy for small hepatocellular carcinoma?

  • Purpose: This study was conducted to compare clinical outcomes and treatment-related toxicities after stereotactic body radiation therapy (SBRT) with two different dose regimens for small hepatocellular carcinomas (HCC) ${\leq}3cm$ in size. Materials and Methods: We retrospectively reviewed 44 patients with liver-confined HCC treated between 2009 and 2014 with SBRT. Total doses of 45 Gy (n = 10) or 60 Gy (n = 34) in 3 fractions were prescribed to the 95% isodose line covering 95% of the planning target volume. Rates of local control (LC), intrahepatic failure-free survival (IHFFS), distant metastasis-free survival (DMFS), and overall survival (OS) were calculated using the Kaplan-Meier method. Results: Median follow-up was 29 months (range, 8 to 64 months). Rates at 1 and 3 years were 97.7% and 95.0% for LC, 97.7% and 80.7% for OS, 76% and 40.5% for IHFFS, and 87.3% and 79.5% for DMFS. Five patients (11.4%) experienced degradation of albumin-bilirubin grade, 2 (4.5%) degradation of Child-Pugh score, and 4 (9.1%) grade 3 or greater laboratory abnormalities within 3 months after SBRT. No significant difference was seen in any oncological outcomes or treatment-related toxicities between the two dose regimens. Conclusions: SBRT was highly effective for local control without severe toxicities in patients with HCC smaller than 3 cm. The regimen of a total dose of 45 Gy in 3 fractions was comparable to 60 Gy in efficacy and safety of SBRT for small HCC.

두경부 환자의 3D Printing을 이용한 Silicon Bolus의 유용성 (Usefulness of Silicon Bolus Using 3D Printing of Head and Neck Patients)

  • 구강 및 두경부 암의 방사선치료 시 치료 범위에 피부를 포함하는 경우가 많으며 이때 볼루스의 사용이 빈번해진다. 특히 턱 부분의 요철로 인하여 환자의 적용 시 선량 불확실성을 제공한다. 본 연구에서는 3D Printing을 이용하여 Gel 볼루스와 Poly lactic acid(PLA), Silicon을 적용한 환자 맞춤형 볼루스를 제작하여 물성 특성을 확인하고, 제작된 볼루스와 치료계획의 불일치성을 확인하며, 실제 방사선 선량 전달시 발생하는 선량불확실성을 측정하였다. 그 결과 일반적인 요철 부위에는 PLA 재질의 볼루스가 안정적이며, 요철이 심하거나 환자의 체형이 자주 바뀔 수 있는 환자의 경우 Silicon 재질의 볼루스가 유용할 것으로 사료된다.

Helical tomotherapy for spine oligometastases from gastrointestinal malignancies

  • Purpose: This study evaluated the treatment effectiveness and proper radiation dose of helical tomotherapy (HT) in spine oligometastases from gastrointestinal cancers. Materials and Methods: From 2006 to 2010, 20 gastrointestinal cancer patients were treated with HT for spine oligometastases (31 spine lesions). The gross tumor volume (GTV) was the tumor evident from magnetic resonance imaging images fused with simulation computed tomography images. Clinical target volume (CTV) encompassed involved vertebral bodies or dorsal elements. We assumed that the planning target volume was equal to the CTV. We assessed local control rate after HT for 31 spine metastases. Pain response was scored by using a numeric pain intensity scale (NPIS, from 0 to 10). Results: Spine metastatic lesions were treated with median dose of 40 Gy (range, 24 to 51 Gy) and median 5 Gy per fraction (range, 2.5 to 8 Gy) to GTV with median 8 fractions (range, 3 to 20 fraction). Median biologically equivalent dose (BED, ${\alpha}/{\beta}$ = 10 Gy) was 52 $Gy_{10}$ (range, 37.5 to 76.8 $Gy_{10}$) to GTV. Six month local control rate for spine metastasis was 90.3%. Overall infield failure rate was 15% and outfield failure rate was 75%. Most patients showed pain relief after HT (93.8%). Median local recurrence free survival was 3 months. BED over 57 $Gy_{10}$ and oligometastases were identified as prognostic factors associated with improved local progression free survival (p = 0.012, P = 0.041). Conclusion: HT was capable of delivering higher BED to metastatic lesions in close proximity of the spinal cord. Spine metastases from gastrointestinal tumors were sensitive to high dose radiation, and BED (${\alpha}/{\beta}$ = 10 Gy) higher than 57 $Gy_{10}$ could improve local control.

자궁경부암의 고선량율 강내 방사선치료 시 부작용을 줄이기 위한 적정 치료 자세의 연구 (Study of Patient's Position to Reduce Late Complications in High Dose Rate Intracavitary Radiation of the Uterine Cervix Cancer)

  • 목적 : 방사선 방광염 및 방사선 직장염은 자궁경부암의 방사선 치료시에 흔히 문제가 되는 만성 부작용이다. 저자들은 자궁경부암의 자궁 강내 방사선치료시에 직장과 방광의 방사선량을 줄일 수 있는 환자의 치료자세를 규명하고자 하였다. 대상 및 방법 : 13예의 환자에서 환자의 방광과 직장에 도뇨관을 삽입한 후에 조영제로 팽대부를 팽대한다. tandem과 두 개의 ovoid를 삽입한 후에 쇄석위와 앙와위에서 semi- orthogonal로 AP, Lat 사진을 촬영한다. 사진에서 ICRU Report 38 권장을 약간 변경한 방법에 따라 방광과 직장의 대표점을 구하고 A point 의 방사선량이 400cGy일 때의 방광과 직장의 방사선량을 각 치료자세에서 구한다. 또한 cervical os 로부터 방광과 직장까지의 거리도 각각 계산한다. 결과 : 직장 방사선량의 평균은 쇄석위에서 240.7cGy 앙와위에서 278.3cGy였고 방광 방사선량의 평균은 쇄석위에서 303.5cGy 앙와위에서 255.8cGy 였다. 쇄석위의 직장선량이 앙와위의 직장선량보다 한계적으로 유의하게 낮았고, 쇄석위의 방광선량은 앙와위의 방광선량보다 유의하게 높았다. 자궁경부의 external os 로부터 직장까지의 평균거리는 쇄석위에서 35.2mm 앙와위에서 32.3mm 였고, 자궁경부의 external os 로부터 방광까지의 평균거리는 쇄석위에서 30.4 mm 앙와위에서 34.0mm 였다. 직장까지의 거리는 쇄석위의 경우가 앙와위보다 유의하게 멀었고, 방광까지의 거리는 쇄석위의 경우가 앙와위보다 유의하게 가까웠다. 결론 : 자궁경부암의 강내치료 시 쇄석위로 치료할 경우 직장선량을 줄일 수 있었고 앙와위로 치료할 경우 방광선량을 줄일 수 있었다.

Comparison of clinical efficacy in epidural steroid injections through transforaminal or parasagittal approaches

  • Background: The transforaminal (TF) epidural steroid injection (ESI) is suggested as more effective than the interlaminar (IL) route due to higher delivery of medication at the anterior epidural space. However, serious complications such as spinal cord injury and permanent neural injury have been reported. The purpose of this study is to evaluate and compare the clinical effectiveness, technical ease, and safety of the TF and parasagittal IL (PIL) ESI. Methods: A total of 72 patients were randomized to either the PIL group (n = 41) or the TF group (n = 31) under fluoroscopic guidance. Patients were evaluated for effective pain relief by the numerical rating scale (NRS) and Oswestry Disability Index (ODI) (%) before and 2 weeks after the ESI. The presence of concordant paresthesia, anterior epidural spread, total procedure time, and exposed radiation dose were also evaluated. Results: Both the PIL and TF approach produced similar clinically significant improvements in pain and level of disability. Among the 72 patients, 27 PIL (66%) and 20 TF (64%) patients showed concordant paresthesia while 14 (34%) and 11 (36%) patients in the same respective order showed disconcordant or no paresthesia. Radiation dose and total procedure time required were compared; the PIL group showed a significantly lower radiation dose ($30.2{\pm}12$ vs. $80.8{\pm}26.8$ [$Cgy/cm^2$]) and shorter procedure time ($96.2{\pm}31$ vs. $141.6{\pm}30$ seconds). Conclusions: ESI under fluoroscopic guidance with PIL or TF approach were effective in reducing the NRS and ODI. PIL ESI was a technically easier and simple method compared to TF ESI.
