마취관련 정보제공이 편도선 절제술 환자의 불안, 혈당, 코티졸 및 에피네프린에 미치는 효과 (The Effect of Anesthetic Information on Pre-operative Anxiety, Glucose, Cortisol and Epinephrine of Patients Undergoing Tonsillectomy)

  • Purpose: The purpose of this study was to ascertain the effects of anesthetic information on preoperative state anxiety, plasma glucose, cortisol and epinephrine for patients under tonsillectomy and to provide generic data with anesthetic nursing intervention. Method: Data were collected from 60 patients who had tonsillectomy from November 1, 2003 to April 30, 2004. The group were divided into experimental group and control group: an experiment group of 30 patients was provided with an anesthetic information prepared by the researchers; a control group of another 30 patients was provided with general information. Data were analyzed through chi-squared test, t-test, repeated measure ANOVA using SPSS Program(version 12.0). Result: There were no significant difference between the experimental group and the control group in pre-operative state anxiety, glucose and cortisol. However, there were statistically significant difference between the above two groups in pre-operative pre-operative blood pressure(p=.001), heart beat(p=.000), and epinephrine(p=.035). Conclusion: The authors, with the results of this study, concluded that the preoperative anesthetic information for patients under tonsillectomy had direct effect on lessening pre-operative anxiety.

  • Purpose: The purpose of this study was to ascertain the effects of preoperative information on postoperative state anxiety, plasma cortisol, and pain for patients under total knee arthroplasty, and to provide generic data with nursing intervention for total knee arthroplasty. Methods: Data were collected from 34 patients who had total knee arthroplasty from January 3, 2003 to January 15, 2004. An experiment group of 17 patients was provided with pre-operative information prepared by the researchers; a control group of another 17 patients was provided with general information. Data were analyzed through Chi-squared test, t-test, paired t-test and ANCOVA using SPSS WIN 11.0. Results: There was no significant differences between the experiment group and the control group in post operative state anxiety(p=.612). However, there was statistically a significant difference between the above two groups in post operative plasma cortisol(p=.012). There was a statistically significant difference between the above two groups in post operative pain(p=.041). Conclusion: According to the results of the study, the authors concluded that the preoperative information for patients under total knee arthroplasty had the effect on the decrease of postoperative plasma cortisol and pain.

정형외과 환자의 관여도에 따른 병원급식 만족도 분석 (The Assessment of Foodservice Satisfaction by Orthopedic Patients according to their Involvement)

  • The purposes of this study were to evaluate the quality of hospital food services in view of patients in orthopedic wards and accomplish the quality improvement in hospital foodservice operations. Quantitative questionnaires for patients containing foodservice satisfaction and demographic information were developed. A survey of 8 general hospitals was undertaken and detailed information was collected from 290 patients in orthopedic wards. The collected data were processed using the SAS PC 6.12 for descriptive analysis, t-test. In demographic information of patients, 32.6% was over 50 years old and 31% was hospitalized over 30 days. 80% of patients was taking normal diet. 47.7% and 47.9% of patients showed moderate appetite and moderate pain respectively. The overall satisfaction score for patients was 3.24 out of 5, showing slightly higher level than the average score(3.00). According to foodservice involvement scores of patients, they were divided into two groups which were high involved group and low involved group. Two groups showed significant differences in taste of meals, variety of menu, punctuality of meal times, temperature of meals and portion size. The foodservice involvement factor which affected significantly patient foodservice satisfaction was 'kindness of foodservice staff'.

Information Technology Usage to Improve the Perceived Quality of Healthcare Service

  • The concept of Health Related Quality of Life and its determinants have evolved since the 1980s. Although many researchers have published articles of technology usage in hospitals and the installation of technology based healthcare system, the research about applying the information technology to improve the patients' perceived quality of healthcare services is still limited. In general, services are deeds, processes and performances that are essentially concerns of the consumer. The healthcare service quality depends on tangible factors, such as equipments, facilities, and the quality of hospital staff and also the intangible ones. The main purpose of this work is to establish new model and find out the contribution of information technology to enhance the patients' perceived quality of healthcare service. We attempted to examine the main information technology related factors in 3 aspects, namely quality of information, the technology accessibility and the community that can improve patients' perceived quality of healthcare services. Offline and online questionnaires were used to measure the patients' perceived quality and were distributed to 384 people in 2 countries, Laos and South Korea. A principle component analysis and multiple regressions were used to verify our model. Results show that the use of information technology has partial positive effect on patient-physician interaction in both countries. However, patient knowledge and patient autonomy which are the 2 dimensions of patient-physician interaction has significant positive effect on patients' perceived quality of healthcare service.

상태정보에 따른 체내삽입형 장치를 부착한 유-헬스케어 환자의 프라이버시 보호 프로토콜 (U-Healthcare user's privacy protection protocol with Implantable medical Device of State Information)

  • 최근 유헬스케어 시스템은 IT 기술과 의료서비스가 접목되면서 사용 범위가 점점 넓어지고 있다. 그러나 유헬스케어 시스템 중 체내삽입형 장치를 사용하는 환자의 경우, 환자의 프라이버시 예방과 안전한 접근 제어에 대한 대비책이 마련되어 있지 않아 그에 따른 프라이버시 보호 문제가 대두되고 있다. 본 논문에서는 체내삽입형 장치를 사용한 환자의 프라이버시를 보장하기 위해서 환자 정보를 가상화한 후 환자의 상태값과 수행값을 동기화함으로써 제3자의 불법적 접근을 예방할 수 있는 환자 프라이버시 보호 프로토콜을 제안한다. 제안된 프라이버시 보호 프로토콜은 사전에 관리 서버에 등록된 병원(의사, 간호사, 약국 등)의 권한정보에 따라 체내삽입형 장치를 부착한 환자의 개인정보의 열람범위를 제한하여 병원관계자의 불법적 업무 수행을 예방한다. 특히, 체내삽입형 장치와 게이트웨이 역할을 하는 가상장치는 접근 허가가 승인된 환자 정보 이외에 허가받지 않은 정보에 대해서 제 3자가 쉽게 접근하지 못하도록 환자의 상태값과 수행값을 동기화함으로써 환자의 프라이버시 관리의 효율성을 향상시키고 있다.

시청각매체를 이용한 정보제공 프로그램이 백내장 수술 환자의 불안과 교육만족도에 미치는 효과 (The Effects of Information Service Program by Media on Anxiety and Educational Satisfaction in Patients with Cataract Surgery)

  • Purpose: The purpose of this research was to identify the effects of providing an information service program by media on preoperative anxiety and postoperative educational satisfaction in patients with cataract surgery. Methods: For this study 52 patients who visited the ophthalmology clinic at P Hospital in B metropolitan city and who were scheduled to undergo one-day cataract surgery under local anesthesia were recruited. The 52 patients were assigned to the experimental group (25) or the control group (27). This study was conducted from August 27, 2015 to May 29, 2016. Results: Following the provision of cataract surgery related information service by media, the experimental group had a lower mean score for anxiety and higher mean score for educational satisfaction compared to the control group of patients. Conclusion: Results indicate that information service program by media "Goodbye Cataract" is useful educational materials for cataract patients in outpatient department and outpatient operating rooms.

Concept Analysis of Health Literacy for Patients with Cardiovascular Disease using Hybrid Model

  • Purpose: The purpose of this study is to provide a clear definition of the health literacy for patients with cardiovascular disease by analyzing the dimensions and properties using Hybrid concept analysis. Methods: The concept of health literacy of patients with cardiovascular disease was analyzed according to the cyclic process of theoretical phase-field work phase-final analysis phase presented in the Hybrid model. We reviewed 26 literatures and conducted in-depth interviews with 13 patients with cardiovascular disease. Results: The concept of health literacy in cardiovascular patients is derived from two dimensions and five attributes. Literacy skills, health information search ability and health information utilization skills were derived as attributes in the individual functional dimension, while active communication with the medical team and utilization of health information support resources were derived at the interrelational dimension. It is defined as the individualized and integrated ability of an individual to explore and utilize the various health information needed to make appropriate health decisions during the chronic course after diagnosis of cardiovascular disease, to communicate proactively with medical staffs and to utilize support resources. Conclusion: This study will contribute to the development and related research of health literacy measurement tools that can be used in cardiovascular nursing practice based on the attributes and indicators of health literacy for patients with cardiovascular disease.

정보제공이 영구형 심박조율기 이식환자의 불안, 지식 및 치료지시이행에 미치는 효과 (Effect of Providing Information on Anxiety, Knowledge and Compliance of Patients with a Permanent Pacemaker)

  • Purpose: This study was undertaken to identify the effect of providing information on anxiety, knowledge and compliance in permanent pacemaker patients. Method: A quasi experimental design with non-equivalent control group and non-synchronized design was used. The subjects of this study were 50 patients who had received permanent pacemaker implantation at a university hospital in Seoul. They were divided into an experimental group of 22 patients who received education and a control group of 28 patients. The education was composed of group education(twice) and individualized reinforcement education(once) using an education booklet. Results: Anxiety decreased in the experimental group. Knowledge significantly increased in the experimental group compared to that in the control group. Compliance significantly increased in the experimental group. Conclusion: It can be concluded that providing information is effective for reducing anxiety, increasing knowledge and improving compliance of the permanent pacemaker patients.

자궁 적출 수술 환자를 대상으로 한 통증 자가 조절기 관련 구체적 정보 제공의 효과 (Effects of Provision of Concrete Information about Patient-controlled Analgesia in Hysterectomy Patients)

  • Purpose: This study was to investigate the effects of the provision of concrete information about patient-controlled analgesia (PCA) in hysterectomy patients. Methods: Study design was a nonequivalent control group non-synchronized pre- and post-test design. Sixty subjects participated were assigned to experimental group (30 patients) or control group (30 patients) at one university hospital. Concrete information about PCA was composed of three sections: explanation with a leaflet, practice of using PCA, and question and answer session. Results: The experimental group who received concrete information about PCA before surgery had statistically higher knowledge level about PCA, more positive attitude toward pain control analgesia, a lower pain score, and a higher satisfaction level of the use of PCA post-surgery compared to the control group who received general information before surgery. Conclusion: Provision of concrete information about PCA was an effective nursing intervention that reduced post-operative pain for patients and increased their satisfaction with using PCA. It is recommended that concrete information about PCA be provided by nurses to promote the use of PCA and consequently reduce patient's pain post-surgery.

  • Purpose: The purpose of this study was to investigate effects of providing written information for coronary artery disease on health behavior compliance related self-efficacy, knowledge of disease, anxiety, and educational satisfaction depending on patients' health literacy. Methods: The participants in this study were 40 patients who underwent coronary angiography or coronary intervention, and depending on the level of health literacy 30 patients were high group and 10 patients were low group. Each group was evaluated on health behavior compliance related self-efficacy, knowledge of disease, anxiety, and educational satisfaction with providing written information. Results: By providing written information in the group with high health literacy, there was a significant difference in health behavior compliance related self-efficacy, knowledge of disease, but there was no significant difference in anxiety variable. On the other hand, there was no significant difference in health behavior compliance related self-efficacy, knowledge of disease, and anxiety by providing written information in the group with low health literacy. Also, there was no significant difference in the educational satisfaction between high and low group of health literacy after providing written information. Conclusion: It is necessary to develop educational materials that can be applied to clinical nursing field with considering health literacy of patients with coronary artery disease. In addition to providing written information, it is also necessary to develop other educational intervention programs such as video and personalized counseling that may be helpful for coronary patients with low health literacy and investigate their effectiveness.
