Objectives: This study aimed to investigate gender differences in the Korean Medical Community. Methods: Participants in the research study included 90 students regularly attending the fourth grade in Korean Medicine College, of which 48 were male and 42 were female. Results: 1. Patient's preferences for female KMD (Korean Medicine Doctor) had the lowest response rate in Korean Medical Society. Preferences for male KMD were due to factors including comfort level, rationality, good training, and patient's preference. 2. Patient's preferences for female KMD had the highest response rate in Gynecology, and Pediatrics. On the other hand, patient's preferences for female KMD had the lowest response rate in the other 7 specialty subjects. 3. The reasons for gender discrimination against male KMD were lack of subtlety, lack of empathy, authoritative attitude, and lack of patient management skills. The reasons for gender discrimination, against female KMD were childcare burden, social prejudices, lack of physical strength, and housekeeping burdens. Conclusions: The study results indicated that there was a lower preference for female KMD than male KMD among the Korean Medical Society and in patients' consciousness. Male-centered culture of Korean Medical Society had no role in this finding, but masculine image for KMD jobs and work-family double burden for female KMD were important contributing factors.
PURPOSE. To assess the clinical performance of monolithic CAD-CAM lithium disilicate glass-ceramic (LDGC) crowns and metal-ceramic (MC) crowns provided by predoctoral students. This study also assessed the effects of patient and provider-related factors on their clinical performance as well as patient preference for these types of crowns. MATERIALS AND METHODS. Twenty-five patients who received 50 crowns (25 LDGC CAD-CAM and 25 MC) provided by predoctoral students were retrospectively examined. LDGC CAD-CAM crowns were milled in-house using the CEREC Bluecam system and cemented with either RelyX Unicem or Calibra Esthetic resin cements. MC crowns were cemented with RelyX Unicem cement. Clinical assessment of the crowns and the supporting periodontal structures were performed following the modified California Dental Association (CDA) criteria. Patients' preference was recorded using a visual analog scale (VAS). The results were statistically analyzed using log-rank test, Pearson Chi-squared test and Kaplan-Meier survival analysis. RESULTS. Twelve complications were observed in the MC crown group (9-esthetic, 2-technical and 1-biological). In comparison, 2 complications in the LDGC CAD-CAM crown group were observed (1-technical and 1-esthetic). The 6-year cumulative survival rates for MC crowns and LDGC CAD-CAM were 90.8% and 96%, respectively, whereas the success rates were 83.4% and 96%, respectively. Overall, patients preferred the esthetic outcomes of LDGC CAD-CAM crowns over MC crowns. CONCLUSION. The high survival and success rates, low number of complications, and the high level of patients' acceptance of monolithic LDGC CAD-CAM crowns lend them well as predictable and viable alternatives to the "gold standard" MC crowns.
This study has been conducted to estimate the degree of cognition of military hospital services according to experience of military hospital services for serviceman, and to find out some factors affecting the medical services satisfaction. Questionnaires were given to 600 persons during October 5th though 24th 2009. All 600 questionnaires distributed affair, but 343 cases were collected. Statistical analysis methods used in this dissertation are t-test, Logistic Regression Analysis and other basic statistics such as frequency, percentage, and standard deviation using SPSS ver, 17.0. Purpose: Hospitals for patients and their guardians can, from the concept of healing, be removed from just gaining profits, but suggest a future-oriented direction for the hospital. Accordingly, there have been studies related to the selection of hospitals, but most were related to preference and satisfaction, and only recently did research from the concept of tradeoffs of factors for selection began to grow rapidly. Methods: From this context, this study evaluates the level of importance for factors of selecting hospitals using the analytical hierarchy process, and identifies the correlation with users, gender, age group, and outpatient features in order to identify the difference of awareness among different groups for selecting hospitals. In the factors for selection 26 factors in six categories were set through studies of preceding research, and after surveying 144 people, the following results were attained. Results: 1) The overall analysis results were found in the order of medical level, medical service, and fame, and low for facilities, which is similar to the cases of preceding studies. 2) For user analysis, it was similar between patients and guardians, but there was a slight difference in awareness among nurses, who are also medical service providers. Nurses showed relatively high level of importance in direct factors such as medical technologies and medical services, while guardians of patients showed higher importance in indirect factors such as facility environments and convenience. 3) Women showed higher assessments of importance levels in environmental factors, while men in physical factors. 4) The older the age group, the lower level importance there was on medical level, while the importance on fame reduced the further the commute to the hospital was.
Purpose: The objective of this study was to publicly report the hospital-level surgical volume for 7 types of surgery including gastrectomy. Also, to investigate the changes in patient behaviors after the public reporting among patients with gastrectomy. Methods: This study used data from the National Health Insurance Service Cohort. The data comprised of 2,214 patients who were diagnosed with gastric cancer and underwent gastrectomy during 2004-2012. An interrupted time series analysis was performed to investigate the association between patients' choice and public reporting. Results: 79.27% of the patients visited a hospital with high surgical volume. The time trend after introduction of public reporting was positively associated with visiting a high volume hospital (per 1 month, RR: 1.004, p=0.0329). However, after adjusting the health policies by reducing copayment, public reporting on surgical volume was not associated with visiting a high volume hospital. Sub-group analyses had also similar results. Conclusion: Patients were more affected by policies on economic support than on public reporting, and the changes in treatment options may have been affected by the increasing preference for large size hospitals. Thus, public reporting did not significantly improve the options available for patients and their decision making on health care utilization.
The purpose of this study was to compare the needs among the patients of hyperthyroid, malignant tumor and normal adult to clarify the level of the need for the patient of hyperthyroid. We analysed relationships between their personal characteristics and their needs to contribute comprehensive nursing care for the patient of hyperthyroid. The . subjects were chosen from 60 persons who visited for general physical examination at Ewha Woman's University Medical Center., 72 patients having diagnosis with hyperthyroidism, and 72 patients admitted with malignant tumors at atomic neuclear medical center. The data was collected from September 1 to December 31, 1980, and the study population was selected by means of convenience sampling. The data was analysed by means of the standard deviation and analysis of variance. The results were as follows: 1. In the patients having hyperthyroid, male to female ratio was 1 :13.5, and incidence was peak at the age groups of 20 to 40. 2. In compare with the needs among 3 groups, there are significant relationships: the patient of cancer has shown the highest score with Achievement, Aggression and Dominance, the patient of hyperthyroid shown higher and the normal adult shown low score. .In the need related with sex, there is significant relationship: the normal adult has shown highest score, the patient having hyperthyroid shown the higher score and the patient of cancer shown the low score. 3. Tn compare with Whang's Edward personal preference schedule, the Hyperthyroidism shown relatively higher needs for aggression, dominance, exhibitionism and sex.
This descriptive study was done to identify the relationship between coping style and trait anxiety in patients undergoing cardiac catheterization. The subjects selected were 69 patients who were scheduled for cardiac catheterization in S hospital from , November 1st, 1994 to May 30th, 1995. Data collection was done using Spielberger's Trait Anxiety Scale, Bendig's Short Form of the Manifest Anxiety Scale, Crowne & Morlowe's Social Desirability Scale, Millers Behavioral Style Scale, Information Subscale of Krantz Health Opinion Survey and Visual Analog Scale about informaton seeking behavior. The summary of the findings is as follows : 1. No significant relationship was found between the general characteristics of age, sex, marital status, level of education and occupation, and the variables coping style and trait anxiety. 2. The mean of trait anxiety scores for the Spielberger Scale was 46.68. It is slightly above average. The subjects were classified as to personality type: truely low anxious 25 (36.2%), highly anxious 25 (36.2%), and repressing 16 (23.2%), using a combination of scores from the Bendig Short Form Anxiety Scale and the Crowne Marlowe Social Desirability Scales. 3. No significant relationship was found between Spielberger s trait anxiety and Bendig's personality type. 4. Each subject's preference for either monitor or blunter coping strategy was assessed via the Miller Behavioral Style Scale. The proportion of Monitors (47.8%) and Blunters (52.2%) among the subjects were similar. The means of preference for information measured on the Krantz's subscale and Visual Analog Scale were 4.18, 80.79 respectively. Because these scores appeared above average, subjects tended to be higher seeker an information rather than avoiding it. 5. There was no significant relationship between Miller's Behavioral Style and Spielberger's trait anxiety. 6. There was no significant relationships between Bendig's personality types. and the preference on information of Krantz's and Visual Analog Scale. and Miller's Behavioral Styles. But 56.0% of the subjects who were highly anxious used Monitor strategy for coping. Findings from the current study do not permit any relationship inferences between coping style and trait anxiety. Patients undergoing cardiac catheterization must receive interventions on stress reduction because the cardiac 'catheterization creats a situation of psychological stress. Therefore, the nursing intervention of providing information about cardiac catheterization have to be given based on each patient's coping style and on their trait anxiety.
Applying effective coping strategies and reducing powerlessness for the chronic pain adjustment of musculoskeletal patients were researched for basic data in the development of nursing intervention. The subjects were 99 musculoskeletal patients with chronic pain. Data was gathered with direct interview using a questionnaire. With the SAS tool. data were analyzed for percentage. Pearson correlation. t-test. and ANOVA according to characteristics of variables. Internal consistency(alpha) coefficients were .91 for coping scales and .71 for powerlessness scales. The nurses should also develop for the nursing intervention of physical therapy and educational programs.
Purpose: To evaluate the effects of low-dose intravenous N-acetylcysteine on the prevention of contrast-induced nephropathy (CIN) in patients undergoing computed tomography (CT). Methods: All patients presenting to our emergency department and undergoing CT with intravenous contrast media between August 2014 and April 2016 were retrospectively enrolled. We included hospitalized patients with renal dysfunction [estimated glomerular filtration rate (GFR) between 30 and $89mL/min/1.73m^2$]. A 600-mg injection of N-acetylcysteine was given to patients once before and once immediately after CT, depending on the preference of physician. The primary outcome was CIN defined as an increase in creatinine level of ${\geq}25%$ or ${\geq}0.5mg/dL$ from the baseline within 48 to 72 hours after CT. A trained person blindly reviewed all medical records. Results: Of the 1903 admitted patients, CIN occurred in 9.8% of patients who received 1200 mg intravenous N-acetylcysteine (24/244) and 6.8% of patients who did not (113/1659, p=0.090). In a multivariable regression analysis, N-acetylcystine was not relevant to the prevention of CIN (odds ratio=1.42 [95% CI, 0.90-2.26]). Even in the stratified analysis using the propensity score matching, N-acetylcysteine was irrelevant (GFR 30-59: odds ratio=1.06 [95% CI, 0.43-2.60]; GFR 60-89: odds ratio=1.76 [95% CI, 0.75-4.14]). After adjustment, crystalloids were significantly associated with the reduction in CIN compared with dextrose water (odds ratio=0.60 [95% CI, 0.37-0.97]). Conclusion: No effect was found when low-dose intravenous N-acetylcysteine was used to prevent CIN. However, there seems to be an association between crystalloids and reduction in CIN.
Purpose: The biggest problem of wound healing is a possible occurrence of lesion. Especially, in the case of patients who have a skin injury around exposed body parts, if their treatment period drag on for long time, they can suffer from aftereffects and the costs can be passed on to a society. Therefore, in this research, we investigated the need to develop the effective medicine and appliances for the patients by examining which therapy methods are being applying to the skin damage and what is the advantage and limit by evaluating the patient's satisfaction level. Methods: We carried out an online and offline survey targeting medical teams in order to analyze device for wound care. A total of 125 medical teams applied to the research, and investigate the level of customer satisfaction. Results: The moist dressings are the most used method for wound healing. When it comes to the level of customer satisfaction, biological dressing product also has a high satisfaction level. However its high cost tends to limit the use. Conclusion: This research reached a conclusion that it is need to develop a low cost and high efficiency wound care product considering the fact that its high cost and low efficiency induced economic problems. Generally, it is needed to develop a product for skin regeneration based on biological technologies, not a product just for damage cure.
