Analysis of external environmental factors affecting patient transport time

  • Emergency transport is directly related to the life of the patient, and rapid transport to the hospital is crucial. However, external environmental factors such as traffic or weather, interfere with hospital transport. In this study, we investigated the external environment affecting hospital transport time. We examined the transfer time and patient treatment time of emergency patients in an area of northern Gyeonggi-do from 2018 to 2020. Diagnosis after arrival at the hospital was used, and on-site treatment time was measured from paramedic arrival time at the scene to departure. Furthermore, we examined whether there was a correlation between the time paramedics left the scene and hospital arrival time through the reason for the delay as recorded in the emergency log. Traffic jams had the greatest impact on patient transport, while transport delays occurred due to heavy rain, but not snow. Among injured patients, electrical accidents were the most problematic in terms of on-site treatment time. This was because a lot of first aid is needed in electrical accidents. It must be necessary to mobilize two ambulances in an emergency through the expansion of infrastructure, prepare a plan for rapid transport in heavy rain, and implement strong laws against transport obstruction.

119구급대원의 근골격계 증상과 환자운반 작업의 인간공학적 평가 (The Evaluation of Musculoskeletal Symptom and Patient Transport Work of 119 EMTs by Ergonomics Tools)

  • 본 연구는 119구급대원의 근골격계 증상 호소율을 파악하고 증상 유소견자들에 대한 작업성 근골격계질환(WMSDs)을 유발하는 환자운반 작업에 대한 인간공학적 평가를 통해 작업 위험도를 파악하고자 하였다. 이를 위해 근골격계 유해요인 조사지침(KOSHA Code H-30-2008)을 활용한 설문도구를 이용하여 근골격계 증상 호소율을 파악하였으며, 환자운반 작업의 위험도는 인간공학적 평가도구인 OWAS, RULA, REBA를 이용하여 평가하였다. 연구결과 근골격계 증상 유경험자는 60.9%였으며 허리 부위의 증상 자각율(36.1%)이 가장 높았고 WMSDs을 유발하는 가장 큰 원인은 환자운반 작업(48.4%)이었다. 환자운반 작업의 인간공학적 위험도 평가 결과 주 들것을 구급차에 싣고 내리기, 들 것 장비를 이용하여 환자를 들어올리는 작업에서 OWAS는 3단계, RULA와 REBA는 3~4단계 수준이었고, 응급용들것으로 계단 이송, 척추고정판으로 차량내 환자 이동, 업거나 안고 계단이나 경사로를 이동하는 작업에서 OWAS, RULA, REBA 모두 3~4단계로 작업자세의 개선이 필요한 것으로 평가되어 119구급대원의 WMSDs 예방을 위해서는 즉각적인 작업자세의 개선이 요구된다.

  • 저자들은 2003년 2월 18일 대구지하철 중앙로역에서 발생한 화재 시의 환자 이송현황의 분석을 통하여 다음과 같은 결론을 얻었다. 환자들이 이송된 의료기관은 주로 의료 기관의 규모가 크고, 이송 거리가 짧은 기관 중심으로 이송이 이루어졌다. 경증의 환자가 가깝고 규모가 큰 응급의료기관으로 이송됨에 따라, 중증의 환자가 멀거나 규모가 적은 응급의료기관으로 이송되어야 하는 상황도 있었으며, 병원도착전에 사망한 환자들은 거리에 상관없이 이송되었다. 따라서 적절한 중증도분류에 따른 적절한 의료기관으로의 이송이 향후 시행되어야 하며, 이를 위해서 초기 피난 및 대응과 이송방법과의 연계, 재해시 이송의 지역적 지침 확보 및 재해 정보와 응급의료기관 정보와의 연계 등이 향후 수정, 연구되어야 한다.

Interhospital Transport System for Critically Ill Patients: Mobile Extracorporeal Membrane Oxygenation without a Ventilator

  • Background: Extracorporeal membrane oxygenation (ECMO) has been successfully used as a method for the interhospital transportation of critically ill patients. In South Korea, a well-established ECMO interhospital transport system is lacking due to limited resources. We developed a simplified ECMO transport system without mechanical ventilation for use by public emergency medical services. Methods: Eighteen patients utilized our ECMO transport system from December 2011 to September 2015. We retrospectively analyzed the indications for ECMO, the patient status during transport, and the patient outcomes. Results: All transport was conducted on the ground by ambulance. The distances covered ranged from 26 to 408 km (mean, $65.9{\pm}88.1km$) and the average transport time was $56.1{\pm}57.3minutes$ (range, 30 to 280 minutes). All patients were transported without adverse events. After transport, 4 patients (22.2%) underwent lung transplantation because of interstitial lung disease. Eight patients who had severe acute respiratory distress syndrome showed recovery of heart and lung function after ECMO therapy. A total of 13 patients (70.6%) were successfully taken off ECMO, and 11 patients (61.1%) survived. Conclusion: Our ECMO transport system without mechanical ventilation can be considered a safe and useful method for interhospital transport and could be a good alternative option for ECMO transport in Korean hospitals with limited resources.

119 구급 서비스의 미이송 시간 분석 (Analysis of response time for patient non-transport by 119 ambulance services)

  • Purpose: The purpose of the study was to investigate the causes of non-transport in 119 ambulance services and to describe the difference in response time according to the reason for non-transport. Methods: This study analyzed 42,415 non-transport cases out of 123,158 cases using prehospital care reports in a metropolitan city. The Kruskal-Wallis test and the Wilcoxon-Mann-Whitney test were used to 6,857 cases for which time was recorded. Results: Non-transportation in 20${\bigcirc}{\bigcirc}$ comprised 33.1% of all ambulance services. The reason for non-transport were other reasons (25.5%), cancellation (23.8%), and moving to other vehicles (21.7%). There were differences in ambulance service times according to the reasons for non-transport. The activation interval was the longest (2.68 minutes) in the absence of the patient, and the response interval was the shortest (4.96 minutes) among the cancelled case. The total interval was the shortest time (21.97 minutes) in the cancellation cases and the longest time among the death cases (32.23 minutes). Conclusion: It is important to suggest the direction of development of emergency services by identifying the reasons for non-transport by ambulance services and describing the response time according to the reason for non-transport.

부산 지역 민간 응급 이송업체의 환자 이송 현황 분석 (Analysis of patient transfer status of private emergency ambulance services in Busan)

  • Purpose: This study was conducted as a direct investigation of the data in the dispatch logbooks and status of patient transportation provided by private emergency transport companies in Busan. Methods: This study was conducted using SPSS 23.0 version for a total of 1,000 processed records of private emergency ambulance services in Busan from September 23, 2017 to November 5, 2019. Results: First, 100% of the emergency patient transfers by private emergency ambulances were carried out between medical institutions; 76.4% of all transfer patients had emergency conditions, and 86.0% had serious diseases. Second, 59.3% of the emergency patients were located at distances less than 10 km and 43.2%, at more than 10 km from the medical institutions. Third, 63.5% of the passengers were accompanied by first-class emergency rescuers according to the severity of the condition. Fourth, 92.7% of the reasons for the selection of medical institutions were transferred to places where professional care was available, accounting for most of the reasons for the selection. Finally, the medical institutions were selected according to the severity of the patient's condition; 76.5% patients were transported to institutions with a large number of doctors, and 42.9% of those were transported to specialized care institutions. Conclusion: This study collected data from 1,000 dispatch records of private emergency transport companies in Busan; these records reflect the government's policies to improve the emergency patient transfer system. The current status of emergency patient transfer offered by private transport companies was analyzed. All of the emergency patient transfers were carried out between medical institutions, and 76% of the transferred patients had emergency conditions.

중환자실 간호사가 인식한 병원 내 중환자 이송 현황 조사 (Intrahospital Transport of Critically Ill Patients: Critical Care Nurses' Perceptions)

  • Purpose : This study aimed to identify the status of intrahospital transport (IHT) of critically ill patients and provide baseline data to form recommendations for safer transport. Methods : Data were collected from 141 intensive care unit (ICU) nurses who attended national conferences between February and August, 2018. The collected data were analyzed using descriptive statistics and ANOVA, and post-hoc analysis was conducted with the Scheffé and Games-Howell tests. Results : Of the nurses surveyed, 61.7% answered that their workplace had a transport guideline. In terms of the experience of ICU nurses, 31.2% of respondents answered that they had received training on IHT. This result indicated that the degree of implementation of the guidelines for IHT was generally high, but some, including guidelines on personnel, equipment, and monitoring, were not. Guidelines of IHT were well observed when the institutes had specific guidelines for IHT of critically ill patients with specified transport grades, a scoring system to assess stability of a patient, a checklist and a protocol for action in case of problems, and when healthcare providers were provided with training. Conclusion : These results suggest that organized infrastructure, such as a transport guideline with transport grades, a checklist to improve the implementation of guidelines, and a protocol for coping with a problem, should be provided for safe transport. Additionally, effective education and evaluation to improve the competency of staff participating in the transport of patients will help reduce the occurrence of adverse events in intensive care transport in hospitals and promote patient safety.

구급대원의 재난대응역량에 관한 연구 (Study on disaster response competencies of 119 EMTs)

  • Purpose: This study was conducted to investigate the disaster response competencies of 119 emergency medical technicians (EMTs). Methods: The subjects of this study were 226 119 EMT's at the fire station located in G province. The questionnaire consisted of 5 sub-factors and 29 items for measuring disaster response competencies and the collected data were analyzed using SPSS program. Results: Disaster response competency comprising immediate response, patient triage, patient treatment, patient transport, and collaboration support were 3.48, 3.64, 3.52, 3.64, and 3.16, respectively. Advanced EMTs scored significantly higher in patient triage (p<.001), treatment (p<.001), and transport (p=.022) competency than basic EMTs. Conclusion: In order to improve disaster response competency, it is necessary not only to enhance individual ability but also to continue disaster management training with related organizations.

  • 도서 지역이라는 울릉도의 특성상 응급 환자의 이송 수단으로 헬리콥터는 필수적이다. 울릉도 지역의 인구 분포의 변화가 고령화가 심화됨에 따라 헬리콥터의 이송 빈도는 더욱 증가될 것으로 예상된다. 그러나, 응급 환자 이송용 헬리콥터가 없어 이송 시간동안의 응급처치 등에 장애가 있으므로, 동해안에 환자 이송의 역할을 가진 헬리콥터의 배치가 필요하다고 생각된다. 아울러 최근 감소세에 들어섰던 울릉군 지역의 인구가 증가하고 있고, 독도 열풍에 따른 관광객의 증가가 현저하므로 그에 따른 울릉군의 적절한 보건 정책이 필요하다고 생각된다

  • Purpose: The purpose of this study was to understand the perception of medical staff to propose an infected patient transport robot as a means of responding to infectious diseases. Methods: The data collected through the survey was analyzed through AHP analysis. The measurement tools used in this study were derived through the SERVQUAL model and Focus Group Interview(FGI), and consisted of four detailed questions for each of five classes: tangible, reliability, responsiveness, assurance, and empathy. Results: As a result of the study, there are concerns about risk factors that may occur in areas where medical staff intervention is minimized. Above all, we confirmed the consensus that safety should be the top priority during the process of robots to transport patients. In particular, highlighted were the resolution of device errors that may occur during the process for transporting patients and easy provision of the first aid. Additionally, the ability to monitor patients and suppress infection factors turned out to be important, which was directly related to the simplification of the role of medical staff and work efficiency. Conclusion: As one of the means of effectively controlling infectious diseases in a pandemic situation, a robot to transport the infected patient was considered. However, in order to commercialize this, specific verification of the safety of medical staff and patients is needed, and empirical data on providing the first aid, patient monitoring, and infection factor suppression should be presented.
