본 연구는 119구급대원을 대상으로 환자평가 수행도에 대한 교육 전 후를 비교함으로써, 구급대원을 대상으로 한 적절한 교육방법 및 기초자료를 제시하기 위해 진행하였다. 전국 4개 시 도 119구급대원 26명을 대상으로 하였으며, 연구는 2015년 12월 14일부터 17일까지 시행하였다. 구급대원이 직접 모의환자를 대상으로 환자평가 및 보고를 시행하도록 하였고, 환자평가를 진행하는 동안 각각의 증상(두통, 가슴통증, 호흡곤란)에 대한 체크리스트(checklist)를 사용하여 교육 전 후를 평가하였다. 연구 결과는 중앙값(사분위값)으로 기술 분석하였고, 교육 전 환자평가 수행도는 100점 만점에 66점(54.00-80.25)이었으나 교육 후 88.5점(80.00-93.00)으로 유의한 상승을 보였다(p<0.001). 환자평가 수행도 중 가슴통증 증례 5개의 항목, 두통 증례 7개의 항목, 호흡곤란의 증례 9개의 항목에서 교육 후에 유의한 증가를 보였고, 환자와의 관계형성에서는 구급대원을 다시 만나고 싶은 정도의 평가점수가 유의한 상승을 보였다(p<0.01). 또한 환자상태 보고 적절성의 모든 평가항목은 교육 후 수행도가 유의하게 상승하였다(p<0.01). 본 연구의 결과는 시뮬레이션 실습에 의한 교육이 119구급대원들의 환자평가 수행도 개선에 필요하다는 것을 보여준다. 따라서 119구급대원에 대한 실질적이고 체계적인 환자평가 교육은 필요하며, 지속적이고 많은 교육의 기회가 주어져야 할 것이다.
Considering the rapid growth of elderly population and the increase of medical demand for aged, many of geriatric hospitals are needed in Korea. On the other hand, supplies of hospitals for acute care are considered to be excessive. So many of small and medium hospitals facing management problems have been converged to geriatric hospitals. These attempts are considered resonable for efficient utilization of health care resources in Korea. This paper aims to identify the concept of geriatric hospital and to analyze the problems and then to seek the alternatives for architectural planning on the basis of surveying the geriatric hospitals converged from hospitals for acute care.
There are various ways to manage the patient flow of the hospital outpatient system. However, it is difficult to apply many implementation solutions to the real outpatient system at once. Because first, the expected effects of each different solution are very much depend on the real situation of the system and applied other solutions, and second, owing to the limited resources, each solution should be implemented according to the priority. In order to overcome these difficulties, this paper focuses on proposing the comprehensive subset of implementation solutions, which is one of the most effective among various kinds of subsets, and verifying the effects of it. The comprehensive subset of solutions is derived from conducting design of experiments and simulation which determine the optimum set of different solutions and analyze the particular interactions and priority order among them. This implementation strategy can solve the difficulties of applying different kinds of various solutions to the hospital outpatient system.
Predicting the number of ICU patients holds significant importance, serving as a critical aspect in efficiently allocating resources, ensuring high-quality care for critically ill individuals, and implementing effective public health strategies to mitigate the impact of diseases. This research focuses on estimating ICU patient numbers through the development of a simple mathematical model. Utilizing data on confirmed COVID-19 cases and deaths, this model becomes a valuable tool for predicting and managing ICU resource requirements during the ongoing pandemic. By incorporating historical data on infected individuals and fatalities from previous weeks, we establish a straightforward equation. We found the substantial impact of the delay in infected individuals, particularly those occurring more than five weeks earlier, on the accuracy of ICU predictions. Proactively preparing for potential surges in severe cases becomes feasible by forecasting the demand for intensive care beds, ultimately improving patient outcomes and preventing excessive strain on medical facilities.
Purpose : This concept analysis identified attributes and defined triage competency among emergency nurses. Method : Walker and Avant's approach was used to guide the concept analysis. A literature review was completed including 26 studies, 5 reports of related associations, and 5 books. Results : The concept of triage competency in emergency nurses was identified as five attributes: clinical judgment, expert assessment, management of medical resources, timely decision, and communication. Antecedents of the concept were triage education and emergency room experience. The consequences of the concept were efficiency of care, patient rating, and safety. Triage competency in emergency nurses was defined as the comprehensive ability to prioritize patients' urgency and allocate limited medical resources. Conclusion : This study is meaningful since it clarified triage competency among emergency nurses. The attributes and empirical indicators of this study will likely lay the foundation for development of triage competency metrics.
Objectives: The purpose of this study was to identify resources put into clinical. activities and identify a new RBRVS for oriental medical services in the Korean National Health Insurance. Methods: Based on a survey of physician's time, physical effort & technical skill, mental effort & judgment, and stress that were used for patient treatments, relative input values for the relevant clinical activities were estimated and rearranged in a way to be compared. with the current values in health insurance. Results: We found the actual resource-based relative values for oriental medical services statistically different from the current values, with a narrower variation in value distribution. Conclusions: The findings suggest the C\lrrent RBRVS should be revised to reflect the actual input resources into physicians activities and to avoid a distortion of physicians behavior.
Alkaptonuria (AKU, OMIM: 203500) is a rare autosomal recessive disorder of tyrosine metabolism due to a defect of enzyme activity, homogentisate 1,2-dioxygenase (HGD). The patients with AKU initially presented with dark urine discoloration, and ochronosis and arthritis develop after third decades of life. With advances of Internet resources, web-based health seekers for rare disease are increasing. Here, we report the case of an 18-year-old boy with AKU who visited our center due to dark black urine based on self-diagnosis via web searching of this rare condition. Compound heterozygous mutations in HGD gene, IVS5+3A>C and IVS12+6T>C were identified and both of mutations were detected in his parents. Our case illustrates the utility of publicly available Internet resources for diagnosis of rare disease.
These days rapid improvement in Mobile phones and their multimedia limits made them powerful enough to manage complicated tasks. Image processing related support for mobile devices is extremely comprehensive in wireless telemedicine. A basic challenge is how to get best quality of image with the limited screen size and resources of mobile phones. This paper deals with image processing features (capturing rendering and zooming in and out) of Mobile Media API and Advanced Multimedia Supplements (MMAPI and AMS) developed for Mobile Java Platform and customized algorithm is designed to keep all image task cost efficient by using minimum device resources and memory. This scenario is driven by the need for evaluation of a distant patient that cannot be moved to the expert.
Objectives: The purpose of this research is to examine the effects of acupuncture treatment on Hwa-byung patient's insomnia. Methods: The study was performed through a patient-assessor blind, randomized, placebo-controlled trial in which the volunteers, data collectors and analysts were unaware of individuals who were receiving the treatment. A total of thirty-seven volunteers were divided into 2 groups. 18 subjects were placed into a trial group and 19 subjects into a control group by a randomization table. The trial group was treated on bilateral Shigu, Ahnmyun, B62 (Shinmaek), and K6 (Chohae), while the control group was not given any other treatment. The ISI(Insomnia Severity Scale) was measured as the 1st evaluative instrument, and then a comparative analysis was conducted by comparing the results with those measured by a Likert scale for major symptoms of Hwa-byung, PSQI, Hwa-byung scale, BDI, STAI, and STAXI. Results: In the ISI, PSQI, and the Likert scale regarding major symptoms of Hwa-byung, Hwa-byung scale, BDI, and STAI, more significant decreases were found in those concerning the trial group compared to those of the control group from a statistical perspective. However, in the STAXI, regarding the trial group, there was no significant decrease compared to the control group. Conclusions: The results suggest that acupuncture treatment is effective in the treatment regarding Hwa-byung patients who suffer from insomnia.
This study provides information and suggest a direction for the future research needs of medical attire as a high value-added industry. A total of 69 studies on medical attire were categorized and analyzed for the study subject, study method, and study locations. As per the time period, until the mid-80s, almost no research on medical attire was conducted; however, the number has increased as of late. The distribution by subject was in the order of status investigation, design development, material research, textile design, and sizing system, and especially there was significant focus on wearing status and design development that has rapidly increased over the last 10 years. The distribution by target was in the order of adult patients, elderly patients, doctors, nurses, and juvenile patients. There are many studies on the wearing status and design development of adult patient attire as well as on the textile design and sizing system study for doctor's attire. The distribution by study method was in the order of survey and interview, research and development, dressing evaluation, real resources analysis, and physical property experiments. Through the results of the analysis on the study subject and target, it was found that the wearing status investigation for adult patient attire through a survey and interview was most active. The distribution by study location was in the order of Seoul, Incheon - Gyeonggi, Busan - Ulsan - Gyeongnam, and suggested that a study on patient attire in Dajeon - Chungcheong and Jeonbuk was inadequate.
