An integrated approach with homeopathic medicine and electro-acupuncture in anaesthesiology during breast cancer surgery: Case reports

  • This study investigates the effect of a combination of homeopathic medicine and electro- acupuncture in two patients with breast cancer and severe liver disease who could not receive standard anaesthesia therapy due to liver problems. Specifically, measurable and quantifiable parameters were used to evaluate whether an integrated approach-consisting of electro- acupuncture and a homeopathic medicine diluted above Avogadro's limit (that is, above a potency of 12CH) during the pre-surgical, surgical and post-surgical phases -can improve general well-being of a patient undergoing breast cancer surgery. In breast cancer surgery, we employed an integrated approach consisting of induction with hypnotics and muscle relaxants, followed by maintenance with anaesthetic gas, combined with a homeopathic treatment (Arnica montana 15CH and Apis mellifica 15CH) before and after surgery and an electro- acupuncture treatment performed in the pre- and post-surgical phases without any analgesic/pain relieving medications. Both of the patients treated with the integrated approach improved their overall condition without need for other common pain relieving medicines. Additionally, thanks to their rapid awakening, the patients were not relocated to a protected area and the hospitalization was shorter. A multidisciplinary approach incorporating homeopathic medicine and electro-acupuncture can be a solution for patients who need or ask about a different and/or safer alternative to the standard treatment. This approach can offer a safe, much less expensive, non-invasive and viable alternative for such cases. Moreover it can be useful for an opioids free anesthesia.

  • 본 연구는 진료기록에 대한 일반인의 인식과 태도를 오픈노트(Open Notes)운동을 중심으로 알아보고, 우리나라 임상진료현장에서 오픈노트제도의 도입 가능성을 확인하는 것을 목적으로 한다. 연구 결과를 요약하면 다음과 같다. 일반인들은 주로 인터넷을 통해 건강정보를 얻고 있으며, 병원의 진료정보와 관련하여서는 보험회사에 제출하기 위한 진료비관련 기록을 주로 이용하였다. 또한 진료기록에 대해서는 의사나 병원이 위조 혹은 변조할 가능성이 있다고 인식하고 있었으며, 대부분의 응답자가 진료기록을 언제든 확인할 수 있다면 병원이나 의사에 대해 더 신뢰할 수 있다고 응답하였다. 한편 오픈노트 운동에 대해서는 대부분 좋은 아이디어라고 생각하고, 우리나라에서도 오픈노트제도가 시행되어야 한다는데 동의하였으며, 오픈 노트제도가 시행된다면 참여하겠다고 응답하였다. 결론적으로, 진료기록의 투명성을 추구하는 오픈노트제도의 도입은 의사-환자 간 신뢰에 기여하여, 의사-환자 간 커뮤니케이션에도 긍정적인 영향을 기대할 수 있을 것이다.

치과내원환자의 감염관리에 관한 인식도 조사 (About dentistry infection from dentistry medical institution recognition research of patient)

  • Objectives : The research which sees collected information about dentistry infection management recognition of the patients and the infection management which the medical treatment consumer does in necessity is what, grasped and satisfaction and became the fundamental data which makes the hospital will be able to trust about dentistry image rise and dentistry environment of the medical treatment consumer who assists a dentistry character executed. Methods : From 2009 April 1st until May 28th 233 people common people who have the experience which visits a Gyeongsangnam-do area dentistry the object with the entry question law which sleeps did and the data analysis SPSS, 12.0 For Window under using produced each question item by frequency and a ratio, the analytical degree of infection management One - Way ANOVA stamps and executed t-test. Is high there was a possibility of knowing the thing about the recording infection where the educational background will be high and considers statistically the difference which was visible Conclusions : The education which is active and continuous is necessary about infection management and sets the guides which are standardized to the patients and about infection management and about infection prevention and exposes from dentistry practices well to emboss the thing is thought that there is a necessity to plant a trust feeling about the dentistry where the patients assist.

한국의 국가적 이미지가 의료관광품질의 기대 형성에 미치는 영향 (Effects of Country-image on Expectation of Medical Tour)

  • Medical tourism is a contemporary phenomenon which has its root in both tourism and health service. Especially, Korea's medical tourism market has grown dramatically after activation of foreign patient attraction law. The purpose of this study is to find out which is the most influential factors among political, economic, relative and national image in case of Japanese tourists when they purchase Korean medical tourism. This study estimates the Japanese tourists' Trust in the Korean health service quality and tourism service quality. It means perception of medical tourism. The results are as follows; The political, relative and economic image not have any significant influences on tourism service and trust in health service quality. And just the national image has a positive influence on tourism service and trust in health service quality. Tourism service quality is related with trust in the health service quality. Trust in health service quality is only related with purchase intention of medical tourism. The implications of this study are: First, the national image factors such as thoughtfulness, high education and polite attitude should be utilized as a Korea medical tourism marketing strategy to differentiate itself from the other foreign country in Japan. Next, the differentiated national image in Japan will be the most important factors for Korean hospital and tourism companies to attract medical tourists.

임상 치과위생사의 직무만족 경험과 일의 의미에 대한 질적 연구 (A qualitative study on experiences of job satisfaction and meaning of work among clinical dental hygienists)

  • Objectives: This study explored job satisfaction experience and meaning of work in dental hygienists. Methods: In-depth interviews were conducted with 13 clinical dental hygienists who had been working in dental clinics for more than 10 years. The analysis method used was developed by Giorgi, including all four steps. Results: The results of the interview were categorized into five central meanings: recognition and trust (including patient's trust, trust in the boss, recognition in the hospital, and intimacy), growth (growth of the hospital, growth in jobs, and problem-solving skills), professional pride (self-esteem, extension of work and changes of awareness, and being able to help others), reward (financial compensation and psychological rewards), and meaning of life given by work (the process of maturing, self-representation, and vitality of life). Conclusions: Based on this study, we need to develop tools to measure dental hygienists' level of job satisfaction as well as follow-up research on ways to improve it.

The art of diabetes care: guidelines for a holistic approach to human and social factors

  • A holistic approach to diabetes considers patient preferences, emotional health, living conditions, and other contextual factors, in addition to medication selection. Human and social factors influence treatment adherence and clinical outcomes. Social issues, cost of care, out-of-pocket expenses, pill burden (number and frequency), and injectable drugs such as insulin, can affect adherence. Clinicians can ask about these contextual factors when discussing treatment options with patients. Patients' emotional health can also affect diabetes self-care. Social stressors such as family issues may impair self-care behaviors. Diabetes can also lead to emotional stress. Diabetes distress correlates with worse glycemic control and lower overall well-being. Patient-centered communication can build the foundation of a trusting relationship with the clinician. Respect for patient preferences and fears can build trust. Relevant communication skills include asking open-ended questions, expressing empathy, active listening, and exploring the patient's perspective. Glycemic goals must be personalized based on frailty, the risk of hypoglycemia, and healthy life expectancy. Lifestyle counseling requires a nonjudgmental approach and tactfulness. The art of diabetes care rests on clinicians perceiving a patient's emotional state. Tailoring the level of advice and diabetes targets based on a patient's personal and contextual factors requires mindfulness by clinicians.

공감에 대한 개념분석 (Concept Analysis of Empathy)

  • Purpose: This study was done to analyze and clarify the concept of empathy in nursing. Methods: Using the concept analysis framework of Walker & Avant 68 articles about empathy were selected for this study. Results: Defining attributes of empathy included recognizing others' perspective and thought, understanding others' emotion, and communicating with them. The antecedents of empathy included contact with patients, respect and trust for patients, a certain level of cognitive ability, and communication skills. Consequences of empathy were increasing patient self-understanding, self-awareness as a therapeutic subject and improving quality of nursing. The concept was clarified through presentation of model, borderline, related, and contrary cases. Conclusion: The results of this study are meaningful in that the concept and attributes of empathy are explored in the area of nursing. Also, based on results of this study, development of standardized tools to measure patient-perceived empathy and measure the three attributes of empathy is recommended as well as the development of educational programs to enhance empathy.

  • Purpose: This descriptive study was designed to explore the clinical nurse's ethical value regarding human life. Method: Data were collected from September to October, 2002. Study subjects were 527 clinical nurses working in General Hospital as tertiary located in Seoul. Ethical value was measured with questionnaire developed by researchers and consisted on items regarding ethical value on human life. Result: Among the items, most nurses highly agree with the item, "When a patient requests his/her health care provider to keep his/her personal secret, the health care provider is obliged to do so." and "When a patient asks for information on his/her medicinal and dietary contents, his/her wish must be granted." Most clinical nurses mainly agree with the item. "Health care providers must always be honest to the patient and/or his/her family". However, most nurses disagree with the item, "When a patient is on the verge of death after an accident, it is justifiable to soothe his/her family by saying 'he/she is OK' instead of telling them the truth, in order to avoid a sudden shock befalling on them". Conclusion: It is intensifying the notion of ethical underpinning for human rights, truthfulness is essential to a trust relationship under what circumstances. Also most clinical nurses agree with that It is essential to trust in the nurse-patient relationship, patients have the right to know and it is the ethical thing to do as health care provider.

Barriers to Low Vision Services and Challenges Faced by The Providers in Pakistan

There were two objectives of the study, first was to identify the barriers as perceived by the patients and providers to access the low vision services and second was to identify the challenges faced by the main providers. Study design. Structured questionnaire based interviews of patients and providers Methodology. To find out the barriers to access of low vision services, the interviews based on structured questionnaire were conducted for two patient groups. The first group consisted of 97 visually impaired individuals attending the department of low vision services at Al-Shifa Trust Eye Hospital Rawalpindi while the second group included 56 visually impaired individuals attending the four rehabilitation centers/schools for the blind in Rawalpindi/Islamabad. To identify the barriers as perceived by the main providers of low vision services and challenges faced by them the interviews based on structured questionnaire were conducted for 19 low vision service providers. Results. From patients point of view, major barrier to low vision services identified was inability to visit hospital /rehabilitation center alone - 29.8% in hospital group and 33.9% in rehabilitation centers group, while the lack of social support, lack of family support, cost of travelling, long distance, afford ability, hesitation in using devices and lack of satisfaction were other important barriers identified. From providers' point of view, major barrier to uptake of services was the need for repeated follow-ups. Optometrists were the main provider of low vision services contributing to 47.4% of the providers. The major challenge faced by the providers was motivation of patients to use low vision devices. Conclusion. The major barrier to low vision services according to the patients is inability to visit the hospital alone, while according to providers, it is the need for repeated follow up which proves major barrier towards uptake of services. The motivation is the major challenge faced by providers, majority of which are optometrists.

Chemotherapeutic Response and Survival for Patients With an Anal Squamous Cell Carcinoma and Low Hemoglobin Levels

  • Purpose: Anemia is associated with poor treatment results for a variety of cancers. The effect of low hemoglobin levels on long-term outcomes after the treatment of patients with an anal squamous cell carcinoma (SCC) remains unclear. For that reason, this study aimed to investigate the effect of anemia on treatment outcomes following chemoradiation for an anal SCC. Methods: This was a retrospective study of all patients who underwent curative treatment for an anal SCC between 2009 and 2015 at 2 trusts in the United Kingdom. Data were collated from prospectively collected cancer databases and were cross-checked with operating-room records and records in the hospitals' patient management systems. Results: We identified 103 patients with a median age of 63 years (range, 36-84 years). The median overall survival was 39 months (range, 9-90 months), and the disease-free survival was 36 months (range, 2-90 months). During the follow-up period, 16.5% patients died and 13.6% patients developed recurrence. Twenty-two people were anemic prior to treatment, with a female preponderance (20 of 22). No differences in disease-free survival (P = 0.74) and overall survival (P = 0.12) were noted between patients with anemia and those with normal hemoglobin levels. On regression the analysis, the combination of anemia, the presence of a defunctioning colostomy, lymph-node involvement and higher tumor stage correlated with poor overall survival. Conclusion: In this study, anemia did not influence disease-free survival or overall survival. We suggest that the interaction between anemia and survival is more complex than previously demonstrated and potentially reliant on other coexisting factors.
