중소병원 종사자의 환자안전관리 중요성과 환자안전문화 인식이 안전수행에 미치는 영향 (Influence of Perception of Importance of Patient Safety Management and Culture on of Small and Medium-sized Hospital Employees' Safety Performance)

  • Purpose: This study aimed to investigate the influence of the perception of the importance of patient safety management and culture on employees' safety performance in small and medium -sized hospitals. Methods: The participants comprised 119 hospital employees, including nurses, doctors, and medical technicians. Data were analyzed through descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and multiple regression analysis using the SPSS program. Results: The mean safety performance was 4.09±0.34, mean safety compliance was 4.12±0.44, and safety participation was 4.06±0.38. There were significant differences in safety performance by gender and job. Safety performance and its assocation with both perception of importance on patient safety management and, perception of patient safety culture showed a positive correlation. Safety performance was influenced by the perception of patient safety culture. The explanatory power was 15.7%. Conclusion: Based on these results, improving the perception of patient safety culture is necessary to increase safety performance. To this end developing and applying an interprofessional safety performance education program for employees in small and medium-sized hospitals is vital.

종합병원 간호사가 인식한 환자안전문화와 소진이 안전관리활동에 미치는 영향 (Effect of General Hospital Nurses' Perception of Patient Safety Culture and Burnout on Safety Management Activities)

  • Purpose: The aim of this study was to examine effects of patient safety culture and burnout on safety management activities with a focus on clinical experience of nurses in general hospitals. Methods: Self-administered questionnaires were given to nurses in a general hospital in C Province, and 107 questionnaires were used for final analysis. Collected data were analyzed using SPSS/WIN 21.0 Program for t-test, ANOVA, Pearson correlation coefficients, and multiple regression. Results: The highest score as perceived by general hospital nurses for patient safety culture was for 'Immediate superior/Manager' (3.84), for burnout, the highest score was for 'Emotional exhaustion' (4.13), and for safety management activities, the highest score was for 'Prevention of infection' (3.96). Patient safety culture and safety management activities perceived by general hospital nurses showed significant positive correlations (r=.35 p<.001). The correlations between burnout and safety management activities perceived by the nurses showed significant negative correlations (r=-.37, p<.001). Results of hierarchical regression analysis conducted to identify factors that affect safety management activities showed that patient safety culture (${\beta}=.40$ p<.001) was effective for controlling safety management activities. Conclusion: The findings indicate a need to build a patient safety culture that fits the characteristics and situations of various hospitals.

간호대학생의 환자안전관리활동: 계획된 행위이론을 중심으로 (Patient Safety Management Activities of Nursing University Students: Focus on the Theory of Planned Behavior)

  • Purpose: This study was undertaken to present an effective plan for the development of an educational program and a strategy to promote patient safety management activities for nursing students by identifying factors that affect these activities based on the theory of planned behavior. Methods: A self-report questionnaire was distributed to 300 nursing students who had clinical practice experience at three nursing colleges in Daejeon, Gyeongbuk, and Jeonbuk. The significance of the model fit, and the path effect was confirmed by confirmatory factor analysis. Results: The hypothetical model for patient safety management activities was appropriate. Among the 5 pathways, 4 were significant. It was found that behavioral intention had a direct influence on patient safety management activities, and perceived behavioral control and attitude had an influence on behavioral intention. Conclusion: To strengthen the perceived behavioral control of nursing students' patient safety management activities, it is necessary to analyze and remove obstacles and provide education that reflects the characteristics of the subject's health problems. In addition, through self-directed learning involving simulation practice, nursing students should be exposed to patient safety accidents, so that they can recognize the risks early and solve problems through critical thinking while bringing about the necessary changes in their attitude.

응급구조과 학생의 환자안전에 대한 지식 및 태도가 환자안전관리 자신감에 미치는 영향 (Knowledge, attitude and confidence on patient safety management among emergency medical technician students)

  • Purpose: This study evaluated the effects of knowledge and attitude on confidence about patient safety management among emergency medical technician (EMT) students. Methods: A self-reported questionnaire was completed by 174 EMT students in two cities from May to June 2016. Data were analyzed using PASW WIN 20.0 for descriptive statistics, t-test, one way analysis of variance, and hierarchical multiple regression. Results: The students' confidence about patient safety management was 3.33 (on a 5-point scale). Confidence varied according to average academic grade (p=.015), experience in patient safety education, and experience in writing medical error reports. After adjusting for sex, average academic grade, and experience in patient safety education, knowledge and attitudes about patient safety increased a student's confidence in patient safety management by 19%. Conclusion: Patient safety education should be included in programs on health promotion and be provided as basic information in intervention programs for EMTs.

환자안전 전담인력의 환자안전관리 역량강화 방안: 질적연구 (Measures to Strengthen Patient Safety Management Competencies for Patient Safety Coordinators: A Qualitative Research)

  • Purpose: This study aimed to identify strategies to enhance the competencies of patient safety coordinators in Korea. Methods: Fourteen participants from nine hospitals were interviewed between May and November 2022. Qualitative content analysis was used to analyze the data. Results: As for the strategies to enhance patient safety management competency, 3 themes and 11 sub-themes were derived. The first theme was 'Having individual competence as a patient safety coordinator', and the sub-themes were 'Communication skills with members', 'Flexible thinking from multiple perspectives', and 'Preparing for administrative work competencies that they had not experienced as a nurse.' The second theme was 'Responding strategically to promote improvement activities', and the sub-themes for it were 'Multi-angle approach to the problem', 'A careful approach so as not to be taken as criticism in the field', 'Increasing the possibility of improvement activities through awareness', 'Activating the network between patient safety coordinators', and 'Expanding learning opportunities through patient safety case analysis.' The third theme was 'Obtaining support to facilitate patient safety activities', and the sub-themes for this were 'Improving staff awareness of patient safety', 'Providing a training course for nurse professional of patient safety', and 'Expanding the manpower allocation standard of patient safety coordinators.' Conclusion: This study explored personal competencies such as document writing and computer utilization capabilities, focused on ways to improve the field of patient safety management, and emphasized the need for organizational and political support.

간호대학생의 환자안전간호 관련요인에 대한 메타분석 (Meta-Analysis of Factors Related to Patient Safety Nursing in Nursing University Students)

  • 본 연구의 목적은 간호대학생의 환자안전간호와 관련된 요인을 융·복합적 측면에서 체계적으로 고찰하고, 메타분석을 통해 관련 요인의 효과크기를 파악하는 것이다. 연구방법은 문헌검색은 Medline, Embases, CINAHL, DBpia, Research Information Service System(Riss), Korean Studies Information Service System(Kiss)을 이용하였으며, 국외 데이터베이스는 MeSH용어와 Emtree를 활용하여 검색하였다. 검색식은 [(patient safety or patient harm or safety management) and (students, nursing)] or [(patient safety or patient harm or safety management) and (education, nursing, graduate)] 이었다. 문헌선정은 PRISMA(Preferred Reporting Items for Systematic Reviews and Meta-Analyses)를 이용하였다. 연구결과 관련 요인으로는 간호수행, 지식, 태도, 자신감, 인식 및 인지 등이 확인되었고, 그 중 환자안전간호 수행과 관련성이 높은 요인에는 자신감, 태도, 인식, 지식 순으로 파악되었다.

환자안전 관리를 위한 위험요인 분석 (Analysis of Risk Factors for Patient Safety Management)

  • Purpose: This is a pilot study to identify patient safety risk factors and strategies for patient safety management perceived by nurses. Methods: Data were collected and analyzed with an open questionnaire from April to May 2005, targeted on 100 nurses working in two hospitals. The issues were 'what are risk factors for patients, nurses, and other medical practitioners? How do they prevent with the aftermath of risk factors, causes of incidents?' For data analysis, types and frequency of risk factors were worked out, using the Australian Incident Monitoring System Taxonomy. Results: The types of patient safety risk factor perceived by nurses were as follows ; therapeutic devices or equipment, infrastructure and services (29.5%), nosocomial infections (16.3%), clinical processes or procedures (15.4%), behavior, human performance, violence, aggression, security and safety (12.2%), therapeutic agents (9.7%), injuries and pressure ulcers (8.7%), logistics, organization, documentation, and infrastructure technology (5.6%). Strategies for patient safety included training of prevention of infection, education about safety management for patients and medical professionals, establishment of reporting system, culture of care, pre-elimination of risk factors, cooperative system among employees, and sharing information. Conclusion: These results will be used to provide evidences for patient safety management and educational program.

일 지역 종합병원 간호사의 환자안전문화 인식이 안전간호활동에 미치는 영향 (Effects of Perceived Patient Safety Culture on Safety Nursing Activities in the General Hospital Nurse's)

  • Purpose: This descriptive correlation study was done to identify how perception of patient safety culture of general hospital nurses affects safety during nursing activities. Data from this study should provide information on management of patient safety as well as improvement in patient safety. Method: Participants in this study were 357 clinical nurses working in a general hospital in M city which had two medical evaluations. A survey was conducted to gather the data. Results: The score for perception of patient safety culture of the general hospital nurses was 3.42, out of a possible 5 points, and the score for safety care activities was 3.90. There was a statistically significant positive relationship between the nurses' perception of patient safety culture and their safety care activities, Perception of patient safety culture, Supervisor/manager, communication and procedures, and frequency of accident reporting were factors that impacted significantly on safety nursing activity. Conclusion: The results of the study indicate that patient safety cultural perception significantly affects the safety of nursing activities and thus systematic educational strategies to increase perception should be provided to increase the level of patient safety culture. Also, other specific methods that increase the level of patient safety culture should be considered.

수술실 간호사의 환자안전관리활동 영향 요인 (Factors Influencing on Patient Safety Management Activities in Operating Room Nurses)

  • 본 연구는 수술실 간호사의 직무 스트레스와 조직몰입, 환자안전관리활동 정도를 파악하고, 환자안전관리활동 정도에 영향을 미치는 요인을 확인하기 위한 서술적 조사연구이다. 대상자는 G광역시 소재 1개의 대학병원과 9개 종합병원 수술실에 근무하는 간호사로 대상자 선정기준에 적합한 총 136명이었으며 자료수집기간은 2014년 7월 25일부터 8월 7일까지이었다. 수집된 자료는 SPSS/WIN 21.0 program을 이용하여 기술통계, Independent t-test, one- way ANOVA, Pearson's correlation, multiple regression으로 분석하였다. 연구결과는 다음과 같다. 대상자의 직무 스트레스 정도는 5점 만점에 3.76점, 조직몰입 정도는 5점 만점에 3.09점, 환자안전관리활동 정도는 5점 만점에 4.15점이었다. 수술실 간호사의 환자안전관리활동 정도는 대상자의 근무경력, 적성, 근무만족도와 환자안전교육 횟수에 따라서 통계적으로 유의한 차이가 있었다. 환자안전관리활동은 조직몰입과 유의한 양의 상관관계가 있었고, 수술실 간호사의 환자안전관리활동에 영향을 미치는 요인은 조직몰입과 환자안전교육 횟수로, 이들 변수의 설명력은 23%이었다. 이상의 연구결과를 통해 조직몰입과 환자안전교육이 수술실 간호사의 환자안전관리활동을 증진시키는 주요 영향요인으로 확인되었다. 따라서, 수술실의 환자안전관리를 위해서 수술실 간호사의 조직몰입을 향상시키는 다양한 중재프로그램 개발과 환자안전관리활동을 위한 지속적이고 체계적인 환자안전교육의 강화가 필요하다고 사료된다.

안전과학 패러다임의 전환과 환자안전의 개선 (What Will We Learn from the Paradigm Shift in Safety Science for Improving Patient Safety?)

  • Patient safety remains one of the most important health care issues in Korea. To improve patient safety, we have introduced concepts from the field of safety science such as the Swiss cheese model, and adopted several methodologies previously used in other industries, including incident reporting systems, root cause analysis, and failure mode and effects analysis. This approach has enabled substantial progress in patient safety to be made through undertaking patient safety improvement activities in hospitals that are systems-based, rather than individual-based. However, these methods have the shared limitation of focusing on negative consequences of patient safety. Therefore, the paradigm shift from Safety I to Safety-II in safety science becomes the focus of our discussion. We believe that Safety-II will complement, rather than replace, Safety-I in the discipline of patient safety. In order to continuously advance patient safety practices in Korea, it is necessary that Korea keeps abreast of the recent global trends and development in safety science. In addition, more focus should be placed on testing the feasibility of new patient safety approaches in real-world situations.
