간호사의 직무스트레스, 환자안전문화와 환자안전관리 활동 -간호·간병통합서비스병동과 일반병동 비교- (Nurses' Job Stress, Patient Safety Culture, and Patient Safety Management Activities -Comparing Comprehensive Nursing Care Service Wards with General Wards-)

  • Purpose: This study aims to compare and analysis job stress, patient safety culture, and patient safety management activities of nurses in comprehensive nursing care service wards and general wards. Methods: Through an online survey conducted on nurses with more than three months of working experience at five general hospitals in the metropolitan area, 156 participants's data were used for 𝜒2-test, t-test, ANOVA using SPSS ver. 26.0. Result: There were no significant differences in the nurses' job stress and patient safety management activities between the comprehensive nursing care service wards and general wards. However, the patient safety culture of nurses in the comprehensive nursing care service ward was significantly lower than that of general ward. Conclusion: Patient safety culture should be promoted through continuous regular patient safety education, training, and organizational support to increase patient safety management activities. Additionally, institutional programs should be prepared to reduce job stress of nurses at the frontline of patient safety.

중소병원간호사의 환자안전관리활동에 대한 연구 (A Study on Patient Safety Management Activities in Small and Medium Hospitals Nurses')

  • 본 연구는 중소병원간호사의 환자안전관리활동에 영향을 미치는 요인을 파악함으로써 중소병원 내 환자안전에 대한 기초자료를 제시하고 환자안전관리활동 강화 프로그램 개발에 필요한 자료를 제공하고자 수행되었다. 연구대상자는 100-300명 미만 중소병원 5곳에 근무하는 총 191명의 간호사이며 자가보고식 설문지를 이용하여 자료를 수집하였다. 자료분석은 기술통계, t-test, ANOVA, 상관분석과 다중회귀분석을 시행하였다. 본 연구 결과, 환자안전문화인식, 환자안전태도 및 자기효능감은 중소병원간호사의 환자안전관리활동과 유의한 양적 상관관계를 나타내었다. 회귀분석결과, 중소병원간호사의 환자안전관리활동에 영향을 미치는 요인은 환자안전태도(β=.39, p<.001), 자기효능감(β=.25, p<.001), 및 환자안전문화인식(β=.17, p=.006)으로 나타났으며, 이 변수들의 설명력은 41.8%였다. 따라서, 중소병원간호사의 환자안전관리활동 증진을 위해 환자안전태도, 자기효능감 및 환자안전문화인식을 향상시킬 수 있는 교육프로그램 개발할 것을 제안한다.

종합병원 간호사의 환자안전문화인식과 조직몰입이 환자안전관리활동에 미치는 영향 (The Effects of Patient Safety Culture Perception and Organizational Commitment on Patient Safety Management Activities in General Hospital Nurses)

  • 본 연구는 종합병원 간호사의 환자안전관리활동에 환자안전문화인식과 조직몰입이 미치는 영향을 검증하고자 시도되었다. 자료는 2017년 8월 2일부터 8월 16일까지 종합병원 간호사 191명을 대상으로 구조화된 설문지를 이용하여 자료수집하였고, SPSS WIN Version 22.0을 이용하여 분석을 실시하였다. 연구결과, 종합병원 간호사의 환자안전관리활동은 환자안 전문화인식과 조직몰입과는 양의 상관관계가 있는 것으로 나타났고, 환자안전관리활동의 영향요인은 환자안전문화인식, 조직몰입 및 환자안전교육 경험으로 나타났으며, 이들 변수의 설명력은 33.3%였다. 따라서, 종합병원 간호사의 환자안전관리 활동을 증진시키기 위해서는 환자안전문화인식, 조직몰입 및 환자안전교육을 고려하여 중재프로그램을 개발하고 적용하는 것이 필요하다고 사료된다.

병원간호사의 환자안전문화에 대한 인식과 안전간호활동에 관한 연구 (A Study on Hospital Nurses' Perception of Patient Safety Culture and Safety Care Activity)

  • Purpose: The purpose of this study was to investigate perception of patient safety culture and safety care activity among hospital nurses and to identify factors associated with the safety care activity. Methods: A total of 399 nurses working at secondary or tertiary hospitals in B city were participated in. Data were collected using 'Questionnaire on Patient Safety' and 'Safety Care Activity Questionnaire'. Results: The mean score of patient safety culture was 3.41 out of possible 5. The mean score of safety care activity was 4.40 out of possible 5. There was a positive relation between the perception of patient safety culture and the safety care activity. Through stepwise regression analysis with 22.4% of accountability, it was found that the perception of the safety care activity was associated with communication process, a sub-domain of safety culture, marital status, experience of incidence reporting, and level of patient safety. Conclusion: The findings show the importance of communication among nurses to improve the safety care activity. To provide high quality of care for patients, it is necessary to educate nurses on fire safety, patient education, and medication safety and improve their communication skills.

중소 정형외과병원 간호사의 환자안전문화인식, 환자안전지식과 환자안전간호수행 (Perceptions of Patient Safety Culture, Safety Care Knowledge and Activity among Nurses at an Orthopedic Hospital)

  • Purpose: The purpose of this study was to identify the factors affecting the perceptions of patient safety culture, safety care knowledge, and safety care activity among nurses at orthopedic hospitals. Methods: Data were collected during Feb. 16 and Feb. 26, 2017, from 195 nurses of 9 small to medium sized orthopedic hospitals. Questionnaires about patient safety culture, safety care knowledge and activity were used. Data were analyzed by descriptive statistics, t-test, ANOVA, $Scheff\acute{e}$ test, Pearson's correlation coefficient, and stepwise multiple regression. Results: The safety care activity was positively correlated with perceptions of patient safety culture (r=.50, p<.001) and knowledge (r=.48, p<.001). Factors that had influence on the safety care activity were the patient safety culture (${\beta}=.30$, p<.001), age (${\beta}=.27$, p<.001), and knowledge of the safety activity (${\beta}=.21$, p=.004). The patient safety care activity was explained 36.6% by those factors. Conclusion: To enhance the patient safety care activity, it should be provided the environment and open communication for the perceptions of patient safety culture and the in service education program for safety care knowledge.

Ensuring Patient Safety in Pediatric Dental Care

  • This review aims to examine safety concerns in pediatric dental care and underscore the need for comprehensive patient safety initiatives within the Korean Academy of Pediatric Dentistry. Drawing insights from the prevailing patient safety policies of the American Academy of Pediatric Dentistry, case reports, and systematic reviews, this review elucidates issues such as dental fires during sedation, ocular complications from local anesthesia, and surgical emphysema. This review highlights the significance of safety toolkits encompassing infection control, medical error reduction, dental unit waterline infection, and nitrous oxide safety in pediatric dental settings, underscoring the need to foster a safety culture. Furthermore, this study explores the curriculum for pediatric dentistry residency programs, emphasizing concepts such as high-reliability organizations and mortality and morbidity conferences. The study suggests the need for initiatives to enhance patient safety, including establishing safety committees, expanding reporting systems, policy development, and supporting research related to patient safety. In conclusion, this study underlines key messages, emphasizing the utmost priority of patient safety, acknowledging the inevitability of human error, promoting effective communication, and cultivating a patient safety culture. These principles are vital for advancing patient safety in pediatric dental care and improving outcomes among pediatric patients.

중소 병원 간호사의 환자안전문화에 대한 인식과 안전간호활동 수행 정도 (Perception of Patient Safety Culture and Safety Care Activity among Nurses in Small-Medium Sized General Hospitals)

  • Purpose: This study was done to identify the factors affecting the perception of patient-safety-culture and the level of safety-care-activity among nurses in small-medium sized general hospitals. Method: Data were collected during April and May 2011, from 241 nurses of five hospitals. A hospital survey questionnaire on patient-safety-culture and safety-care-activity was used. Collected data were analyzed using descriptive statistics, Pearson correlation, t-test, ANOVA, Scheffe test and multiple-regression. Results: There were significant differences in the level of perception of patient-safety-culture according to the nurses' age, type of hospital, position, work department, and knowing whether there was a Patient-Safety committee in their hospitals. Nurses with higher perceived level of the patient-safety-culture performed more safety-care-activities. Factors influencing on the safety-care-activities were general patient safety, having had safety-education, patient-to-nurse ratio, employment status, and the level of reporting medical errors. These factors explained 22.9% of the safety-care-activity. Conclusions: The study findings suggest that in order to improve the nurses' perceived level of patient-safety-culture and safety-care-activity, the hospitals need to establish patient-safety committees and communication systems, and openness to reporting medical errors are needed. Better work conditions to ensure appropriate work time, regulate patient-to-nurse ratio, and nursing education standards and criteria, are also required.

중소병원 간호사의 사건보고태도와 환자안전간호활동의 관계에서 환자안전관리 중요성 인식의 매개효과 (Mediating Effects of Perceptions Regarding the Importance of Patient Safety Management on the Relationship between Incident Reporting Attitudes and Patient Safety Care Activities for Nurses in Small- and Medium-sized General Hospitals)

  • Purpose : The purpose of this study was to examine the mediating effect of perceptions regarding the importance of patient safety management in the relationship between incident reporting attitudes and patient safety care activities for nurses in small-and medium-sized general hospitals. The objective was to provide a basis for planning tailored training programs aimed at improving patient safety care activities. Methods : This study was conducted with 187 participants in small- and medium-sized general hospitals in K city in South Korea from March 15 to March 31, 2019. The data collected from participants were analyzed using descriptive statistics, a t-test, ANOVA, Pearson's correlation coefficients, and a multiple regression using IBM SPSS/WIN 21.0 software. Results : Patient safety care activities were found to be correlated with incident reporting attitudes (r=.27, p < .001) and perceptions of the importance of patient safety management (r=.59, p < .001). Further, perceptions of the importance of patient safety management had a complete mediating effect (${\beta}=.409$, p < .001) on the relationship between incident reporting attitudes and patient safety care activities. Conclusion : Based on the findings of this study, tailored training programs regarding patient safety care activities focused on boosting perceptions of the importance of patient safety management are highly recommended to improve nurses' patient safety care activities in small- and medium-sized general hospitals.

환자안전 전담인력의 환자안전관리 역량강화 방안: 질적연구 (Measures to Strengthen Patient Safety Management Competencies for Patient Safety Coordinators: A Qualitative Research)

  • Purpose: This study aimed to identify strategies to enhance the competencies of patient safety coordinators in Korea. Methods: Fourteen participants from nine hospitals were interviewed between May and November 2022. Qualitative content analysis was used to analyze the data. Results: As for the strategies to enhance patient safety management competency, 3 themes and 11 sub-themes were derived. The first theme was 'Having individual competence as a patient safety coordinator', and the sub-themes were 'Communication skills with members', 'Flexible thinking from multiple perspectives', and 'Preparing for administrative work competencies that they had not experienced as a nurse.' The second theme was 'Responding strategically to promote improvement activities', and the sub-themes for it were 'Multi-angle approach to the problem', 'A careful approach so as not to be taken as criticism in the field', 'Increasing the possibility of improvement activities through awareness', 'Activating the network between patient safety coordinators', and 'Expanding learning opportunities through patient safety case analysis.' The third theme was 'Obtaining support to facilitate patient safety activities', and the sub-themes for this were 'Improving staff awareness of patient safety', 'Providing a training course for nurse professional of patient safety', and 'Expanding the manpower allocation standard of patient safety coordinators.' Conclusion: This study explored personal competencies such as document writing and computer utilization capabilities, focused on ways to improve the field of patient safety management, and emphasized the need for organizational and political support.

재활전문병원 간호사의 환자안전문화에 대한 인식과 안전간호활동 수행의 관계 (Relationship between Perception of Patient Safety Culture and Performance for Safety Care Activity in Rehabilitation Hospital Nurse)

  • Purpose: This study was to examine the relationships between nurse's perception of patient safety culture and performance for safety nursing activities at rehabilitation hospitals. Methods: This study applied a descriptive research design. Participants were 194 nurses who have provided nursing services for more than 6 months at 4 rehabilitation hospitals located in B metropolitan city. Data were analyzed using descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficients and multiple regression. Results: There was a positive correlation between the awareness of patient safety culture and safety nursing activity. Multivariate analysis showed that hospital work environment, experience of education, hospital climate, frequency of reported events, and marital status were significantly associated with the safety nursing activity. Overall, approximately 23.1% of total variability in the safety nursing activity could be explained by the 5 variables ($R^2=0.231$, p<.005). Conclusion: Nurses at rehabilitation hospitals are relatively positive about patient safety culture. Therefore, we need to develop safety education programs at the level of organization in order to improve patient safety through performing effective safety nursing activities in addition to increase awareness of patient safety culture among nurses. Furthermore, we need hospital's strategies at the system level for open communication and outcome reports regarding patient safety.
