This study is aimed to assess the general population's attitude toward the continuity of medical care and its related factors. Self administered questionnaire was performed on the 1,120 office workers in the C city, Gang-won province. The questionnaire included the attitude of the continuity of medical institute, the intention of medical service use on a given case, and the variables of the related factors. 58.8% of the total respondents agreed to sustaining treatment without changing medical institutes; on the other hand, 41.2% showed negative attitude. In case that a patient would gain a recommendation of a surgery, hospitalization, or a specific examination, the total respondents' 84.9%, 61.8%, and 50.8% of each recommended situation said that they would visit another doctor and gain a diagnosis. As a result of multiple logistic analysis of determinant factor on continuity, reliability of doctors was statistically significant factor. In order to reduce wastefully used medical resources and offer well-qualified medical service, a system of second opinion among peer group or beforehand agreement could be possibly adopted. In addition, improving the image and reliability of a doctor could be an important factor to make better the behavior of medical service shopping; therefore, an effort to improve the relationship between a doctor and a patient, and restore the reliability of doctors should be paralleled.

Purpose: This study was aimed to provide information on the awareness of patient safety culture and safety care activities among operating room (OR) nurses and to analyze the factors influencing the safety care activities. Methods: For this descriptive research, self-reported questionnaires were administered to 168 OR nurses who were working at the university-affiliated and general hospitals. The collected data were analyzed using descriptive statistics, t-test, ANOVA, $Scheff{\grave{e}}$ test, Pearson's correlation coefficient and Stepwise multiple regression with SPSS/WIN 17.0. Results: The mean score of the awareness on patient safety culture was 3.27 out of 5 points and that of safety care activity was 4.31 out of 5. The statistically significant difference was found between experience of safety education and the awareness on patient safety culture. Also, the scores of safety care activities were significantly different according to OR nurses' position, education levels, and experience of safety education. There was a positive correlation between the awareness of patient safety culture and safety care activity. Their explanatory power on safety care activity was 8.8%, which includes working environment in operating room 3.3% and nursing position 5.5%. Conclusion: Compared to the level of patient safety activities, the OR nurses' awareness on patient safety culture was low. Given the specific characteristics and conditions in each hospital, it needs to increase the OR nurses' awareness on patient safety culture and activities related to patient safety.
Purposes: This study was performed to understand the recognition about accreditation motives, support within the hospital and accreditation survey process for the hospitals that participated in the accreditation program and to find out whether these factors are related to hospital management performances, so that the study can suggest plans for activation and development of the accreditation program. Methodology: This study was performed targeting 98 hospitals answered th the survey among 189 acute care hospitals that acquired accreditation from December 2010 to February 2014. For data analysis, frequency analysis, ${\chi}^2$-test, reliability analysis, ANOVA, Kruskal-Wallis H test and multiple regression analysis with SPSS 21.0 were used. Findings: The hospitals that had staff in charge of patient safety had bigger hospital culture change than those that didn't have(p<.05). In addition, the hospital culture change was bigger as internal motives were bigger, and as CEO's will was bigger(p<.05). Meanwhile, as maintenance rate of internal management after accreditation was higher, and as CEO's will was bigger, process improvement level was higher(p<.05). The quality improvement and patient satisfaction level were higher as CEO's will was bigger, and as suitability of survey process was recognized to be high(p<.05). As a result of analyzing the factors that affect hospital management performances with hospital culture change, process improvement and quality improvement combined, as internal management maintenance rate was higher, as CEO's will was bigger, and as suitability of survey process was higher, performances were higher(p<.05). Practical Implications: Hospitals need to reinforce internal motives to improve internal competences such as the whole system maintenance opportunity and staff training. In addition, the will of hospital director is most important, and if there is hospital director's interest in quality improvement and improvement intention definitely, employees voluntarily participate in and cooperate with the accreditation program, so that prompt medical service provision and high quality of medical services can be guaranteed, leading to hospitals' management performances.
The purpose of this study was to identify the of patient safety related to perception of Hospital environment, Organizational culture, Reporting system and Factors Influencing on Perception of Medical Errors Report Related to Patient Safety of Healthcare Workers in a General Hospital. Healthcare Workers in a General Hospital in B City who signed on the written consent participated in this study between February 12 and 28, 2017. A total of 244 copies were collected and were analyzed using the SPSS 22.0 program. The result mean score perception of Hospital environment was 3.26(${\pm}0.31$)point on a scale of 0 to 5, and Organizational culture 3.74(${\pm}0.54$)point, Reporting system 3.64(${\pm}0.57$)point. Factors influencing on perception of medical errors report is sex(${\beta}=.137$, p=.023), Type of occupation(${\beta}=289$, p=.001), Department of Nursing(${\beta}=-.196$, p=.023), Hospital environment(${\beta}=.327$, p=<.001), Organizational culture(${\beta}=.288$, p=<.001). Therefore management and hospital management efforts should be made to establish a system that enables healthcare workers to report without fear of medical errors reporting, and appropriate staffing and open communication.
Purpose: The purpose of this study is to understand and analyze the current status of dementia management and rehabilitation services in Busan, South Korea, in response to the rapid increase of people with dementia due to the aging of the population. Methods: To investigate the current status of dementia rehabilitation in Busan, a survey was disseminated to dementia safety centers and day- and night-care centers in 16 districts/counties in Busan. Of the 209 day- and night-care centers, 23 institutes were registered in the National Health Insurance Service and received the highest grade (Grade A) in the institute evaluation that was implemented in each district. A telephone interview was conducted, and survey questions were related to the existence of an ongoing dementia rehabilitation program, program presenter, number of participants, progress method, program time, program duration, program contents, and participation path. Results: Dementia safety centers were implementing dementia prevention program, cognitive enhancing program, dementia program, self-help meeting and education program for family of dementia patient. The majority of the presenters of all four dementia-related programs were occupational therapists. The highest number of participants in the dementia prevention program was 15, and the highest number of participants in the cognitive enhancing program, dementia program, self-help meeting and education program for family of dementia patient was 10. All institutes' programs delivered group therapy. As for the time and frequency of the program, most dementia program included three-hour sessions five times a week. Most dementia prevention program, cognitive enhancing program, self-help meeting and education program for family of dementia patient included 60-minute sessions once a week. The most frequently observed program duration for the dementia prevention program and cognitive enhancing program was six months, and the most frequently observed duration for the dementia program was three months. Lastly, study participants most often reported that self-help meeting and education program for family of dementia patient lasted for two months. Among day- and night-care centers in Busan, programs related to cognition were implemented in 18 institutes, and the majority of the program presenters were social workers. Conclusion: In response to the rapidly growing number of dementia patients due to the aging of the population, this study examined the current status of dementia rehabilitation in Busan. The study results underscore the need to develop systems that consider the circumstances in Busan and continuously and systematically support dementia programs.
Surgeons occasionally encounter a patient with a gastric cancer invading an adjacent organ, such as the pancreas, liver, or transverse colon. Although there is no established guideline for treatment of invasive gastric cancer, combined resection with radical gastrectomy is conventionally performed for curative purposes. We recently treated a patient with a large gastric cancer invading the abdominal wall, which was initially diagnosed as a simple abdominal wall abscess. Computed tomography showed that an abscess had formed adjacent to the greater curvature of the stomach. During surgery, we made an incision on the abdominal wall to drain the abscess, and performed curative total gastrectomy with partial excision of the involved abdominal wall. The patient received intensive treatment and wound management postoperatively with no surgery-related adverse events. However, the patient could not receive adjuvant chemotherapy and expired on the 82nd postoperative day.

Purpose : The purpose of this study was to identify status and characteristics of patients who have been readmitted to ICU, and to analyze risk factors associated with the readmission to ICU within 48hours. Method: Data were collected from patient's electronic medical reports from one hospital in B city. Participants were 2,937 patients aged 18 years old or older admitted to the ICU. Data were analyzed using odd ratios (ORs) from multivariate logistic regressions. Results: 2.2% of the 2,937 patients were early readmitted to ICU. Risk factors for early readmission to ICU were existence of respiratory disease, use of mechanical ventilator, and duration of hospitalization (longer). Conclusion: The assessment on the respiratory system of the patient who will be discharged from the ICU was identified as an important nursing activity. Therefore, the respiratory system management and education should be actively conducted. In addition, early ICU readmission may be prevented and decreased if a link was built to share the information on patient condition between the ICU and general wards.
Rectal prolapse is encountered in all domestic animal species but clinical report regarding the condition in the elk is limited. A 10-year-old elk bull weighing 400 kg was referred to the Large Animal Hospital of Kyungpook National University with clinical signs including intermittent tenesmus related to rectal prolapse and anorexia for the previous 5 days. Type II rectal prolapse was diagnosed based on the history and clinical signs. An emergency resection and anastomosis including a purse string suture was performed under general intravenous (IV) anesthesia in field to prevent injury of the patient and the staff. The patient recovered uneventfully after the surgery without excitement. A systemic antibiotic and an anti-inflammatory drug were given to prevent postoperative complication and relieve pain. The follow up on the patient after 4 weeks of the surgery showed that the prolapse recurred due to intermittent straining after 2 weeks of the surgery. The patient was on the glucocorticoid for the following 3 days but was finally euthanized owing to the exacerbation of the prolapse 1 week after the recurrence. This report describes a rare case of rectal prolapse in an elk bull and the clinical outcome of the surgical repair in detail.
Objectives : The purpose of this study is to seek analysis of related factors for dental fear and to know the fear component affecting the treatment of the dental treatment. Methods : Total of 320 females and males aging from 10 to 40 years old with dental treatments done from May to July 2010 have participated with self-entry survey method utilizing the Dental Fear Survey questionnaire. The data received was analyzed using the descriptive statistic, t-test, ANOVA and multiple correlation analysis with level of significance as p<0.05. Results : The dental fear factor were significantly high in woman by $2.79{\pm}0.92$ and in 40's age group by $2.82{\pm}0.84$ (p<0.05). Participants with frequent tooth ache, experiencing pain while treating the tooth ache and breaking the dental treatment appointment showed higher fear factor. Thoughts of having poor dental health condition caused bad influence on fear factor. Overall, the factors affecting the fear of the dental treatments were gender, an ache while treating, number of missed appointment, and distrust of the dentist, stimulation factor were the factors affecting the treatment. Conclusions : The fear factors in the dental treatment were higher with gender, related characteristics of the oral health and distrust of the dentist. Therefore accurately recognizing the sensitive patient with dental treatment, proper management of the toothache, effort to increase the creditability of the doctors and separately treating patient customized for each individual patient will reduce the fear of the dental treatment.

Pain is a public health problem affecting more than half of cancer patients. Despite the success of the protocols currently used, pain cannot still be reduced satisfactorily in the large majority of patients. In order to improve pain management, all healthcare professionals involved with pain should have sufficient knowledge on pain assessment and treatment, and should inform patients to prevent patient-related barriers.
