The present study investigated the actual conditions of physical therapy rooms at long-term hospitals in Korea and conducted a comparative analysis to develop an efficient floor plan and facility improvement measures. 1. At hospitals surveyed, physical therapy services were used at a high frequency but they did not have enough space for rehab treatment and long paths of patient flow were found to make patient management inconvenient. Therefore, physical therapy units should be conveniently located both in terms of distance and direction so as to be accessible from patient rooms or wards. The space should be organized in a concentrated layout for efficiency of physical therapy, and floor planning for therapy units should ensure the best possible viewing angle to therapists. 2. With regard to the disease characteristics of patients, many physical therapy rooms were in difficult circumstances because of poor facilities, so they need to secure skilled personnel, supplement apparatuses and equipment and have rooms for functional recovery, hydrotherapy and operation treatment. In addition, each of the curtained or partitioned areas for treatment should be set up with consideration for the amount of space taken up by medical equipment. The area under each bed should be designed for patient convenience so that it can be used as storage space for patient's belongings and shoes. 3. Patients complained about the lack of physical therapy space, resting places or exercise areas and demanded the expansion of rehab programs and facilities. Physical therapy facilities need to be improved for patient privacy and effective natural ventilation. 4. At most of the long-term hospitals surveyed, physical therapy units were found to have small areas and treatment equipment and devices were insufficient compared to the number of patients. Therefore, it is required to secure more space (at least 138.24 sq. meters per 100 beds) and improve facilities for better physical therapy services.
Objective : To evaluate the effect of Oriental Medical Treatment on a patient with Hypoxic Ischemic Encephalopathy followed by Cardiopulmonary Resuscitation Method : We applied various methodology of Oriental Medical Treatment including Acupuncture, Electroacupuncture, Physical treatment, Herbal Medicine, Moxibustion treatment and Western medication as well. Result: Herbal medicine was applied on the basis of the patient's history. We applied formular to remove phelgm as a pathogenic factor after Hypoxic Ischemic Encephalopathy. At the same time, considering the patient spent more than a month in ICU lacking appropriate nutrition, we used formuli on the basis of 'Deficiency of Spleen' focusing to vitalize the function of digestive system. As the condition of the patient changed, we also adapted formular accordingly. We prescribed Herbal medication to strengthen Yin and Yang equally as she got hospitalized for long time. Also we applied Acupuncture treatment and Moxibustion treatment to control Qi flow. The general condition of the patient got better with successful removal of Foley catheter and elevated Glasgow Coma scale. We used Electroacupuncture, Physical treatment and Western medication at the same to get maximized effect on relaxing the contracted muscle. According to the Modified Ashworth Scale (MAS), we have some changes in muscle spasticity but later, the effect was not that significant. Conclusion : We had a patient with Hypoxic Ischemic Encephalopathy followed by Cardiopulmonary Resuscitation. In the management of Hypoxic Ischemic Encephalopathy, Conservative treatments are the mainstream but there are not many alternatives. Therefore, We suggest that Oriental medical approach may contribute to the management of Hypoxic Ischemic Encephalopathy.
본 연구에서는 임상실습경험이 있는 간호대학생을 대상으로 인터넷 윤리의식, 자기통제력, 간호전문직관 및 환자개인정보보호 인식간의 관계를 살펴보고, 정보화 시태의 흐름을 반영하여 환자개인정보 보호 인식 향상프로그램 개발을 위한 기초자료를 제공하고자 시도되었다. 자료수집기간은 2023년 11월 20일부터 12월 15일까지이며. 대전광역시와 청주시 소재 D대학교 및 C대학교에 재학중이며 본 연구에 참여 동의한 간호대학생 142명의 설문지가 분석되었다. 분석결과 간호대학생의 환자개인정보 보호인식에 미치는 영향요인은 간호전문직관과 자기통제력으로 나타났으며, 이들의 변수에 대한 설명력은 24%였다. 결론적으로 간호대학생의 환자개인정보 보호인식을 향상시키기 위해서는 인터넷 윤리의식을 증진시킬 수 있는 방안을 모색하고 간호전문직관을 높일 수 있는 교육 프로그램개발이 이루어져야 할것으로 생각된다.
This study was designed and undertaken to find out the effectiveness of VAS for evaluation of general anesthetic postoperative pulmonary-function. We compared the degree of perceived pulmonary function recovery with peak expiratory flow at postoperative 72 hours of subjects. The subjects of this study were collected 38 patients who had received upper abdominal operation in St. Paul Hospital, Catholic University Medical College, and Kangnam Scared Heart Hospital and Kangdong Scared Heart Hospital, Hallym University. Data collection period was from June 15th, to August 7th, 1992. The degree of pulmonary recovery function was measured with peak expiratory at 72 hours postoperatively. The degree of perceived pulmonary function of the patient was measured with ten points visual analog scale at 72 hours postoperatively. Peak expiratory flow and visual analog score was analyzed with Pearson correlation. Peak expiratory flow was expressed as a ratio of preoperative value. The result was as follows : The recovery of pulmonary function and the degree of perceived pulmonary function of the patient at 72hours postoperatively was revealed high correlation (r=.84). The above result suggested that patients with general anesthetic upper abdominal surgery should evaluate recovery of pulmonary function making use of VAS. We know that VAS is very useful in postoperative patients. We perceived that VAS is to take up a positive attitude of patients. Nurse should furnish the nursing care objectively and scientifically to patients. As VAS was economic and simple, VAS should be adviced for wider application.
Purpose: The temperature of a warm fluid infused into a patient is lowered because the line that allows the fluid to be infused into the patient is exposed to room air. This study evaluated the effects of aluminum foil used as an insulator surrounding the fluid infusion lines when using warm crystalloid fluids to treat traumatic shock patients. Methods: The study measured the differences in fluid temperature between infusion lines with and without the aluminum-foil insulation. We used 1L of normal saline at $40^{\circ}C$ as the infusion fluid, and the fluid infusion line was 200 cm long. The differences in temperature were measured for various fluid flow rates from 12,000 mL/min to 100 mL/min. We performed three experiments at each flow rate. Results: The results showed the differences in temperature between the groups with and without the aluminum insulation were significant for flow rates above 100 mL/min. Conclusion: Hypothermia in trauma patients results in many adverse complications such as peripheral vascular constriction, tissue hypoxia, metabolic acidosis, heart dysfunction and so on. Thus, the use of warm fluids and blood components is essential to reduce the probability of hypothermia. This study showed the aluminum foil wrapped around the infusion line had an insulator effect. As a result, such a wrapping can be used to avoid the adverse effects of hypothermia.
전 인구의 노령화와 더불어 그 위험성이 커지는 질환 중 하나인 뇌졸중은 원인의 80%정도가 대뇌관류, 즉 뇌로 흐르는 혈류량이 줄어드는 문제로 발생한다. 이러한 뇌졸중의 가장 대표적인 예방법과 치료법중 하나가 대뇌관류를 증강하는 것 이다. 하지만 기존의 대뇌 관류 증강 방식이 주로 침습적인 방법으로 행해져 왔기 때문에, 환자에게 부담을 주고, 다수의 합병증을 유발한다는 문제점이 지적되었다. 이에 본 논문에서는 이러한 침습적 대뇌 관류 증강 방법의 단점을 보완하고자, 비침습적으로 대뇌관류를 증강 시키는 장치를 제안한다. 비침습적 대뇌 관류 증강장치는 환자의 사지에 압박패드를 장착한 후 각 사지의 혈압을 정확히 측정한 후, 측정된 혈압을 바탕으로 사지압박을 실시하여 대뇌관류증강을 이룬다. 이는 기존의 침습적 대뇌 관류 증강 방법과는 비교해 대뇌관류 증가율이 상대적으로 조금 부족할 수 있지만 환자에게 부담이 되지 않고 대뇌혈류량을 의미 있게 증가시키기 때문에 환자에게 선택적으로 사용될 수 있다.

We applied Qi-diagnosis to the main study to lay the foundation and framework for research and education about the Korean Medicine. The authors try to describe systemically and specifically the Qi-diagnosis that the authors are using in clinical diagnosis ane treatment so that anyone can use it. The authors have been able to grasp the flow of human bio-energy through years of training. It has had many effects by applying the Qi-diagnosis to patients. The steps of the bio-energy flow have become objective. And the authors have been applied to acupuncture, herbal medicine, moxibustion, bruising treatment and anthrax anesthesia in clinical through the Qi-diagnosis. Also, it is applied to the life management of patients. It is applied to arts such as music therapy and art therapy. The deeper the depth of the Qi-diagnosis, the greater the opportunity to utilize the Qi-diagnosis. The Qi-diagnosis is the origin of the korean medicine. It was able to make diagnosis and treatment correct and to establish clues that the medical problems would be solved through the Qi-diagnosis. In order to do so, the diagnostician must be able to feel the auricular flow of the body accurately and objectively. In addition, he must have a comprehensive understanding of the overall framework of medicine. As a result, diagnosis and treatment of the patient as well as general problems of the patient can be identified and advised, so comprehensive treatment is possible. And it is not only a specific person can do it, but it is a diagnostic method that anyone can take if they take the basic steps step by step.
Objective : The authors analyze prospectively the result of transcranial doppler[TCD] in normal pressure hydrocephalus and compared its cerebral blood flow parameters to radionuclide cerebrospinal fluid[CSF] flow study, postoperative brain computed tomography[CT] findings and clinical outcome, and studied the relationship between cerebral hemodynamics and clinical performance. Methods : Twenty five patients with hydrocephalus undertook pre- and post-operative TCD but only preoperative CSF flow study was performed. Mean flow velocity[Vm], pulsatility index[PI] and resistance index[RI] were assessed through TCD and changes in ventricle size and cortical gyral atrophy were checked through brain CT. Results : Postoperative hydrocephalus showed an increase in Vm[ACA P=0.037, MCA P=0.034], decrease in PI[ACA P=0.019, MCA P=0.017] and decrease in RI [ACA P=0.017, MCA P=0.021] compared to preoperative TCD parameters in the postoperative improvement group. In the postoperative improvement group, postoperative TCD parameters correlated with CSF flow study grade [Vm : $R^2=-0.75$, PI : $R^2=0.86$, RI : $R^2=0.78$] and ventriculocranial ratio change correlated with PI change [$R^2=0.73$]. The convexity gyral atrophy and initial TCD parameters showed close relationship to outcome. Conclusion : PI and RI can be used as an indicator of post operative prognosis, and with the addition of CSF flow study values, can also be used as a tool to predict pre-op and post-op patient status and successful shunt surgery.
Background: It had been reported by authors that linear polarized infrared light radiation (Superizer: SL) near the stellate ganglion had a similar effect on the change of skin temperature of hand compared with the stellate ganglion block (SGB). We hypothesized that this was due to dilatation of vessels and an increased blood flow. The aim of this study was to measure the velocity of blood flow in peripheral vessels after linear polarized infrared light radiation near the stellate ganglion and to compare the effect of SL with that of SGB using local anesthetics. Methods: Forty patients whose clinical criteria were matched for the symptoms of SGB were selected for study. We radiated the stellate ganglion by linear polarized infrared light radiation and measured the blood flow of radial artery using Ultrasound Doppler blood flow meter before and after 10, 20 and 30 minutes post-radiation. After 3 days, SGB was performed using 8 ml of 1% mepivacaine to the same patient, and the radial artery blood flow was measured in the same manner. Results: The blood flow velocity was increased by 40% and 27% at 10 min and 20 min after SL and by 42% and 41% at 10 min and 20 min after SGB. However, there was no statistically significant difference in blood flow velocity between SGL and SGB. Conclusions: We could conclude that linear polarized radiation is a clinically simple and useful noninvasive therapeutic tool in clinical area.
Objective : The cerebrospinal fluid(CSF) pulsates within the craniospinal axis in response to rhythmic cerebral blood volume variation during the cardiac cycle. The aim of this study is to characterize the normal and abnormal CSF flow and its waveforms in the cervical spinal subarachnoid space. Methods : The magnetic resonance(MR) images were obtained with 1.5 T(GE Signa, GE Medical Systems, Milwaukee, USA) unit using the 2 dimensional cine PC(phase contrast) sequence with cardiac gating and gradient recalled echo imaging. This pulse sequence yielded 16 quantitative flow-encoded images per cardiac cycle. Sagittal and axial images of the cervical spinal CSF space were obtained, and target sites were analyzed for characteristic CSF flow (TR=50ms, TE=12.5-15ms). The region of interest(ROI) was 1mm 3 in volume. Twenty six persons were included in this study : 10 healthy volunteers and 16 patients with cervical myelopathy. The post-operative cine MR study were also done in five patients. Results : The normal CSF pulsation dynamics in the cervical spine showed discrete systolic and diastolic components. The CSF flow revealed a sine wave pattern, in which the systolic phase was shorter than the diastolic phase(ratio=2 : 3). The patient group revealed decreased amplitudes of the CSF flow and irregularly distored flow waves. The systolic phase was elongated in the ROI above the stenotic level, whereas the diastolic phase was lengthened below the level. In the postoperative images, the abnormal pattern and amplitude were found to be corrected. Conclusion : From these results, the authors believe that the CSF flow study provides valuable informations regarding the extent of cervical stenosis and may be useful for the surgical planning and post-operative evaluation.
