Rhinolith: An incidental radiographic finding

  • Rhinoliths are foreign bodies composed of mineralised deposits in the nasal cavity that are rarely encountered in routine dental practice. The current report presents a case of a 77-year-old female patient who visited a dental treatment centre for a routine dental check-up and prophylaxis. On the clinical examination, the patient did not report any breathing difficulty, nasal obstruction, or any form of discharge. Panoramic radiography showed a horizontally magnified radiopaque structure in the right nasal fossa region. Cone-beam computed tomography (CBCT) revealed a hyperdense entity with a laminated presentation in the right nasal fossa. The hyperdense entity was diagnosed as a rhinolith based on the radiographic findings. Although the patient was asymptomatic, a referral was made to an ear, nose, and throat surgeon for a further evaluation. This report highlights the importance of CBCT imaging in the diagnosis of soft-tissue calcifications.

  • Purpose : This study was to develop the nursing database for gastric cancer patients for clinical application. Method : Nursing data that development of this data base is comprehensive connected with gastric cancer patient nursing process frame to foundation as classification. Result : Each stage was processed based on the System Development Life Cycle. At the Strategy Planning stage, gastric cancer patient nursing process were analyzed. At the system Analysis Stage, database flowchart was drawn up based on frame of nursing process was drawn up. At the system Design Stage, a system was developed based on the flowchart and named the Nursing Database. The Nursing Database consisted of the patient's Basic Information, Patient's Nursing History, Discharge summary, Nursing Assessment, Nursing Diagnosis, Nursing Intervention/activity, Nursing Evaluation, Statics, Code Registration. Each element in flowchart was coded and made into a database. Nursing Assessment classified according to Gorden's Health Pattern Typology, and nursing diagnosis draws the standard 27 name of Hanguls and connected with nursing assessment. Nursing intervention and nursing activity draw 192 of thing that present in NIC, connected this with nursing assessment. Nursing evaluation is linked with nursing assessment, diagnosis and intervention by achievement availability of nursing goals. Conclusion : The biggest advantage of this database nursing process that can manage nursing information exactly and rapidly to foundation be.

  • A very rare case of mycotic aneurysm in the descending thoracic aorta due to salmonellosis was treated in our hospital The patient was a 62 year-old male who 48 days before the operation was admitted to the depart,cent of internal medicine complaining of fever. nausla. vomiting, and loose stool. He was treated for 35 days and discharged. Three days after discharge, however, the patient was readmitted to the hospital comE긴ainin프 of $\ulcorner$i포ht uppe$\ulcorner$ 킥uadrant abdominal pain. Fever developed on the third hospiti린 day, and on the eighth hospital day, the patient complAined of back pain and epigastric pale. A simple chest x-ray showed evidence of hemothorax in the left plueral space, and therefore, computed tomography of the chest was done. The patient was diagnosed as a ruptured mycotic aneurysm of the descending thoracic aorta, and was transferred to our Department of Thoracic and CArdiovascular Surgery. The aneurysm and infected tissues was widely debrided, and the site was then patched with a Dacron graft. Salmonella choleraesuis was identified in the blood and aneurysm cultures, and antibiotics were administered for weeks according to the sensitivity of the organism. The patient experienced no complication thereafter and for the last three months since the operation, he has been leading a healthy and normal social life.

  • The purpose of the present research is to evaluate the therapeutic ingredients-empathy, warmth, and intimacy - as perceived by patient in the relationship with nurses during his hospitalization. "Relationship Questionnaire"developed by C. B. Truax during 1963 was administered from September 1976 to December 1976 to a sampling of 61 discharged patients, 20 were received by mail questionnaire, 19 were successfully interviewed by their doctor at a regular psychotherapy meeting, 22 were contacted by nurses or psychiatric residents at discharge day. Findings showed that 1) the total mean score of therapeutic ingredients of nurses was 9. 54 ; 2)the total mean score of empathy, warmth and intimacy was 10. 10, 9.57, and 9.00 : 3) there was the significant relationship between therapeutic ingredients and other variables-sex, education, number of admission, helpfulness ratings of admission and, present health condition- : 4) there was no significant. relationship between therapeutic ingredients as patient′s perception and age, duration of admission, or motivation of admission, or follow - up rare ; 5) there was no relationship among empathy, warmth and intimacy ; 6) tile ranking of the important psychiatric personnel who were marked as the helper at the treatment and understanding for patient′s recovery, was psychiatric doctor and nurse and then nurse student and then nurse aid.

Chronic Central Post-Stroke Pain Treated with Scalp Acupuncture and Traditional Korean Medicine: A Case Report

  • A patient with central post-stroke pain was treated for 4 weeks with scalp acupuncture and traditional Korean medicine (following a cerebral infarction in 2013). The patient presented at Chungju hospital in January 2019 with left side weakness and tingling, numbness in the left hemisphere, chronic pain and dysarthria. Initially, herbal medicine, acupuncture, pharmacupunture, indirect moxibustion, and physiotherapy were administered together with Western medicine, with no improvement in the patient's condition. On Day 5, scalp electroacupuncture (MS1, MS5, MS10, MS11) was introduced. The numbness feeling in the patient's head resolved, and the pain in his upper body decreased. Grip force difference between the left and right hand improved from 3 kg to 0-0.5 kg. Sleep disturbance was resolved after 4 weeks treatment, and his average numeric rating scale score for pain improved from an admission score of 10, to a discharge score of 5. The patient could walk unaided after treatment.

A Case Report of a Patient Diagnosed with Diabetic Ketoacidosis Accompanied by Hyperammonemia from Systemic Inflammation (고암모니아혈증을 동반한 전신성 염증으로 인한 당뇨병성 케톤산증 환자 치험 1례)

  • A 42-year-old male patient with prolonged throat pain and discomfort, dry mouth, and general weakness and recently diagnosed with tonsillitis, pulmonary embolism, and venous thrombosis was admitted to the internal Korean medicine department. A sudden onset of diabetic ketoacidosis with hyperammonemia was diagnosed on the second day of treatment. During admission, the patient received insulin therapy, hydration, and traditional Korean medicine treatment, including herbal medicine. Subjective symptom change was evaluated daily and blood glucose level checked five times per day. At discharge, the patient's fasting and postprandial blood sugar levels were adequate. After an additional two weeks of herbal treatment, the symptoms were significantly ameliorated. Thus, having been admitted with dysregulated glucose metabolism leading to a hyperglycemic crisis after a series of inflammatory events, the patient showed symptomatic improvements and decreased blood glucose after 18 days of hospitalization and treatment.

A Case Report of Valproic Acid-Induced Tremor by Korean Traditional Medicine (Valproic Acid 투여로 인한 약인성 떨림 환자의 한방 치료 치험 1례)

  • A 56-year-old male patient with a sudden onset of tremor and involuntary movement of right upper and lower extremities, head, and voice was diagnosed as having a drug-induced tremor, with valproic acid being the culprit drug. The patient had undergone admission treatment at an internal Korean medicine department with herbal medicine according to constitutional diagnosis, acupuncture, moxibustion, cupping, and rehabilitation. The change in the severity of tremor was assessed with the Fahn Tolosa Marine scale. After discharge, the patient took herbal medicine for two more months. After 20 days of admission and 2 months of treatment with herbal medicine, the patient's tremor improved. In this case, the patient with valproic acid-induced tremor showed improvement in symptoms after 20 days of hospitalization and further reduction of tremor and improvement of quality of life were confirmed through follow-up for 2 months.

Analysis on the situation of inpatients with pressure ulcer by patient safety indicators (환자안전 지표에 의한 욕창발생 현황 분석)

  • In this study, we analyzed situation and length of stay(LOS) variations of inpatients with pressure ulcer using patient safety indicators developed by the United States Agency for Healthcare Research and Quality(AHRQ) and proposed management of medical quality and development of policy. The dataset was taken from 1,373 database of the hospital discharge injury survey from 2005 to 2008. Analysis method was used frequency and chi-square test, ANOVA, multiple linear regression analysis. In result, distribution of inpatients with pressure ulcer by sex were 52.5%(male), 47.5%(female), respectively and aged $65{\geqq}years$ was the highest in age group. LOS of inpatients with nervous system principal disease was the longest. Independent variables which were statistically associated with LOS of inpatients with pressure ulcer were year, sex, insurance type, bed size, operation, principal diagnosis. Therefore, hospital should develop the standardized strategy and guidelines to manage pressure ulcer inpatients efficiently and apply it into the medical information system.

A Case of a Patient with Pontine Infarction Complaining of Discomfort in Both Legs (양 하지 불편감을 호소하는 교뇌 경색 환자에 대한 한의 치험 1례)

  • Objective: This study examined the effectiveness of Korean medical treatment for an 81-year-old female patient with pontine infarction complaining of discomfort in both legs. Methods: The patient was hospitalized for 45 days and treated with herbal medicine (mainly Bohyulanshin-tang [補血安神湯], pharmacopuncture, acupuncture, electroacupuncture, and moxibustion). We used the International Restless Legs Scale (IRLS) and Numerical Rating Scale (NRS) and checked the duration of the symptoms to evaluate the clinical effects of the treatment. Results: After 45 days of hospitalization, the IRLS score decreased from 33 to 19, with the lowest score of 8 recorded on the twenty-seventh day after discharge. The NRS score and the duration of symptoms also improved, and the treatment effect continued following discharge. Conclusions: This study suggests that Korean medical treatment may be effective for the treatment of restless legs.

A Case Report of Korean Medicine Treatment for A Patient with Variant Angina and Asthma Symptoms (천식 증상을 가진 변형 협심증 환자에 대한 한방치료 증례보고)

  • Objective: The purpose of this case study was to report the symptom improvement effect of integrative Korean medicine treatment based on Pyongsim Solution: Cheonwangbosim-dan in a patient with variant angina and asthma symptoms. Methods: This 68-year-old male patient was treated with Pyongsim Solution, Gamihyangsayukgunja-tang, Gwakhyangjunggi-san, acupuncture, pharmacopuncture, and cupping therapy for 24 days. The European Quality of Life-5 Dimensions-5 Levels profile, Numerical Rating Scale score, and pulse rate per minute were measured on admission, at two weeks of hospitalization, and upon discharge to assess the clinical outcomes. Results: After treatment, the "mobility" dimension profile of the European Quality of Life-5 Dimensions-5 Levels changed from 1 to 1, "self-care" changed from 1 to 1, "usual activity" changed from 3 to 2, "pain/discomfort" changed from 4 to 3, and "anxiety/depression" changed from 3 to 2. The Numerical Rating Scale score for chest pain decreased from 7 to 5, chest tightness decreased from 6 to 4, sleep disturbance decreased from 6 to 3, polydipsia decreased from 5 to 3, and stomach discomfort decreased from 5 to 3. All the profiles of these five dimensions were unchanged for 11 days in the period between the two weeks of hospitalization and discharge. There were also no changes in the profiles of either "mobility" or "self-care" throughout the entire 24-day hospitalization period. Conclusion: This case report shows that integrative Korean medicine treatment based on Pyongsim Solution can improve the symptoms of patients with variant angina and mild intermittent asthma, suggesting that Pyongsim Solution could be an important herbal medication for treatment.
