A Case Study of Soyangin Patient with Hyperhidrosis Treated Successfully with Hyungbangsabaek-san(荊防瀉白散) (신열두통(身熱頭痛) 망음증(亡陰證)으로 진단하여 형방사백산(荊防瀉白散)으로 호전된 국소다한증(多汗症) 환자 3례 보고)

  • Objectives The aim of this study was to report significant improvement of hyperhidrosis after treatment with Hyungbangsabaek-san (荊防瀉白散), in a Soyangin Mangeum Symptomatic pattern Patient. Methods The patients were diagnosed with Soyang Sin-Yeol Du-Tong Mang-Eum Symptomatology(身熱頭痛亡陰證) and treated with Hyungbangsabaek-san(荊防瀉白散). The primary outcome measure for this study were sweat using a questionnaire with visual analogue scale(VAS). Secondary outcome assessment included change of odinary symptoms such as patient's sleep, feces and digestion. Results The symptoms of hyperhidrosis disappeared by the end of the accure period without side effect. Original symptoms were also changed. Conclusions This result show Hyungbangsabaek-san(荊防瀉白散) can be used to treat hyperhidrosis in a Soyangin Heat-related diarrhea accompanied by headache Mangeum(身熱頭痛亡陰) symptomatic pattern patient. Meaning and process of hyperhidrosis are different according to Sasang Constitutions.

Mild Bradykinesia Due to an Injury of Corticofugal-Tract from Secondary Motor Area in a Patient with Traumatic Brain Injury

  • Objectives: We report on a patient who showed mild bradykinesia due to injury of the corticofugal tract (CFT) from the secondary motor area following direct head trauma, which was demonstrated on diffusion tensor tractography (DTT). Case summary: A 58-year-old male patient underwent conservative management for subarachnoid hemorrhages caused by direct head trauma resulting from a fall from six-meter height at the department of neurosurgery of a local hospital. His Glasgow Coma Scale score was 3. He developed mildly slow movements following the head trauma and visited the rehabilitation department of a university hospital at ten weeks after the fall. The patient exhibited mild bradykinesia during walking and arm movements with mild weakness in all four extremities (G/G-). Results: On ten-week DTT, narrowing of the right CFT from the supplementary motor area (SMA-CFT), and partial tearing of the left SMA-CFT, left CFTs from the dorsal premotor cortex (dPMC-CFT) and both corticospinal tracts (CSTs) at the subcortical white matter were observed. Conclusion: This case demonstrated abnormalities in both CSTs (partial tearing at the subcortical white matter and narrowing), both SMA-CFTs (narrowing and partial tearing) and left dPMC-CFT. We believe our findings suggest the necessity of assessment of the CFTs from the secondary motor area for patients with unexplained bradykinesia following direct head trauma.

Case Report of Chemotherapy Induced Peripheral Neuropathy Treated with Korean Medicine (항암화학요법으로 인한 말초신경병증 환자의 한방치험 1례)

  • The purpose of this study is to report a clinical case of a patient with CIPN (Chemotherapy Induced Peripheral Neuropathy) successfully treated with Korean medicine interventions including acupuncture, herbal medicine and moxibustion. The patient is a female with CIPN who was diagnosed with ascending colon cancer and suffering from paresthesia of extremities after chemotherapy of platinum compounds. The patient was treated mainly with electroacupuncture (Ex-LE 10), indirect moxibustion and herbal medicine (Ucha-Shinki-hwan granule). The clinical outcomes were measured by NCI CTCAE grade, EORTC QLQ-C30 (CIPN20) scale and patient's assessment of symptoms. In this case, the NCI CTCAE grade was improved from II to I. The scores of global health status and functional scale in QLQ-C30 were increased and symptom scale was decreased. Especially, this case shows a noticeable decrease in sensory scale in QLQ-CIPN20. In patient's general assessment, the scale was changed from 10 to 5. Korean medical interventions including acupuncture, herbal medicine and moxibustion could potentially be an effective treatment for CIPN if further researches are conducted.

Completeness of Emergency Medical Service Activity Report by Paramedics (119 구급대원의 구급활동일지 기록 충실도)

  • This study evaluated the completeness of emergency medical service (EMS) activity reports. In all, 67,830 cases of normal transfers were statistically analyzed using IBM SPSS Statistics version 22, and statistical significance was set at p <0.5. The 119EMT_2 certificate was omitted in 50,037 (73.8%) cases, followed by time-related items in 1,227 (1.8%) cases. In the primary assessment of vital signs, systolic blood pressure was omitted and erroneous in 1,218 (1.9%) and 1,129 (1.8%) cases, respectively. In the secondary assessment, the completeness of all vital sign items was approximately 70%. As the severity of the patient's condition increased, the errors in the EMS activity report also increased, at a significant level (p=.00). Paramedics must be aware of the importance of completing the activity report.

  • Resricted opening of the mouth in children can be derived from a variety af extra and intra articular causes. A 5-year-old female patient with an elongated right coronoid process and with congenital missing teeth came to clinic for wearing of esthetic denture. A proper medical consultation and laboratory test had done for assessment a systemic syndrome related to her dental anomalies. The result was that her systemic findings dindn't accord specific signs and symptoms of Hallermann-Streiff syndrome. Unilateral coronoidectomy was fulfilled to improve mouth opening and subsequently endodontic and restorative procedure. Maxillary partial denture was delivered for esthetic problem. The periodic recall medical/dental check-up are recommended.

Improvement of Pneumonia in a Patient with Non-Small Cell Lung Cancer Treated with Herbal Medicine after Cessation of Antibiotics - a Case Report

  • A 73-year-old non-small cell lung cancer (NSCLC) patient admitted due to cough, sputum, and dyspnea, aggravated a week ago. She was diagnosed as pneumonia based on the assessment of inflammation markers, chest X-ray and sputum culture. Computed tomography (CT) scan was conducted to exclude malignant tumor metastasis. At the initiation of treatment, considering underlying disease and inflammation marker level, herbal medicine and antibiotics were concurrently used and antibiotics had been discontinued after 10days. Using the monotherapy of herbal medicine in the next 6 days, chest X-ray showed remarkably decreased infiltration in right middle lung and right lower lung. This case represented additional improvement of chest X-ray when treated only with herb medicine after termination of antibiotic therapy and demonstrated the possibility of applying herbal medicine in patients with limited use of antibiotics.

  • Objectives: The purpose of this study is to evaluate the clinical effectiveness of acupotomy in the treatment of various facial atrophic scars except acne scars. Methods: Four patients with facial atrophic scars were treated by superficial dermal undermining with acupotomy. They were treated six times, and evaluated by comparing before and after photos, PGA(Physician's Global assessment), SESES(The Stony Brook Scar Evaluation Score) and patient satisfaction. Results and Conclusions: After observing case photos, PGA, SBSES, patient's satisfaction, it can be considered that acupotomy is an effective treatment modality for facial atrophic scars and it has no significant side effects.

  • Jarcho-Levin syndrome is a congenital disorder characterized by several vertebral and costal anomalies. Other abnormalities have also been described, including neural tube defects, Arnold-Chiari malformation, renal/urinary tract abnormalities, hydrocephalus, hydroureteronephrosis, and meningomyelocele. We describe a spondylocostal dysplasia form of Jarcho-Levin syndrome that was prenatally diagnosed at 11 weeks of gestation and surviving. Although the patient had sporadic-type Jarcho-Levin syndrome, with normal karyotype and no family history of disease, the assessment of inheritance patterns and genetic counseling for the parents was important to inform them about the potential risks.

  • Objectives : To get to know the effects of placental extract and laser theraphy for Temporomandibular Joint Disorder, we have tried a injection of placental extract and laser theraphy into 聽宮(SI19), 聽會(GB02) for patient with Temporomandibular Joint Disorder. Methods : In this case clinical assessment of temporomandibular joint dysfunction is assessed by the VAS score and the Korean Version of Research Diagnostic Criteria for Temporomandibular Disorders(RDC/TMD). Results : The VAS score was significantly decreased after Placental extract herbal acupuncture and laser theraphy Conclusion : Placental extract herbal acupuncture and laser theraphy effectively resolved pain for Temporomandibular Disorders.

Evaluation of Clinical Competence in Plastic Surgery using OSCE(Objective Structured Clinical Examination): 3-Year Experience (객관구조화진료시험을 이용한 성형외과학 진료능력평가: 3년간의 경험)

  • This is the evaluation report of clinical competence of undergraduate medical students in Plastic Surgery field using OSCE(Objective Structured Clinical Examination) executed in three years. OSCE comprises of assessment of subject, choice of clinical stations sampling, identification of components of clinical competence to be evaluated, the level of performance required, development of specification table, editing of OSCE presentation page and assessment of practicability and results exploitation. About fifty students were examined annually. Seven station stimuli with simulated patient participation were carried out. The mean OSCE score was $82.3{\pm}6.19$. The reliability of the total station was 0.72. The examination shows a positive response to the OSCE. Our experience shows OSCE's feasibility for Plastic Surgery during the initial course of education. Referring to our experiences, the Korean Society of Plastic and Reconstructive Surgeons (KPRS) applied the OSCE test for Korean Plastic Surgery Board Certification
