후복막강 임파종에 의한 좌골신경통 (Sciatica from Retroperitoneal Lymphoma)

  • Back pain with sciatica is a common clinical syndrome that may be the expression of an uncommon pathological process. We describe a case of retroperitoneal lymphoma mimicking herniated lumbar disc. 62 year old male patient with a clinical picture of sciatica as a first manifestation of retroperitoneal lymphoma. His pain was not response to conventional therapy, and progressively worsened. We reevaluated him and found huge retroperitoneal lymphoma in his pelvic cavity. So we discuss how to identify the few who had extra attention. It is important to rule out occult pathology in patient with an atypical presentation. In assessing a patient with a low back pain, accurate evaluation require a logical, step by step method.

  • The joint dysfunction is the major cause of pain in trunk and extremities according to J. McM. Mennell who mentioned that there were no pathological changes that was found in the painful joint. The referred pain due to joint dysfunction has to be differentiated from other painful conditions, particularly of the neuromusculoskeletal system. Most of the pain in the lower extremities is due to the sacroiliac joint dysfunction according to Setsuo Hakata who had developed arthrokinematic approach(AKA) for sacroiliac joint in 1989. AKA for sacroiliac dysfunction is made up of distraction & gliding technics that are no violent to recover the disturbed intra-articular movement and result in immediate relief of pain.

  • Pain and sensory disorder resulting from injury to peripheral nerves of the face and jaws are a major source of patient dissatisfaction and suffering. The majority of patient who sustain injuries to the peripheral sensory nerves of the face and jaws experience a slow but orderly return of sensation that is functional and tolerable in quality, if not "normal". For many patients, however, the long-term effects are a source of aggravation, and for a few, a significant cause of suffering. Common complaints relate to reduced sensory information causing embarrassing food accumulations or drooling, biting a burning the lip or tongue, and difficulty in performing routine activities such as shaving and apply makeup. For some patients posttraumatic symptoms become pathological and frankly painful. The predominent pain components are (1) numbing anesthesia dolorosa pain, (2) triggered neuralgiaform pain, (3) burning, aching causalgiaform pain, and (4) phantom pain. This is a report of cases about posttraumatic pain syndrome associated with dental treatment in a psychologically disabled patient.

Text-Mining of Online Discourse to Characterize the Nature of Pain in Low Back Pain

  • PURPOSE: Text-mining has been shown to be useful for understanding the clinical characteristics and patients' concerns regarding a specific disease. Low back pain (LBP) is the most common disease in modern society and has a wide variety of causes and symptoms. On the other hand, it is difficult to understand the clinical characteristics and the needs as well as demands of patients with LBP because of the various clinical characteristics. This study examined online texts on LBP to determine of text-mining can help better understand general characteristics of LBP and its specific elements. METHODS: Online data from www.spine-health.com were used for text-mining. Keyword frequency analysis was performed first on the complete text of postings (full-text analysis). Only the sentences containing the highest frequency word, pain, were selected. Next, texts including the sentences were used to re-analyze the keyword frequency (pain-text analysis). RESULTS: Keyword frequency analysis showed that pain is of utmost concern. Full-text analysis was dominated by structural, pathological, and therapeutic words, whereas pain-text analysis was related mainly to the location and quality of the pain. CONCLUSION: The present study indicated that text-mining for a specific element (keyword) of a particular disease could enhance the understanding of the specific aspect of the disease. This suggests that a consideration of the text source is required when interpreting the results. Clinically, the present results suggest that clinicians pay more attention to the pain a patient is experiencing, and provide information based on medical knowledge.

  • Although painful conditions of varying degrees of severity involving the soft tissues (i .e., muscles, tendons, ligaments, periosteum and peripheral nerves) occur frequently, their underlying pathogenesis is poorly understood. The term peripheral neuropathic pain has recently been suggested to embrace the combination of positive and negative symptoms in patients whose pain is due to pathological changes or dysfunction in peripheral nerves or nerve root. The spinal nerve root, because of its vulnerable position, is very easily prone to injury from pressure, stretch, angulation, and friction. Therefore, not a few of musculoskeletal chronic pains are result of nerve root dysfunction. Neuropathic changes due to nerve root dysfunction are primarily in soft tissue especially muscle, tendon and joint. It shows tenderness over muscle motor points and palpable muscle contracture bands and restricted Joint range. Careful palpation and physical examination is the important tool that, be abne to detect all of these phenomena.

  • Objective : The purpose of this study was to evaluate water distribution differences of the right and left low limb in patients who had low back pain with sciatica or not. Methods : Water distribution differences of the right and left low limb by body composition(INBODY 2.0, Biospace, seoul, korea) were analysed in the views of groups(low back pain with and without sciatica) and duration of disease. Results : 1. In the case of low back pain patient, there was no significance in variance of water distribution of the right and left lower limbs regardless of suffering period, but significance difference in patients that have suffered low back pain with one side sciatica showed according to suffering period. 2. Within 2 weeks, there was no significant deflection between low back pain with and without sciatica in water distribution of the right and left lower limbs, More than 2 weeks significant deflection showed. Conclusion : Loss of lower limb's muscle that ensue in contracting a disease period was observed in Low Back Pain with Sciatica. The continuos studies about pathological change of lower limb in low back pain have to be perfomed.

Selective Neurotomy of Sacral Lateral Branches for Pain of Sacroiliac Joint Dysfunction

  • Objective : The sacroiliac joint complex is often related with functionally incapacitating pain in old aged people. The purpose of this study is to delineate the investigation strategies and to determine the long-term effect of radiofrequency [RF] neurotomies for pain arising from sacroiliac Joint dysfunction[SIJD]. Methods : Sixteen patients were diagnosed as having chronic pain from SIJD by comparative controlled blocks on L5 dorsal rami, sacroiliac Joints and deep interosseous ligaments. After confirming the positive response [more than 50% of pain relief], sensory stimulation was applied to detect the 'pathological' branches. Subsequently, RF neurotomies were performed on the selected nerve branches. Surgical outcome was graded as successful, moderate improvement, and failure after a 6month follow-up period. Results : Stimulation intensity was 0.45V to elicit pain response in the L5 dorsal rami and lateral sacral branches. The number of RF-lesioned nerve branches was 6per patient. The average number of lesions for each branch was 1.3. Most commonly selected branches were L5 dorsal ramus [88%] and S2-upper division [88%]. Ten patients [63%] reported a successful outcome according to the outcome criteria after 6months of follow-up, and five patients [31%] reported complete relief [100%]. Five patients [31%] showed moderate improvements. One patient reported failure. Conclusion : RF neurotomy of lateral sacral branches is an excellent treatment modality for the pain due to SIJD, provided that comparative controlled block shows a positive response.

Correlation Analysis of Pressure Pain Threshold and Muscle Thickness in Individuals with Non-Specific Low Back Pain

  • Objective: Low back pain (LBP) is a symptom that accounts for a large proportion of musculoskeletal pain. Among them, non-specific LBP (NSLBP) means that the pathological cause is unknown, and belongs to the most common LBP. Studies on the mechanism of muscle control in LBP are insufficient, and quantitative studies are needed. Design: Observational cross-sectional study design Methods: A Thirty participants with NSLBP symptoms were enrolled, and their pressure pain thresholds (PPT) and muscle and fat thickness were measured. Participants measured the paraspinal muscles (PM) of the thoracic and lumbar spine and medial hamstring (semitendinosus) on the dominant and non-dominant sides in the prone position. Results: Among the variables that were significant in the correlation analysis, PM of the thoracic and lumbar spine showed a significant relationship in the PPT ([thoracic spine PM]=1.141+0.912 [lumbar spine PM]). Also, there was a significant relationship between the lumbar spine PM in the PPT and the thoracic spine PM in the muscle thickness ([lumbar spine PM of PPT]=4.057+0.117 [thoracic spine PM of muscle thickness]) Conclusions: Although there was no muscle imbalance according to the dominant and non-dominant side, there is a correlation between the pressure pain threshold and the muscle thickness between the paraspinal muscles of the thoracic spine and the lumbar spine.

요통환자의 보행패턴에 관한 연구 (A Study of Gait Patterns in Patients with Low Back Pain)

  • Gait is a highly complex activity in which many variables can be observed and measured. Walking is a repetitious sequence of limb to move the body and to maintain stability. Normal gait is rhythmic and characterized by alternating propulsive and retropulsive motions of the lower extremities. Pathological gait patterns have four functional categories (deformity, muscle weakness, impaired control, pain). The purpose of this study was to assess the quantitive gait variables(the width of the base, length of a step, stride length, cadence, velocity) in patients with low back pain. Patients walked more slowly, took shorter steps and did not show the symmetrical gait patterns.

Paeoniflorin ameliorates neuropathic pain-induced depression-like behaviors in mice by inhibiting hippocampal neuroinflammation activated via TLR4/NF-κB pathway

  • Neuropathic pain (NP) that contributes to the comorbidity between pain and depression is a clinical dilemma. Neuroinflammatory responses are known to have potentially important roles in the initiation of NP and depressive mood. In this study, we aimed to investigate the effects of paeoniflorin (PF) on NP-induced depression-like behaviors by targeting the hippocampal neuroinflammation through the toll-like receptor 4 (TLR4)/nuclear factor-kappa B (NF-κB) signaling pathway. We used a murine model of NP caused by unilateral sciatic nerve cuffing (Cuff). PF was injected intraperitoneally once a day for a total of 14 days. Pain and depression-like behavior changes were evaluated via behavioral tests. Pathological changes in the hippocampus of mice were observed by H&E staining. The levels of proinflammatory cytokines in the hippocampus were detected using ELISA. Activated microglia were measured by immunohistochemical staining. The TLR4/NF-κB signaling pathway-associated protein expression in the hippocampus was detected by western blotting. We found that the PF could significantly alleviate Cuff-induced hyperalgesia and depressive behaviors, lessen the pathological damage to the hippocampal cell, reduce proinflammatory cytokines levels, and inhibit microglial over-activation. Furthermore, PF downregulated the expression levels of TLR4/NF-κB signaling pathway-related proteins in the hippocampus. These results indicate that PF is an effective drug for improving the comorbidity between NP and depression.
