류미티스관절염 (Rheumatoid Arthritis)

  • Anatomy: Advanced knowledges of cellular and molecular biology led to the development of therapies of rheumatoid arthritis(RA). Rheumatoid arthritis (RA) is a chronic, recurrent, systemic inflammatory disease and results in major deformity or dysfunction of joints. Etiology: Rheumatoid arthritis is now concevied as autoimmune disease. There have been many trials to define the immunological changes in rheumatoid arthritis. But now pathogenesis and significance of immunoglobulin complement and rheumatoid factor are not full accepted. Syndrome: Joints are characteristically involved with early inflammatory changes in the synovial membrane, peripheral portions of the articular cartilage, and lation tissue(pannus) forms, covers, and erodes the articular cartilage, bone and ligaments within the jiont capsule. Inflammatory changes also occur in tendon sheaths(tenosynovitis), and if subjected to a lot of friction, the tendons may fray or rupture. Extra-articular pathological changes sometimes occur, these include rheumatoid nodules, atrophy and fibrosis of muscles, and mild cardiac changes. Treatment: Tumor necrosis factor(TNF) inhibitor for the treatment of rheumatoid arthritis(RA) induces not only significant improvement of symptoms and signs of RA but also substantial inhibition of progressive joint damage.

  • Behçet's disease (BD) is an inflammatory systemic disorder with oral and genital ulcers, as well as ophthalmologic and cutaneous symptoms. Neurological manifestations in BD represent between 2.2% to 50% of the cases. The 25-year-old male patient, diagnosed with BD three years earlier, was admitted to our clinic with complaints of recurrent headaches. Tumor-like-parenchimal involvement was detected on a cranial magnetic resonance imaging. The lesion was removed surgically and then he suffered from right hemiparesis and epilepsy. Pathological examination of the lesion noted a demyelinating non-tumoural etiology. A neuro-Beh$\check{g}$et's case with parenchymal involvement has been examined in light of the literature, in terms of a tumor and a demyelinating disease differential diagnosis.

Fibrous Dysplasia of the Clivus

  • Fibrous dysplasia (FD) of craniofacial structures is well documented, however, its involvement of the clivus is seldom described. We report a case of clival FD in a young man who presented with headache localized to the occipital area. The radiological studies revealed a monostotic disease confined to the clivus, with typical findings of hypo intensity on magnetic resonance images and ground-glass density on computed tomography. The diagnosis of FD was confirmed on pathological examination of specimens taken through transsphenoidal surgery. The patient showed reduction of symptoms and no change of residual lesion on follow-up imaging taken 2.5 years later after surgery. This study includes clinical aspect, radiographic appearance, differential diagnosis and treatment strategy of this rare skull base lesion.

Occurrence of Anthracnose on Welsh Onion Caused by Colletotrichum circinans

  • Anthracnose occurred frequently on leaf sheaths of Welsh onions grown in Gangwha island, Korea in November, 2007. The disease incidence was as high as 30% in five fields investigated. A total of 20 single spore isolates of Colletotrichum species were obtained from the affected plants, and all the isolates were identified as Colletotrichum circinans based on their morphological and cultural characteristics. Three isolates of the fungus caused anthracnose symptoms on the leaf sheaths of Welsh onions by artificial inoculation, which were similar to those observed during the field survey. In this study, the mycological and pathological characteristics of C. circinans identified as causing anthracnose of Welsh onions are clarified.

  • A mixed infection of Haemophilus paragallinarum (Hpg), Mycoplasma gallisepticum (MG) and M. synoviae (Ms) was detected in layers of a poultry farm in Iwate prefecture in Japan by pathological, serological and bacteriological investigation. Hpg strains were isolated from three of five birds investigated and all strains were identified to be type C. The Hpg isolates were more susceptible in vitro to a combination of sulfamonomethoxine and ormetoprim (Ektecin) than each of sulfamethoxasol, sulfamonomethoxine, oxytetracycline, tetracycline, streptomycin, erythromycin and thianphinicol. After a total of six days' medication of 1% feed additive Ektecin, symptoms of infectious coryza of hens in the farm almost disappeared and no Hpg was detected even from birds showing nasal discharge.

Cerebellar Schistosomiasis: A Case Report with Clinical Analysis

  • The authors report here a rare case of cerebellar schistosomiasis identified by pathological diagnosis, lacking extracranial involvement. The clinical symptoms included headache, dizziness, and nausea. Studies in blood were normal and no parasite eggs were detected in stool. Computed tomography of brains showed hypodense signal, and magnetic resonance imaging showed isointense signal on T1-weighted images, hyperintense signal on T2-weighted images, and intensely enhancing nodules in the right cerebellum after intravenous administration of gadolinium. A high-grade glioma was suspected, and an operation was performed. The pathologic examination of the biopsy specimen revealed schistosomal granulomas scattered within the parenchyma of the cerebellum. The definitive diagnosis was cerebellar schistosomiasis japonica. A standard use of praziquantel and corticosteroid drugs was applied, and the prognosis was good. When the pattern of imaging examinations is present as mentioned above, a diagnosis of brain schistosomiasis should be considered.

폐 Aspergillosis 14 예 보고 (The clinical study of pulmonary aspergillosis -A report of 14 cases-)

  • 14 cases of pulmonary aspergillosis were managed surgically over a 6 year period in the Dept. of Thoracic Surgery Inje Medical College, Paik Hospital were reviewed. 1. Male, female ratio was 9:5 male predominant and 2,3,4 decade were prevalent. 2. The most prevalent chief complaint was hemoptysis, 9 cases [63%] and other symptoms were chronic productive cough 4 cases. 3. Preoperative diagnostic accuracy ratio was 0.21 [3 cases only]. 4. Right upper lobe was most common involving site, 7 cases [5[%], 13 lobectomy and 2 segmentectomy were performed. 5. Pathological underlying diseases were pulmonary tuberculosis 8 cases [57%], bronchiectasis and lung abscess was 1 case, no underlying disease were 4 cases. 6. One major postoperative complication was symptomatic residual dead space which was managed by thoracoplasty.

  • The studies were carried out to diagnose of disease chickens that were raised at the chicken farms located in Chung -nam province and evaluated potential of mold growth inhibition of antifungal agent, Mold-X. The diseased 25 chickens that were shown clinical sign such as anorexia, respiratory symptoms, were suspected aspergillosis. The results obtained were summarized as follows ; 1. The diseased thickens were diagnosed aspergillosis according to clinical sign, pathological finding, isolation of etiological agent. 2. The growth of mold was conciderable inhibited by the Mold-X of 300 ppm level.

  • Four Humboldt penguins were imported from overseas in March, 1975. Soon after arrival at the Zoological Garden of Chang Gyeong Won in Seoul, two penguins became ill with symptoms of severe dysnnoea, vomiting, diarrhea and mild convulsion. They died on the second day of illness. The main pathological finding was bronchitis which accompanied a caseous airsacculitis on the left lung. A massive melanin pigmentation in the intertubular tissue of the testis was found accidentally.

  • This study largely compared the general concept, examination, and treatment of various manual therapy techniques. These various manual therapy techniques, however, are still in the developing stage, so no one approach is yet perfect. Clinically, manual therapy techniques are widely being used to treat the dysfunctional neuromusculoskeletal with the common practice of mobilization, manipulation, exercise, and patient education. Anyone of the above approaches must not be selectively chosen as the best method. Manual therapists should treat patients in the clinic with the full knowledge of these proper manual therapy techniques depending on the patient's symptoms in each of the anatomical, biomechanical, and pathological views.
