Papillary Fibroelastoma of the Aortic Valve: Discovered by Chance with Intraoperative Transesophageal Echocardiography - A case report - (수술실 내 경식도 심장초음파검사에서 우연히 발견된 유두상 섬유탄력종 - 1예 보고 -)

  • Papillary fibroelastoma is a rare benign cardiac tumor with an elevated risk for embolization and most papillary fibroelastomas do not cause symptoms. In this report, we describe a case of previous undiagnosed masses of the aortic valve that were incidentally found on intraoperative transesophageal echocardiography during coronary artery bypass surgery. Upon surgery, masses were found on the left and right aortic cusps and the pathological findings were consistent with a papillary fibroelastoma.

The Effect of Total Radiation Dose on Normal Spinal Cord of Hybrid Mice -Early Pathological Changes- (총 방사선 조사량이 잡종 백색 마우스의 정상 척수에 미치는 영향)

  • Radiation myelitis is a rather rare, but irreversible fatal complication. Etiology, pathologic change, clinical symptoms and the method of diagnosis have been studied with animal experiments and human necropsies. In spite of massive studies, pathogenesis of post-irradiation myelitis and the level of tolerance dose still remain controversial. Thoracolumbar spine of 110 hybrid mice were irradiated with orthovoltage x-ray machine. Mild capillary congestion and axonal swelling were observed in 1,000 rad irradiated specimens. Focal necrosis in 3,500 rad specimens, fragmentation of neural tissue in 4,000 rad specimens were also observed. These results suggest that 5,000 rad is not a completely safe tolerable dose which have been accepted and we cannot exclude direct radiation damage to news tissue as the causative pathology of radiation myelitis in addition to blood vessel damage.

Partial nucleotide sequencing and phylogenetic analyses of Newcastle disease virus and infectious bursal disease virus isolated in South Korea

  • The present study was conducted to investigate the genetic profile of two prevalent avian pathogens in Korea namely, Newcastle disease virus (NDV) and infectious bursal disease virus (IBDV). Two farms located in Yeongi-gun, Chungnam were selected for this study. The two viruses were isolated from various organs (spleen, trachea, bursa of Fabricius) of deceased chickens that showed clinical symptoms of Newcastle Disease or Infectious bursal disease like swelling and congestion of the F bursa, facial edema, lacrimation, greenish yellow diarrhea as well as pathological signs like airsacculitis, haemorrhages in the intestines and so on. For analysis of NDV and IBDV, a 466 and 435 base pair fragments corresponding to the HN and VP2 regions which are highly conserved among related strains of NDV and IBDV, respectively, were amplified by RT-PCR and analyzed by sequencing. Comparison of the VP2 region showed a $99.3\%$ homology between the Korean IBDV isolate and the BJ836-attenuated vaccine strain. In contrast, the HN region of the Korean NDV isolate only has an 83 to $84\%$ homology with the vaccine strains LaSota, B1 and VGGA. Our findings reveal that the prevalent NDV strain in Korea is genetically different from the vaccine strains and may explain the recent outbreaks of Newcastle disease in the region.

  • The aim of this study was to suggest the standard process in developing Questionnaire of Pattern Identification (QPI). The process in developing QPI was researched from validated and developed questionnaire and the standard process in developing QPI was suggested through review of the experts in research, statistics and clinics. Check list was also provided. The number of QPI reviewed in this research was 17(4 in disease in Korea Medicine, 5 in Pathological symptoms, 6 in Sasang constitutional Diagnosis, and 2 in etc), The standard process in developing QPI consisted of 11 phage and 33 check lists. 1) Composition of Research Member(3check lists), 2)Set up of the Aim(5), 3) Review for advanced research(3), 4) Finding an Important Index(3), 5) Review of item selection(4), 6) Developing the questions using items(5), 7) Developing Draft of Questionnaire(2), 8) 1st Survey of Reliability and Validity(2), 9) Revision and Correction of Item(1), 10) 2st Survey of Reliability and Validity(2), 11) Completion and Application(3). This study suggests the standard process in developing QPI for the first time in Korea. This following step may help A new QPI development.

Characterization of Tunicamycin as Anti-obesity Agent

  • Adipocytes undergo adipocyte stress in the excessive presence of lipid. Adipocyte stress accompanies the typical signs of endoplasmic reticulum (ER) stress: unfolded protein response and overexpression of molecular chaperones. Apoptotic induction in adipocytes is known as a good strategy for treating obesity. The drug "tunicamycin" was tested for its therapeutic potential in inducing apoptosis on differentiating adipocytes of 3T3-L1. When the 3T3-L1 cells, stimulated for adipogenesis, were treated with tunicamycin, they showed typical ER stress symptoms. Despite progression in ER stress, however, the differentiated 3T3-L1 hardly proceeded to apoptosis based on the CHOP protein expression and FACS analysis. This is very different from C2C12, the myogenic counterpart of 3T3-L1, which showed significant apoptosis along with ER stress. This study also characterizes a potential mechanism whereby adipocyte may avoid apoptosis to sustain the pathological state of obesity. The level of GRP94 expression significantly upholds in 3T3-L1 under tunicamycin treatment compared to preadipocytes and C2C-12. When GRP94 expression was inhibited by siRNA, 3T3-L1 showed a higher level of CHOP expression compared to C2C12 cells. In conclusion, adipocytes exert an anti-apoptotic mechanism under ER stress caused by tunicamycin; thus, apoptotic induction in adipocyte is not a viable anti-obesity option. The unusual level of GRP94 may serve as a key role whereby adipocytes reach to the obesity level circumventing the apoptosis.

A Study of the Peripheral Neuropathy among the Workers Exposed to Carbon Disulfide (이황화탄소에 폭로된 근로자들의 말초신경병증에 관한 연구)

  • Neurotoxicity in the workplace may occur with exposure to scores of chemicals. Although large acute outbreaks of the occupational neurological disease are rare, the incidence of occupational neurotoxicity in its subtler aspects is unknown. A working knowledge of both the major occupational neurotoxic solvents and the tools used by cliniical neurologists and neurotoxicologists to evaluate neurotoxicity in working populations is a necessity fur the occupational physician. To investigate the effects of carbon disulfide($CS_2$) on the peripheral nerve system using the nervous conduction study, 105 male workers working in the spinning room of a viscose rayon factory were examined and compared with a sex and age matched, unexposed 105 male controls using t-test analysis. 72.4% of $CS_2$-exposed workers complained of neurological symptoms, and the abnormal cases in nerve conduction study were 48.6%. The abnormal cases of nerve conduction study increased in number according as the age and duration of exposure increased. In this study, asymptomatic workers were confirmed to have subclinical neuropathy by nerve conduction study. Also as there were abnormal cases even in its duration of exposure below 4 years, nerve conduction study turned out to be ways of discovering of early peripheral neuropathy. In nerve conduction study, the amplitude, velocity, F-wave latency and H-reflex of the motor and sensory nerves in both upper and lower extremities were significant different between $CS_2$-exposed workers and the controls. From the pathological viewpoint, both segmental and axonal degenerations were assumed in this study.

Novalis Radiosurgery of Primary Central Nervous System Lymphoma in Elderly Patients : Preliminary Results

  • Objective : Primary central nervous system lymphoma[PCNSL] is a non-Hodgkin's lymphoma arising in the central nervous system. Combined chemotherapy and radiation therapy is the standard treatment for PCNSL. However, treatment induced neurotoxicity is a major problem especially in elderly patients. Methods : From May 2001 to April 2004, elderly five patients with PCNSL confirmed via pathological examination were underwent Novalis radiosurgery in authors' institution, who were investigated retrospectively. Of these patients, 2 were male and 3 were female, with a mean age 68 years old [range $65{\sim}73$]. The number of lesions was 1 in 2 patients, 2 in 2, and 3 in 1. The mean follow up period was 12.6 months [range $8{\sim}16$]. Results : Clinical symptoms and signs in all patients markedly improved within 1 weeks after Novalis radiosurgery. The Karnofsky performance status score was also improved from a pre-radiosurgery average of 68 to a post-radiosurgery one of 82. All of treated lesions showed a partial or complete regression of the original mass. There have been no complication following Novalis radiosurgery. Conclusion : The preliminary results of our experience indicate that radiosurgery with Novalis provides a safe and effective therapeutic alternative treatment of PCNSL in elderly patients.

A Case Report of Integrative Korean Medicine Treatment of a Patient with a Compression Fracture Arising from Metastasis of Prostate Cancer to the Spine (전립선암의 척추전이로 인해 압박골절이 발생한 환자에 대한 한방처치 1례)

  • Objective: The aim of this study was to report the effects of traditional Korean medicine as a treatment for the pain of a compression fracture in a patient with metastatic prostate cancer. Methods: A 60-year-old male patient with a compression fracture was treated with herbal medicines, acupuncture, and pharmacopuncture for 25 days. His chief complaints were severe low back pain, pain and numbness in both legs, and right groin pain. The effect of treatment was evaluated by the numerical rating scale (NRS) score for pain, the Oswestry Disability Index (ODI) score, and blood test results (CBC-diff count, CRP, ESR, etc.). Results: After 25 days of inpatient treatment, the patient's pain was controlled. The NRS and ODI scores were lower and most pathological symptoms had decreased. Conclusions: Traditional Korean medicine can be a solution for patients with compression fractures arising from metastasis of prostate cancer to the spine.

The Agreement in Cold-Heat and Health Status among Sasang Constitutional Experts in Diagnosis of Sasang Pathological Symptoms (사상의학 병증진단에 있어서 사상의학 전문가의 한열 특성 및 건강상태 평가에 관한 일치도 연구)

  • Objectives In Sasang Constitutional medicine(SCM), it is an important factor to assess the degree of cold/heat and healthy status. We purposed to check the agreement among the experts on the assessment of cold/heat and healthy status. Methods Six SCM experts and 10 healthy volunteers were recruited for this study. The SCM experts had an interviewed with each volunteer in the similar condition like that of the actual clinic. And then the expert evaluated each volunteer's cold/heat and healthy status. Intraclass correlation coefficient(ICC) was used to measure the interrater reliability and interrater agreement among the experts. Results Two volunteers are excluded due to the missing data, therefore eight persons' data (3 males and 5 females) were included in our analysis. The mean of age and BMI are 21.73(${\pm}2.32$) and 29.63(${\pm}4.66$), respectively. In health status, cold status and heat status, the ICC was 0.789, 0.904, 0.925, respectively. Conclusions We found that the assessment of cold/heat and healthy status was continued reliable among SCM experts. The assessment of cold/heat status was more consistent than the assessment of healthy status.

Long-term Retrospective Clinical Study Comparing Submerged Type with External Hex Connection and Non-submerged Type with Internal Morse Taper Connection Implants

  • Purpose: This study was aimed to compare the survival and success rates, and long-term crestal bone loss according to the use of 2 connection types of dental implants (submerged-USII and non-submerged-SSII; Osstem $Implant^{(R)}$) by analyzing the change in alveolar bone height after 1 year under load and during final follow-up period. Materials and Methods: Between December 2004 and August 2008, patients with two types of Osstem implants (USII and SSII) were retrieved retrospectively. A total of 92 patients with 284 implants (USII=60, SSII=224) was finally selected. Their mean follow-up period was 7.5 years. The mesial and distal alveolar crestal bone changes were measured using radiographic images and the average was calculated at 1 year after loading and during final follow-up period. Result: Among the 284 implants, 4 USII and 7 SSII implants were removed, indicating 93.3% and 96.9% survival rates. Of the survived implants, mean crestal bone loss 1 year after loading was 0.39 mm for USII and 0.19 mm for SSII (P=0.018). During the final follow-up, mean crestal bone loss was 0.63 mm and 0.35 mm for USII and SSII, respectively, without statistical significance (P=0.092). According to the criteria for the success and failure of the implant by Albreksson and colleagues, final success rate was estimated as 86.7% for USII and 91.5% for SSII, respectively. Conclusion At 1 year after loading, the average crestal bone loss was significantly different between USII and SSII; however, both types met the criteria for implant success. During the final follow-up, both groups showed insignificant bone resorption patterns and did not show any pathological clinical symptoms. Therefore, both implants exhibited high long-term stability.
