수근관증후군 : 자기공명영상과신경전도검사의 상관 관계 (Carpal Tunnel Syndrome : Correlation between Magnetic Resonance Imaging and Nerve Conduction Study)

  • Purpose : Carpal tunnel syndrome (CTS) is a disorder of median nerve at wrist. It is usually diagnosed through clinical manifestation and nerve conduction study (NCS). However, sometimes, NCS does not provide a reliable evidence to reach the diagnosis. Thus, authors performed this study to determine whether NCS was correlated with specific parameters measured on magnetic resonance imaging (MRI) which might become a potential complemental diagnostic tool. Methods : We performed MRI in 34 wrists of 18 patients with clinical manifestations of CTS and pathologic nerve conduction values and analyzed them at levels of the distal radioulnar joint, pisiform and hook of hamate, Results : Increase in the cross-sectional area of the median nerve at the pisiform level and flattening, increased signal intensity, and contrast enhancement of the median nerve at levels of the pisiform and hook of hamate were statistically significant. Change in cross sectional areas between the distal radioulnar joint and hamate and the signal intensities at levels of pisiform and hamate were well correlated with the median nerve conduction velocity. Conclusions : Characteristic MRI findings in CTS reported previously were well demonstrated and some of MRI parameters are well correlated with nerve conduction study. MRI, despite cost, may help in evaluating CTS.

성대구증의 형태에 따른 음향학적 분석 및 치료 결과 (Voice Analysis and Treatment Result According to Configuration of Sulcus Vocalis)

  • Background and Objectives : Sulcus vocalis could be classified into type I, type IIa, and type IIb. There have been a little reports about voice quality and treatment results related with types of sulcus vocalis. The authors conducted an analysis of voice and treatment according to different types of sulcus vocalis. Materials and Methods : This study was based on a retrospective chart review. The sulcus types were classified into type I and type II. Objective and subjective voice assessments were analyzed. Patients were treated individually with voice therapy, percutaneous steroid injection, and injection laryngoplasty. Comparison was performed on the voice difference between type I group and type II group, and between pre-treatment and post-treatment of each types. Results : One hundred and one patients were enrolled into this study, and 49 patients were type I and 52 patients were type II. Type I group showed longer mean maximal phonation time (MPT) than type II group, although other voice parameters didn't show any difference between two groups. Even after the management, almost all of the voice parameters didn't show improvement except MPT of type II group. Conclusion:Although the type I sulcus has been known as a non-pathologic lesion, it can result in some degree of voice change and discomfort, and thus need an active management. In this study, voice therapy, percutaneous steroid injection, and injection laryngoplasty showed limited effect to the both types of sulcus vocalis. Further studies for management of sulcus vocalis were needed.

종자골 절제 후 족무지 관절의 운동 변화 (Range of Motion of Great Toe after Sesamoidectomy: A Cadaveric Study)

  • Purpose: Regardless of potential and actual complications, the sesamoidectomy either tibial side or fibular side or both, had been used as a surgical option for various pathologic conditions. The objective of this cadaveric study was to identify the changes of range of motion of great toe after sesamoidectomy. Material and Methods: Eight fresh cadaver legs were used. The angular changes of the hallucal articulations were measured by traction of the flexor hallucis longus tendon at the proximal border of fibro-osseous tarsal tunnel and by traction of the extensor hallucis longus tendon at the superior border of inferior extensor retinaculum. The measurement started at neutral position and proceeded to the maximum for respective tendons. After sesamoidectomy either partial or total, same procedures were repeated and the angular changes were measured. Results: In flexion of great toe, there were significant metatarsophalangeal angular differences at 1 cm traction in total sesamoidectomy and lateral sesamoidectomy. In extension of great toe, there were significant metatarsophalangeal angular differences at more than 2 cm traction in total sesamoidectomy. In other measurements, there were no significant angular changes of the hallucal articulations. Conclusion: The sesamoidectomy resulted in change of motion of great toe. Statistical analysis showed that the significant increases in the initial flexion and maximal extension occurred with total sesamoidectomy and the significant increase in the initial flexion occurred with lateral sesamoidectomy.

Cognitive Dysfunction and Hippocampal Damage Induced by Hypoxic-Ischemic Brain Injury and Prolonged Febrile Convulsions in Immature Rats

  • Objective : Perinatal hypoxic-ischemic encephalopathy (HIE) and prolonged febrile seizures (pFS) are common neurologic problems that occur during childhood. However, there is insufficient evidence from experimental studies to conclude that pFS directly induces hippocampal injury. We studied cognitive function and histological changes in a rat model and investigated which among pFS, HIE, or a dual pathologic effect is most detrimental to the health of children. Methods : A rat model of HIE at postnatal day (PD) 7 and a pFS model at PD10 were used. Behavioral and cognitive functions were investigated by means of weekly open field tests from postnatal week (PW) 3 to PW7, and by daily testing with the Morris water maze test at PW8. Pathological changes in the hippocampus were observed in the control, pFS, HIE, and HIE+pFS groups at PW9. Results : The HIE priming group showed a seizure-prone state. The Morris water maze test revealed a decline in cognitive function in the HIE and HIE+pFS groups compared with the pFS and control groups. Additionally, the HIE and HIE+pFS groups showed significant hippocampal neuronal damage, astrogliosis, and volume loss, after maturation. The pFS alone induced minimal hippocampal neuronal damage without astrogliosis or volume loss. Conclusion : Our findings suggest that pFS alone causes no considerable memory or behavioral impairment, or cellular change. In contrast, HIE results in lasting memory impairment and neuronal damage, gliosis, and tissue loss. These findings may contribute to the understanding of the developing brain concerning conditions caused by HIE or pFS.

Intradural Extramedullary and Subcutaneous Tumors in Neonate : Atypical Myxoid Spindle Cell Neoplasm

  • Tumors of the central nervous system are common in the pediatric population and constitute the second most prevalent tumor type in children. Within this group, spinal cord tumors are relatively rare and account for 1 to 10% of all pediatric central nervous system tumors. We describe a very rare case of an intradural extramedullary spinal cord tumor with a subcutaneous mass and discuss its clinical presentation, pathogenesis, and treatment. A male infant was delivered normally, with uneventful development. At 16 days post-delivery, his family took him to a pediatrician because of a mass on his upper back. Magnetic resonance imaging of the thoracic spine revealed a well-demarcated soft-tissue mass with central cystic change or necrosis at the subcutaneous layer of the posterior back (T2-7 level). Another mass was found with a fat component at the spinal canal of the T1-3 level, which was intradural extramedullary space. After six weeks, the spinal cord tumor and subcutaneous mass were grossly total resected; pathologic findings indicated an atypical myxoid spindle cell neoplasm, possibly nerve sheath in type. The final diagnosis of the mass was an atypical myxoid spindle cell neoplasm. The postoperative course was uneventful, and the patient was discharged after nine days without any neurological deficit. We report a rare case of an intradural extramedullary spinal tumor with subcutaneous mass in a neonate. It is necessary to monitor the patient's status by examining consecutive radiologic images, and the symptoms and neurological changes should be observed strictly during long-term follow-up.

The efficacy of aspartate aminotransferase-to-platelet ratio index for assessing hepatic fibrosis in childhood nonalcoholic steatohepatitis for medical practice

  • Purpose: Childhood obesity is associated with nonalcoholic fatty liver disease (NAFLD), and it has become one of the most common causes of childhood chronic liver diseases which significant as a cause of liver related mortality and morbidity in children in the United States. The development of simpler and easier clinical indices for medical practice is needed to identify advanced hepatic fibrosis in childhood NAFLD instead of invasive method like liver biopsy. FibroScan and aspartate aminotransferase (AST)-to-platelet ratio index (APRI) have been proposed as a simple and noninvasive predictor to evaluate hepatic fibrosis in several liver diseases. APRI could be a good alternative to detect pathologic change in childhood NAFLD. The purpose of this study is to validate the efficacy of APRI for assessing hepatic fibrosis in childhood NAFLD based on FibroScan. Methods: This study included 23 children with NAFLD who underwent FibroScan. Clinical, laboratory and radiological evaluation including APRI was performed. To confirm the result of this study, 6 patients received liver biopsy. Results: Factors associated with hepatic fibrosis (stiffness measurement >5.9 kPa Fibroscan) were triglyceride, AST, alanine aminotransferase, platelet count, APRI and collagen IV. In multivariate analysis, APRI were correlated with hepatic fibrosis (>5.9 kPa). In receiver operating characteristics curve, APRI of meaningful fibrosis (cutoff value, 0.4669; area under the receiver operating characteristics, 0.875) presented sensitivity of 94%, specificity of 66%, positive predictive value of 94%, and negative predictive value of 64%. Conclusion: APRI might be a noninvasive, simple, and readily available method for medical practice to predict hepatic fibrosis of childhood NAFLD.

UVB조사기의 홍반 발생 특성과 안전성에 관한 연구 (The Study of Characteristic of Induced Erythema and Safety by UVB Lamp)

  • Purpose: The present study purposed to examine induced characteristic or erythema and safety by medium wave ultra violet(UVB) lamp. Methods: We compared sunshine and UVB lamp using spectroradiometer and UV radiometer. For measuring sunshine irradiation, we used spectoradiometer and detected from 8 to 18 o'clock every each hour on the beach, playground and rooftop of a 5 story building. The subjects for erythema examination were 5 healthy subjects who have no pathologic history of photosensitivity reaction, psoriasis and vitiligo. They were exposed to UVB radiation at the abdominal area for 2 hours and after irradiation, we observed the change of skin color every 12 hours over a period of 1 week. Results: Between sunshine and UVB lamp, sunshine had higher data on the chromaticity coordinates, dominant and peak wavelength, bandwidth and purity than the UVB lamp but on the color temperature, brightness the UVB lamp had higher data than the sunshine. In comparison of sunshine and UVB lamp, UVB lamp irradiated constantly such as $3.9-4.4{\mu}W/cm^2$ at a distance of 100cm between bed and lamp which was same as early morning irradiation on the sunshine. The erythema didn't appear to any subject. Conclusion: This results suggest that the UVB lamp has lower irradiance as much as early sunshine. Therefore the UVB lamp had no influence of inducing erythema at a distance of 100cm between bed and lamp.

대하치료(帶下治療)에 사용(使用)되는 온이약(溫裏藥)이 질내(膣內) 미생물(微生物)에 미치는 영향(影響) (Antimicrobial effects of the interior warming herbs on vaginal microbe)

  • Purpose: This study was conducted to investigate the antimicrobial effects of the interior warming herbs on vaginal microbes. Methods: Staphylococcus aureus, Methicillin-resistant Staphylococcus aureus, Candida albicans and Gardnerella vaginalis were used for vaginitis-induced microbes. Lactobacillus gasseri, Streptococcus spp. and Escherichia coli HB101 were used for normal vaginal florae. And herbs for warming the interior (Zingiberis Rhizoma, Alpiniae Officinari Rhizoma, Aconiti Tuber, Anethi Fructus, Evodiae Fructus, Cinnamomi Cortex Spissus, Caryophylli Flos, Aconiti Tube, Zanthoxyli Pericarpium, Piperis Longi Fructus, Piperis Nigri Fructus) were used in this study. Antimicrobial activities were tested by the change of optical densities (OD) and colony test in vitro. Results: In the results of vaginitis-induced microbes, Anethi Fructus was decreased the OD values on MRSA and Gardnerella vaginalis and Aconiti Tuber was decreased on MRSA. There were no viable MRSA and Gardnerella vaginalis colony forming against Evodiae Fructus, Staphylococcus aureus and MRSA colony forming against Piperis Longi Fructus, Staphylococcus aureus colony forming against Piperis Nigri Fructus and MRSA colony forming against Zanthoxyli Pericarpium. In the results of normal vaginal florae, Zingiberis Rhizoma was decreased the OD values on Streptococcus spp. and all normal vaginal florae were showed viable colony forming against all experimental herbs. Conclusion: According to these results, we can suggest that some kinds of interior warming herbs have antimicrobial effects on vaginal microbes. So there might be needed to make furthermore studies to seek the herbs which have selective antimicrobial effect on pathologic vaginal microbes.

PET/CT planning during chemoradiotherapy for esophageal cancer

  • Purpose: To evaluate the usefulness of positron emission tomography/computed tomography (PET/CT) for field modification during radiotherapy in esophageal cancer. Materials and Methods: We conducted a retrospective study on 33 patients that underwent chemoradiotherapy (CRT). Pathologic findings were squamous cell carcinoma in 32 patients and adenocarcinoma in 1 patient. All patients underwent PET/CT scans before and during CRT (after receiving 40 Gy and before a 20 Gy boost dose). Response evaluation was determined by PET/CT using metabolic tumor volume (MTV), total glycolytic activity (TGA), MTV ratio (rMTV) and TGA ratio (rTGA), or determined by CT. rMTV and rTGA were reduction ratio of MTV and TGA between before and during CRT, respectively. Results: Significant decreases in MTV ($MTV_{2.5}$: mean 70.09%, p < 0.001) and TGA ($TGAV_{2.5}$: mean 79.08%, p < 0.001) were found between before and during CRT. Median $rMTV_{2.5}$ was 0.299 (range, 0 to 0.98) and median $rTGAV_{2.5}$ was 0.209 (range, 0 to 0.92). During CRT, PET/CT detected newly developed distant metastasis in 1 patient, and this resulted in a treatment strategy change. At a median 4 months (range, 0 to 12 months) after completion of CRT, 8 patients (24.2%) achieved clinically complete response, 11 (33.3%) partial response, 5 (15.2%) stable disease, and 9 (27.3%) disease progression. $SUV_{max}$ (p = 0.029), $rMTV_{50%}$ (p = 0.016), $rMTV_{75%}$ (p = 0.023) on intra-treatment PET were found to correlate with complete clinical response. Conclusion: PET/CT during CRT can provide additional information useful for radiotherapy planning and offer the potential for tumor response evaluation during CRT. $rMTV_{50%}$ during CRT was found to be a useful predictor of clinical response.
