Monitoring of Methicillin-resistant Staphylococcus aureus in Nasal Swabs Obtained from Dental Clinic Healthcare Providers and Medical Environment Nurses

  • The aims of this study were to investigate the nosocomial infection route of methicillin-resistant Staphylococcus aureus (MRSA) and explore preventative methods for this pathogen that involve blocking its dispersion. We cultured MRSA from nasal cavity swabs collected between June and July 2008 that we obtained from eight dental healthcare providers, 32 nurses and the sputum specimens of two patients from our hospital. In addition, we used VITEK 2 equipment to measure drug sensitivity, and we further performed biochemical testing and pulse-field gel electrophoresis (PFGE) to isolate MRSA colonies. The incidence of these bacteria on the nasal swabs was 25.0% from dental clinic healthcare providers, 13.6% from the internal medicine ward nurses and 30.0% from intensive care unit nurses. Moreover, MRSA was detectable in sputum specimens of ward patients. The antimicrobial agents resistance and partial PFGE types of MRSA showed a similar pattern. We suggest from these analyses that nasal cavity infection by MRSA could occur by cross contamination between healthcare providers and patients which underscores the importance of stringent MRSA management practices.

  • Dental implant restoration in partial or full edentulous state has become the standard treatment in recent years. Bone graft with guided bone regeneration technique has been regarded as one of the most reliable methods to restore the bone defect area due to periodontal disease or dental trauma. Bone graft materials and membrane are the essential component of guided bone regeneration; however, a variety of bone graft materials confuse us in implant dentistry. Autogenous bone is the recognized standards in implant dentistry owing to its osteogenesis potential. Despite of its disadvantages, grafting autogenous bone is the most reliable methods. Even though the development of new bone grafts materials, autogenous bone is useful in exposed implant thread and total lack of buccal or lingual bone. Allogenic, xenogenic and synthetic bone have the osteoconductive and osteoinductive potential. These materials could be used successfully in self-contained cavity such as sinus cavity and three-wall defects. In this article, application of bone graft material is suggested according to the function of bone graft materials.

  • A 64-year-old male was admitted due to abruptly developed, severe dyspnea via local clinic. He had been a heavy smoker and alcoholic for a long time. Chest PA showed huge haziness in right upper lung field. Sputum culture for bacteriology was positive for Klebsiella pneumoniae. Immediately, appropriate antibiotics were administered and artificial ventilation was started. On 40th hospital day, simple chest roentgenogram taken due to sudden aggravated dyspnea showed marked hyperlucency in right upper lung field, suggestive of rupture of abscess cavity and resultant pneumothorax. At that time, chest tube was inserted but air leakage from the chest tube persisted. Chest CT scan taken after chest tube insertion showed the tube inserted into a thin-walled cavity in the above lesion. on 84th hospital day, right upper lobectomy with decortication was performed. Pathologically, cavittary lung abscess was diagnosed on the findings of partial re-epithelialization of ciliated columnar epithelium with severe pulmonary vascular occlusion and extensive fibrous pleural adhesions.

Verrucous carcinoma arising from a previous cystic lesion: a case report

  • Background: Verrucous carcinoma (VC) accounts for 1-10% of cases of squamous cell carcinoma (SCC) in the oral cavity, and 75% of VC occur in the oral cavity. Only 3% of primary intraosseous squamous cell carcinomas (PIOSCC), which means SCC occurring primarily in the bone, are VC. Verrucous carcinoma arising from odontogenic cysts (OC) is very rare, with only seven cases reported to date. Case presentation: This study reported a case of a patient who underwent partial maxillectomy and neck dissection for VC that occurred in the right anterior maxilla. The patient was admitted to the emergency department at our institution 8 years ago and showed cystic lesions in the anterior maxilla on facial computed tomography (CT) images. Treatment through other departments including assessment of laceration in the mental region and only suture was performed. This report highlights a very rare case of VC in the right anterior maxilla arising from a previous cystic lesion. Conclusions: Since PIOSCC can arise from OC, appropriate treatment of intraosseous cysts and regular radiologic evaluation are necesssary. Surgical exicision of the primary lesion without neck dissection can lead to good prognosis for patients with primary intraosseous verrucous carcinoma.

Design of a Variable Resonator for the Sacred Bell of the Great King Seongdeok (성덕대왕신종을 위한 가변형 명동의 설계)

  • This study proposes a design model of the variable type resonator which corrects the temperature variance according to the season, in order to maximize the resonance effect in the Sacred bell of the Great King Seongdeok. In the bell, the 1st natural frequency (64 Hz) and the 2nd natural frequency (168 Hz) are the most important partial tones. Resonance conditions of the two components are determined for the internal acoustic cavity system, which consists of bell body cavity, gap and the resonator. Acoustic frequency response characteristics of the internal cavity are determined by the boundary element analysis using SYSNOISE. As an external factor, temperature variance according to the season largely influences the resonance condition and the length of the resonator should be controlled to maximize the resonance effect. As a measure, this study proposes a design model of the variable type resonator for the Sacred Bell of the Great King Seongdeok, which can control the length at the belfry according to the season.

  • Based on the recently developed resonance scattering theory for elastic waves, a relationship between the stress components, which may be measured using ultrasonic transducers, of partial waves scattered from cylindrical fluid scatterer, cavity, and resonance scatterer has been derived. The computed resonance scattered stresses exhibit frequency behaviors similar to the corresponding scattering coefficients: particularly, abrupt changes in phase by 180°near the resonant frequencies. By studying the behavior of pressure in the fluid scatterer, the physics of the theory has been further understood. Using the method studied and developed in this paper, nondestructive characterization of fluid inclusions in elastic media is expected to become more reliable.

  • Partial discharges(PD)in air insulated electric power apparatus often lead to deterioration of solid insulation by electron bombardments and electrochemical reaction. The PD caused to reduce the life time of power apparatus and to increase power losses. Thus understanding and classification of PD patterns in air are very important to discern sources of PD. In this paper, PD in air by using statistical methods was investigated. We classified air discharges, corona, surface discharges and cavity discharges by Kohonen network. For classification of PD patterns, we used statistical operators and parameters such as skewness$(S^+,\; S^-),\; kurtosis(K^+, K^-),\; mean phase(AP^+, AP^-)$, cross-correlation factor(CC) and asymmetry derived from the mean pulse-height phase distribution$(H_{avg}(\phi))$, the max pulse-height phase distribution $(H_{qmax}(\phi))$, the pulse count phase distribution $(H_n(\phi))$ and the pulse height vs. Repetition rate $(H_q(n))$.

  • Partial discharge(PD) in air insulated electric power systems is responsible for considerable power lossesfrom high voltage transmission lines. PD in air often leads to deterioration of insulation by the combined action of the discharge ions bombarding the surface and the action of chemical compounds that are formed by the discharge and may give rise to interference in ommunication systems. PD can indicate incipient failure. Thus understanding and classification of PD in air is very important to discern source of PD. In this paper, we investigated PD in air by using statical method. We classified air discharge with corona, surface discharge and cavity discharge by source of discharge. we used the mean pulse-height phase distribution $H_{qmean}(\psi)$, the max pulse-height phase distribution $H_{qmax}(\psi)$ , the pulse count phase distribution $H_n(\psi)$ and the max pulse height vs. repetition rate $H_{q}(n)$ for analysis PD pattern. We used statistical operators, such as skewness(S+. S-1, kurtosis(K+, K-), mean phase(AP+. AP-), cross-correlation factor(CC) and asymmetry from the distribution.

A Study on the Relationships of Oral Status and Self-perceived Oral Health Status in Elderly (일부 노인의 구강내 상태와 자가인지 구강건강상태와의 관련성)

  • Purpose: The recent proliferation of the aging population, oral health care is closely related to the mental health of the elderly is very important being recognized. Provide basic data for improving the quality of life for the elderly, to determine whether the oral health of the elderly whether oral self-perception of health and any related wish. Methods: Surveys to May 20 from April 15, 2011, subjects were elderly and living in Daejeon and Chungnam 277 people were examined. All the statistics using SPSS 18.0 for the determination of statistical significance, and the significance level was 0.05. Results: 1. Recognized, the most common oral health, self-aware bad the state (46.2%) there was no significant difference in the general characteristics according to self-Oral health status, remaining natural dimensions or the intra-oral prosthetic, there was no significant difference. More teeth, residual phase, in accordance with intra-oral prosthetic mandibular Not all dentures, partial dentures, full dentures net self-perception of oral health status better recognized. 2. According to the type of partial dentures, oral health status self-phase, mixed mandibular removable, fixed, fixed, removable partial dentures in order to recognize that good oral health, self-aware state, showed between them, there was no significant difference. 3. Effects of Loess, Rhodobacter, and Bacillus Mixture on the Trees Cavity Treatment (수목 공동치료를 위한 황토, Rhodobacter, Bacillus 조합의 효과)

  • The symptoms such as weaken shape, partial and full death in nurse- and old- trees have been mostly caused by the occurrence of cavity. In general, the adaption of poly-urethane has been practised routinely in the way of treatment in cavity. However, the use of poly-urethane was not an effective, but it was related to pollution materials. Here, the experiment was conducted to identify the effects of the treatments of loess or micro-organisms for the alternatives of poly-urethane. The cavities which occurred in Zelkova serrata, Prunus yedoensis and Quercus myrsinaefolia were treated by poly-urethane, loess, mixtured loess and micro-organism (Rhadobacter), and mixtured loess and micro-organism (Rhodobacter + bacillus). As the results of this experiment, it was shown that the treatment of mixtured loess and micro-organism (Rhodobacter + bacillus) induced to develop greater callus formation in the cavity up to 97% in Zelkova serrata, up to 87% in Prunus yedoensis, and by 73% in Quercus myrsinaefolia than the treatment of poly-urethane. But the only loess and mixtured loess and micro-organism (Rhadobacter) was a similar or lower callus formation ratio than poly-urethane.
