Evaluation of Gait Assistive Devices in Patients with Parkinson's Disease

  • Objective: There are no guidelines for choosing appropriate gait assistive devices. The aim of this study was to evaluate gait assistive devices in patients with Parkinson's disease. Method: We evaluated 15 individuals with Parkinson's disease who did or did not use one of two different devices including canes and two-wheeled walkers. Data were collected using the GAITRite system. Results: Participants in the group using canes and two-wheeled walkers had significantly increased double support time and decreased gait velocity, normalized gait velocity, and stride length compared with those who did not. Participants who used a two-wheeled walker had significantly decreased gait velocity, normalized gait velocity, and stride length compared with those who used a cane. Furthermore, participants who used a two-wheeled walker had significantly decreased coefficients of variation for step time, stride length, and swing time compared with those who used a cane. Conclusion: Our results indicated that the two-wheeled walker offered the most consistent advantages for decreasing the risk of falling.

  • In the last decades patient's suffering with Parkinson's disease is increasing at a rapid rate and as per prediction it will grow more rapidly as old age population is increasing at a rapid rate through out the world. As the performance of wearable sensor based approach reached to a new height as well as powerful machine learning technique provides more accurate result these combination has been widely used for assessment of various neurological diseases. ON state is the state where the effect of medicine is present and OFF state the effect of medicine is reduced or not present at all. Classification of ON/OFF state for the Parkinson's disease is important because the patients could injure them self due to freezing of gait and gait related problems in the OFF state. in this paper wearable sensor based approach has been used to collect the data in ON and OFF state and machine learning techniques are used to automate the classification based on the gait pattern. Supervised machine learning techniques able to provide 97.6% accuracy while classifying the ON/OFF state.

Function and dysfunction of leucine-rich repeat kinase 2 (LRRK2): Parkinson's disease and beyond

  • Mutations in leucine-rich repeat kinase 2 (LRRK2) are the most common cause of familial Parkinson's disease (PD). As such, functions and dysfunctions of LRRK2 in PD have been the subject of extensive investigation. In addition to PD, increasing evidence is suggesting that LRRK2 is associated with a wide range of diseases. Genome-wide association studies have implicated LRRK2 in Crohn's disease (CD) and leprosy, and the carriers with pathogenic mutations of LRRK2 show increased risk to develop particular types of cancer. LRRK2 mutations are rarely found in Alzheimer's disease (AD), but LRRK2 might play a part in tauopathies. The association of LRRK2 with the pathogenesis of apparently unrelated diseases remains enigmatic, but it might be related to the yet unknown diverse functions of LRRK2. Here, we reviewed current knowledge on the link between LRRK2 and several diseases, including PD, AD, CD, leprosy, and cancer, and discussed the possibility of targeting LRRK2 in such diseases. 뇌 자기공명영상에서 대뇌 피질 두께 분석법을 이용한 파킨슨병의 경도인지장애 신경심리검사와 신경해부학적 상관관계 (Correlation of the Neuropsychological Screening Battery (NSB) and Neuroanatomy for the Parkinson's Disease with Mild Cognitive Impairment by Using the Analysis of Cerebral Cortex Thickness in the Brain MRI)

  • 본 연구는 경도인지장애가 동반된 환자를 대상으로 뇌 자기공명영상에서 인지 영역에 대한 대뇌 피질 두께를 측정하고 신경심리검사 결과와 비교하여 신경해부학적 상관관계를 규명하고자 하였다. 이를 위하여 파킨슨병으로 최초 진단 받고 신경심리검사를 시행한 78명(경도인지장애군: 39명; 비인지장애군: 39명)과 정상인 그룹 32명을 선정하였다. 신경심리검사에서 경도인지장애군과 비인지장애군의 상관관계와 신경심리검사와 뇌 자기공명영상에서 대뇌 피질 두께의 상관관계는 독립표본 T 검증 또는 피어슨 상관분석을 실시하였으며 수집된 자료의 유의 수준은 p<0.05에서 검증하였다. 결론적으로 경도인지장애를 동반한 파킨슨병 환자는 뇌 자기공명영상에서 양측 쐐기압소엽과 우하측두엽의 대뇌 피질 두께가 통계적으로 유의하게 감소하였으며 인지 기능 평가를 위한 신경심리검사에서 시공간 기능, 언어 및 시각 기억력 기능이 저하되었다. 특히 언어 및 시각 기억력 영역에 대한 신경심리검사와 신경해부학적으로 좌측 쐐기앞소엽에서 상관관계가 있었다.

16주간의 복합운동이 파킨슨병 환자의 혈중 도파민 농도, 기능적 체력 및 삶의 질에 미치는 영향 (Effects of 16-week Combined Exercise on Blood Dopamine Concentrations, Functional Fitness and Qol in Patients with Parkinson's Disease)

  • 본 연구의 목적은 파킨슨병 환자를 대상으로 16주간 복합운동을 수행하였을 때 혈중 도파민 농도, 기능적 체력 및 삶의 질에 어떠한 영향을 미치는지를 규명하는데 있다. 이를 위해 본 연구에 참여한 60대 이상의 파킨슨병 환자 24명을 운동군(n=12)과 통제군(n=12)으로 분류한 후 운동군을 대상으로 16주 동안 주 2회, 회기 당 70분의 복합운동을 실시하였다. 복합운동 실시 전·후 항목별 차의 비교를 위해 그룹 내 차이는 대응 t 검정, 그룹 간 차이는 독립 t 검정, 상호작용 검증을 위해 이원 반복측정 분산분석을 사용하였으며, 각 항목별 통계적 유의수준은 .05로 설정하였다. 그 결과 첫째, 도파민 농도는 운동군(p<.01)이 16주간 운동 전보다 운동 후 유의하게 증가하였고, 통제군의 경우 유의한 차이가 나타나지 않았다. 둘째, 기능적 체력에서 악력은 운동군(p<.001)과 통제군(p<.05) 모두 16주간 운동 전보다 운동 후 유의하게 증가하였고, 덤벨들기(p<.001), 의자 일었다 앉기(p<.001), 3m 걸어갔다 돌아오기(p<.01)가 16주간 운동 전보다 운동 후 운동군에서 유의한 차이를 보였고, 통제군은 유의한 차이가 나타나지 않았다. 셋째, 삶의 질은 운동군(p<.001)이 16주간 운동 전보다 운동 후 유의하게 증가하였다. 이상의 결과를 종합해 볼 때 16주간의 복합운동은 파킨슨병 환자에게 매우 효과적임을 알 수 있었으며, 도파민 농도를 증가시키고 근력, 근지구력, 평형성 및 보행능력 등 기능적 체력과 삶의 질을 향상시키는데 도움을 줄 수 있다고 사료된다.

파킨슨씨 병 환자와 정상인의 입각기시간과 상비율의 비교 (Comparison of the Total Stance Time And the Phase Ratio in Parkinson's Disease Patients And Normal Subjects)

  • The purpose of this study is to investigate the gait characteristics in Parkinson's disease patients. Specifically, the total stance time and the ratio of each stance phase (heel strike, mid-stance, propulsion) are analyzed from the foot-pressure measurement system which requires low cost and small space compared to the conventional gait analysis system. The gait characteristics were analyzed in 23 Parkinson's disease patients (before and after L-dopa medication), 34 elderly (sixties) normal subjects and 21 young (twenties) normal subjects. Bradykinesia global score (self-developed score of slowness of body movement) of patients before medication was determined to see the relationship between the score and the gait characteristics. The total stance time was greater in the erde. of patients, elderly, youngs (p<0.05). The phase ratio of heel strike and propulsion was smaller and that of mid-stance was greater in the order of patients, elderly, youngs (p<0.05). However, there was no significant difference in the above gait characteristics of patients before and after medication. There was a tendency, though statistically non-significant, that the total stance time is longer and the propulsion phase ratio is shorter in patients with greater Bradikinesia global scale, and this tendency was relieved after medication.

파킨슨병의 신경영상 기법을 이용한 침치료 임상 연구 논문에 대한 고찰 (Review of Studies on Clinical Trials of Acupuncture Treatment for Parkinson's Disease Using Neuroimaging Methods)

  • Objectives: The purpose of this study was to investigate the current state of clinical trials for acupuncture treatment of Parkinson's disease (PD) using neuroimaging methods. Methods: We searched for clinical trial studies of acupuncture treatment for Parkinson's disease that used neuroimaging methods in the MEDLINE (PubMed) database. The identified studies were then selected manually based on inclusion criteria. We subsequently analyzed the characteristics of the selected articles and reviewed the neural substrates of acupuncture treatment in PD. Results: Eight studies were included. The most frequently applied modality for PD was functional MRI. The most frequently selected acupoint for PD was GV34. Several studies showed that acupuncture treatment could improve the symptoms of PD. Through the analysis, we demonstrated that the neuroimaging method could capture the neural substrates associated with PD and that acupuncture treatment may affect the activation of brain areas that are impaired in PD in a different way than sham acupuncture. Conclusions: Applying a neuroimaging method could be helpful in clinical trials of acupuncture treatment for PD, and more efforts should be made in this area.

파킨슨병 환자 부양자의 부양부담에 영향을 미치는 요인 (Factors Affecting Caregiver Burden in Caregivers of Patients with Parkinson's Disease)

  • Purpose: Depression is a common sign of suffering among the patients with Parkinson's disease (PD). Frequent and severe neuropsychiatric symptoms lead to high levels of distress in patients and their caregivers, which results in the high levels of caregiver burden. The aim of this study was to determine the predictors of caregiver burden in caregivers of the patients with Parkinson's disease (PD). Methods: The study included 183 consecutive PD patients and their caregivers. Patients were assessed using the Hoehn and Yahr scale, Mini Mental State Examination (MMSE), Beck Depression Inventory (BDI), and Schwab and England Activities of Daily Living Scale. Caregivers' depressive symptoms were evaluated using the Caregiver Burden Inventor (CBI), BDI, and World Health Organization Quality of Life Scale. Results: All of the patients reported one or more neuropsychiatric symptoms. Patients' and caregivers' depressive symptoms, caregivers' age and education, time for caregiving, and quality of life were significantly associated with the increased caregiver burden. After controlling the level of education as a potential confounding variable, depression in both patients and caregivers, time for caregiving, and quality of life explained 45.6% of the variance in caregiver burden. Conclusion: Substantial attention needs to be given to the early identification of depression in PD patients and their caregivers to improve caregivers' quality of life and burden.

The Effects of Walking and Turning and Treadmill Training on Postural Balance and Walking in People with Parkinson's Disease

  • Purpose: The aim of our study was to evaluate the therapeutic effects of walking and turning plus treadmill training on the functional balance and walking ability of individuals with Parkinson's disease (PD). Methods: Twenty-four participants with Stage 1 to 3 ($2.13{\pm}0.64$) PD based on the Hoehn and Yahr scale were randomly allocated to the experimental group (EG) and control group (CG), with 12 participants in each group. The measured outcomes included the motor subscale of the unified Parkinson's disease rating scale (UPDRS-M), the Berg balance scale (BBS), the Timed Up and Go (TUG) test, the 10-meter walk test (10MWT), and the 6-minute walk test (6MWT). Results: Pre-to-post intervention improvements were noted for all the outcome measures for both groups (p < 0.05). Post intervention, there was a significant improvement in the EG compared to the CG for the following measured outcomes (p < 0.05): UPDRS-M (p = 0.021; 95% CI, 0.081-6.519), BBS (p = 0.042; 95% CI, 1.375-4.541), TUG (p = 0.034; 95% CI, -3.315--0.143), 10MWT (p = 0.011; 95% CI, -2.032--0.289), and 6MWT (p = 0.002; 95% CI, 24.39-91.273). Conclusion: Our study suggests that walking and turning plus treadmill training improves balance and walking compared to treadmill training only in patients with PD.

Current Status of Intervention Studies on Acupuncture for Parkinson's Disease

  • Objectives : The purpose of this study was to investigate the effect of acupuncture treatment (AT) in the tendency of increase of the need for AT for the treatment of Parkinson's disease (PD) worldwide and to investigate the advancements in AT research in Korea and the future directions of research on this topic. Methods : Until May 2017, the PubMed, Scopus, Medline, and four Korean databases were searched. The searched keywords were "Parkinson's disease", "Acupuncture", and "Intervention study". The intervention groups from all screened original studies were analyzed and the methods used to determine the effect of AT on PD were examined. Results : A total of 17 studies were grouped by country on the basis of the first author's position, of which 10 studies were conducted in China, four in the United States, two in Korea, and one in Brazil. The most common type of intervention was electroacupuncture (nine studies), followed by AT (six studies), and a combination of AT and bee venom AT (two studies). The most frequently used acupoints in AT were Baihui (GV20), Taichong (LR3), Zusanli (ST36), Sanyinjiao (SP6), and Yanglingquan (GB34). The most commonly used tool for evaluation of PD was the Unified Parkinson's Disease Rating Scale III, which assesses motor functions. Conclusion : The screened studies reported that there were no adverse effects of AT on drug therapy, and AT reduced the dose of drugs used in PD treatment. Future studies on PD treatment with AT should use the acupoints GV20, LR3, ST36, SP6, and GB34, and the meridians Gallbladder meridian and Governor Vessel. Clinical studies on PD should use CONSORT or STRICTA to ensure the quality of national studies and allow the development of new tools for the assessment of the effect of AT on PD using the above criteria.
