The Treatment Result of Antituberculous Chemotherapy Followed by Surgical Excisions in Tuberculous Cervical Lymphadenitis (경부 결핵성 림프절염에서 외과적 절제수술후 항결핵제 요법시의 치료 성적)

  • Objectives: Tuberculous cervical lymphadenitis is a frequently recurring disease when treated with chemotherapy alone without enough surgical removal of the tuberculous lesions. Authors reviewed retrospectively the treatment result of antituberculous chemotherapy following almost complete surgical removal of tuberculous foci in the neck. Materials and Methods: A retrospective clinical review and analysis was made in 127 cases of tuberculous cervical lymphadenitis patients treated during the past 10 years from 1989 to 1998 at the Department of General Surgery, Inje University Paik Hospital, Pusan. Results: 1) The peak age incidence was the 2nd decade(37.8%), and female was predominated over male by 2.3:1. 2) The time interval from the onset of symptoms to the first visit was less than 3 months in 60.6% of the patient. 3) The location of lymphadenitis was the right neck in 60%, the left neck 34%, and bilateral in 6% of the patient. 4) Signs on the first visit showed solitary masses(60%), abscess(25%) and both mixed(15%). 5) 25 patients(19%) had present or past history of tuberculosis; pulmonary tuberculosis 12 patients, tuberculous lymphadenitis 10 patients, and others 3 patients. 6) Locations of tuberculous lymphadenitis were posterior cervical triangle 70, supraclavicular 51, submandibular 19, anterior triangle 16 and others 4 cases. 7) The principle of treatment of cervical lymphadenitis was surgical management followed by chemotherapy. Surgical procedures were excision(s), curettage and drainage of abscess, combination of both, and biopsy in 60%, 22%, 12% and 6% respectively. Mean duration of antituberculous medication was 9 months after surgery. 8) The rate of recurrent and persistent tuberculous lymphadenitis was 9% in 4 years follow up. Conclusion: Tuberculous cervical lymphadenitis is a frequently recurring disease in young adult when only antituberculous chemotherapy was employed without almost complete removal of the lesions. It is considered that antituberculous medications for 6-9 months after removing the foci at a maximal extent by surgical excision and curettage will reduce the recurrence rate or persistence of tuberculous lymphadenitis.

Healthcare Utilization and Expenditure Depending on the Types of Private Health Insurance in Korea (민간의료보험 가입 및 가입유형별 의료이용 특성 분석)

  • Since the introduction of National Health Insurance(NHI) in 1977, it has grown rapidly and contributed to extend patient's access to the health care services. However, limited coverage for health care services of NHI has been ongoing challenge and private health insurance(PHI) has been rising as an alternative source of enhancing coverage and saving out-of-pocket(OOP) expenditure for patients. In this study, after controlling for socio-demographic, economic, health related variables, we identified the patients' healthcare utilization and subsequent OOP expenditure depending on their PHI enrollment and their enrollment types(fixed benefit, indemnity, fixed benefit plus indemnity). Data were collected from the 2010 Korean Health Panel. The unit of analysis was a member of household(n=13,324). Of the 13,324 cases, 70.7% of patients held PHI, in detail, fixed benefit(47.0%), indemnity(3.6%), fixed benefit plus indemnity(20.1%). Major findings showd that patients who enrolled in PHI used more outpatient services(outpatient visit, number of physician visit, number of examination) and spent more OOP expenditure than non-PHI patients. There were also differences of healthcare utilization and OOP expenditure among the types of PHI. In addition, PHI patients used more inpatient services(inpatient use, number of hospitalization, LOS), but there was no significant difference between PHI and non-PHI patients with regard to the OOP expenditure. Thus, we could not find any distinct relationship between the types of PHI and patients' tertiary hospital use. Policy-makers should need careful political deliberation for monitoring the effect of PHI on health care utilization and subsequent expenditure not only to improve patients' coverage but also to save their OOP expenditures.

An Efficient k-D tree Traversal Algorithm for Ray Tracing on a GPU (GPU상에서 동작하는 Ray Tracing을 위한 효과적인 k-D tree 탐색 알고리즘)

  • This paper proposes an effective k-D tree traversal algorithm for ray tracing on a GPU. The previous k-D tree traverse algorithm based on GPU uses bottom-up searching from a leaf to the root after failing to find the ray intersected primitive in the leaf node. During the bottom-up search the algorithm decides the current node is visited or not from the parent node. In such a way, we need to visit the parent node which was already visited and the duplicated bounding box intersection tests. The new k-D tree traverse algorithm reduces the brother and parent duplicated visit by using an efficient method which decides whether the brother node is already visited or not during the bottom-up search. Also the algorithm take place bounding box intersection tests only for the nodes which is not yet done. As a result our experiment shows the new algorithm is about 30% faster than the previous.

Effects of AIF on Knee Osteoarthritis Patients: Double-blind, Randomized Placebo-controlled Study

  • Anti-inflammatory factor(AIF) is a water soluble extract of three herbs, Panax notoginseng(Burk.) F. H. Chen, Rehmannia glutinosa Libosch and Eleutherococcus senticosus. The present study aimed to investigate the safety and efficacy of herb extracts, AIF, on Korean knee osteoarthritis patients for six weeks. Fifty seven patients with knee osteoarthritis, ranging from 43 to 73 years of age, who fulfilled the "American College of Rheumatology"(ACR) classification of idiopathic osteoarthritis of knee and radiographic criteria were randomly selected and enrolled for the study. After initial screening and resting period, two capsules each of AIF(Each capsule contains; 400 mg) and similar identical placebo were administered twice a day to both groups. Pain intensity at second, fourth, and sixth weeks of study as well as one week after discontinuation of drugs was assessed by using 100 mm visual analogue scale(VAS). Changes in the Korean version of the Western Ontario and McMaster Universities(K-WOMAC) index score were compared at the initiation and completion of the study. VAS assessed by patients were significantly reduced(at visit 2; $54.64{\pm}14.72$, at visit 4, $37.32{\pm}16.58$, p<0.001) after AIF administration. Results showed an improvement in the physical function of K-WOMAC scale which was significantly higher(p=0.013) in AIF than placebo group, and decreases of total K-WOMAC score were also significantly higher(p=0.030) in AIF groups than placebo group. No serious adverse effect was observed, and there was no difference in incidence of adverse effect between AIF and placebo groups. In this population of Korean patients with knee osteoarthritis, AIF was found to be safe, tolerable and effective for symptomatic improvement of pain and physical function.

A Study on Self-care Behavior Types of Hypertensives : Q-methodological Approach (고혈압환자의 자가간호행위 유형에 관한 연구 : Q 방법론 적용)

  • Essential hypertension is a typical chronic disease requiring adequate and continuous management. And many studies supported that self-care was the essential factor to promote the wellbeing. The purpose of this study is to identify and understand the behavior patterns of self-care in hypertensives. As a research method, 35 Q-statements were collected through Individual interviews and review of the related literatures. 21 subjects were interviewed and the data were analyzed by the PC QUANL program with principal component analysis. There were 6 different self-care types classified as follows 1) Type 1 was the self-oriented control type, monitoring the blood pressure and taking the low salt diet. But they didn't take the anti -hypertensive drug and visit the health agency regularly. 2) Type 2 was the stress-control type. Their main activities were meditation to relieve the stress and communication with family. 3) Type 3 was called daily-life control type. This type tried to make their mind comfort and think positively. They also preferred walking and exercise regularly. 4) Type 4 was the medical-oriented control type, taking the anti-hypertensive drug, visiting the medical personnel and following the medical regimens. 5) Type 5 was the medication-oriented type. They only took the anti -hypertensive drug regularly and didn't any other self-care like as monitoring the blood pressure, taking the low salt diet and exercise. 6) Type 6 was called non-medication control type. This type had no medication, but tried to visit the health agency and health personnel. From the above results, it can be concluded that the self-care types were very various and self-care education have to provide individually according to the characteristics of self-care type. Another repeated study can be recommended to improve the nursing intervention the self-care behavior in chronic patient like as diabetics or rheumatoid arthritis.

  • The Youngsan River Restoration Project(YRRP) is to bring back the original appearance of the stream and to create a new waterfront. This project, which id part of the Four Major Rivers Restoration Project, began in 2009, and Juksan and Seungchon beams was completed, and waterway of Youngsan river also was constructed. Both sides of the river, bike trails, waterfront park of 70 places, including the composition and the amenities were installed. The purpose of this study is to analyze the useage behaviors and satisfaction on visitors of waterfront parks in Yeongsan River. Many visitors visit waterfront in order to walking and biking, there were many people who visit family and hobbyists club units. Visitors showed a relatively high level of satisfaction about the waterfront in the new composition such as bike trails, walking roads, and ecological parks created by the YRRP. However, the major problems was the lack of amenities in the waterfront.

The Association between Health Examination and Personal Medical Cost through Panel Survey (건강검진이 개인 의료비지출에 미치는 영향)

  • Background: This paper describes the relationship and effect of health examination on personal medical cost by identifying the difference of the cost for medical care in physician visit between the population without and with health examination. Methods: After classifying into three cohorts in which, independent variables were designed according to the Andersen's behavioral model, the association of personal medical cost for medical care and prescription drugs which is dependent variable was analyzed by t-test and Mann-Whitney test for description and gamma regression model for inference. Results: In personal average medical cost, the population with health examination paid significantly more than without health examination, 11.6% more in cohort 2008, 26.6% more in cohort 2009, and 48.0% more in combined cohort. The odds ratio on medical expenditure of outpatients with health examination was 1.067, 1.126, 1.398 significantly in cohort 2008, 2009, and combined cohort respectively, comparing to the group without health examination. In independent variables, that is female, the elderly, never married, non-working, non-metropolitan, the higher family income, the smaller family size, people with disability, the people with chronic disease, and people with health examination have significantly being paid more tendency showing positive association with medical cost. Conclusion: This result showed that medical expenditure in physician visit has been increased after taking a health examination. Therefore reasonable limitation of getting preventive medical service is suggested to avoid medical shopping around and reduce being repeated health examination by unifying control to find out easily the clinical results from various medical facilities.

A classificatory study about the tendency of the patient who visited oriental medical hospital for growth treatment (한방병원에 내원하는 성장 환자의 유형 분류)

  • Objectives : Patients who visit oriental medical hospital for growth treatment are increasing. So we aimed to classify the tendency of the patients. Methods : We studied 231 patients who visited Oriental medical hospital for growth treatment from January 2004 to August 2005. We classified sex ratio, height percentile, symptom form of the Oriental medicine, age ratio and developed complication of patients. We used X-ray, endoscope for nasal cavity, blood sample, the Standard Growth Table made by the Korean Association of Pediatrics, 1998. Results : The results which were classified as follows; boys were 50.2 percentile, girls were 49.8 percentile. The classification according to age stage resulted as follows; infant stage 1.3 percentile, preschool age 13.4%, school age 28.6%, rapid growth stage 22.9 percentile, puberty 33.8 percentile. The classification according to height percentile resulted as follows; 3 percentile or under 12.1%, 25 percentile or under 48.9%, 50 percentile or under 25.6%, 75 percentile or under 10.8%, 75 percentile excess 2.6%. The classification according to disease resulted as follows; paranasal sinusitis 12.1 percentile, allergic rhinitis 10 percentile, atopic dermatitis 5.6 percentile, nocturia 3 percentile, convulsion or tic disorder 2.2 percentile, precocious puberty 1.3 percentile, Tuner syndrome 0.9 percentile, developmental disability 0.4 percentile. Conclusions : Sex ratio of children patients who visited Orienatal medical hospital were nearly the same. 13.4% of hospital visit children patients were taller than average. Most of the male children patients were school ages while the female were in puberty. 35.5% of them have developed complication as follows; paranasal sinusitis, allergic rhinitis, atopic dermatitis, nocturia, convulsion or tic disorder, precocious puberty, Tuner syndrome and developmental disability.

  • This study analyzes how expectations, perceived performance and expectation-disdonfirmation of the recreation-forest visitors affect their satisfaction and revisit using path analysis. These results which would maximize visitors' satisfaction and revisit will enhance recreational forest management. As the results, factors of expectation-disconfirmation positively effected visit satisfaction are experience-facilities, natural resources, access and valley factors. In addition, natural resources factor effects positively their selection-satisfaction. Also, visit-satisfaction and selection-satisfaction effect positively to their revisit intention. This result show seeking the visitors' satisfaction through recreation-forest manager try to expectation-disconfirmation of experience-facilities, natural resources, access and valley factors exchange to positive expectation-disconfirmation.

Association between Vibration Exposure and Skeletal Muscle Mass Index in a Single University Hospital Health Check-up (일개 대학 병원 건강 검진 수검자에서 진동 노출과 골격근 지수의 관련성)

  • Objectives: The aim of this study is to evaluate the association between vibration exposure and skeletal muscle mass index through a single university health check-up. Methods: We used data from 134,067 male subjects who received a general health check-up or vibration exposure health check-up out of the 1,515,322 people who underwent medical check-up at a local university hospital from 2002 to 2018. Pearson correlation analysis was conducted for comparing the association between skeletal muscle mass index and demographic and hematological variables in both groups. Mixed linear model analysis after controlling demographic and hematological variables was used to analyze the differences in skeletal muscle mass index between groups at every visit over 10 years. Results: In the Pearson correlation test, the variables that showed different results when comparing the two groups were C-reactive protein (p=0.001) and glycated hemoglobin (p=0.002) in the vibration exposure group and erythrocyte sedimentation rate (p<0.001) and vitamin D (p<0.001) in the general group. After the adjustment of demographic and hematologic variables, the skeletal muscle mass index at every visit was markedly decreased in the vibration exposure group (p<0.001). Conclusions: In the vibration exposure group, the skeletal muscle mass index showed a tendency to decrease markedly over time compared to the general health check-up group, which showed that C-reactive protein and glycated hemoglobin would have an influence on skeletal muscle index in male workers exposed to vibration.
