세대 간 거주근접성과 부모의 정신 및 신체 건강 : 부모 연령, 성인자녀 성별의 조절효과분석 (The effects of residential proximity on parents' mental and physical health: Parental age and the adult child's gender as moderators)

  • Objectives: This study examined the effects of residential proximity to adult children on the mental and physical health of middle-aged and older parents. The study also evaluated whether the parental age and gender of the adult child in closest proximity to the parent might moderate the association. Method: Data were drawn from five waves of the Korean Longitudinal Study of Aging(2006-2014). The analytic sample consisted of 7,359 parents aged 45 or older who had at least one non-coresidential adult child aged 19 or older. The analyses were conducted by estimating a series of fixed effects models while adjusting for the nested structure of the data. Results: The results showed that first, a closer distance between an adult child and the parent was generally associated with the parent experiencing a decrease in depressive symptoms. Yet, the mental health benefit was smaller for parents aged 65 years or older whose closest living adult child was a son. Second, a closer distance was observed to affect chronic illness only among middle-aged parents (aged 45 to 64). When the closest living adult child was a daughter, the middle-aged parent experienced deterioration in their chronic illness. On the other hand, the opposite pattern was observed when the closest living adult child was a son. Conclusions: The parental age and gender of the adult child in closest proximity to the parent might have varying effects on parents' health. A closer distance between an adult child and their parent has a positive impact on the mental health of the parent as a whole, whereas the effect of living closer was mixed in relation to the parental physical health.

주의력결핍 과잉행동장애와 우울장애 아동 집단의 부모 양육태도 (Parents' Rearing Attitude of Children with Attention-Deficit Hyperactivity Disorder and Depressive Disorder)

  • Objectives: This study was conducted to compare the mothers' rearing attitude of ADHD children and that of children with depressive disorder. Methods: The participant consisted of 58 school-aged children diagnosed as ADHD, 14 children diagnosed as depressive disorder based on DSM-IV criteria. Normal control group consisted of 34 school-aged children who are free of any diagnosis of psychiatric disorders. Parental Acceptance-Rejection Questionnaire, Parenting Style Questionnaire, Children's Depression Inventory, Marital Satisfaction Scale, Beck's Depression Inventory were administered to all children and their mothers. Results: The parents of ADHD children showed more aggressive/hostile, neglecting/indifferent and less warm/affectionate parenting styles to their children than those of the depression and control groups. The depressive group perceived their parents as more rejecting than the control groups. Conclusion: The externalizing symptoms of ADHD might provoke parental distress and make it difficult for the parents to show positive rearing attitude toward their children. The depressive children might be more sensitive and perceptive to the negative sign of their parents' rearing attitude.

지역 저소득층 아동의 우울증상과 주의력결핍-과잉행동증상에서 사회경제적 요소의 관련성 (Association of Psychosocial Factors in Developing Childhood Depression and ADHD in a Community Low Income Family Children)

  • Ovjectives: As the attention devoted to children's mental health increases, medical costs and burdens mount as well. In the present study, we evaluated the association between socioeconomic status(SES) and major child psychiatric symptoms. Methods: The subjects of this study were children of recruited from a mental health screening program in the Seoul Sungbuk mental health center over the course of 3 months. To establish the SES of each child, we collected data about each child's medical insurance, years of parental education, household income, family structure and housing. 149 children & parents completed questionnaires including the Childhood Depression Inventory(CDI) and the Korean Attention Deficit Hyperactivity Disorder Rating Scale(K-ARS). Results: The mean K-ARS-P score was $12.1{\pm}11.1$ and the suspected prevalence of ADHD was 20.8%(n=31). The mean cm score was $12.9{\pm}7.9$, and the prevalence of suspected depression was 16.8%(25). Depressive symptoms and ADHD symptoms were both more severe than those observed in a previous epidemiologic study in Korea. Depressive symptoms were more closely associated with family SES status. Conclusion: SES status is one of the most important risk factors in the development of major child psychiatric symptoms. In our study we found that depressive symptoms in particular were most tightly associated with psychosocial factors. Evaluation of the risk factors, early screening and intervention for low SES children would be valuable mental health management tactics to implement in a community mental health system.

Mood and Emotional Changes After PEERS® Program in Parents of Young Adults With Autism Spectrum Disorder

  • Objectives: This study examined the changes in anxiety and depressive symptoms in the parents of Korean adults with autism spectrum disorder (ASD) after participating in the Korean version of the Program for the Education and Enrichment of Relational Skills for Young Adults (PEERS® -YA-K). Methods: Forty-six parents were enrolled (mean age 54.7 years; 22 fathers and 24 mothers), of whom 27 participated in the PEERS® - YA-K as social coaches. Participants completed self-report scales, including the Test of Young Adult Social Skills Knowledge (TYASSK), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), and Symptom Check-List-90-R (SCL-90-R). The scales were administered three times: before and after the 16-week program and 4 months after the program ended. Differences between participant variables at pretreatment, post-treatment, and follow-up were analyzed using paired-sample t-tests. Results: Participants showed a significant improvement in their social skills knowledge (TYASSK) (p<0.01). There was a significant improvement in the BAI and BDI scores of parents with severe depressive and anxiety symptoms at the baseline (p<0.05). Paternal paranoia and maternal hostility results also significantly improved on the SCL-90-R. Conclusion: This study suggests that PEERS® -YA-K can reduce parental anxiety and depressive symptoms. To the best of our knowledge, this is the first to compare the degree of depression and anxiety after PEERS® -YA-K in parents of adults with ASD.

뇌전증 소아청소년 환아와 가족의 삶의 질에 영향을 주는 요인에 관한 연구 (A Study on the Factors Affecting Quality of Life in Children and Adolescents with Epilepsy and Their Families)

  • Objectives The aim of this study was to investigate the impact of clinical and psychological factors on the quality of life of children and adolescents with epilepsy and their families. Methods Children and adolescents with epilepsy and their families (n = 63, age range = 6-17 years) completed questionnaires on epilepsy-related variables, quality of life, children's depressive symptoms, children's anxiety, children's behavioral problems, children's attention problems, parental stress, and parental anxiety. Stepwise regression analysis was performed to determine the significant predictive variables that affect quality of life. Results In the correlational analysis, children's attention problems (r = 0.363, p = 0.004), parental anxiety (r = 0.377, p = 0.003), parental stress (r = 0.564, p < 0.001), and children's behavioral problems (r = 0.503, p < 0.001) showed a significant correlation with quality of life. Parental stress (β = 0.415, p = 0.001, adjusted R2 = 0.345) and children's behavioral problems (β = 0.285, p = 0.02, adjusted R2 = 0.345) were significantly related to the quality of life. Conclusions Clinicians should pay attention to parental stress and children's behavioral problems, which affect quality of life in families with pediatric epilepsy.

뇌전증 소아청소년 환아의 부모 스트레스에 영향을 주는 요인 (Factors Affecting the Parental Stress of Children and Adolescents with Epilepsy)

  • 본 연구는 뇌전증 소아청소년 환아의 부모 스트레스에 대한 임상 및 심리적 요인의 영향을 조사하고자 하였다. 방 법 뇌전증이 있는 소아청소년(n=90, age range 6~17 years)을 대상으로 뇌전증 관련 임상 변수를 조사하였고, 환아에게 우울 및 불안 증상 설문지를 작성하도록 하였으며 지능검사를 시행하였다. 환아의 부모를 대상으로 부모의 스트레스, 부모 불안 및 환아의 주의력 문제, 행동 문제에 대한 설문을 작성하도록 하였다. 뇌전증 환아 부모의 스트레스와 변수들간의 연관성을 확인하기 위해 경향성 분석, 일원분산분석, 피어슨 상관분석으로 단변량 분석을 시행하였고, 부모의 스트레스에 영향을 미치는 독립 변수를 찾기 위해 단계적 회귀분석을 수행하였다. 결 과 상관 분석에서 발작의 치료 기간(r=0.253, p=0.016), 환아의 지능(r=-0.544, p<0.001), 주의력 문제(r=0.602, p<0.001), 우울 증상(r=0.335, p=0.002), 불안 증상(r=0.306, p=0.004), 행동 문제(r=0.618, p<0.001) 및 부모의 불안(r=0.478, p<0.001)은 부모의 스트레스와 유의한 상관 관계를 나타냈다. 부모의 스트레스에 영향을 미치는 독립변수는 환아의 행동 문제(β=0.241, p=0.010) 및 지능(β=-0.472, p<0.001) 그리고 부모의 불안(β=0.426, p<0.001)으로 나타났으며 부모의 스트레스에 대해 61.9%의 설명력을 보여주었다. 결 론 뇌전증을 가진 소아청소년 환아에서 부모의 스트레스는 환아의 행동문제와 인지기능 그리고 부모의 불안과 연관성이 높으며 따라서 임상의는 이에 주의를 기울이고 관리하는 것이 필요하겠다. 뇌전증 환아 부모의 스트레스를 경감시키기 위해서 뇌전증 환아의 지능과 행동문제를 조기에 평가하고 부모의 불안감에 대해서도 면밀하게 관찰하여 향후 뇌전증 치료에 반영하는 연구가 필요할 것으로 예상된다.

신체적 장애아 가족의 스트레스와 적응 과정에 관한 고찰 (Stress and Adaptation in Family with Physical Disabled Children)

  • The purpose of this study was to describe the relationship between family stress and adaptation in families with a disabled child through literature review using McCubbin's Double ABCX family crisis framework. The literature review focused on (1) family stress and factors affecting family stress, (2) the critical individual, familial and social resources which families acquire and employ over time in managing crisis situation. (3) the changes in definition and meaning families develop in an effort to make sense out of their predicament. (4) the coping strategies families employ. and (5) the range of outcomes of these family efforts The results showed that families reported financial difficulties and the burden of care-giving demands as major family stressors. Siblings of disabled children manifested depressive symptoms and social isolation. but was not consistent study results. The parents' views of the cause of the disabling condition fundamentally affected their behavior toward their disabled child. Especially. the fathers' views of the child's characteristics made the greatest contribution to positive changes in the mothers' perceptions. The term perceived social support refers to the cognitive appraisal by individuals that they are cared for and valued, that significant others are available to them if needed, and that they are satisfied with their interpersonal relationships. The perceived social support was more protective than social support source. network size and network density. Parental adaptation was found to be related to the child's communication competence rather than family coping strategies proposed by Lazarus and Folkman. One study results showed that there was no difference in depressive symptoms and physical health between mothers with a disabled child and those without all though mothers with a disabled child had negative attitudes and perceived themselves as having significantly less social support and lower family functioning. But a longitudinal study revealed decreases in the negative impact of the child and increases in sibling and overall family adaptation.

The Relationships among Postpartum Fatigue, Depressive Mood, Self-care Agency, and Self-care Action of First-time Mothers in Bangladesh

  • Purpose: Postpartum fatigue can impact maternal well-being and has been associated with levels of perceived self-care. This study aimed to examine the relationship among fatigue, depressive mood, self-care agency, and self-care action among postpartum women in Bangladesh. Methods: A descriptive cross sectional survey was done with 124 first-time mothers from two tertiary hospitals in Dhaka, Bangladesh. The Modified Fatigue Symptoms checklist, Denyes' Self Care Instrument, the Edinburgh Postnatal Depression Scale, and items on sociodemographic and delivery-related characteristics, were used in Bengali via translation and back-translation process. Results: High fatigue levels were found in 18.5% (n=23) and 73.4% had possible depression (n=91). There was a significant negative relationship between fatigue and self-care agency (r=-.31, p<.001), and self-care action (r=-.21, p<.05). Fatigue differed by level of self-care agency (t=4.06, p<.001), self-care action (t=2.36, p=.023), newborn's APGAR score (t=-2.93, p=.004), parental preparation class participation (F=15.53, p<.001), and postpartum depressive mood (t=-4.64, p<.001). Conclusion: Findings suggest that high level of self-care efficacy and behaviors can contribute to fatigue management, and highlight the need for practical interventions to better prepare mothers for postpartum self-care, which may, in turn, alleviate postpartum fatigue.

대학생이 지각한 부모의 과보호와 우울증상의 관계 : 회피대처의 조절효과 (The Relations between Parents' Overprotection and Depressive Symptoms in University Students : The Moderating Effects of Avoidance Coping)

  • 본 연구는 대학생이 지각한 부모의 과보호와 우울 증상에 미치는 영향에서 회피대처의 조절효과를 규명하고자 한 연구이다. 연구 대상은 지방 지역의 대학생 230명을 대상으로 자기보고식 설문조사를 실시하였다. 본 연구에 사용된 측정도구는 부모양육태도, 우울, 회피대처의 척도였다. 분석방법은 SPSS 21.0 통계프로그램을 이용하여 빈도와 백분율을 산출하였고, 내적합치도 계수(Cronbach α)를 구하였다. 본 연구문제를 검증하기 위해 상관분석과 위계적 중다회귀분석을 실시하였고, 밝혀진 결과를 요약하면 다음과 같다. 첫째, 부모의 과보호와 우울증상, 회피대처는 각 변인 간 모두 정적상관을 나타냈다. 둘째, 부모의 과보호와 우울증상의 관계에서 회피대처는 조절효과를 주는 변인임을 확인하였다.

한국비행 청소년의 가정환경 및 개인내적 특성 (THE CHARACTERISTICS OF THEIR FAMILY ENVIRONMENT AND CHARACTER TRAIT AMONG DELINQUENT ADOLESCENTS IN KOREA)

  • 본 연구의 목적은 비행청소년의 가정환경, 개인내적 특성을 규명함으로써 청소년 비행행동의 원인요인을 탐색하기 위함이다. 본 연구는 조사연구로써 자료수집방법은 설문지조사법, 학생생활기록부, 소년원생활기록부 참조와 필요시 면담방법을 병행하였다. 연구대상자는 현재 중학교 1학년에서 고등학교 3학년까지 재학중인 학생 청소년 1,236명과 소년원, 분류심사원에 재소중인 비행, 범죄청소년 707명을 선정하였으며 연령범위는 12세에서 18세 사이였다. 표본추출방법은 비례층화표본추출법을 적용하여 지역(서울-지방) 및 조사기관의 유형(중학교, 고등학교, 소년원, 분류심사원)을 함께 고려하여 조사대상집단을 무작위로 선정하였다. 총표집 대상자 1,943명중 불충분한 응답자 80명을 제외한 1,863명을 연구대상으로 하여 응답률은 95.9%(비행군:92.9%, 학생군:97.6%)이었다. 자료처리는 연구자와 보조연구원들이 면담, 설문지법, 학교생활기록부나 기타 소년원재원기록에서 얻은 자료를 종합검토하여 불충분한 자료는 제거한후 SAS 프로그램을 통하여 분석하였다. 본 연구에서 사용한 통계방법은 Chi-square 검정과 주성분 분석등이었다. 본 연구에서 제시한 가설 검정 결과는 다음과 같다. 1) 비행청소년의 가정환경은 학생청소년의 가정환경보다 보다 더 역기능적이었다. 즉 비행청소년은 학생청소년에 비해 부모의 자녀 양육방식이 일관성이 적었으며 가정안정도는 낮았고 부모-자녀 관계 및 가족간의 관계도 원만하지 않았으며 가족원의 가정만족도도 낮았다. 그러나 본 조사결과에서는 비행청소년의 모친이 학생청소년의 모친보다 사회활동이 낮게 나타나 비행청소년의 모친이 학생청소년의 모친보다 사회활동이 높을 것이다는 가설은 지지받지 못하였다. 2) 비행청소년의 성격특성은 학생청소년의 성격보다 더 부적응적이었다. 즉 비행청소년은 학생청소년에 비해 욕구좌절, 반사회적 성격양상, 정신신체증상호소성향, 우울성향은 높은 반면 사회성향은 낮았다.
