경구강 적출이 가능했던 부인강 이형성종 1예 (A Case of Parapharyngeal Pleomorphic Adenoma Removal Intraorally)

  • Primary tumors of the parapharyngeal space are rare and account for only 0.5% of head and neck neoplasm. About 80% of parapharyngeal tumors are benign and 20% are malignant. Parapharyngeal space is classified into the Prestyloid space and the Poststyloid space. The Poststyloid tumors are usually benign lesions such as neurogenic tumors, paragangliomas, vascular tumors, or aneurisms. The origins of prestyloid tumors are much more diverse pathology, the pleomorphic adenoma in parotid deep lobe is most common type. Several surgical approaches have been introduced for management of parapharyngeal tumor, such as transcervical, transparotid-transcervical and the transcervical-transmandibular approaches. This paper is aimed to present a large parapharyngeal space tumor removed via transoral approach. It is possible to remove easier by using microdebrider. The pathologic diagnosis was pleomorphic adenoma.

Diagnostic and management challenge of concurrent tongue squamous cell carcinoma with an unknown parapharyngeal mass

  • There are very few case reports of the diagnosis and management of concurrent oral cavity and parapharyngeal space tumors. We present a case involving a 49-year-old female who presented with oral cavity squamous cell carcinoma confirmed by biopsy. Initial diagnostic workup revealed a concurrent parapharyngeal mass. Diagnostic studies and surgical therapy were tailored to account for both pathological entities. The patient was treated with a combination of surgery and adjuvant therapy. The surgical strategy was designed to address both lesions simultaneously. One year post-surgery, the patient had good response to therapy with no evidence of persistent or recurrent disease. This report discusses the outcome and treatment of a rare case of concurrent squamous cell carcinoma with a complicating parapharyngeal space tumor. It explores the diagnostic process, comprehensive workup, and the surgical management.

  • Parapharyngeal space tumors are extremely rare accounting for about 0.5% of all head and neck tumors and treat mostly by surgical removal. Due to their inherent location, they present with varied non-specific signs and symptoms, resulting in a delay in diagnosis and unnecessary procedures, such as a 'tonsillectomy' or 'incision and drainage' of a 'quinsy'. We recently confirmed a case about a the neurilemnoma on parapharyngeal space during tonsillectomy. To present our experience with the transoral approach for parapharyngeal space tumor and describe our technique for removal of these neoplasms. Although parapharyngeal space tumours are uncommon, recognizing them would enable the correct sequence of investigations, instead of unnecessary procedures resulting in an increased morbidity for the patient.

  • The meningiomas are slowly growing tumors arising from meningoepithelial cells and they generally occur in the intracranial space. Tumor extension to the extracranial structures occurred in about 20 percent of intracranial meningiomas. Meningiomas extending to the cervical area are uncommom. Beacuse these patients complain of localized symptoms, sometimes those tumors have been operated as cervical tumors. In this report, we incidentally found a meningioma in the left parapharyngeal space in a 63-year-old female, who was confirmed by histopathologic diagnosis to have meningioma. We report herewith this case with the review of literatures.

  • A new surgical approach to the area of the infratemporal fossa and parapharyngeal space is described. This approach results in a wide-field exposure of the infratemporal fossa, pterygomaxillary space and parapharyngeal space. We used two osteotomies on the patient's mandible and temporary resection of zygomatic arch for superior margin of tumor. Lower lip splitting was not needed because the incision was started in the frontal scalp, curved in front of and below the external auditary canal, and extended anteriorly to the greater horn of hyoid bone on the neck along a skin crease. We had good results without sacrifice of the facial nerve, mandibular function and sensory supply of the face and oral cavity.

부인두강 종물로 발현된 갑상선 유두상암종 (Thyroid Papillary Carcinoma Presenting as a Parapharyngeal Mass)

  • An unusual case of nodal metastases from thyroid neoplasm known as parapharyngeal space mass is likely to be overlooked. And identification of the primary lesion by excisional biopsy calls for a secondary operation. Therefore, it is important to be aware of the possible lymphatic spread of the thyroid neoplasm to the parapharyngeal space. In this case, completion thyroidectomy should be considered. Here, we present a case of thyroid papillary carcinoma masquerading as a parapharyngeal space tumor. The mass was removed by transcervical approach and pathologically diagnosed as a metastatic thyroid papillary carcinoma. Successful results were obtained after additional completion thyroidectomy.

측두하강 및 측인두강의 원발성 종양 (Primary Tumors of the Infratemporal and Parapharyngeal Spaces)

  • There is a difficulty in complete removal of tumor developing at the parapharyngeal space or infratemporal space where they are just under the skull base. We have experienced 3 cases of malignant tumors and 5 cases of benign tumors arising from the parapharyngeal spaces and/or infratemporal space since 1976. We hereby present 8 cases of primary tumors arising in these spaces with a review of anatomical relationship between above spaces, the characteristic symptoms, diagnostic technics, and surgical approaches which should be considered in their management.

Difficult airway management in a patient with a parapharyngeal tumor

  • A 47-year-old man was referred to the operating room to treat a dentigenous cyst of the mandibular bone. Initial assessment of the airway was considered normal. However, after the induction of anesthesia, we could not intubate the patient due to severe distortion of the glottis. Fiberoptic bronchoscopy and video laryngoscopy were not effective. Intubation using a retrograde wire technique was successful. After the conclusion of surgery, the patient recovered without any complications. Subsequent magnetic resonance imaging of the patient's neck showed a $6{\times}4{\times}8.6cm$ heterogeneous T2 hyperintense, T1 isointense well-enhancing mass in the prestyloid parapharyngeal space. The patient was scheduled for excision of the mass. We planned awake intubation with fiberoptic bronchoscopy. The procedure was successful and the patient recovered without complications. Anesthetic induction can decrease the muscle tone of the airway and increase airway distortion. Therefore, careful airway assessment is necessary.

두경부악성종양에서의 전산화단층촬영의 진단적 가치 -비인두업성종양을 중심으로- (The Diagnostic Value of Computed Tomography in Bead and Neck Cancer fart I : Nasopharyngeal Carcinoma)

  • The CT findings of 46 cases of nasopharyngeal cancer were analyzed and compared with clinical tumor staging. They are composed of 28 cases $(61\%)$ of squamous cell carcinoma, 13cases $(28\%)$ of undifferentiated carcinoma, 4 cases $(9\%)$of lymphoma and 1 case $(2\%)$ of adenoid cystic carcinoma. The results were as follows : 1. The most common CT findings of nasopharyngeal cancers are air·way asymmetry including obliteration of Rosenmuller fossa, orifice of Eustachian tube and asymmetric obliteration of parapharyngeal fat. 2. Other involved anatomic sites are carotid sheath area, oropharynx, paranasal sinuses especially sphenoid sinus, cervical lymph nodes, nasal cavity and skull base or middle cranial fossa. 3. CT does significantly influence on the tumor staging of the nasopharynx cancers, but has a definite value in evaluating deep tissue invasion of the cancers especially to parapharyngeal space or carotid sheath area. 4. CT seems to be essential for staging work-up, estimating the prognosis, and assessing the effect of radiotherapy of the nasopharyngeal cancer because it clearly shows the whole extent of the tumors including deep tissue invasion.

부인두 공간에서 발생한 악성 말초 신경초 종양 1예 (A Case of Malignant Peripheral Nerve Sheath Tumor in Parapharyngeal Space)

  • The malignant peripheral nerve sheath tumor is an extremely rare soft tissue sarcoma. It is a highly malignant sarcoma, which is locally invasive, frequently leading to multiple recurrences and eventual metastatic spread. The peak incidence of disease is known to occur sporadically between the age of 20s and 50s, and is usually associated with the neurofibromatosis type I. In human body, the trunk and extremities are the most commonly involved sites, with only 8-14% of all lesions appearing in the head and neck region. We present a case of malignant peripheral nerve sheath tumor involving the right parapharynx in a 48-year-old patient who complained of headaches in the right parietal area and of dysphagia that aggravated over a month. After surgery, tumor was finally diagnosed as malignant peripheral nerve sheath tumor by histopathologic examinations. The authors report a case of malignant peripheral nerve sheath tumor in the right parapharynx with a review of the literature.
