Retrobulbar abscess is a rare, but severe complication of paranasal sinusitis. The clinical presentations are eyelid swelling, erythema, proptosis, conjunctival chemosis, restricted ocular movement, and decreased visual acuity. Diagnostic methods available for evaluating retrobulbar abscess include sinus X-ray, ultrasonography, computed tomography (CT), and bacterial culture. For the treatment of retrobulbar abscess, immediate surgical drainage and systemic antibiotic therapy are needed. Proper diagnosis and treatments are necessary for preventing visual loss, cavernous sinus thrombosis, subdural abscess, and other lifethreatening complications. A patient, a 30-year-old man, was admitted to our hospital because of progressive eyelid swelling, erythema, ptosis and decreased visual acuity on the right eye after endodontic treatment. The sinusitis occurred secondary to the infection from an upper molar tooth. The spread of the infection led to the orbit via ethmoidal sinus and posterior orbital wall. Immediate surgical intervention was performed and systemic antibiotics was administrated. The symptoms and signs are improved after treatments, so we present our case with a brief review of the literature.
Chronic cough is one of the most common respiratory symptoms, especially in children. And it can be the sale presenting manifestation of bronchial asthma. Although most coughs are self limiting, chronic cough often proves to be a frustrating problem. It is commonly defined as a persistent or recurrent cough exceeding 3weeks duration. The post nasal syndrome has been determined to be the most common cause of chronic cough, followed by asthma, chronic bronchitis, gastroesophageal reflux and bronchiectasis. This study was performed at both City-Oriental Medicine Hospital and Pundang Cha Oriental Medicine Hospital from January,1,1998 to November 31,1998, and 114 children with chronic cough persisting for longer than 3 weeks were evaluated. We investigated the clinical findings and evaluated the etiology in children with chronic cough syndrome including: type of cough (with or with out sputum and daily onset) and associated signs & symptoms. The results were as follows: The most common cause of chronic cough was asthma with sinusitis (27.2%); The second and third were post nasal drip syndrome(22.8%) and bronchial hypereactivitic cough(14.9%). The other causes included asthma, paranasal sinusitis, bronchitis and rhinitis. Therefore, in the diagnostic and therapeutic approach to this symptom, it should be considered that the cause of this type of cough is either bronchus and pulmonary disease or that associated with another problem, especially sinusitis, gastroesophageal reflux and allergic disease. Also, in infants and toddlers, congenital abnormaly should be considered.
Pituitary abscess is a very rare disorder, since antibiotics therapy has become widely available. Clinically and radiologically, the preoperative diagnosis of pituitary abscess is difficult because of its features which may be identical to those of any space-occupying lesions of the sella turcica. A combination of clinical features, such as meningitis, paranasal sinusitis and panhypopituitarism with intrasellar cystic lesion with homogeneous ring enhancement on computed tomography or magnetic resonance image should raise the suspicion of a pituitary abscess. Drainage of the abscess through trans-sphenoidal approach, appropriate antibiotics therapy, endocrine assessment and hormonal replacement should bring complete recovery to the patient. The authors review the literature and report two cases of primary pituitary abscess.
Osteomas of the paranasal sinuses rarely lead to intracranial manifestations. The authors report an unusual case of a frontal sinus osteoma leading to subdural emyema formation. A 19-year-old man presented with headache and fever one month after minor facial trauma. Neuroradiological studies revealed subdural empyema in left frontal lobe with moderate cerebral edema and a osteoma in the left frontal sinus with sinusitis of maxillary sinus. The patient was surgically treated in one stage operation of decompressive craniectomy, removal of subdural empyema with frontal sinus osteoma, and endoscopic sinus surgery via cranial and nasal route. The patient recovered very well after surgery and postoperative antibiotic therapy. The etiology of intracranial infection and the treatment strategy are to be discussed.
Purpose : The purpose of this study was to determine the prevalence of sinus opacification among dental paediatric patients. Materials and methods : Two hundred and eight Cone Beam Computed Tomography (CBCT) scans of dental patients under the age of 18 were reviewed for sinus opacification. Patients with any sinus-related signs or symptoms were excluded. Results : The overall prevalence of sinus opacification was 48.1 %. The ethmoid (28.4%) and maxillary (27.8%) sinuses were most frequently affected. There were no statistically significant differences for both age and gender. Conclusion : The high prevalence of sinus opacification in asymptomatic children emphasizes the necessity of clinical correlation. (Korean J Oral Maxillofac Radiol 2008; 38 : 219-23)
Purpose : The purpose of this study was to determine the prevalence of sinus opacification among adult dental patients. Materials and Methods : Five hundred and sixty-four Cone Beam Computed Tomography (CBCT) scans of dental patients over the age of 18 were reviewed for sinus opacification. Opacification was graded as clear, mild, moderate or severe. Patients with any sinus-related signs or symptoms were excluded. Results : Sinus opacification in one or more sinuses was found in 59.2% of the patients. The sinus opacification was mild in 49.8%, moderate in 8.3%, and severe in 1.1%. The maxillary (37.7%) and ethmoid (37.4%) sinuses were most frequently affected. The prevalence was higher in the older age group and showed a male predomination (p<0.05). Conclusion : Sinus opacification in asymptomatic adults is very common and emphasizes the importance of clinical correlation before deciding on the final diagnosis and treatment.

In the treatment of Nasal Polyps. which are produced with allergic rhinitis, chronic paranasal sinusitis at clinic. Herb-med., acupuncture and some medicine for external application have been used. The symptoms, however, has taken a turn for the better in temporary, the nasal polyps was not removed in completely. So the origin treatment was impossible by the use of herb-med.. acupuncture and medicine. Although a surgical operation (excision) of western-medicine is effective for a specified period of time (about 6 months), the nasal polyps tend to recur. Because of that the second and third operation might be needed, so we cannot consider the operation as the treatment of nasal polyps. The study was observed to investigate the nasal polyps treatment, and to search practical attempt for the clinic application through a literature study on Bisikyuk and Bithch which are similar with nasal polyps. As results, the practical application of external treatment for the nasal polyps treatment in clinic might be more necessary, and internal use of the Herb-med. with the external treatment might be treated as supplementary treatment.

Background: Allergic rhinitis(AR) is a heterogeneous disorder that despite its high prevalence is often undiagnosed. It is characterized by one or more symptoms including sneezing, itching, nasal congestion, and rhinorrhea. And it is frequently accompanied by symptoms involving the eyes, ears, and throat, including postnasal drainage. There are many different causes of rhinitis in children and adults. Approximately 50$\%$ of all cases of rhinitis are caused by allergy. In the case of rhinitis caused by allergens, symptoms arise as a result of inflammation induced by a gamma globulin E-mediated immune response to specific allergens such as pollens, molds, animal dander, and dust mites. The immune response involves the release of inflammatory mediators and the activation and recruitment of cells to the nasal mucosa. AR is similar to 鼻?, hypersensitive rhinitis in Oriental Medicine. I think hypersensitive rhinitis is including of AR, vasomotor rhinitis and non-allergic rhinitis related with eosinophil increased and so on. Purpose: To perform a clinical analysis of hypersensitive rhinitis including allergic rhinitis and estimate the efficacy of Oriental Medical treatment. Objective: We studied 96 patients who had visited our hospital with complaints of nasal symptoms from March 2000 to February 2002; they had the signs more than 2 - nasal obstruction, watery discharge, sneezing and eye or nasal itching. Parameters Observed & Methods: We treated them with acupuncture & herb-medication. Sometime they used aroma oil or external medicine. 1) the distribution of sex & age groups 2) the clinical type based on duration & the severity of symptom 3) the breakdown of complication & pasl history of Otolaryngologic or allergic disease 4) the clinical assessment and classification of rhinitis(sneezers and runners & blockers) 5) the associated symptoms and signs 6) the classification of Byeonjeung 7) the classification of prescriptions and 8) the efficacy of treatment. Result: 1. In the clinical type of based on duration, the intermittent type was 42.7$\%$ and the persistent was 57.3$\%$. 2. We observed the severity of symptoms based on the quality of life. The mild type was 24.0$\%$ and the moderate-severe was 76.0$\%$. 3. In the clinical assessment and classification of rhinitis, the sneezers and runners type was 69.8$\%$ and the blockers was 30.2$\%$. 4. The most common family history with otolaryngologic or allergic disease were allergic rhinitis(17.7$\%$), urticaria, paranasal sinusitis and T.B.(3.1$\%$). 5. The most common past history with otolaryngologic or allergic disease were paranasal sinusitis(14.6$\%$), atopic dermatitis and asthma(8.3$\%$). It was 31.3$\%$ they had a family history and 44.8$\%$, past history. 6. The most common complication was paranasal sinusitis(15.6$\%$). In decreasing order the others were otitis media with effusion(9.4$\%$), GERD and headache(6.3$\%$), asthma, bronchitis, nasal bleeding and allergic dermatitis(5.2$\%$). 7. Classification through Byeonjeung : ⅰ) 39 cases(34.9$\%$) were classified as showing Deficiency syndrome. The insuffficiency of Qi was 17.7$\%$, deficiency of Kidney-Yang, 12.5$\%$ and Lung-Cold, 10.4$\%$. ⅱ) 57 cases(59.4$\%$) were classified as showing Excess syndrome. The Fever of YangMing-meridian was 35.4$\%$, Lung-Fever, 24.0$\%$. 8. The efficacy of treatments showed: an improvement in 22cases(22.9$\%$); an improvement partly in 24 cases(25.0$\%$); no real improvement or changes in 16 cases(16.7$\%$); and couldn't check the results 18cases(18.6$\%$). Conclusion: We suggest that this study could be utilized as a standard of clinical Oriental Medical treatment when we treat hypersensitive rhinitis including allergic rhinitis.

Purpose: This study aimed to assess texture analysis(TA) of cone-beam computed tomography (CBCT) images as a quantitative tool for the differential diagnosis of odontogenic and non-odontogenic maxillary sinusitis(OS and NOS, respectively). Materials and Methods: CBCT images of 40 patients diagnosed with OS (N=20) and NOS (N=20) were evaluated. The gray level co-occurrence (GLCM) matrix parameters, and gray level run length matrix texture (GLRLM) parameters were extracted using manually placed regions of interest on lesion images. Seven texture parameters were calculated using GLCM and 4 parameters using GLRLM. The Mann-Whitney test was used for comparisons between the groups, and the Levene test was performed to confirm the homogeneity of variance (α=5%). Results: The results showed statistically significant differences(P<0.05) between the OS and NOS patients regarding 3 TA parameters. NOS patients presented higher values for contrast, while OS patients presented higher values for correlation and inverse difference moment. Greater textural homogeneity was observed in the OS patients than in the NOS patients, with statistically significant differences in standard deviations between the groups for correlation, sum of squares, sum of entropy, and entropy. Conclusion: TA enabled quantitative differentiation between OS and NOS on CBCT images by using the parameters of contrast, correlation, and inverse difference moment.

Purpose: Maxillary sinus elevation has been widely used to enable insertion of endosseous implants in severely resorbed maxilla. Maxillary sinusitis after this procedure was considered to be the major drawback, therefore, preoperative evaluation of paranasal sinus is considered to be important. In order to evaluate the condition of the sinus, we used Waters' projection. In this study, asymptomatic patients were evaluated by Waters' view, and compared to timing to assess the sinus cavity. Methods: The retrospective study was based on 14 patients who were performed sinus elevation surgery in Seoul National University Dental Hospital. These patients did not show any signs of maxillary sinusitis. These patients were taken Waters' view at preoperative, postoperative 1 day, 3 months, 6 months. In Waters' view, presence of air fluid level, radiopacity of sinus wall, or radiopacity of entire maxillary sinus were evaluated. The density, and sinus dimension changes were assessed using Adobe Photoshop CS5$^{(R)}$ (Adobe Systems Inc., San Jose, CA, USA). Results: Findings of Waters views in patients with clear maxillary sinus at preoperative time were followed by elevated sinus floor with transplanted bone, mucosal swelling, and air fluid level. At postoperative 3 months, and 6 months, the radiographic findings were similar to preoperative state. By contrast, patients with preoperative mucosal swelling, or haziness in sinus cavity showed radiopacity entire sinus in Waters' view. In cases of the patients who were treated with simultaneous treatment to mucosal swelling, good status of sinus cavity were found. Conclusion: Although Waters' projections provide the limited information, and is less sensitive method compared with computed tomography, it is simple, easy, and economical method to assess of maxillary sinus. We suggest using Waters' view as radiographic routine tool for evaluation of sinus condition, especially in the sinus elevation surgery.
