Rheumatoid arthritis (RA) is a connective tissue disease involving the larynx in 30 % of the patients. Foreign body sensation, hoarseness, and cough are common symptoms in laryngeal involvement. An urgent tracheostomy is required when acute airway obstruction occurs in case of bilateral vocal fold paralysis. The most common cause of bilateral vocal fold paralysis in RA patients is a cricoarytenoid joint arthritis. Laryngeal nerve degeneration is rare cause of bilateral vocal fold paralysis in RA patients. In this case report, an emergent tracheostomy was performed on a 64-years-old male patient with acute dyspnea, and concurrent involvement of RA on laryngeal nerve and cricoarytenoid joint was revealed by laryngeal electromyography and histopathology. The vocal fold mobility was restored after 3-months medical treatment.
Herpes zoster presents clinically with cutaneous vesicular eruption and pain along the dermatome, but it can sometimes cause muscular paralysis. When the disease involves cervical root, it is included in the differential diagnosis of shoulder diseases. A sixty-six year old patient, complaining of severe pain and weakness of his left shoulder, was referred to the authors as having a partial tear of the supraspinatus tendon on MRI. However, the authors found out a paralysis of the sixth cervical root in the patient by electrophysiologic studies, noting that the patient had been affected with a herpes eruption in the neck and arm two months before. Zoster paresis has been reported to be associated with the cutaneous eruption within two weeks of its onset, making its diagnosis not so difficult. The authors report a case of delayed-onset muscular paralysis after cutaneous herpes zoster, which presented just like a rotator cuff tear.
Hypoglossal nerve palsy is an uncommon neurologic disorder. We report a 67-year-old Korean male with tongue paralysis due to hypoglossal nerve palsy. He had complaints associated only with tongue paralysis and was treated with herbal medicine and electric acupuncture for 12 days. We evaluated his tongue paralysis severity by a numeric rating scale (NRS) and the angle of tongue deviation. After 12 days of treatment, the NRS score showed improvement of his complaints, and the angle of tongue deviation was decreased. Korean medicine could therefore be an effective treatment choice for hypoglossal nerve palsy.
The purpose of this study was to examine the effect of needle-embedding therapy to facial paralysis patient after acoustic neuroma surgery. The patient was treated by basic oriental medicine treatment. We evaluated the effect of Needle-Embedding Therapy by using Yanagihara's unweighted grading system and subjective satisfaction score. The final Yanagihara's score was higher than before the treatment. The patient was satisfied after Needle-Embedding Therapy. Needle-Embedding Therapy was effective against facial paralysis after acoustic neuroma surgery. Further studies will be required to identify the beneficial effect of Needle-Embedding Therapy in facial paralysis after acoustic neuroma surgery.
Facial nerve palsy is not an uncommon disease encountered at the department of otolaryngology or pain clinic. It usually occurs following viral infection(Bell's palsy, BP). However, it is rare to develop bilaterally, and that not simultaneously. We experienced a rare case of bilateral facial nerve palsy. Patient first experienced left side paralysis, then right side paralysis approximately two months later. We treated the patient with serial bilateral stellate ganglion blocks(SGB). When left side paralysis improved, we performed unilatertal SGB for right side paralysis. Patient was also treated with intermittent electro-acupuncture stimulations, to right side first, then left side. Four months of treatment provided good results. This may be the first case, in Korea, of facial nerve paralysis bilaterally within a certain interval, treated with serial SGB and electro-acupuncture stimulation. However, the etiology of this bilateral BP was not fully confirmed as being a cause of viral origin.
Purpose: The purpose of study was to determine the effects of muscle reeducation training in patients with acute facial nerve paralysis. Methods: Thirty patients were randomly assigned to either the experimental group (n=15) or the control group (n=15). The experimental group received muscle reeducation training for 20 minutes after electrotherapy and the control group received conventional electrotherapy for only 40 minutes. Therapeutic intervention for each group was performed four times per week for four weeks. The patients were measured for recovery of paralysis using the House-Brackmann Grading System (H-B grade), the Movement Distance of Mouth, Nasolabial Angle (NA), and Facial Disability Index (FDI). Results: In within group comparison, the experimental group showed significant improvements for all variables (p<0.01). In comparison between two groups, the experimental group showed relatively greater significant improvements for all variables (p<0.01). Conclusion: These findings suggest that muscle reeducation training is more effective than conventional therapy in improving the condition of patients with facial nerve paralysis. In particular, the results of this study indicate that muscle reeducation training can be recommended by clinicians since it provides more benefits.
Bilateral vocal cord paralysis can result in severe airway compromise. Over the years a variety of arytenoidectomy procedures have bee described, and one or more of these have been the gold standard for many years. A widely accepted treatment is endoscopic laser total arytenoidectomy. However, vocal results are usually poor. Objective : To evaluate the effect of treatment of endoscopic laser medial partial arytenoidectorny for bilateral vocal cord paralysis Material and Methods : We performed endoscopic medial partial arytenoidceomy with $CO_2$laser for 3 patients with bilateral vocal cord paralysis. The $CO_2$laser is operated with a continuous 7-watt beam in superpulse mode. We compared degree of dyspnea and glottic area of pre-operation with those of post-operation for 3 patients. We analysed aerodynamic study pre-operatively and post-operatively for 1 patient. Results The symptom of dyspnea was improved markedly and the glottic area was widened from 34% to 50% compared with that of pre-operation. The voice quality was slightly decreased. Tracheotomy was not necessary for not-tracheotomized patient and decanulation was possible for tracheotomized patient post-operatively. Cunclusion: The endoscopic laser medial partial arytenoidectomy is a convient and effective method for opening the posterior glottic airway.
Objectives : This study was to evaluate the effect of electroacupuncture on symptoms of peripheral facial paralysis. Methods : We investigated 34 cases of patients with Peripheral Facial Paralysis. We flip a coin and divide into two groups. Electroacupuncture was performed at one group, and the other group didn't do it. we evaluated the treatment effect of each group by using Gross Grading System of House-Brackmann, Yanagihara's Unweighed Grading System. Results : 1. In Pain back of the ear, we found that two groups have no significant differences. 2. As a result of evaluation by using Gross Grading System of House-Brackmann, Yanagihara's Unweighed Grading System, treatment score after 2weeks was marked higher than that before treatment and treatment score after 4weeks treatment was higher than treatment score after 2weeks within each group. 3. After 4weeks treatment, electroacupuncture group showed significant difference on Gross Grading System of House-Brackmann, Yanagihara's Unweighed Grading System compared with acupuncture group. Conclusion : These results suggested that electroacupuncture treatment should be more effective in the patient with peripheral facial paralysis.
Objectives : This study was to compare the effect of electroacupuncture therapy on peripheral facial paralysis according to the wave forms. Methods : We investigated 56 cases of inpatients with Peripheral Facial Paralysis, and divided them into two groups, One group was treated by electroacupuncture on interrupted wave, and the other was treated by electroacupuncture on dense-dispersed wave. We evaluated the effect of electroacupuncture in each group by using Gross Grading System of House-Brackmann and Yanagihara's Unweighed Grading System. Results : 1. In the interrupted wave and the dense-dispersed wave, compared with baseline, at final, Gross Grading System of House-Brackmann was significantly decreased and Yanagihara's Unweighed Grading System was significantly increased. 2. At final, Interrupted wave showed significant decrease on the Gross Grading System of House-Brackmann and significant increase on Yanagihara's Unweighed Grading System compared with dense-dispersed wave. Conclusions : These results suggested that interrupted wave should be more useful for improving symptoms related with peripheral facial paralysis than dense-dispersed wave.

Recent reports have described several cases of double muscle transfers to restore natural, symmetrical smiles in patients with long-standing facial paralysis. However, these complex procedures sometimes result in cheek bulkiness owing to the double muscle transfer. We present the case of a 67-year-old woman with long-standing facial paralysis, who underwent two-stage facial reanimation using two superficial subslips of the serratus anterior muscle innervated by the masseteric and contralateral facial nerves via a sural nerve graft. Each muscle subslip was transferred to the upper lip and oral commissures, which were oriented in different directions. Furthermore, a horizontal fascia lata graft was added at the lower lip to prevent deformities such as lower lip elongation and deviation. Voluntary contraction was noted at roughly 4 months, and a spontaneous smile without biting was noted 8 months postoperatively. At 18 months after surgery, the patient demonstrated a spontaneous symmetrical smile with adequate excursion of the lower lip, upper lip, and oral commissure, without cheek bulkiness. Dual-innervated muscle transfer using two multivector superficial subslips of the serratus anterior muscle may be a good option for long-standing facial paralysis, as it can achieve a symmetrical smile that can be performed voluntarily and spontaneously.
