Bilateral Alternating Bell's Palsy Treated with Stellate Ganglion Block -A case report- (양측 교대형 안면신경 마비의 치험 -증례 보고-)

  • Facial nerve paralysis is a common pain clinical diagnosis. But ipsilateral or contralateral recurrent facial paralysis is found in about 2.6~19.5% of facial paralysis and especially bilateral facial paralysis is rare. While idiopathic facial paralysis is the most common diagnosis, a comprehensive evaluation must be completed prior to this diagnosis in patients with bilateral facial paralysis. A representative case of bilateral alternating facial paralysis treated with stellate ganglion block (SGB) is presented. A 57 years old male patient who had the onset of a right facial paralysis 7 months ago visited pain clinic. Five months after the onset of right facial paralysis, as it was improving, he developed a left facial paralysis. He had history of hypertension, diabetus mellitus and pain episode on mastoid process before facial paralysis developed. Electrical test showed incomplete neuropathy on both side and computed tomography (CT) scan was normal. He was treated with SGB, physical theraphy and aspirin medication. After 25 times SGB, he was recovered almost completely.

The Clinical Study on Measurement of Foot Reflex Zone Acupoint Detection of Facial Paralysis Patients by Acupoints Detector (경혈탐측기를 이용한 말초성 안면신경마비환자의 족부반사구 변화에 대한 임상적 고찰)

  • Objectives : We investigate the characteristics of foot reflex zone acupoint of facial paralysis patients. Methods : In order to make a comparison between facial nerve paralysis patient group and non-facial paralysis group, we measured foot reflex zone acupoint detection in both group of 18 patients who were diagnosticated to facial nerve paralysis and 18 persons who were not. Results : 1. In comparing the means of the foot reflex zone, the measurements of facial nerve paralysis group is different significantly from non-facial paralysis group(p<0.05). 2. The measurement of detection of foot reflex zone acupoints, such as hypophysis(垂體), nose(鼻), cerebrum(大腦), neck(頸項), Trapezius muscle(僧帽筋), eye(眼) and ear(耳) of the facial nerve paralysis group is different significantly in comparison with non-facial paralysis group(p<0.05). But the measurement of detection of foot reflex zone acupoints, such as trigeminal nerve(三叉神經), cerebellum (小腦), kidney(腎), ureter(輸尿管) and urinary bladder(膀胱) of the facial nerve paralysis group is not defferent significantly in comparison with non-facial paralysis group(p>0.05). Conclusions : The results suggest that foot reflex zone can be used in the diagnosis and treatment of facial nerve paralysis.

Case of Vocal Cord Paralysis And Upper Limb Motor Paralysis Caused by Herpes Zoster (대상포진에 의해 발생한 성대마비 및 상지운동신경마비 1례)

  • Motor paralysis by herpes zoster is rare. The incidence of herpetic motor involvement was reported to be between 0.5-31%. This case was reported to evaluate the effects of oriental medicine therapy on vocal cord paralysis and upper limb motor paralysis caused by herpes zoster. The subject was a 48-year-old man who suffering from hoarseness, right upper limb paralysis, right upper limb weakness, right post ear pain, headache. After our treatment, clinical symptoms were improved and VAS score significantly decreased. This study suggests that herbal medicine and acupuncture complex therapy is probably effective in the treatment of vocal cord paralysis and upper limb motor paralysis.

  • Even though a rapid development of modem medical science, paralysis disease is a highly dangerous and murderous disease. Shin et al. (1978) constructed the diagnosis expert system which identify a type of the paralysis disease from symptoms of a paralysis disease patients by using the canonical discriminant analysis. The decision tree-based analysis, however, has advantages over the method used in Shin et al. (1998), such as it does not need assumptions - linearity and normality, and suggest appropriate diagnosis procedure which is easily explained. In this paper, we applied the decision tree to construct the model which Identify a type of the paralysis disease.

  • Hypokalemic periodic paralysis(HypoPP) is characterized by an abrupt onset of flaccid paralysis with a clear mentality, but muscles of speech and swallowing are usually spared. We report on three patients who suffered attacks of acute paralysis. After exclusion of central nervous system involvement, the patients showing hypokalemia was diagnosed as hypokalemic periodic paralysis, which was completely reversible on parenteral potassium substitution.

Unilateral vocal cord paralysis after open heart surgery -A report of 2 cases- (개심술후 발생한 일측성 성대마비 -2례 보고-)

  • We have experienced 2 cases vocal cord paralysis after open heart surgery. One was a postoperatively developed right unilateral vocal cord paralysis after prosthetic mitral valve replacement with tricuspid valve annuloplasty. The other was a postoperative left unilateral vocal cord paralysis after prosthetic aortic and mitral valve replacement with tricuspid annuloplasty. They were intubated for forty-eight and seventy-two hours but after extubation complained of hoarseness, aphonia, anxiety, and ineffective coughing Indirect laryngoscopy performed at about postoperative one week, revealed partial paralysis and decreased mobility of the vocal cord. After active phonation therapy, symptoms were improved gradually and in the follow up indirect laryngoscopy, the vocal cord paralysis was improved. The symptoms were recovered completely at about postoperative one month in both. The cause of vocal cord paralysis after open heart surgery may be any retraction or stretching injury to the recurrent laryngeal nerve, especially right side, during median sternotomy retraction and open heart operation procedures. As a result, avoid of excessive spread of median sternotomy retractor and excessive manipulation and retraction of the heart during open heart procedures will reduce the occurrence of the vocal cord paralysis.

Phrenic Nerve Paralysis Complicating Topical Cardiac Hypothermia During Open Heart Surgery - A Report of 4 Cases- (개심술후 합병되는 횡경신경 손상 -4례 보고-)

  • Because of increasing myocardial damage by normothermic arrest, most of cardiac surgeons now uses many kinds of method reducing myocardial injury, such as systemic hypothermia, topical cooling and cold cardioplegic solutions. And phrenic nerve paralysis has been reported with the use of iced slush for topical cooling. So we reviewed the preoperative and postoperative chest X-rays of 54 patients undergoing open heart surgery with the use of iced slush for topical cooling to find phrenic nerve paralysis. Four of 54 patients were known to have phrenic nerve paralysis. The first time known to develop phrenic nerve paralysis was from POD 4 1 day to POD 4 3 day and the phrenic nerve paralysis resolved within a month postoperatively except one. A patient have had phrenic nerve paralysis persistently over 7 months. And the effect of unilateral phrenic nerve paralysis was of no clinical significance.

  • A 10-year-old castrated male Domestic Shorthair cat visited a veterinary medical teaching hospital for emergency dyspnea. The cat was hypoxic and hypotensive, and stridorous respiration was remarkable. Visual inspection confirmed laryngeal paralysis and a lack of mobility of the left larynx. Megaesophagus, aspiration pneumonia, cranial mediastinal mass, and positive Tensilon test results using neostigmine were observed, indicating acquired myasthenia gravis secondary to thymic neoplasia. After 10 minutes of neostigmine 0.02 mg/kg IV administration, laryngeal paralysis and dyspnea resolved. Histopathlogical examination for the cranial mediastinal mass after surgical resection confirmed malignant thymoma. Here, we report a case of acquired myasthenia gravis in a cat with a malignant thymoma that presented with life-threatening dyspnea due to laryngeal paralysis. Feline laryngeal paralysis is uncommon, and myasthenia gravis, a cause of laryngeal paralysis in cats, has not yet been reported. Myasthenia gravis should be considered in cats with laryngeal paralysis.

  • Facial nerve paralysis(or Bell's palsy) which commonly occurs unilaterally, gives rise to paralysis of facial expression muscle. This condition is classified into symptomatic facial nerve paralysis due to intracranial tumor, post operative trauma, etc. and idiopathic facial nerve paralysis. To explain the etiology of idiopathic facial nerve paralysis, many hypothesis including ischemic theory, viral infection, exposure to cold, immune theory etc. were suggested, but there is no agreement at this point. The method to evaluate the facial nerve paralysis, when it occurs, consists of three stage scale method, image thechnics like CT and MRI, laboratory test to examine the antibody titers of viral infection, neurophysiologic test to evaluate the degree and prognosis of paralysis. Treatment includes medication, stellate ganglion block(SGB), surgery, physical therapy and other home care therapy. In medication, systemic steroids, vitamins, vasodilating-drug and ATP drugs were used. SGB was also used repeatedly to attempt the improvement of circulation and to stimulate the recovery of nerve function. Physical therapy including electric acupuncture stimulation therapy(EAST) and hot pack was used to prevent the muscle atrophy. When No response was showed to this conservative therapies, surgery was considered. After treating two patients complaining of Bell's palsy with medication(systemic steroids) and EAST, favorable result was obtained. so author report the case of facial nerve paralysis.

Clinical Study of Bell's Palsy with DITI and Nerve Conduction Test(EN0G and EMG) (DITI 및 전기신경전도검사(EN0G 및 EMG)를 이용한 구안와사(Bell's palsy) 환자에 대한 임상적 고찰)

  • The clinical data and thermographic imaging were analyzed on the 38 cases of Bell's palsy who were treated admission in the Oriental Medicine Hospital of Wonkwang University from January 2002 to May 2003. 38 patients with Bell's palsy were within one week after the onset of the paralysis, and thermal type in the DITI were hypo or hyper generally. Nerve conduction test(ENOG and EMG) examined in two weeks after onset. We studied interaction effect between thermal type and paralysis grade on admission day. We studied each main effect ; paralysis grade on admission day - nerve conduction test, nerve conduction test - thermal type, thermal type - paralysis grade after 4 weeks, paralysis grade on admission day - paralysis grade after 4 weeks, sasang constitution - nerve conduction test. The following results were obtained that interaction effect between thermal type and paralysis grade on admission day showed no significance, each main effect ; paralysis grade on admission day - nerve conduction test, paralysis grade on admission day - paralysis grade after 4 weeks, showed significance, each main effect ; nerve conduction test - thermal type, thermal type - paralysis grade after 4 weeks, sasang constitution - nerve conduction test, showed no significance.
