A Study on the Relationship between Emotion Related to Pap Smear and Continuous Participation in Pap Smear Screening in Married Korean Women (기혼여성의 Pap 도말검사 경험관련 정서와 검진 참여정도와의 관계연구)

  • The purpose of this study was to determine the relationship between emotion related to Pap smear and continuous participation in Pap smear screening by married women. The subjects for this study were 209 married women who had at least one previous Pap smear. The data were collected using a self reported survey questionnaire which was developed by the researchers. It's Cronbach's alpha score ranged from .8799 to .9118. Data analysis was done by descriptive statistics. ANOVA, t-test and Pearson correlation using the SPSS Win program. The results are as follows: 1. The mean for continuous participation in Pap smear screening over the past three years was 1.95. 2. The mean score for negative emotions related to Pap smear screening was 3.2. 3. The mean score for positive emotions related to Pap smear screening was 3.9 4. Positive correlations were found between positive emotion and continuous participation in Pap smear screening (r=.417, p<.05). A weak negative correlation was found between negative emotion and continuous participation in Pap smear screening (r=-.213, p<.05). In conclusion, negative and positive emotions had a significant effect on continuous participation in the Pap smear screening among married women. This suggests that reducing negative emotions and activating messages which relate to positive emotions may be an effective strategy for increasing continuous participation in Pap smear screening.

Factors Influencing on Cervical Cancer Screening Acceptance Using Pap Smear in Women Over 20 Years Old (20세 이상 여성들의 Pap 도말검사를 통한 자궁암 검진 수용도의 영향요인)

  • This study was attempted to identify the factors influencing on cervical cancer screening acceptance using Pap smear in women over 20 years old. The subjects of this study were 192 screening acceptance group who visited health examination center. Data were collected by interview and self-administered questionnaire from April 22 through May 24, 2013. The results of this study as follows. Firstly, the response rate(45.8%) of non-screening acceptance group who is not a religion was significantly higher than the response rate(32.3%) of screening acceptance group($X^2=4.82$, p<.05). Secondly, the response rate(61.5%) of non-screening acceptance group who performed breast-feeding was significantly higher than the response rate(46.9%) of screening acceptance group($X^2=0.61$, p<.05). Thirdly, screening acceptance group who performed other screening was significantly higher 4.62 times than non-screening acceptance group(95% Cl=1.15-9.84, P=0.006). Based on the results of this study, it necessary to provide more cervical cancer screening information which can change the negative attitude among the public toward cancer screening acceptance using Pap smear.

Factors related to Continuous Participation in the Pap Smear Screening among Korean Women: using a Structural Model (한국여성의 Pap 도말검사 지속적 참여행위에 관한 설명모델)

  • The purpose of this study was to develop an explanatory model to predict factors related to continuous participation in the Pap smear screening among Korean women. A hypothetical model was constructed on the basis of Health Belief Model and extensive review of literature on the Pap smear screening. Exogeneous variables included in this model were knowledge, perceived sensitivity, perceived severity, negative and positive emotional responses and professional support from physicians and nurses. Endogeneous variables were threat to cervical cancer, perceived benefit of the Pap smear screening, perceived barrier, and the final outcome variable was continuous participation in the Pap smear screening. The hypothetical model was tested with an empirical study. The data was collected from 623 married women whose age range was 24 - 83 using a self-reported survey questionnaire which was developed by the researcher. It's Cronbach's alpha score ranged from .6478 to .9118. Data was collected at different locations in Seoul; a university hospital, a local health center, and apartment complexes. Data analysis was done using SPSS 7.5 WIN Program for descriptive statistics and LISREL 8.12a WIN Program for covariance structural analysis. In conclusion, threat, perceived benefit, perceived barrier, positive emotion and professional support had a significant direct effect on continuous participation in the Pap smear screening among Korean women. The results of this study also showed that perceived barrier had the most significant direct effect on continuous participation in the Pap smear screening while negative emotional response had the most significant direct effect on perceived barrier. It can be suggested that decreasing perceived barrier by reducing negative emotional responses may be the most effective strategies for increasing continuous participation in the Pap smear screening among married Korean women.

Relating Factors in Pap Smears by Stages of Change among Married Nurses (기혼간호사 대상 자궁경부세포 검진의 변화 단계에 따른 관련요인)

  • Purpose: This study was to examine the differences in health belief variables and HPV (Human Papillomavirus) knowledge in Pap smears according to the stages of change using Trans-theoretical Model (TTM) among married nurses. Methods: The research design was a cross sectional survey. Participants were 387 married nurses working in general hospital located metropolitan city. Measurement variables were perceived threatening, perceived severity, benefits of Pap, HPV knowledge and stages of the change in Pap smear. Results: Classified stages were pre-contemplation 3.9%, contemplation 17.8%, relapsing 29.5%, action 31.0%, and maintenance 17.8%. Accuracy rate of HPV knowledge were low ranging from 15% to 45%. Among the variables, perceived threatening (F=3.56, p=.007), perceived severity (F=9.73, p<.001) and HPV knowledge (F=7.52, p<.001) were significantly different by stages of change. Conclusion: Application of TTM to Pap smears was efficient to know the nurse's level of health behaviors, Health belief variables and HPV knowledge were main factors to identify the stages of change. The continuation of Pap smears and the education in relation to HPV knowledge should be encouraged for married nurses.

Liquid-Based Cytology Using $MonoPrep2^{TM}$ System in Cervicovaginal Cytology: Comparative Study with Conventional Pap Smear and Histology (고식적 직접 도말법과 $MonoPrep2^{TM}$ system 법에 의한 자궁경부질 세포검사 성적의 비교 검토)

  • We compared the diagnostic accuracy of liquid-based cervicovaginal cytology using $MonoPrep2^{TM}$ system (Monogen, Herndon, Virginia, USA), a manual system based on membrane filtration method, with conventional Pap smear. Study population included 92 patients visiting the gynecologic department under the suspicion of uterine cervical disease. In thirty of them, surgical biopsy was performed. $MonoPrep2^{TM}$ system provided well-preserved monolayer specimen with good nuclear morphology. However, about 19% of specimens were inadequate to interpret due to low cellularity. The detection rate of abnormal cells more than ASCUS (atypical squamous cells of unknown significance) was 23.9% and higher than 19.4 % of conventional Pap smear. Diagnostic concordance rate with conventional Pap smear was 81%, and severe discordance rate influencing on the management of patient was 7.6 %. Among these seven cases, $MonoPrep2^{TM}$ system was more diagnostic only in four. In comparison with histology, the sensitivity of diagnosis of $MonoPrep2^{TM}$ system was 78.9% and slightly higher than 73.5% of conventional Pap smear. However, the specificity was 81.1% and lower than 90.9% of Pap smear. In conclusion, $MonoPrep2^{TM}$ system provided diagnostic accuracies similar to the conventional Pap smear. The inexpertness of slide preparation and the low cellularity were considered to endow a limitation in more accurate evaluation.

The Relationship between Conical Pap. Smear Findings and Related Factors for Uterine Cervical Cancer in Ullungdo Females (울릉도 여성들의 자궁경부 세포학적 검사소견과 관련요인과의 관계)

  • To study the incidence and epidemiological factors of uterine cervical cancer in medical underserved area females, the questionnaire survey and Pap. smear for uterine cervical cancer was done on total 330 women who lived in Ullungdo from 5th to 12th August, 1998. The results were summarized as follows: The age distribution of subjects was 50s (24.5%), 60s (24.5%) and their educational level was “no schooling” (14.2%) and “elementary school” (42.7%). The first coital age of subjects was 19∼21yrs (30.0%), 16∼18yrs (13.9%) and the first pregnancy age was 22∼24yrs (36.7%) and 19∼21yrs (30.0%). The frequency of total pregnancy of subjects was over 5 times (52.1%). The frequency of total delivery was “3∼4 times” (35.5%) and “5∼6 times” (15.2%). 68.8% of subjects had experience of abortion and 80.0% of their husband were on the phimosis. 172 (52.1%) subjects had gynecological symptoms, their symptoms were leukorrhea (48.3%), pruritus (21.5%) and leukorrhea with pruritus (20.3%). 63.9% of total subjects have been received Pap. smear and the frequency of their Pap. smear was “only 1 time” (44.1 %), “irregularly” (30.3%) and the reason of respondents who have not been received Pap. smea. was “no specific symptom” (51.3%). Among the 330 women screened there were negative (45.8%), inflammation (47.3%), trichomoniasis and candidiasis (1.8%), atypical cells (4.5%) and dysplasia (0.6%).

A Study on Nucleus Extraction of Uterine Cervical Pap-Smears (자궁 경부진 핵 추출에 관한 연구)

  • If detected early enough, cervical cantor may have a good survival rate due to its preneoplastic state. However, the process is so time consuming that a medical expert can handle only a small amount of such examinations. In this paper, we propose a new nucleus extraction algorithm for uterine cervical pap smears in order to mitigate such burdens of medical experts. In the preneoplastic state cytodiagnosis images, it is important to differentiate three main areas - background, cytoplasm and nucleus. Thus, we apply lighting compensation and $3{\times}3$ mask of B channel in order to restore damaged image and remove noises respectively. The cell object is obtained from those clean binarized images with Grossfire algorithm. When there are clusters of cells, the target nucleus can be obtained with repetitive binarization of R channel brightness. In our experiment of using uterine cervical pap smears of 400 magnifications that is common in the diagnostic cytology, our method is able to extract 40 nucleus out of 45 population successfully.

Risk Factors of Cervical Cancer and Results of Cervix Cytology Screening in Chungnam Province, Korea, 1995-1999 (최근 5년간(1995-1999) 충남지역자궁경부세포진 검진결과 및 이상소견위험요인분석)

  • The purpose of this study was to evaluate results of cervix cytology screening in the community and to determine the risk factors of cervical abnormality. Branch of Daejeon city and Chungnam Province, The Planed Parenthood Federation of Korea had conducted cervical cancer screening of 146,848 married women in Chungnam province from 1995 to 1999. Cervical cancer screening was Pap smear using cytolgic brush swab by trained nurse. Women who had abnormal finding of 1st Pap smear screening were followed re- examination and 2nd close examination. Crude prevalence rate of cytologic abnormalities for 1st screening results was 0.63% in 1995- 1999(1995 0.68%, 1996 0.59%, 1997 0.70%, 1998 0.56%, 1999 0.62%). Crude prevalence rate of above class III for 1st screening results was 0.61%, but crude prevalence rate of above class III for the results of re- examination and 2nd close examination was 2.2/ 1000. The false-positive rate of class III, IV and V for positive findings were defined above class II(cervicitis) results were 52.6%, 26.9% and 19.0%, respectively. And the false- positive rate of class III, IV and V for positive findings were defined above class III(dysplasia) results were 75.3%, 46.2% and 47.6%, respectively. Major predictors of risk factors for abnormal results of cervix screening on the multiple logistic regression were age, educational attainment and living area. The false-positive rate of cervix cytology screening in the community were highest result so cervix cytology screening should be improve for better diagnostic power. And the finding of logistic regression would be understand within the limit of experimental trials on the relationship between cervical disease and risk factors.

The Usefulness of $SurePath^{TM}$ Liquid-Based Smear in Sono-Guided Thyroid Fine Needle Aspiration; a Comparison of a Conventional Smear and $SurePath^{TM}$ Liquid-Based Cytology (초음파 유도 갑삼샘 세침흡인 세포검사에서 $SurePath^{TM}$ 액상세포검사의 유용성; 고식적 도말검사와 $SurePath^{TM}$ 액상세포검사와의 비교)

  • Sono-guided fine needle aspiration (FNA) of the thyroid is widely used, but the aspirated samples are typically not well preserved and low cellularity makes diagnosis difficult in many cases. The object of the current study is to evaluate the adequacy and diagnostic accuracy of the use of $SurePath^{TM}$ liquid-based cytology (SP-LBC) in the sonoguided fine needle aspiration of the thyroid nodule and to compare its use with that of the use of a conventional smear (CS). A total of 172 sono-guided FNAs of thyroid nodules from April to June, 2006 were prepared by the use of the split method with either SP-LBC or CS; the samples were stained with the use of hematoxylin-eosin (H&E) and Papanicolaou (Pap) stains. A cyto-histological correlation was performed in 69 (30 SP and 39 CS) cases that had been histologically confirmed. The rate of producing unsatisfactory slides by the use of the SP-LBC method (9.3%) was less than that of the use of the CS method (20.9%). The diagnostic accuracy of the SP method (93.3%) was better than that of the CS method (85.3%). The sensitivity and specificity of the SP method (94.4% and 92.3%) was better than that of the CS method (83.3% and 70%), respectively (p < 0.05). The CS of sono-guided aspirated specimens had some unavoidable limitations related to inadequate sampling such as a bloody background, low cellularity and an indication that some clinicians smeared many useless slides (averaging four to ten slides), and that most slides showed only blood that included few follicular cells. The SP method resulted in more thinly smeared slides and showed cleaner background and greater cellularity than the use of the CS method. Each follicular cell shows superior nuclear detail, and more distinct cytoplasmic features than with the use of the CS method. SP-LBC appears to be an easy, highly accurate, and reliable cytological method for employ for a diagnostic approach of thyroid disease and thyroid nodules. The SP-LBC method is a suitable alternative to the CS method to overcome diagnostic difficulties.

Prevalence of Human Papillomavirus by DNA Chip Test in Women (여성에 있어 DNA 칩검사에 의한 인유두종바이러스 감염률의 조사)

  • We determined the prevalence of human papillomavirus (HPV) by DNA chip test in 549 women and cytologic diagnosis. 237 of 549 women (43.17%) subjected with HPV DNA Chip examination were found positive for HPV. 210 (88.60%, High group) were infected with high-risk HPV types. 17 (7.17%, Low group) were infected with low-risk HPV types (6, 11, 40, 44, 70) and 17 (7.17%, Mixed group) were infected with mixed types. According to age, in their twenties, thirties, forties, fifties and over sixties, the prevalence of infection with high-risk HPV types were 1.26% (3/237), 15.61% (37/237), 31.65% (75/237), 23.21% (55/237), and 13.92% (33/237), respectively. In the Low and Mixed group, percentages of infection with HPV were significantly lower than that of the High group. On the comparison of cytologic diagnosis (224 women) by Pap smear and DNA chip positive (237 women) for HPV, 132 out of 194 cases in the High group (68.04%) suffered cervical lesions with ASCUS (atypical squamous cells of undetermined significance, 7.22%), LSIL (low grade squamous intraepithelial lesion, 15.98%), HSIL (high grade SIL, 23.20%) and ICC (invasive cervical cancer, 21.65%). The Low group (14/224 women) showed 1 case of ASCUS and 6 cases of LSIL, whereas the Mixed group (4/224 women) had only 2 cases of ASCUS. According to the HPV subtypes, the high-risk types 16 and 18 induced 26 and 7 cases of ICC, respectively, whereas other HPV subtypes induced lower or no ICC incidence. In conclusion, the present data imply that the prevalence of HPV was 43.17%, high-risk HPV type 16 is a major factor, which causes precancerous and/or cervical cancer in woman and that HPV DNA chip is an accurate and useful tool for detecting HPV.
