Osteomyelitis in an Osteopathia Striata with Cranial Sclerosis Patient

  • Osteopathia striata with cranial sclerosis (OS-CS) is characterized by linear bone dysplasia at the long bone radiographically and sclerotic change at the cranium. The purpose of this case report is to study the symptoms and treatments of osteomyelitis in a patient with OS-CS. A 41-year-old patient had pus discharge from a fistula at the mental region and increase in radiolucencies with sequestra in panoramic radiograph images. Computed tomography (CT) as well as radiograph images for the whole skeleton were taken. The patient was diagnosed with OS-CS. Sequestrectomy and fistulectomy were performed. The patient recovered and no relapse occurred within six months after surgery. For diagnosis of OS-CS, CT and additional radiograph images for the whole skeleton are required. Because of the increased bone density, this patient is prone to relapse after sequestrectomy. Therefore, the surgeon must minimize trauma with the least incision and exfoliation, and preoperative antibiotics.

Multiple fibromyxomas of the jaws: A case report

  • Fibromyxoma of the jaw is a rare benign mesenchymal odontogenic tumor with locally aggressive behavior. In the present report, a 13-year-old female patient presented to our university hospital with delayed eruption of some of her teeth. A panoramic radiograph taken at the initial examination revealed four pericoronal radiolucencies related to the four third molars. Thereafter, a magnetic resonance imaging (MRI) examination was performed. After the surgical removal of these molars, the microscopic examination diagnosed the four lesions as fibromyxomas. Here, we have discussed the clinical, panoramic radiography, MRI, and histopathological findings of the case.

  • The author has evaluated the panoramic image clarity of the midfacial anatomic structures in dry skull according to the skull position. The radiopaque markers were attached to the anatomic structures: infraorbial rim, upper and lower borders of zygomatic arch, pterygomaxillary fissure, lateral pterygoid plate, pyriform aperture of nasal cavity, lateral wall of maxilla, orbital floor, infraorbital foramen, and nasal floor. Position of the skull were divided into four groups. standard, 25mm forward, chin-down, chin-up position. The results were as follows: 1. The pyriform aperture of nasal cavity, lateral wall of the maxilla, orbital floor, infraorbital foramen and nasal floor did net cast any discernible image. 2. Nearly all images of midfacial structures were blurred in the chin-up position. 3. The forward position provided good visualization of the maxillary sinus. 4. The chin-down position provided good visualization of the zygomatic arch, pterygomaxillary fissue, and lateral pterygoid plate.

External root resorption after orthodontic treatment: a study of contributing factors

  • Purpose : The purpose of this study was to examine the patient- and treatment-related etiologic factors of external root resorption. Materials and Methods : This study consisted of 163 patients who had completed orthodontic treatments and taken the pre- and post-treatment panoramic and lateral cephalometric radiographs. The length of tooth was measured from the tooth apex to the incisal edge or cusp tip on the panoramic radiograph. Overbite and overjet were measured from the pre- and post-treatment lateral cephalometric radiographs. The root resorption of each tooth and the factors of malocclusion were analyzed with an analysis of variance. A paired t test was performed to compare the mean amount of root resorption between male and female, between extraction and non-extraction cases, and between surgery and non-surgery groups. Correlation coefficients were measured to assess the relationship between the amount of root resorption and the age in which the orthodontic treatment started, the degree of changes in overbite and overjet, and the duration of treatment. Results : Maxillary central incisor was the most resorbed tooth, followed by the maxillary lateral incisor, the mandibular central incisor, and the mandibular lateral incisor. The history of tooth extraction was significantly associated with the root resorption. The duration of orthodontic treatment was positively correlated with the amount of root resorption. Conclusion : These findings show that orthodontic treatment should be carefully performed in patients who need the treatment for a long period and with a pre-treatment extraction of teeth.

  • Panoramic radiographs are a relatively simple technique that is commonly used in all dental specialties. In panoramic radiographs, in addition to the formation of real images of metal objects, ghost images may also form, and these ghost images can hinder an accurate diagnosis and interfere with the accuracy of radiology reports. Dentists must understand the formation of these images in order to avoid making incorrect radiographic diagnoses. Therefore, the present study sought to present a study of the formation of panoramic radiograph ghost images caused by metal objects in the head and neck region of a dry skull, as well as to report a clinical case in order to warn dentists about ghost images and to raise awareness thereof. An understanding of the principles of the formation of ghost images in panoramic radiographs helps prevent incorrect diagnoses.

Radiographic evaluation of the maxillary sinus prior to dental implant therapy: A comparison between two-dimensional and three-dimensional radiographic imaging

  • Purpose: This study was performed to evaluate the diagnostic efficacy of panoramic radiography and cone-beam computed tomography (CBCT) in detecting sinus pathology. Materials and Methods: This study was based on a retrospective evaluation of patients who had undergone both a panoramic radiograph and a CBCT exam. A total of 100 maxillary sinuses were evaluated. Four examiners with various levels of expertise evaluated the images using a five-point scoring system. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the diagnostic efficacy of the two modalities. The image analysis was repeated twice, with at least two weeks between the evaluation sessions. Interobserver reliability was assessed using Cronbach's alpha, and intraobserver reliability was assessed using Cohen's kappa. Results: Maxillary sinus pathology was detected in 72% of the patients. High interobserver and intraobserver reliability were observed for both imaging modalities and among the four examiners. Statistical analyses using ROC curves demonstrated that the CBCT images had a larger area under the curve (0.940) than the panoramic radiographs (0.579). Conclusion: Three-dimensional evaluation of the sinus with CBCT was significantly more reliable in detecting pathology than panoramic imaging.

Age Estimation with Panoramic Radiomorphometric Parameters Using Generalized Linear Models

  • Purpose: The purpose of the present study was to investigate the correlation between age and 34 radiomorphometric parameters on panoramic radiographs, and to provide generalized linear models (GLMs) as a non-invasive, inexpensive, and accurate method to the forensic judgement of living individual's age. Methods: The study included 417 digital panoramic radiographs of Korean individuals (178 males and 239 females, mean age: 32.57±17.81 years). Considering the skeletal differences between the sexes, GLMs were obtained separately according to sex, as well as across the total sample. For statistical analysis and to predict the accuracy of the new GLMs, root mean squared error (RMSE) and adjusted R-squared (R2) were calculated. Results: The adjusted R2-values of the developed GLMs in the total sample, and male and female groups were 0.623, 0.637, and 0.660, respectively (p<0.001), while the allowable RMSE values were 8.80, 8.42, and 8.53 years, respectively. In the GLM of the total sample, the most influential predictor of greater age was decreased pulp area in the #36 first molar (beta=-26.52; p<0.01), followed by the presence of periodontitis (beta=10.24; p<0.01). In males, the most influential factor was the presence of periodontitis (beta=9.20; p<0.05), followed by the number of full veneer crowns (beta=2.19; p<0.001). In females, the most influential predictor was the presence of periodontitis (beta=18.10; p<0.001), followed by the tooth area of the #16 first molar (beta=-11.57; p<0.001). Conclusions: We established acceptable GLM for each sex and found out the predictors necessary to age estimation which can be easily found in panoramic radiographs. Our study provides reference that parameters such as the area of tooth and pulp, the number of teeth treated, and the presence of periodontitis should be considered in estimating age.

A retrospective institutional study of human age determination by evaluating the pulp length and width ratio of the maxillary lateral incisor on panoramic radiographs in Indonesian subjects

  • Purpose: The pulp length to width (PL/W) ratio of the maxillary lateral incisor can be used as an age determination method. This study aimed to investigate the correlation between the PL/W ratio of the maxillary lateral incisor on panoramic radiographs and human chronological age in Indonesian subjects. Materials and Methods: This study analyzed with 134 maxillary lateral incisors on 113 panoramic radiographs from patients who visited the Oral and Maxillofacial Radiology Unit of Dental Hospital Universitas Padjadjaran, Bandung, Jawa Barat, Indonesia, from 2013 to 2018 (age range: between 11 to 60 years). The pulp length was measured from the pulp chamber roof to the apical foramen, and the pulp width was measured on the cervical area of the cementoenamel junction in millimeters using Fiji ImageJ open-source software. Simple linear regression (in SPSS) was used to analyze the results. The reliability of the observers was evaluated. Results: The PL/W ratio of the maxillary lateral incisor was significantly correlated with chronological age (P<0.01). No statistically significant difference was found in the PL/W ratio between the left and right maxillary lateral incisors(P=0.333). There was a very strong correlation (r=0.939) between the PL/W ratio of the maxillary lateral incisor and human chronological age, with the following formula: age= -3.057+1.875×PL/W ratio (R2=0.882, standard error of estimate: 4.659). Conclusion: The PL/W ratio of the maxillary lateral incisor on panoramic radiograph can be used for age determination in Indonesian subjects.

Comparison of Multi-Label U-Net and Mask R-CNN for panoramic radiograph segmentation to detect periodontitis

  • Purpose: Periodontitis, the most prevalent chronic inflammatory condition affecting teeth-supporting tissues, is diagnosed and classified through clinical and radiographic examinations. The staging of periodontitis using panoramic radiographs provides information for designing computer-assisted diagnostic systems. Performing image segmentation in periodontitis is required for image processing in diagnostic applications. This study evaluated image segmentation for periodontitis staging based on deep learning approaches. Materials and Methods: Multi-Label U-Net and Mask R-CNN models were compared for image segmentation to detect periodontitis using 100 digital panoramic radiographs. Normal conditions and 4 stages of periodontitis were annotated on these panoramic radiographs. A total of 1100 original and augmented images were then randomly divided into a training (75%) dataset to produce segmentation models and a testing (25%) dataset to determine the evaluation metrics of the segmentation models. Results: The performance of the segmentation models against the radiographic diagnosis of periodontitis conducted by a dentist was described by evaluation metrics(i.e., dice coefficient and intersection-over-union [IoU] score). MultiLabel U-Net achieved a dice coefficient of 0.96 and an IoU score of 0.97. Meanwhile, Mask R-CNN attained a dice coefficient of 0.87 and an IoU score of 0.74. U-Net showed the characteristic of semantic segmentation, and Mask R-CNN performed instance segmentation with accuracy, precision, recall, and F1-score values of 95%, 85.6%, 88.2%, and 86.6%, respectively. Conclusion: Multi-Label U-Net produced superior image segmentation to that of Mask R-CNN. The authors recommend integrating it with other techniques to develop hybrid models for automatic periodontitis detection.

양성 종양과 유사한 형태를 가진 발육성 타액선 골결손 (Benign Tumor-Like Developmental Salivary Gland Defect)

  • 50세 여자 환자와 남자 환자가 파노라마 방사선사진에서 우연히 발견된 무증상의 하악 구치부의 방사선투과성 병소로 인해 의뢰되었다. 임상검사에서 특이할 만한 소견은 발견되지 않았다. 파노라마 방사선사진에서 각각 우측과 좌측 하악 구치부에서 경계가 비교적 명확하며, 과골성 변연을 수반하지 않는 원형의 방사선투과성 병소가 관찰되었다. 병소 내부는 다방성의 양상이었으며, 또한 병소 부위를 주행하는 하악관의 불연속성 소견이 관찰되었다. 이에 양성 종양을 배제하기 위해서 조영증강 전산화단층사진을 촬영하였다. 전산화단층사진의 축상면에서 불규칙한 경계를 가지는 명확한 설측 함요부가 관찰되었으며, 내부는 지방 및 일부 악하선 조직으로 채워져 있었으나 다른 질환을 의심할 만한 연조직 종괴는 관찰되지 않았다. 최종적으로 비전형적 형태의 발육성 타액선 골결손으로 진단하였으며, 이 중 한 증례에서, 4개월 후의 재검사시 촬영한 파노라마 방사선사진에서 뚜렷한 변화가 관찰되지 않았다. 본 증례들과 같은 비전형적 발육성 타액선 골결손을 다른 질환과 감별하기 위해서는 단면 및 내부 구조물에 대한 보다 정확한 평가가 이루어져야 하므로 일반방사선사진뿐만 아니라 전산화단층영상 또는 자기공명영상이 추가적으로 필요하리라 생각된다.
