측두 하악 관절 장애 환자의 파노라마 영상과 cone beam형 전산화 단층 영상의 비교 (Comparison of bony changes between panoramic radiograph and cone beam computed tomographic images in patients with temporomandibular joint disorders)

  • 본 연구는 교정 진단 및 치료 계획에 어려움을 줄 수 있는 측두 하악 관절 장애 증상을 보이는 환자의 cone beam computed tomography (CBCT) 영상과 파노라마 영상을 비교하여 파노라마 영상의 유용성을 알아보고자 하였다. 2008년 6월부터 2008년 11월까지 측두 하악 관절 부위의 이상 증상으로 치과 의원에 내원한 환자를 대상으로 임상 진단과 파노라마 촬영을 시행 후 CBCT로 촬영한 106명, 212개 관절을 대상으로 영상의 결과를 비교하였다. 2명의 치과의사가 하악 과두의 골변화의 양상을 관찰하여 정상(normal), 편평화(flattening), 골경화(sclerosis), 골증식체(osteophyte), 침식(erosion)으로 나누었다. 그 결과로 첫째, 검사자간 신뢰도에서 파노라마(weighted kappa: 0.714), CBCT (weighted kappa: 0.727) 각각의 영상 진단 일치도가 높았다. 둘째, CBCT 영상에 대한 파노라마 영상의 A 검사자의 민감도는 82.4%, 특이도는 58.1%였으며 B 검사자는 각각 84.3%, 61.5%였다. 셋째, 파노라마 영상과 CBCT 영상이 5% 유의수준에서 두 영상 간 판독이 동일하지 않았다. 이상의 결과는 파노라마 영상이 CBCT 영상과 비교할 때 비교적 높은 80% 이상의 민감도를 보여 측두 하악 관절 골 변화의 일차적인 진단수단으로 임상적으로 유용하게 사용될 수 있다는 것과 측두 하악 관절의 골 변화가 파노라마 영상에서 불분명한 경우 CBCT를 사용하였을 때 더욱 정밀한 진단이 될 수 있다는 것을 보여주었다.

파노라마방사선사진에서 석회화가 발견되는 경동맥의 협착 정도 (Stenosis of calcified carotid artery detected on Panoramic Radiography)

  • Purpose : This study aimed to investigate the luminal stenosis of the internal carotid artery with calcification detected on panoramic radiographs. Materials and Methods : This study used fifty carotid arteries of 36 dental patients whose panoramic radiograph and computed tomography angiography (CTA) revealed the presence of carotid artery calcification. A neuroradiologist interpreted CTA to determine the degree of stenosis of the internal carotid arteries. The degree of stenosis was stratified in four stages; normal (no stenosis), mild stenosis (1-49%), moderate stenosis (50-69%) and severe stenosis (70-99%). Results : Among the fifty carotid arteries with calcification detected on both panoramic radiography and CTA, 20 carotid arteries (40%) were normal, 29 carotid arteries (18%) had mild stenosis, 1 carotid artery (2%) had moderate stenosis, and there was none with severe stenosis. Conclusion : Sixty percent of the carotid arteries with calcification detected on both panoramic radiography and CTA had internal luminal stenosis, and two percent had moderate stenosis. When carotid atheroma is detected on panoramic radiograph, it is possible that the dental patient has luminal stenosis of the internal carotid artery.

치아의 순(협)설 경사도 변화에 따라 파노라마 방사선 사진에 나타난 치아 길이 및 각도 변화 (Change in tooth length and angulation on panoramic radiographs taken at different labiolingual and buccolingual inclinations)

  • 파노라마 방사선 사진은 치아 및 주위 조직에 대한 많은 정보를 제공해 주며 교정치료 동안에도 치근의 근원심 경사도나 길이 평가를 위해 흔히 이용되고 있지만 파노라마 방사선 사진에서 보이는 치아의 근원심 각도와 길이는 순(협)설 경사도에 의해 영향을 받는 것으로 알려져 있다. 따라서 본 연구에서는 치아의 근원심 경사도를 정상교합자의 평균 근원심 각도로 유지하면서 순(협)설 경사도만 변화시킬 때 파노라마 방사선 사진에서 각 치아의 근원심 경사도와 길이가 어떻게 변화되는지 알아보고자 하였다. 성인 정상 교합자 30명의 평균 악궁을 기초로 하여 아크릴 모델을 제작한 후 치아를 대신하는. 와이어를 정상 근원심 및 순(협)설 경사도$(I^{\circ})$로 식립하고 여기에 순(협)설 경사도 만 $I-15^P{\circ}$에서 $I+15^{\circ}$까지 $5^{\circ}$씩 변화시켜 파노라마 방사선 사진을 촬영한 후 방사선 사진에 나타난 와이어의 길이와 각도를 각각 계측하였다. 와이어를 정상 근원심 및 순(협)설 각도로 식립했을 때 방사선 사진에서 계측된 길이는 중절치, 측절치, 견치의 경우 실제 길이의 $111%{\sim}117%$의 확대율을 보였고, 소구치와 대구치에서는 $121%{\sim}125%$의 확대율을 보였다. 이 때 근원심 경사도는 그대로 유지하면서 순(협)설 경사도만 $I-15^{\circ}$(순측경사)에서 $I+15^{\circ}$ (설측경사)로 변화시킨 경우 중절치 (p<0.01)와 측절치 (p<0.05)에서만 유의한 확대율 증가를 보였다. 파노라마 방사선 사진에서 치아의 경사도는 대부분 실제 근원심 경사도보다 더 크게 즉 원심경사된 것으로 계측되었고, 이러한 원심경사 경향은 측절치와 견치에서 가장 크게 나타났으며, 제1, 2소구치의 $I+10^{\circ},\;I+15^{\circ}$ 그룹과 제1, 2대구치의 $I+15^{\circ}$ 그룹에서만 실제 근원심 각도보다 더 작게, 즉 근심 경사된 것으로 계측되었다. 또한 순(협)설 경사도의 변화에 따라 파노라마 방사선 사진에서 모든 치아의 근원심 각도가 유의한 변화를 보였는데, 순(협)설 경사도가 작아질수록, 즉 순(협)측 경사될수록 파노라마 방사선 사진에서 더 원심경사되어 나타났다. 따라서 파노라마 방사선 사진에서 중절치와 측절치의 치아 길이와 모든 치아의 근원심 경사도를 평가할 때 치아의 순(협)설 경사도에 따라 영향을 받기 때문에 이를 고려해야 할 것이다.

안양시에서 치과용 파노라마방사선사진의 참고 선량 수준 (Reference dose levels for dental panoramic radiography in Anyang City)

  • Purpose : To measure dose-width product (DWP) values used for dental panoramic radiography in Anyang city, Korea. Materials and Methods : Thirty-six panoramic dental radiographic sets (17 analogue panoramic sets and 19 digital panoramic sets) in 36 dental clinics in Anyang city were included in the study. Each patient's panoramic exposure parameters were simulated and the panoramic radiation doses were measured at the secondary collimator using a Mult-O-Meter (Unfors Instruments, Billdal, Sweden) at each dental clinic during 2006. The third quartile DWP was determined from 310 surface dose measurements on adult. Results : The third quartile DWP for adult panoramic radiograph was 106.7 mGy mm. For analogue and digital panoramic radiograph, 3/4 DWP were 116.8 mGy mm and 72 mGy mm respectively. The overall third quartile DWP of panoramic radiography was 106.7 mGy mm. Conclusion : The measured 3/4 DWPs were higher than the 3/4 DWP of 65 mGy mm recommended by NRPB. Dentists who are operating above the reference dose should lower their panoramic exposure doses below the recommended reference value by changing the exposure parameters and/or their panoramic equipments.

파노라마방사선사진을 이용한 연령별 하악 피질골 두께에 관한 연구 (The relationship between age and the mandibular cortical bone thickness by using panoramic radiograph)

  • Purpose : This study was to determine the relationship between age and the mandibular cortical bone thickness on panoramic radiograph. Materials and Methods : Panoramic radiographs of 360 patients (180 men and 180 women) over 20 years old, who visited the Chonbuk National University Hospital from January to December in 2007, were assessed. The subjects were divided into 5 age groups. Five indices such as cortical bone thickness at the gonion (GI), antegonion (AI), and below the mental foramen (MI), the panoramic mandibular index (PMI), the mandibular cortical index (MCI) were measured on panoramic radiographs. Results : All five indices including GI, AI, MI, PMI, and MCI showed significant differences between third decade and over 8 decade groups (p<0.05). PMI, MI and GI showed significant differences with gender statistically (p<0.05). Conclusion : The mandibular cortical bone thickness showed negative correlation with age, and the value of the thickness (PMI, MI, and GI) was greater in men than in women.

구강검진의 효과 증진을 위한 파노라마방사선사진의 필요성에 관한 연구 (Usefullness of panoramic radiograph for the improvement of periodic oral examination)

  • Purpose : This study was designed to evaluate the efficacy and utility of panoramic radiograph for the improvement of the periodic oral examinations. Materials and Methods : Clinical examinations and panoramic examinations were done for the 242 subjects of oral examinations. The results of panoramic radiograph interpretation were compared with the clinical findings. Two questionnaires were created. One was carried out before the panoramic examination and the other done afterwards, to find out the subjects' cognition and satisfaction for the clinical and panoramic examinations. Results : 1. Panoramic findings showed a higher detection rate of 31.9% for periodontal diseases, and 23.1% for dental caries than clinical findings. 2. The additional abnormalities detected through panoramic examinations were impacted tooth in 81 subjects (33.6%), maxillary sinus abnormalities in 28 subjects (11.6%), condylar abnormalities in 5 subjects (2.1%), congenital and acquired dental anormalies in 59 subjects (24.5%), and other miscellaneous abnormalities in 34 subjects (14.1%). 3. 164 subjects (67.8%) were satisfied with the current periodic oral examination, and 75 subjects (31.1%) hoped for better accuracy. 4. In the first and second questionnaire, 154 subjects (67.0%) and 163 subjects (70.6%) responded respectively that panoramic examination was necessary, and 193 subjects (83.2%) responded that it actually helped. Conclusion : The panoramic examination was revealed to improve the effectiveness of the periodic oral examination and to increase the satisfaction of the subjects of examination.

하악 비대칭과 자기공명영상에서의 측두하악관절 원판변위와의 관계 (THE RELATIONSHIP BETWEEN MANDIBULAR ASYMMETRY AND TEMPOROMANDIBULAR JOINT DISC DISPLACEMENT ON MRI)

  • Purpose: This study was aimed to investigate the relationship between the mandibular asymmetry and the internal derangement of temporomandibular joint. Materials and methods: One hundred and sixty eight patients had been assessed through clinical examinations, panoramic radiographs and magnetic resonance imagings (MRIs), were selected. The samples were classified into three subgroups according to the severity of the mandibular asymmetries in the panoramic radiographs and the status of TMJ discs on the MRI were compared among each groups. Results: In an apparent asymmetry group, there was a significant difference in the number of temporomandibular disk displacement without reduction between the long and short side (66.7%, 18/27 joints on the short side) when the ratio of condylar process and coronoid process was used (p<0.05), but there was no statistically significant difference when the ratio of condyle and ramus was used. Conclusion: The probability of the disc displacement without reduction was higher at the side with relatively shorter condylar process on the panoramic radiograph, and also it might be more effective to use ratio of condylar process and coronoid process in the assessment of mandibular asymmetry. Therefore, a careful assessment on the temporomandibular disorders is necessary to diagnose and establish the treatment plans for the patients with a mandibular asymmetry and the panoramic radiograph can be used effectively on that way.

Quantitative localization of impacted mesiodens using panoramic and periapical radiographs

  • Purpose : The purpose of this study was to evaluate a new technique for localizing impacted mesiodens using its horizontal magnification ratio on panoramic radiographs. Materials and Methods : Location-magnification equation of a panoramic equipment was obtained from horizontal magnification ratio of a metal ball which was located variable positions from the center of image layer at interval of 2 mm. Panoramic radiographs were obtained from a skull phantom with a metal ball which was a substitute for impacted mesiodens and was embedded 10mm(Group 1), 15mm(Group 2), and 20mm(Group 3) posterior to the central incisor. Each group obtained 7 panoramic radiographs at variable positions and one periapical radiograph. Three methods were used to estimate the actual width of the incisors and the balls which were used to calculate the magnification ratio. The methods included using the actual incisor width and the calculated ball width (Method 1), using the actual incisor width and the ball widths measured on periapical radiograph (Method 2), and using the incisor and the ball widths measured on periapical radiograph (Method 3). The location of the metal ball was calculated by using the location-magnification equation. Results : The smallest difference between the calculated and the actual distance was $0.1{\pm}0.7 \;mm$ in Group 1/ Method 3. The largest difference was $-4.2{\pm}1.6 \;mm$ in Group 3/Method 2. In all groups, method 3 was the most accurate. Conclusion : Quantitative localization of impacted mesiodens is possible by using panoramic radiograph.

Panoramic radiological study to identify locally displaced maxillary canines in Bangladeshi population

  • Purpose : This study was performed to determine the prevalence of maxillary canine impaction on a basis of a single panoramic radiograph in Bangladeshi population. Materials and Methods : A random sample of seven hundred panoramic radiographs was collected from the patient record of a dental clinic. All the selected panoramic radiographs were taken from January 2009 to August 2010 by a single panoramic radiograph machine with the same exposure time (19 seconds) for all radiographs. One hundred and twenty panoramic radiographs were excluded to minimize the selection bias. In a dim lit room, an observer assessed the radiographs on a standard radiographic light box. The position of the impacted maxillary canine was recorded in line with the longitudinal axis of a tooth using the edge of a metal ruler. Data were subsequently put on SPSS 11.5 software and chi-square (${\chi}^2$) tests were applied to find out the association. Results : Among 580 panoramic radiographs it was found that impacted maxillary canines were present in only 7 (1.2%) radiographs. A statistical significant difference was found between the age of the patients and the vertical position of the impacted canines (p=0.000) and between the age of the patients and the horizontal position of the impacted canines (p=0.003). Conclusion : The prevalence was found to be low compared with the present study from the limitation of panoramic image. Further study needs to include three-dimensional imaging modality.

파노라마 방사선사진과 cone beam CT에서 제3대구치와 하악관의 관계 평가 (Assessment of the relationship between the mandibular third molar and the mandibular canal using panoramic radiograph and cone beam computed tomography)

  • Purpose : The purpose of this study is to evaluate the position of the mandibular canal in relation to the mandibular third molar by cone beam CT in cases showing a close relationship between the third molar and the mandibular canal on the panoramic radiograph. Materials and Methods : The panoramic images and cone beam CT scans of 87 impacted mandibular third molars in 60 patients were evaluated to assess the tooth relationship to the mandibular canal. The clearness of the canal wall and the vertical depth of the lower third molar were evaluated on panoramic radiographs. The lower third molars were assessed using cone beam CT to determine the proximity and position of the canal relative to the roots. Results : In the 66 cases where the canal wall was unclear on the panoramic radiographs, 58 (87.9%) of the third molars had contact between the canal and root; 34 (51.5%) canals were showed an inferior position and 22 (33.3%) showed a linguoinferior position on cone beam CT. Conclusion : Interruption of the canal wall on panoramic radiographs was highly predictive of contact between the mandibular canal and the third molar. Cross sectional CT may be indicated for localization of the mandibular canal in such cases.
