Analysis of Factors Related to the Use of Korean Medicine Treatment in Patients with Mood Disorders: Based on 2019 Korea Health Panel Annual Data (기분장애 환자에서 한의치료 이용과 관련된 요인분석: 제2기 한국의료패널 자료를 중심으로)

  • Objectives: We used the 2019 Korea Health Panel Annual Data to analyze factors related to visits to Korean medicine (KM) outpatient clinics among patients with mood disorders in Korea. Methods: Individuals aged 19 years or older, with depressive or bipolar disorders, and with a record of using Western medicine (WM) and/or the KM medical service were included. The 266 subjects were classified into the WM group or the integrative medicine (IM) group. The Andersen healthcare utilization model was used to analyze factors that potentially influenced the subjects' healthcare utilization. Binomial logistic regression analysis was used to analyze factors influencing the use of IM medical services. Results: Among the subjects, 75.56% (n=201) were in the WM group, and 24.44% (n=65) were in the IM group. Statistically significant differences were observed in residential areas, total annual income, the presence of disability, and the level of pain/discomfort between the two groups. Regression analysis found that residential areas and pain/discomfort were factors related to the use of IM services. Specifically, reporting "a lot" of pain/discomfort compared to "no" pain/discomfort showed a significant positive relationship with the use of IM (odds ratio=4.57, 95% confidence interval=1.79 to 11.70). Conclusions: This study was the first to analyze the status of KM medical service use and related factors among patients with mood disorders in Korea. The finding that the presence of pain/discomfort was positively correlated with the use of KM services is potentially related to medically unexplained physical symptoms or somatization phenomena.

Analysis of Factors for Heating Period Changes among Greenhouse Grape Farms (시설포도 농가의 가온시기 변화에 미치는 요인 분석)

  • The purpose of this study is to identify the factors that led greenhouse grape farms to delay their heating periods after the coming into force of the Korea-Chile Free Trade Agreement (FTA). Panel data on the cropping (system) changes from 2004 through 2016 were used for the analysis. According to the panel logistic model, the estimated coefficient of the cultivation area was 0.0002, which was statistically significant at the 10% significance level, the estimated coefficient of grape imports was 1.4258, which was statistically significant at the 1% significance level, and the estimated coefficient of the regional dummy was 0.808, which was statistically significant at the 5% significance level. The results indicated that the use of wider cultivation areas, increase in grape imports, and colder climate(in the mid-northern part of Korea) increased the likelihood of delayed heating. The Korean government is offering direct payment programs and business closure support to the greenhouse grape farmers. While these actions can relieve the damage caused by the increase in grape imports, they will not provide the ultimate solution. Various support measures are needed, such as renewing the varieties to meet the changing demand of grape consumers, providing agricultural materials to reduce the heating expenses, and modernizing greenhouse facilities to improve the energy efficiency and reduce the costs.

Analysis of Unmet Medical Needs according to Mental Health (정신건강유형에 따른 미충족 의료 현황 분석)

  • Objectives : This study investigated the prevalence of unmet health care needs among Korean adults and related factors. Methods : The study participants were adults over the age of 20 mental health experience from the Korea Health Panel in 2012(n=4,730). Statistical analysis methods used in this study were the ${\chi}^2$-test, Logistic Regression Analysis and other basic statistics such frequency-and percentage using SPSS version 22.0. Results : (1)Significant variables of stress: Factors were age, economic activity, subjective health status, and activity limitation. (2)Significant variables of depression: Factors were age, income class(low) and activity limitation. (3)Significant variables of suicidal impulse: Factors were age, chronic diseases, income class, and activity limitation. Conclusions : Stress, depression, and suicidal impulse can be unmet medical factors; therefore improvement measures and mental health counseling programs in response to suicide impulses, should be developed. Thus there is a need for a health sciences approach.

A Study on the Work Environment and Location of Family-owned Small Business (소규모 가족기업 소유자의 사업장 위치와 근무환경에 관한 연구)

  • According to recent statistics, the number of family-owned small business have increased. And these growing numbers have created an urgent need for researchers and government to analyse and plan for this population. The purposes of this study were to identify the determining factors of the location of family owned small business and to analyze the factors related to their job satisfaction, and life satisfaction. The data used for this study, were 713 self-employed men and women which were elected from the panel data of 1998 MIPS of Daewoo Economic Research Institute. Statistics performed for the analysis were frequencies, percentiles, t-test, $\varkappa$$^2$, OLS and Logistic analysis. It was found that the person who work at home-base small office were the residents of smaller city, and are older than the office-going attendants. The variable which effects the job satisfaction of the home-based workers was the educational background. And sex was the only factor which affects the job satisfaction of the office-going attendants. It was also found that job satisfaction affects the life satisfaction significantly in both group.

The Effect of Income Transfer on Poverty Rate (소득이전의 빈곤완화 및 빈곤이행 효과에 관한 연구)

  • The primary purpose of this study is to analyze the decreasing effect of public and private income transfers on poverty rate. Two year data of Korean Labor and Income Panel Study (KLIPS, 1998, 1999) are used for the analysis, and 1/2 of median income and 1/3 of mean income are adopted to measure poverty rate. Although private income transfer contributes more to reduce the rate than the public transfer, the main effect for decreasing poverty rate is forced by the wage. Statistically significant variables that affect to the exit of poverty based on the logistic regression analysis are number of family members(-), wage(+), property income(+), social insurance benefit(+), and the transfer income(+). Therefore, the future policy should be more related with the active labor market policy for developing better human resources among the poor family.

The Relationship between the Use of Korean and Western Medicine in treating Musculoskeletal Disease (근골격계 질환에 대한 한방의료기관 이용이 양방의료기관 이용에 미치는 영향 - 한국의료패널 자료를 이용하여 -)

  • Objectives: The aim of this study was to identify the complementary and substitute relationships between the use of Korean medicine (KM) and that of Western medicine (WM) in the treatment of musculoskeletal disease. Methods: We analyzed the 2009 Korea Health Panel dataset. General characteristics and the medical utilization of respondents were analyzed descriptively. Logistic regression, negative binominal regression, and Tobit regression analysis were used to identify the relationships between the use of KM and the use, visit frequency, and expenses of WM, respectively. Results: In the treatment of musculoskeletal disease, KM use and non-herbal treatments with Korean medicine significantly reduced WM use. Herb medication use significantly increased WM visit frequency. There were no significant relationships between KM use and WM expenses. Conclusions: There are substitute relationships between WM use and KM use, especially non-herbal treatments in KM. Therefore we need to develop the clinical protocols of KM and WM treatments in the treatment of musculoskeletal disease for proper distribution medical resources.

Predictors of Cognitive Function Decline of Elderly : Using Living Conditions and Welfare Needs of Older Korean Persons Panel Data (노인의 인지기능 저하 예측요인: 노인실태조사 패널자료를 이용하여)

  • Objectives : The aim of this study was to investigate predictors of cognitive function decline among community dwelling elderly. Methods : Data were analyzed from the Survey of Living Condition of Elderly panel study. Cognitive function was measured with the MMSE-KC at baseline and year 3. The study subjects were 5,464 community dwelling people aged 65 years or older who had no disability at baseline. Logistic regression analysis was used to predict cognitive function decline. Results : From 2008-2011, 4,417(80.8%) elderly people had no cognitive decline, and 1,074(19.2%) showed cognitive function decline measured by the MMSE-KC. After adjusting for demographics and baseline MMSE-KC score, the best predictors for cognitive function decline at 36 months were diabetes mellitus, smoking, low intensity physical activity, relationship with relatives and friends. Conclusions : Health promotion programs that are focused on the elderly are essential in preventing cognitive function decline. Promoting regular physical activity, and social relationships should be included in health promotion for elderly. When treating patients with diabetes, preventing cognitive impairment should be considered through education and counseling.

Impact of Perceived Cancer Risk on the Cancer Screening Rate in the General Korean Population: Results from the Korean Health Panel Survey Data

  • Objective: To investigate the relationship between the perception of cancer risk and likelihood of having undergone cancer screening. Materials and Methods: We used data from the Korean Health Panel Survey from December 2011 onward. Of 3,390 patients who visited a hospital during the previous year, we included data from 2,466 individuals; 924 samples were excluded due to missing data. Logistic regression analysis and the chi square test were used to investigate the association between perceived cancer risk and the likelihood of having undergone cancer screening. Results: For patients who perceived their risk of developing cancer during the next 10 years to be 30-40%, the odds ratio was increased 1.65 fold (95%CI: 1.223, 2.234) compared with those who perceived their risk to be almost zero. Although the difference was not statistically significant, perceiving cancer risk as either extremely low or extremely high appears to be associated with a reduced likelihood of having undergone cancer screening, resulting in an inverted U-shaped relationship. Conclusions: Physicians and researchers should be aware of the importance of the affective component of risk perception. Policies addressing the influence of cancer risk perception should be implemented in South Korea and worldwide.

Relationship between asthma and dental caries in Korea children (한국 아동의 천식과 치아우식과의 관련성)

  • The aim of this study was to confirm the relationship between asthma and dental caries in Korean children. Using the data of the 2017 The Panel Study of Korean Children (PSKC), logistic regression analysis was performed to investigate the association between asthma and dental caries in 331,457 9-year-old children. As a result of this study, asthma was associated with dental caries(unadjusted odds ratio [OR]: 2.75, 95% confidence interval [CI]: 2.63-2.88). In addition, after all confounders were controled, their association became stronger(adjusted odds ratio [aOR]: 2.78, 95% confidence interval [CI]: 2.65-2.91). Further studies are needed to clearly confirm the relationship between respiratory disease and dental caries and their causal relationship in children.

Determinants analysis of uninsured herbal medicine utilization in the Korean Medicine outpatient service (한의 외래에서 첩약을 포함한 비급여 조제 한약 이용결정요인 분석)

  • Objectives : This study aimed to analyze the characteristics of uninsured herbal medicine(UHM) users and the economic and social barriers of UHM utilization. Methods : We used the Korea Health Panel Data, representative national survey on medical utilization and cost, provided by National Health Insurance Service and Korea Institiute of Health and Social Affairs. The frequency analysis was used to identify the characteristics of the respondents, and the cross-analysis (${\chi}^2-test$) was used to verify the relationship between their characteristics and the usage of UHM. In order to analyze the determinants of using the UHM considering the individual's characteristics, logistic regression analysis and multiple regression analysis were conducted for those who used the Korean medicine (KM) outpatient service in 2015. Results : The usage of UHM was significantly lower for those (1) who's age of 20 to 65; (2) who have the university or higher education degree; (3) who live in Jeju province, and (4) who bought the herbal medicine for other health related purposes. On the other hand, the usage of UHM for those (1) who have the first quintile of household income; (2) who have the chronic respiratory disease; (3) who have been taking the medicine for health promotion purpose for more than 3 months and (4) who have purchased the food which has health promotion function was significantly higher than others. The patients who have chronic musculoskeletal diseases accounted the most among the UHM users. Conclusions : There was the considerable inequality in the usage of UHM among household income groups, which provides policy rationale for UHM to be covered by national health insurance. To facilitate the coverage expansion, restrictive covering model can be considered for children and adolescents, or for patients with muskuloskeletal diseases who have the high demand for UHM.
