  • Eagle's syndrome is the term given to symtomatic elogation of the styloid process or mineralization of the stylohyoid or stylomandibular ligament. Since ossification of stylohyoid ligament at cadeva was fist described by De Manchetis in 1652 and Weinlecher described clinical symptom which produced by elongated styloid process and osteotomy of styloid process in 1872, Clinical symtom which include sensation of a foreign body on the pharynx, dysphagai, dysphonia, referred pain, and mouth opening disturbance was termed by Eagle as Eagle's syndrome. then, case reports of Eagle's syndrome are presented. in these cases, the patient's chief complaints included periauricular radiating pain, mouth opening disturbance, foreign body sensation, dysphagia, tenderness on the neck. Through vairous X-ray examination and palpation of tonsillar fossa, elongated styloid process were confirmed. Under the general anesthesia there were successfully removed out via transoral approach technique, described by Eagle. after resented styloid process, they were freed from the symptoms without further complication. Therefor we reported these cases treated by surgical resection of styloid process with good results.

Simultaneous Augmentation Rhinoplasty with Bony Reduction in Nasal Bone Fracture (비골골절 시 골절정복과 동시에 시행된 융비술)

  • Purpose: The nasal bones are the most common fracture sites of the facial bones, and a careful reduction may still result in secondary deformities, such as saddle nose, deviated nose, hump nose etc, requiring secondary cosmetic rhinoplasty. Therefore, this study examined the clinical characteristics of nasal bone fractures to propose guidelines for patient selection and surgical procedures to achieve more satisfactory results and to prevent secondary deformities with simultaneous augmentation rhinoplasty and bony reduction. Methods: The study was based on 26 out of 149 nasal bone fracture patients who underwent simultaneous augmentation rhinoplasty with bony reduction between May 2008 and April 2009. Retrospective analysis was performed according to the clinical data, surgical techniques and postoperative results. Results: Of the 26 patients, there were 15 males and 11 females. The incidence according to the Stranc's classification revealed that 62% of patients were injured by a frontal impact and 38% by a lateral impact. Frontal impact plane I (50%) was the most frequent type. At the follow up, 18 (81.2%) out of 22 patients were satisfied with their postoperative outcome, and the remaining 4 patients were fair. No one was dissatisfied. However, 5 cases in 3 patients (23%) had some complications; minimal implant deviation in 2 cases, minor irregularity on the nasal dorsum in 2 cases and palpable implant movement under palpation in 1 case. None of these cases required surgical correction. Conclusion: With the proper guidance, simultaneous augmentation rhinoplasty with bony reduction can prevent secondary deformities and satisfy the cosmetic outcomes.

Preparation and Management of Recipients in Bovine Embryo Transfer (수정란이식에 있어서 수란우의 준비와 관리)

  • Recipients are an integral part of embryo transfer and they are expensive to maintain as a good recipient. Recipient management is one of the most important components in a successful embryo transfer program. Management includes selection and subsequent care of the animals. A good recipient is basically on "open" cows or heffers whose reproductive tract is capable of receiving one or two embryos and incubating it to term. Potential recipients should be always be healthy and cycling normally ranging from 18 to 23 days. A thorough veterinary examination is recommended for candidate of recipients and cattle for questionable health should be eliminated from the recipient herd. Age and size of recipients are particularly important considerations when heifers are used, because of most embryos available for transfer are from large dams and sires. Body condition can influence a recipient's production, reproduction and health. Obese and underconditioned cattle should be avoided for use. Transfer of fresh embryos especially requires precise synchronization of donors and recipients. For estrus synchronization, PGF$_2$$\alpha$ is injected twice 10 to 12 days apart and short4erm progestagen treatment is applied to potential recipient cattle by coil into vagina (PRID) or ear implant (Synchro-Mate-B). The highest pregnancy results are achieved in recipients at exact synchrony with donors or 12 to 24 hr earlier than donors. Estrus detection is a major factor in breeding efficiency. High accuracy can be achieved by use of heat mount detection alds or by obserbing cattle for 30-minute peroids 3 times daily. Assay progesterone in milk can be used to discrIminate between pregnant and nonprenant recipients. Rectal palpation on day 35 to 70 after is an accurate and safe method of pregnancy diagnosis. Embryonic mortality in recipients may be associated with factors such as high environmental temperature and nutritional or lactational stress in early lactation period. Achievement of short calving interval requires concentrated management activity during the first 90 days following calving. Acceptable candidate for a recipient should be routinely vaccinated for infectious diseases. Proper nutritional programs according to NRC requirements and body condition scoring system for recipient cattles are vital to the ultimate success of an embryo transfer program.r program.

  • The objective of this work was to determine the effect of corpus luteum (CL) grade on pregnancy rate after embryo transfer in Korean cattle and we found that CL development was linked to pregnancy rate. The in vivo derived blastocyst-stage embryos were transferred to 15 recipients synchronized in the estrus cycles. Based on size and palpable characteristics, CLs were categorized into three grade. The grade three CL is not to be identified by rectal palpation. The pregnancy rates tended to increase with the increase in CL size of recipients. In grade one, two, and three, the pregnancy rates were 62.5%, 50.0%, and 0%, respectively. This result suggests that pregnancy rates after embryo transfer might be affected by the CL status of recipients.

  • The objective of this study is to find out a reason why prescriptions have different effects on each patient who has same symptoms or same abdominal examinations and further more to classify the formulas more efficiently. In applying 's prescriptions, it is known that conventional ways such as treatment in accordance with symptoms or abdominal examinations have many advantages and problems reversely. To make up for the problems, additional references like strength of constitutional resistance and location of disease, degree of income and outgo are designed. And the notion in Oriental Medicine embracing aspects mentioned above corresponds to triple energizer. Triple energizer's exuberance-debilitation is able to draw an inference from some factors like density of skin interstices, elasticity of abdomen, appetite, digestive power. According to Exuberance-Debilitation of Triple Energizer, can be divided into five steps: weak(弱)-moderately weak(中弱)-neither weak nor strong(中)-moderately strong(中强)-strong(强). prescriptions would be dealt with those 5 steps, and it would be highly effective and consequently side effects could be reduced. On the basis of this classification method upon formula group, the prescriptions of can be applied more accurately by setting a direction through strength of constitutional resistance and location of disease and combining with existing references like symptoms, palpation and abdominal examinations.

Response of Ovaries and Cyst According to Treatment with Dinoprost or Fenprostalene in Dairy Cows with Ovarian Luteal Cyst (황체낭종우에서 Dinoprost 또는 Fenprostalene 치료에 대한 난소 및 낭종의 반응)

  • This study was carried out to monitor the response of ovaries and cyst according to treatment with dinoprost or fenprostalene in dairy cows with ovarian luteal cyst. Twenty cows were diagnosed as luteal cysts by rectal palpation, ultrasonography and progesterone analysis. The cystic cows were treated with dinoprost or fenprostalene. All the animals were re-examined by ultrasonography and blood was collected for the measurement of plasma progesterone concentration at day 0 (the day of treatment), 3, 13 and 24, respectively. Mean plasma progesterone concentrations on day -11 and day -1 before treatment were 2.1$\pm$0.2 ng/ml and 2.8$\pm$0.3 ng/ml, respectively. On day 3 and day 13 were 0.3$\pm$0.1 ng/ml and 4.3$\pm$0.2 ng/ml, respectively. Mean cystic wall thickness on day -11 and day -1 were 3.2$\pm$0.2 mm and 3.9$\pm$0.2 mm, respectively. And on day 3 was 2.4$\pm$0.3 mm. The responses of luteal cyst after treatment noted during ultrasonography included dramatical degeneration of the luteal tissue of cystic wall on day 3 (all cows), slowly reduction of cyst size (cyst resolution) until last examination (8 cows), complete disappearance on day 13 (7 cows) and no changes of cyst size (5 cows). A group of 10 cows with luteal cysts injected fenprostalene compared with another 10 cows treated dinoprost showed a slightly higher pregnancy rate on first service after initial treatment (50.0 vs 30.0%). But treatment effects of dinoprost or fenprostalene did not significantly different results for each groups. This study suggested that the response of the cyst according to treatment revealed various types. Therefore, veterinarians should have attention on monitoring of the response of cystic ovaries after treatment, specially on no change of cyst size after treatment.

Early rehabilitation treatment helpful in a case of pectus excavatum of a dog (개에서 발생한 누두흉의 초기 재활치료의 효과)

  • A 6-week-old female cocker spaniel, with a history of abasia astasia, was referred to the Department of Laboratory Animal Medicine, Medical Research Center, Yonsei University College of Medicine on February 23, 2000. The ribs inclined downwards with a sharp slope on both lateral sides of the thorax. The animal was diagnosed as pectus excavatum. She showed no signs of dyspnea, dyschezia, or dysuria. There was no heart murmur or sign of neural disease. For rehabilitation treatment, she was given physical exercise therapy 2 to 3 times a day by bending and stretching each articulation of the hind limbs and pressing the costochondral junction in and upward with both hands placed on each lateral side. After a month of therapy, she was able to stand up and walk. The therapy continued and resulted in the slight reformation of a round thoracic wall. The flattened rib cartilages grew more round by palpation and the thoracic cavity distended. Rehabilitation therapy yielded good results, enabling the subject to walk and run normally, as the thoracic cavity was distended by the rounded rib junction.

  • Eagle's syndrome occurs when an elongated styloid process or calcified stylohyoid ligament causes recurrent throat pain or foreign body sensation, dysphagia. It is supposed that this symptoms and signs are due to the compression of the styloid process on some neural and vascular structures. More uncommonly, symptoms such as dysphagia, tinnitus, and otalgia may occur in patients with this syndrome. It may also cause stroke due to the compression of carotid arteries. The compression depends on the size, shape, and orientation of the ossified styloid process Besides, degenerative or inflammatory changes in the tendinous portion of the styloid ligament insertion or rheumatic styloiditis may also cause this syndrome. Diagnosis can usually be made on physical examination by digital palpation of the styloid process in the tonsiller fossa. Three-dimensional computed tomography can utilized for supporting diagnosis. The treatment of Eagle's syndrome is primarily surgical. The styloid process can be shortened through an intraoral and extraoral approach. We report a 62-years-old man with the symptomatology of Eagle's syndrome and literature reviews.

Characteristics of the Palm Prints and Palm Creases According to Sasang Constitutional Types (사상체질 유형에 따른 손바닥문, 손금의 특징)

  • In Sasang constitutional medicine, the classification of Sasang constitutional types (Tae-Yang, Tae-Eum, So-Yang, and So-Eum) is important for the treatment. There are some problems with the methods used for classifying Sasang constitutional types; old methods such as pulse-palpation are not considered objective and recent methods such as immunohematology are considered expensive, painful and time-consuming. To overcome these problems, a body measurement and finger prints analysis were performed. The purpose of this study was to determine whether the palm prints and the palm creases could be helpful in classifying Sasang constitutional types. Thus, we looked for characteristics of the palm prints and the palm creases according to Sasang constitutional types. Before analyzing the palm prints and palm creases, 760 Korean (465 males, 295 females) were surveyed using two kinds of questionnaires for classifying Sasang constitutional types. As there were no Tae-Yang individuals, we were only able to identify the characteristics of the palm prints and the palm creases for Tae-Eum (288 persons), So-Yang (193 persons), and So-Eum (279 persons) individuals. In this study, the terminal points of D, closed crease, and open crease seemed to be helpful in classifying Tae-Eum and So-Eum individuals. Terminal point 11 and closed crease were frequent in Tae-Eum individuals; whereas, terminal point 7 and open crease were frequent in So-Eum individuals. Therefore, the palm prints and the palm creases seem to contribute to the classification of Sasang constitutional types.

Effect of Visual and Palpation Feedback on Muscle Activity of Gluteus Maximus and Motion of Pelvic Rotation during Clam Exercise (크램 운동 시 시각-촉진 되먹임이 큰볼기근 활성도와 골반 회전에 미치는 영향)

  • Purpose: This study was conducted in order to determine the effect of visual and tactile feedback on muscle activity of the gluteus maximus (Gmax) and abdominal muscles and the motion of pelvic rotation during performance of clam exercise (CE). Methods: Thirteen subjects without low back pain were recruited for this study. Each subject was instructed to perform the CE without and with feedback. The subjects were instructed to keep pelvic from rotating backwards by palpating the ASIS and monitoring the pelvic movement by themselves during performance of CE with feedback. The electromyographic (EMG) activities of Gmax and abdominal muscles were collected using surface EMG. Angles of pelvic rotation were measured using a 3-dimensional motion-analysis system. Paired t-tests were used for comparison of EMG activities in each muscle and the angle of pelvic rotation. Results: The EMG activities of all abdominal muscles were not significant between CM without and CM with feedback (p>0.05). The EMG activity of Gmax was significantly greater in CM with feedback compared with CM without feedback (without vs. with feedback; 14.2% vs. 20.7%MVIC) (p<0.05). The angle of pelvic rotation was significantly less in CM with feedback compared with CM without feedback (without vs. with feedback; $15.3^{\circ}$ vs. $10.8^{\circ}$ ) (p<0.05). Conclusion: Therefore, these findings suggest that CM with the visual and tactile feedback is effective in activation of the Gmax and correcting of the uncontrolled lumbopelvic rotation during CE.
