자궁절제술 후 신체적 불편감과 성생활 양상 (Physical Discomforts and Sexual Life Pattern of Women with Hysterectomy)

  • The purposes of this study were to investigate the physical discomforts and sexual life pattern and to identify the relation between the physical discomforts and the satisfaction of sexual life in women with hysterectomy. The subject were 301 women who lived with their spouses from 3 months to 2 years after hysterectomy in S. University Hospital. The data were collected using a self-reported questionnaire by mail, which composed of 25 items of physical discomforts, restarting time and frequency of sexual intercourse, and 10 items of sexual satisfaction, The results were as follows: 1) The mean score of physical discomforts was 13.22 and range of score was 0-45. 1.7% of 301 women had no physical discomforts and 12.0% of them complained of severe physical discomforts such as fatigue, lumbago and pain of extremities. 2) The women with hysterectomy complained of fatigue(76.1%), lumbago(68.8%), pain of extremities(63.5%), weight gain(55.5%), vaginal dryness(50.8%) and symptom of estrogen deficiency such as perspiration (47.5%), flush(41.2%) and palpitation (38.5%). As unusual symptom, numbness of thigh (20.3%) and acne(16.3%) were identified. 3) There was no significant difference between the degree of physical discomforts and the laps of time after hysterectomy. But the score of physical discomforts was lower in women with vaginal hysterectomy than in women with abdominal hysterectomy. 4) The mean score of sexual satisfaction was 33.11 and range of score was 10-50. There was no significant difference between the degree of sexual satisfaction and the lapse of time after hysterectomy. 83.8% of women had not change of sexual life satisfaction after hysterectomy. The women restarted sexual intercourse in average 2.57 months after hysterectomy. 5) There was a negative correlation between the physical discomforts and the sexual satisfaction. In conclusion, nurses should make the discharge educational program of the physical discomforts and the sexual pattern for women with hysterectomy in hospital.

  • Peripheral nerves may be affected or injured for several reasons. Peripheral nerve damage can result from trauma, surgery, anatomical abnormalities, entrapment, systemic diseases, or iatrogenic injuries. Trauma and iatrogenic injuries are the most common causes. The ulnar, median, and radial nerves are the most injured nerves in the upper extremities, while the sciatic and peroneal nerves are the most injured nerves in the lower extremities. The clinical symptoms of peripheral nerve damage include pain, weakness, numbness/ tingling, and paresthesia. Therefore, early diagnosis and appropriate treatment of peripheral nerve injuries are crucial. If a peripheral nerve injury is left untreated, it can lead to severe complications and significant morbidity. The sciatic nerve is one of the most affected nerves. This nerve is generally injured by trauma and iatrogenic causes. Children are more susceptible to trauma than adults. Therefore, sciatic nerve injuries are observed in pediatric patients. When the sciatic nerve is damaged, pain, weakness, sensory loss, and gait disturbances can occur. Therefore, the diagnosis and treatment of sciatic nerve injuries are important to avoid unexpected consequences. Ultrasound can play an important role in the diagnosis of peripheral nerve injury and the follow-up of patients. The aim of this case report is twofold. First, we aimed to emphasize the critical role of ultrasonographic evaluation in the diagnosis of peripheral nerve injuries and pathologies. Second, we aimed to present this case, which has distinguishing features, such as the existence of periostitis ossificans progressiva with sciatic neurotmesis due to a traumatic glass injury.

비소세포폐암의 뇌전이로 인한 갑작스런 수부 통증 및 마비 (A Man Presenting with Sudden Weakness and Pain of the Right Hand, by Non-Small Cell Lung Cancer with Brain Metastases)

  • Unexplained pain and weakness, i.e., without obvious predisposing factors, are often encountered by physiatrists and efforts should be made to determine the cause. A 63-year-old male presented with radiating pain in his right arm and mild weakness of the right hand. An electrodiagnostic examination revealed distal symmetric sensory polyneuropathy in the upper and lower extremities, and denervation potentials in the forearm muscles, which were inconsistent with the cervical spine MRI images and symptoms. A predisposing undiscovered disease was revealed, i.e., squamous cell carcinoma in the lung; brain metastasis affecting the left primary motor cortex was also detected. Therefore, we concluded that the pain and weakness were related to paraneoplastic syndrome and brain metastases of the hand knob. The observed denervation potentials were characterized as trans-synaptic changes in the brain metastasis. This case highlights the importance of unexplainable focal pain and weakness in the increasing prevalence of cancer.

척수 손상 환자의 중추성 통증에 대하여 시행한 Microsurgical DREZotomy의 효과 (Microsurgical DREZotomy for Treatment of Intractable Central Pain in Patient with Spinal Cord Injury)

  • 척수손상 환자에게 동반되는 통증은 흔하지만 치료에 잘 반응하지 않고 환자들의 일상생활에 더욱 장애를 줄 수 있는 합병증의 하나로 알려져 있다. 3년 전 외상성 척수손상으로 인하여 불완전 하지 마비 소견을 보였던 52세 된 남자 환자로 손상 직후부터 발생된 양측 T10위치의 분절통과 손상 1개월 후부터 발생된 양하지 통증이 있어 약물요법, 물리치료, 및 운동치료 등의 여러 가지 치료를 시행하였으나 치료에 잘 반응하지 않아 DREZ(Dorsal Root Entry Zone)otomy 수술을 시행한 후 분전통이 호전된 증례가 있어 이를 보고하고자 하였다. 보존적 치료에 잘 반응하지 않는 척수손상 후 통증의 경우 DREZotomy 수술도 도움이 될 것으로 사료된다.

타입 1형 당뇨 환자에서 급작스런 혈당조절에 의해 발생한 신경염 (Treatment-Induced Neuropathy of Diabetes)

  • Treatment-induced neuropathy in diabetes (TIND) is a reversible neuropathy that occurs in patients with diabetes, usually after a fast improvement in glycemic control. TIND is defined as the sudden onset of neuropathic pain or autonomic dysfunction with a large improvement in glycemic control (glycated hemoglobin [HbA1c] level of ≥ 2% over 3 months). We report the first case of a 24-year-old woman with type 1 diabetes mellitus who developed TIND in Korea. Her HbA1c level had decreased from 16.7% to 7.3% within a 3-month period. She developed acute-onset, severe, and continuous burning pain affecting her back and lower extremities. She was administered tapentadol (50 mg), pregabalin (75 mg), and vitamin B with minerals twice daily for neuropathic pain. She complained of orthostatic hypotension; thus, midodrine (2.5 mg) and anti-embolic stockings were prescribed. She almost completely recovered 6 months after the onset. A physician should be aware of TIND and gradually reduce HbA1c levels to prevent the occurrence of TIDN. They must also try to provide relief from severe pain or autonomic dysfunction and emphasize on an almost complete recovery.

Effect of Sprinter Pattern Bridging Exercise using Theraband on Activation of Lower Extremity and Abdominal Muscle

  • Objective: The purpose of this study was to investigate the effect of sprinter pattern bridging exercise using theraband on activation of lower extremity and abdominal muscle and to find out postures that can effectively improve abdominal and lower extremity muscle strength and increase abdominal stability. Methods: This study was designed as a cross-sectional study. The following research was done with applicants attending S university in Seoul to compare the difference in muscle activity between one-leg-Support bridging exercise and sprinter-pattern bridging exercise using theraband. For 48 study participants, we first measured their MVC. Then, we applied one-leg-support bridging exercise and sprinter-pattern bridging exercise at random order. These data were expressed as the percentage of maximal voluntary contraction (%MVC).Electromyography analysis was performed by measuring the external obliques, internal obliques, biceps femoris, and gluteus maximus. Results: There was a statistically significant increment of muscle activity in external and internal oblique muscle(p<0.001)by sprinter-pattern bridging exercise using theraband. On the lower body, statistically significant increment of muscle activity in biceps femoris and gluteus maximus was found(p<0.05). On the other hand, on erector spinae, there was statistically significant decrease in muscle activity(p<0.05). Conclusions: Efficient treatment is expected when sprinter-pattern bridging exercise using theraband is applied clinically.For patients with chronic knee and ankle pain who have difficulty bearing weight, including low back pain and internal rotation of the femur, starting with a low weight bearing, we think it will be helpful in planning systematic training aimed at progressively strengthening the lower extremities.

Prevalence of MSDs and Postural Risk Assessment in Floor Mopping Activity Through Subjective and Objective Measures

  • Background: Residential and commercial cleaning is a part of our daily routine to maintain sanitation around the environment. Health care of professionals involved in such cleaning activities has become a major concern all over the world. The present study investigates the risk of musculoskeletal disorders in professional cleaners involved in floor mopping tasks. Methods: A cross-sectional study was performed on 132 mopping professionals using a modified Nordic questionnaire. The Pearson correlation test was implemented to study the association of perceived pain with work experience. The muscle strain and postural risk were evaluated by means of three-channel electromyography and real-time motion capture respectively of 15 professionals during floor mopping. Results: Regarding musculoskeletal injuries, risk was reported majorly in the right hand, lower back, left wrist, right shoulder, left biceps, and right wrist of the workers. Work experience had a low negative association with MSDs in the left wrist, right wrist, right elbow, lower back, and right lower arm (p < 0.01). Surface EMG showed occurrence of higher muscle activity in upper trapezius and biceps brachii (BB) muscles of the dominant hand and flexor carpi radialis and BB muscles of the nondominant hand positioned at the upper and lower portion of the mop rod, respectively. Conclusion: Ergonomic mediations should be executed to lessen the observed risk of musculoskeletal injuries in this professional group of workers.

만성 허리 통증 노인의 오스웨스트리 장애지수, 버그 균형 척도, 보행 시 운동학적 움직임과의 상관관계 (Correlation between the Oswestry Disability Index, Berg Balance Scale, and Kinematic Data during Gait Analysis in Elderly People with Chronic Back Pain)

  • PURPOSE: Chronic low back pain is one of the main factors that affect the quality of life and cause problems of gait and balance in elderly people. This study investigated the correlation between the Oswestry disability index (ODI), Berg balance scale (BBS), and kinematic data measured while analyzing the gait of elderly people with chronic back pain. METHODS: A total of 29 subjects participated in this study. The ODI, BBS, and kinematic data of lower extremities were measured while walking. All data were analyzed using Pearson's correlation coefficients and the significance was measured at .05. RESULTS: ODI had a significant correlation with 1, 13, and 14 items of BBS (p < .05), and left hip external rotation and right ankle abduction respectively in the stance and swing phase of gait (p < .01). 13 items of BBS had a significant correlation with the right ankle abduction in the stance phase of gait (p < .01). In addition, 14 items of BBS had a significant correlation with right and left ankle abduction in the stance and swing phase of gait (p < .01). CONCLUSION: While attempting to predict chronic low back pain and balance issues, it may be useful to check the right ankle abduction in the stance and swing phase of gait. In the future, it would be helpful if some simple tests could be designed to assess balance in elderly people with chronic low back pain.

경막외차단 중 발생된 우발적 경막하 스테로이드 주입에 의한 난치성 요추수술 후 실패증후군의 치료 경험 -증례 보고- (Accidental Subdural Steroid Injection during Intended Epidural Block Relieves Intractable Radiculopathy in Failed Back Surgery Syndrome -A case report-)

  • We describe the accidental injection of local anesthetics containing steroid into the subdural space during an attempted lumbar epidural injection for intractable radiculopathy in a patient with failed back surgery syndrome. A 24-year-old man complained of severe radiating pain to left lower extremity and showed a walking disturbance and severe lumbar scoliosis. The MRI finding was a left paramedian recurred disc herniation on L4-5 in a laminectomy state. Several therapeutic modalities such as epidural steroid injection, transforaminal injection, L2 root block, medication, and exercise therapy, etc failed. Initially, during epidural block at L4-5 under fluoroscopic guidance, a railroad track appearance appeared on epidurogram suggesting the presence of a subdural space. A second epidural block was tried at L5-S1. Following confirmation of epidural space upon epidurogram, 6 ml of 0.5% lidocaine including triamcinolone 40 mg was injected. The patient showed signs of the subdural injection including an unexpectedly high sensory block (T2) and a motor weakness of both lower extremities. Following this event, the severe radiculopathy and lumbar scoliosis were improved. Therefore, we conclude that subdural injection of steroid could be helpful in intractable radiculopathy, especially in the failed back surgery syndrome. However, it must be used cautiously with careful patient selection.

Lumbar Disc Herniation Presenting Cauda Equina Syndrome

  • Objective : To determine the relationship between the clinical outcome and the extent of surgical laminectomy for adequate decompression on the cases of cauda equina syndrome, the authors review and analyze their cases and compared with those of literatures. Methods : The authors reviewed 655 patients retrospectively who had underwent surgery on the cases of lumbar disc herniation from January 2000 to December 2004. There were 19 patients [2.9%] who presented for clinical cauda equina syndrome. Among them, we selected and analyzed 15 patients who were treated by unilateral partial hemilaminectomy with discectomy or bilateral partial laminectomy with discectomy, and they had been followed from 5 weeks to 47 months postoperatively [mean, 13.47 months]. The levels of the disc herniations were L4-5 in 8 patients, following L5-S1 in 4 patients and 2 levels [L4-5 and L5-S1] in 3 patients. Motor and sensory recoveries were recorded. Postoperative urinary function recovery Was defined according to Gleave and Macfarlane. Results : In 12 months postoperatively, the bladder function was obtained in 14 of 15 patients[93%] with regaining urinary continence. Thirteen of 15 patients[86%] with preoperative motor weakness of lower extremities were recovered. Sensory deficit of lower extremities, perianal and saddle anesthesia were all recovered. Patients had recovered on lumbosciatic pain and saddle hypesthesia, in turn, motor function and urinary incontinence. Conclusion : In treating cauda equina syndrome, the authors did less extensive surgery, such as unilateral partial hemilaminectomy with discectomy or bilateral partial laminectomy with discectomy for adequate decompression. The outcome is satisfactory and comparable with those of subtotal or total laminectomy.
