지역사회 거주 노인의 성별에 따른 요통 및 좌골신경통 관련 요인 (Gender Differences in Relating Factors of Low Back Pain and Sciatica in Community-dwelling Korean Elderly)

  • Purpose: The aim of this study was to investigate gender differences in risk factors and sleep, depression, and mobility of Korean elderly with and without low back pain and sciatica. Methods: Data were derived from the 2011 Korean National Survey on Older Adults. Participants included 10,674 community-dwelling elderly. The group of elderly subjects with low back pain and sciatica and the group of elderly subjects without low back pain and sciatica were compared according to gender. Result: Age, monthly income, exercise, arthritis, osteoporosis, and providing caring support were significant risk factors for low back pain and sciatica in elderly male subjects. On the other hand, monthly income, living arrangement, arthritis, osteoporosis, providing caring support, and television watching time were significant risk factors in elderly female subjects. Significant differences in depression and mobility according to low back pain and sciatica were observed in both genders. Conclusion: Risk factors for low back pain and sciatica in elderly differed according to gender. Low back pain and sciatica showed an association with depression and mobility. These findings should be considered in planning for nursing intervention for low back pain and sciatica.

  • Purpose: The aim of this study was to investigate the status of visiting nurses assessment and management of chronic musculoskeletal pain in older adults. Methods: The subjects of this study were 285 visiting nurses working at 24 public health centers located in Seoul, Gyeonggi-do, and Gangwon-do. Data were collected using a structured questionnaire from June to July, 2008. The SPSS/WIN 13.0 program was used for data analysis. Results: Of the visiting nurses, 195 (77.4%) reported lack of pain education for nurses. The education of exercise (83.3%) was used most frequently by nurses as a pain intervention. Barriers to effective pain management were inadequate pain management guidelines and lack of pain education for nurses. Conclusion: It is necessary to develop evidence-based nursing interventions for pain management and to develop systematic educational programs for visiting nurses management of chronic musculoskeletal pain.

요통 및 슬관절통을 가진 노인에 대한 수지요법 적용의 효과 (The Effects of Hand Acupuncture Therapy on Pain, ROM, ADL and Depression among Elders with Low Back Pain and Knee Joint Pain)

  • Purpose: The purpose of this study was to identify the effects of hand acupuncture therapy on pain, ROM, ADL, and depression among older people with low back pain and knee joint pain. Methods: The research was a quasi-experimental design using a non-equivalent control group pre-post test. The participants were 40 patients, 18 in the experimental group and 22 in the control group. A pretest and 2 posttest were conducted to measure the main variables. For the experimental group, hand acupuncture therapy, consisting of hand acupuncture and press-pellets based on corresponding points, was given. Results: There were statistically significant differences in pain, ROM in knee joint, and ADL in the experimental group but not in depression compared to the control group over two different times. Conclusion: The hand acupuncture therapy was effective for low back pain, knee joint pain, ROM in knee joint and ADL among the elders in this study. Therefore, the hand acupuncture therapy can be utilized in the field of geriatric nursing as a nursing intervention for older people with low back pain and knee joint pain.

종합병원 간호사의 요통발생 실태와 관련요인 (A Survey on Low Back Pain of General Hospital Nurses)

  • The purpose of the survey was to find out the prevalence rate and related factors for low back pain of nurses and to develop educational program for prevention of the low back pain. The subjects of the survey were 593 nurses of general hospitals in Tae-gu City. The survey was conducted from June 10, 1994 to October 3, 1994. The questionnaire included 34 question items concerning general characteristics, factors related to low back pain, physical nursing activities, and characteristics of low back pain. The results were as follows : 1. Among 593 of subjects, the prevalence rate of low back pain showed 62.1%. 2. In relation of health related life activities and low back pain prevalence, self-reported health state was highly significant(P=.000). 3. In relation of work environmental factors and low back pain prevalence, job satisfaction (P=.026), job stress(P=.020), and workload(P=.002) were significant. 4. In relation of physical nursing activities and low back pain prevalence, bending (P=.000), trunk twisting(P=.003), stretching(P=.006), and pulling and pushing(P=.046) were significant. 5. Physical nursing activities inducing back stress was varicant according to wards. The results of this study pointed out that the subjects' low back pain prevalence was related to the work-related physiologic and psychologic factors. Therefore, for the effective prevention of low back pain, both practicing the body mechanics and raising the morale of the nurses are needed in educational program.

텍스트네트워크분석을 적용한 통증관리 간호연구의 지식구조 (Identification of Knowledge Structure of Pain Management Nursing Research Applying Text Network Analysis)

  • Purpose: This study aimed to explore and compare the knowledge structure of pain management nursing research, between Korea and other countries, applying a text network analysis. Methods: 321 Korean and 6,685 international study abstracts of pain management, published from 2004 to 2017, were collected. Keywords and meaningful morphemes from the abstracts were analyzed and refined, and their co-occurrence matrix was generated. Two networks of 140 and 424 keywords, respectively, of domestic and international studies were analyzed using NetMiner 4.3 software for degree centrality, closeness centrality, betweenness centrality, and eigenvector community analysis. Results: In both Korean and international studies, the most important, core-keywords were "pain," "patient," "pain management," "registered nurses," "care," "cancer," "need," "analgesia," "assessment," and "surgery." While some keywords like "education," "knowledge," and "patient-controlled analgesia" found to be important in Korean studies; "treatment," "hospice palliative care," and "children" were critical keywords in international studies. Three common sub-topic groups found in Korean and international studies were "pain and accompanying symptoms," "target groups of pain management," and "RNs' performance of pain management." It is only in recent years (2016~17), that keywords such as "performance," "attitude," "depression," and "sleep" have become more important in Korean studies than, while keywords such as "assessment," "intervention," "analgesia," and "chronic pain" have become important in international studies. Conclusion: It is suggested that Korean pain-management researchers should expand their concerns to children and adolescents, the elderly, patients with chronic pain, patients in diverse healthcare settings, and patients' use of opioid analgesia. Moreover, researchers need to approach pain-management with a quality of life perspective rather than a mere focus on individual symptoms.

여성 치매 노인의 통증, 신체적 기능, 인지적 기능, 우울, 초조에 관한 연구 (A Study on Pain, Physical Function, Cognitive Function, Depression and Agitation in Elderly Women with Dementia)

  • Purpose: The purpose of this study was to determine the relationship between assessed pain, functional status, and emotional status in elderly women with dementia. Methods: The method was a descriptive correlational design. Subjects were sampled from 75 elderly women with dementia who were resident in nursing home. and their pain, functional status(physical function, cognitive function), emotional status(depression, agitation) were measured. The collected data were analyzed for correlations between pain and functional status and for emotional status using the SPSS 11.0 statistical program. Results: The pain degree of the aged women in dementia were as follows; between 0 and 27 points, average 4.04 points, which was a possible point extent. Looking at the grades in detail items, the wry face expression was shown highest, an average of 0.84 points. The relation with cognitive function was(r=-.259, p<.025) a minus relation. And the relation with physical function was (r=.406, p<.001) a plus one. The relation with depression was (r=.462, p<.001), plus one. And (r=.592, p<.001) a plus relation was found with agitation. Conclusion: Pain is associated with impaired functional and emotional status. Major efforts are needed to improve nursing assessment and management of pain in this cognitive impaired population.

간호사의 암환자 통증관리 및 통증지식에 관한 연구 (Pain management and Pain knowledge of Nurses)

  • Purpose: This study is part of questionnaire survey concerned with the views of nursing staff for cancer patients. This study was done to describe cancer pain management problem, pain management knowledge. Methods: The participants were 188 nurses at cancer ward. Data were collected from July 2003 to August 2003. Data were collected with multiple-choice items and one open-ended question, which were constructed structured questionnaire. The data were analyzed by means of SPSS statistical software and content analysis. Results: Experiences of pain education are 53.7%, pain assessment sheet not use 86%, pain management status are partial. Education need of pain management was 87.8% patients in pain were very often(23.9%) or rather often (35.1%) cancer patients. The nurses are respond to open question. Pain management problems as assessed by nurses are categorized 11 item. The health professional problems are Knowledge deficit pain management, Incontrollable pain, Doctor's busy. The patient problems afraid of narcotics for addiction, side effect, distinguishing between physical and suffering, economical problem. The scores of knowledge about pain was average 16.7 score. The pain knowledge showed significant correlation education need(r=.180, p=.013). The effective variable was need of $education({\beta}=.163)$. Conclusion: It was found that pain knowledge was middle score and pain management problem was multiple. The study highlights the need to increase pain education for health professional. Therefore, This study suggest that health professionals education should be done to improve pain management problem.

수지뜸이 여성의 월경통 완화에 미치는 영향 (The Effect of Hand Moxibustion Therapy on Pain during Menstruation)

  • Purpose: The purpose of this study was to identify the effect of hand moxibustion for reduction of pain during menstruation. Method: Fifteen women were selected as subjects and data collection was conducted from September to December 2003. The Visual Analogue Scale (VAS) and Digital Infrared Thermotraphic Image (DITI) were used to measure the degree of pain and body heat. Result: The results showed that there was a significant reduction of pain during menstruation when hand moxibustion was performed. Also, there were significant differences in the change of body heat. These were left hands (Z=-2.805, p=.005), right hands (Z=-2.805, p=.005) and lower abdomen (Z=-2.803, p=.005). Conclusion: It was found that hand moxibustion decreased pain during menstruation and increased body heat. Therefore, hand moxibustion may be a good method for intervention to decrease pain during menstruation.

통증 개념을 다룬 국내 간호 연구 분석 (An Analysis of Nursing Research on Pain Reported in Korea from 1970 to 1994)

  • This study aimed at analyzing the trend of re-search on pain in Korea, suggesting direction future pain research, and contributing to the use of pain interventions in nursing practice. Research studies on pain were selected from journals of medical and nursing schools, the Korean Nurse, the Korean Nurses' Academic Society Journal, the Central Journal of Medicine, the New Medical Journal, and from theses and dissertations, which were conducted between 1970 and 1994. The total number of the studies was 93. These studies were analyzed for 1) time of publication or presentation, 2) thesis for a degree or nondegree, 3) research design, 4) characteristics of subjects used in each study, 5) measurement tool, 6) types of correlated variables, 7) Korean terms for pain 8) types of nursing interventions, and 9) results of studies. The findings of the analysis can be summerized as follows : 1) The number of studies related to pain has increased rapidly since the early 1980's. The number of experimental research studies related to pain has increased chronologically, but the number of survey research studies related to pain was highest from 1981 to 1985, after that it decreased slowly. 2) The subjects in 19 studies were healthy people and, in 73 studies, patients with various illnesses. Thirty two studies were conducted with surgical patients. 3) Sixty one pain research studies were done for a thesis for a degree and 32 were nondegree research studies. 4) As measurement tools for pain, self- report pain scales were used in 54 studies and more than two tools were used in 28 studies. In the experimental studies, the trend was to use more than two tools. And in the nonexperirnental studies, the trend was to use self-report pain scales only. 5) There were 11 correlational studies. In these studies, the trend was to study anxiety, depression and variables such as intravenous infusion as related to pain.6) In the thirty six experimental studies, the effects of 16 types of nursing interventions weretested. Teaching and information, and relaxation technique were the most popular interventions for pain. 7) In eighteen methodological studies, the majority were studies testing the validity and re-liability of Dr. Lee's Korean Pain Rating Questionnaire. The following suggestions are made based on the above findings : 1) The patterns of these studies related to pain in Korea need to be compared with trends in other countries. 2) Meta - analysis should be done to analyze and integrate the results of various studies. 3) This analysis of pain research is needed to identify the present trend of pain research and to suggest the direction of future pain research, so these patterns of studies should be done in 5 to 10 year intervals. 4) More replicated pain research is needed to prove the effect of nursing interventions and more qualitative research on pain is needed to identify indepth the meaning of pain. 5) Pain researchers should make an effort to apply research result in various clinical settings and try to carry out team research with clinical nurses or with other multidiscipinary researchers.

Assessment of pain and adequacy of pain management in hospitalized cancer patients

  • The author investigated pain experiences of 90 cancer patients and the adequacy of pain treatment they have received during their stay at a large medical center in T city between October 1994 and August 1995. Pain was assessed by the Shortened BPQ and results are summarized as follows: As for ratings of “worst pain” during the 24 hour period, 70% of the patients reported they had “severe” pain. As for ratings on “pain now,” 43% of the cancer patients reported “moderate to severe” pain. Over 46% of the patients reported a pain relief score of 0(not at all) or 1(somewhat) even after receiving pain medication. Adequacy of analgesic treatment was evaluated by comparing the patient's reported level of pain and the analgesic use, namely, the pain management index(PMI). The PMI indicated that 58% of the patients were undertreated for the pain control. In review of nurse's notes. systematic pain assessment was scarcely recorded, although pain documentation appeared in 70% of the notes; and the contents were mostly simple description. In conclusion, the results of patient's pain ratings, the PMI and poor pain documentation in the nurse's notes implied poor pain assessment and management.
