The Therapeutic Value of Ultrasound-Guided Irrigation in Sialodochitis: A Case Report

  • In the present case study, we reported a female patient with sialodochitis fibrinosa in which a conservative ultrasound (US)-guided irrigation method was used to relieve her symptoms. A 30-year-old female patient visited Kyung Hee University Dental Hospital with chief complaints of persistent swelling and discomfort in the left facial area. Her primary symptom was facial swelling that was accompanied by neuropathological symptoms, such as itching, tingling, and warmth that usually continued for 3 to 4 days. During clinical examination of orofacial area, mild swelling and fever were noted in the left face including parotid region, and mild induration could be felt at the corresponding site; sialadenitis of the left parotid gland was tentatively diagnosed. Herein, we performed intraductal irrigation of the left parotid gland in three times per month and prescribed some medication. Thus, her signs and symptoms have been improved, and she did not experience a recurrence for 12-month follow-up to date. This study can support the usefulness of the US-guided irrigation method to treat the sialodochitis fibrinosa without remarkable side effects.

견비통 완화 도인기공체조에 대한 기초적인 연구 (Basic Study on the Doin Gigong Exercise for the Reduction of Shoulder-arm Pain)

  • Gigong exercise is simple form's game for health care. Gigong exercise is the training method in mind and body, and it prevent or cure the disease by becoming larger the human's nature-healing power This syudy intends to identify the effective processing channel of the Doin Gigong Exercise for the reduction of shoulder-arm pain. The processing channel of the Doin Gigong Exercise for the reduction of shoulder-arm pain cinsist of warming, main, finishing exercise. The warming exercise start breathing and concentrate upon shoulder-arm exercise. The upper and lower sides join-hands exercise give help smoothly the Gi-circulation of human body in every direction. The finishing exercise consist of experience to Gi with the object of maintaining human comfortable condition. As mentioned above, the Doin Gigong exercise is effective intervention for the reduction of shoulder-arm pain.

Complementary and Alternative Medicine for treating Low Back Pain with Teaching Exercise: A narrative review

  • Objectives: In modern society, many people have low back pain (LBP) and spinal diseases. About 80% of them experience severe LBP more than once in their lifetime. We can find studies on many Korean medicine-based treatments, such as acupuncture treatment for LBP and exercise therapy, which are effective in reducing the symptoms. This study focuses on the combined effect of both Korean medicine and exercise therapy for treating LBP. Method: For this review, we searched for articles focusing on pain and disability recovery in pre-clinical and clinical studies of extension and flexion exercise therapy related to LBP. The search databases were as follows: PubMed, Google Scholar, and seven Korean electronic databases (Korea Citation Index (KCI), Korean studies Information Service System (KISS), Research Information Service System (RISS), Oriental Medicine Advanced Searching Integrated System (OASIS), DBPIA, National Digital Science Library (NDSL), and KOREAMED). The keywords were as follows: Korean Medicine, back pain, flexion exercise, extension exercise, McKenzie method, McKenzie exercise, Williams' flexion exercise, and Mechanical Diagnosis and Therapy. Results & Conclusions: This review shows the usefulness of flexion and extension exercises for LBP treatment and effective patient education, but further studies are necessary.

두통의 특성에 관한 연구 (A Study on Prevalence and Characteristics of Headache)

  • The purpose of this study is investigation on prevalence and clinical characteristics of headache among 10-39 years men and women. Information could be obtained with the questionnaire, and 1952 respondents completed the questionnaire. All participants were asked if they had experienced headache. Those responding positively were further questioned regarding the frequency, severity, character, location and duration of headache and were questioned regarding the experience of medical cunsultation and taking analgesics. The obtained results were as follows : 1. The life time prevalence of headache was 49.6% among men, 70.8% among women and 61.5% in all. 2. The prevalence of episodic tension type headache, chronic tension type headache, migraine, mixed type headache in this population was 27.1%, 5.0%, 2.8%, 23.1% respectively. 3. Headache duration was classified as less than 1 hour, 1-6 hours, 6012 hours, more than 12 hours for 1 day. The percentage of each other case is 41.8%, 46.2%, 7.4% and 4.4% of headache suffers respectively. 4. As regards the severity of headache, 4.2% of headache suffers had a very mild pain intensity, 17.9% a mild pain, 57.7% a moderate pain, 18.1% a severe pain and 2.2% a quite severe pain. 5. Headache occured at morning in 8.2% of headache suffers, at afternoon in 9.4%, at evening in 18.1%, at night in 4.3% and at anytime in 59.9%. 6. 11.7% of headache suffers among men consulted doctor, 13.9% among women and 13.1% in all. 7. 66.2% of headache suffers didnt have any analgesics because of their pain, 33.8% of headache suffers had analgesics because of their pain.

전자궁적출술후 시행한 정맥내 통증자가조절법의 임상 경험 (Clinical Experience on Intravenous Patient Controlled Analgesia after Total Abdominal Hysterectomy)

  • Background: We studied 150 patients who received intravenous patient controlled analgesia (PCA) after total abdominal hysterectomy to evaluate pain relief, analgesic consumption, patient's satisfaction and side effects. Methods: We made total 40 ml of analgesic mixture with morphine 40 mg, ketorolac 120 mg, droperidol 3 mg and normal saline. Loading/bolus/basal infusion dose and lockout interval was 2 ml, 1.5 ml, 0.5 ml/hr and 10 min, respectively. Numerical rating scale (NRS) pain score, cumulative analgesic consumption, degree of satisfaction, and incidence of side effects were evaluated. Also, correlation of age and edu ion with analgesic consumption were evaluated. Results: The average pain scores using NRS were $3.1{\pm}1.7$ (6 h), $2.1{\pm}1.5$ (24 h), $1.7{\pm}1.5$ (48 h). The average cumulative analgesic consumption were $11.7{\pm}5.0$ ml (6 h), $23.0{\pm}6.7$ ml (24 h), $32.1{\pm}3.7$ ml (48 h). The degree of satisfaction in postoperative pain control was good in 94% of patients. There was no correlation between degree of satisfaction and analgesic consumption. Also age and level of edu ion did not correlated with analgesic consumption. Conclusions: Intravenous PCA with morphine, ketorolac, and droperidol is an effective method of postoperative pain control because it provides adequate pain relief and a few side effects with high patient's satisfaction. However, age and level of education did not correlated with analgesic consumption.

Interventional Pain Management in Rheumatological Diseases - A Three Years Physiatric Experience in a Tertiary Medical College Hospital in Bangladesh

  • Background: Interventional pain management (IPM) is a branch of medical science that deals with management of painful medical conditions using specially equipped X-ray machines and anatomical landmarks. Interventional physiatry is a branch of physical medicine and rehabilitation that treats painful conditions through intervention in peripheral joints, the spine, and soft tissues. Methods: A cross-sectional study was conducted using three years of hospital records (2006 to 2008) from the Physical Medicine and Rehabilitation Department at Chittagong Medical College Hospital in Bangladesh, with a view toward highlighting current interventional pain practice in a tertiary medical college hospital. Result: The maximum amount of intervention was done in degenerative peripheral joint disorders (600, 46.0%), followed by inflammatory joint diseases (300, 23.0%), soft tissue rheumatism (300, 23.0%), and radicular or referred lower back conditions (100, 8.0%). Of the peripheral joints, the knee was the most common site of intervention. Motor stimulation-guided intralesional injection of methylprednisolone into the piriformis muscle was given in 10 cases of piriformis syndrome refractory to both oral medications and therapeutic exercises. Soft tissue rheumatism of unknown etiology was most common in the form of adhesive capsulitis (90, 64.3%), and is discussed separately. Epidural steroid injection was practiced for various causes of lumbar radiculopathy, with the exception of infective discitis. Conclusion: All procedures were performed using anatomical landmarks, as there were no facilities for the C-arm/diagnostic ultrasound required for accurate and safe intervention. A dedicated IPM setup should be a requirement in all PMR departments, to provide better pain management and to reduce the burden on other specialties.

정적 스트레칭이 중년 여성 어깨굳음증 환자의 통증과 운동범위에 미치는 영향 (Effects of Static Stretching on Shoulder Joint Pain and Range of Motion in Middle Aged Women Patients with Frozen Shoulder )

  • Background: This study aimed to determine the effects of static stretching on the pain and range of motion (ROM) of shoulder joints in middle aged women patients with frozen shoulders. Design: One group pretest-posttest design Methods: The participants were 15 middle aged women patients with frozen shoulders in their 40s to 60s. Subjective and objective pain and joint ROM(range of motion) were measured, and the static stretching intervention consisted of 15 minutes of flexion, abduction and external rotation stretching. Results: The results of this study indicated that the daily pain of shoulder joints and pressure pain thresholds of the muscles surrounding the shoulder joints were generally improved after the intervention with the static stretching, for which significant differences were observed (p<0.05). The ROM of shoulder joint flexion, abduction, and external rotation was significantly increased (p<0.05) after the intervention with the static stretching. Conclusion: Static stretching intervention in patients with frozen shoulders relieved shoulder joint pain and had positive effects on the ROM of shoulder joints. Thus, the application of static stretching in middle aged womenpatients who experience severe pain could be effective at enhancing the function of shoulder joints without pain.

극소체중아 어머니의 경험 (Experience of mothers with very low birth weight infant)

  • A phenomenological study is conducted to investigate the experience of mothers with very low birth weight. Sample of 8 mothers participate. Seven theme clusters emerge when the formulated meanings were organized into categories. The participants are 8 mothers who gave birth in the general hospital located in Kyunggi-Province and still live there. The data are collected from the intensive interviews on the experience of mothers whose baby is very low birth weight infant. Data collection is through in depth personal interviews two or three times with each mother. On the basis of the meanings of the experience of mothers with very low birth weight infant, the seven essential themes have been obtained Theme 1:Being surprised and perplexed due to the unexpected sudden delivery Theme 2:Experiencing emotional pain such as anxiety, sorrow, fear, confusion and self-blaming caused by a small and feeble baby. Theme 3: Pouring all their time and efforts into the baby under the acute tension Theme 4:Undergoing intolerable difficulties in confronting reality `Theme 5:Raising the special infant into an normal one by themselves Theme 6: Being on the vigilance for their baby even though they can be relieved Theme 7:Finding out a reward for the difficulties they have experienced The results obtained in this study will enable us to comprehensively understand the experience by mothers whose baby is very low birth weight infant and furthermore to provide a new insight which helps baby.

  • Stellate ganglion block is most commonly used in pain clinic because it is an easy procedure and it has broad indications reported that Angina pectoris, tachyarrhythmia and long QT syndrome (LQTS) are indicated. LQTS is a disorder of the abnormalities of cardiac sympathetic innervation and of myocardial repolarization. LQTS is characterized by marked prolongation of the QT interval, often manifestating as syncope, seizures, or sudden death due to polymorphic ventricular tachyarrhythmia known as torsades de pointes. Treatment of symptomatic patients usually begin with beta blocker. The elective treatment of LQTS patients unresponsive to beta blocker is the left cardiac sympathetic denervation. We report a case of LQTS patient who had received stellate ganglion block.

Use of Intermaxillary Traction Appliances and Exercises to Strengthen the Masticatory Muscles of Patients with Anterior Open Bite Caused by Temporomandibular Joint Osteoarthritis: Case Reports

  • Temporomandibular joint osteoarthritis (TMJ OA) causes destruction of the temporomandibular joint (TMJ) and can lead to occlusal changes such as anterior open bite in some patients. Consequently, these patients may experience difficulty in chewing food items and exhibit abnormal functional habits such as bruxism, preventing healing of the TMJ condyles. Treatment protocols include the use of traction appliances to reduce stress on the condyles. Unstable occlusions can lead to weakness of the masticatory muscles which, in turn, worsen the occlusal changes and complicate pain management. Therefore, the current study evaluated the condition of the masseter muscle using ultrasonography and educated patients on the execution of gum-chewing exercises for muscle strengthening. It also aimed to assess the effects of traction appliances and strengthening exercises on the masticatory muscles of patients with occlusal changes caused by TMJ OA.
