  • Brachial radiculoplexitis is characterized by acute onset of shoulder and arm pain followed by weakness and sensory loss. Brachial radiculoplexitis by herpes zoster is a rare disease, which can be diagnosed by careful history, electrodiagnosis and MRI. It has remained uncertain about clinical characteristics, treatment, and prognosis. Better understanding of this disease helps earlier diagnosis and prompt treatment to minimize neurologic sequale. We present two cases of subacute brachial radiculoplexitis preceded by herpes zoster infection.

  • Herpes zoster is caused when the varicella zoster virus(VZV) that has remained latent since an earlier varicella infection is reactivated with cutaneous and mucous manifestations. They occur in 20% cases in the trigeminal area and typical manifestations are neuralgias simulating dental pain, also vesicles with an erythematous halo located in the territory of the second and third trigemial branch. They erupt on the skin, the lips, tongue, palate and cheeks. With an ever-increasing number of elderly and immunocompromised patients attending the dentist, the dental profession can expect to encounter an increased number of herpes zoster patients. Furthermore, the oral and maxillofacial surgeons must be familiar with the presenting signs and symptoms of patients experiencing the prodromal manifestations and oral complication of herpes zoster of the trigeminal nerve. As presentation of our patient with ulcer on hard palate caused by herpes zoster, current treatment of herpes zoster and post-herpetic neuralgia are discussed.

Clinical Studies on 11 Cases of Facial Nerve Palsy in Herpes Zoster Oticus (이성대상포진으로 인한 안면신경마비환자 11례에 대한 임상적 고찰)

  • Background: Herpes Zoster Oticus, which can be caused from infection by Varicella-zoster virus, is known to be expected poor prognosis, since it has higher possibility of complete injury of facial nerve. Conservative therapy of the Western medicine is effective on the treatment for the herpes zoster and pain. However, its effectiveness on the facial verve palsy has not been proven. Objective : This study was performed to evaluate the treatment of oriental medicine and an analyze prognosis of the facial nerve palsy in Herpes Zoster Oticus. Methods : This retrospctive study was conducted to observe the clinical manifestation of Herpes Zoster Oticus, and the result of electrical diagnostic study on the patients with the facial nerve palsy in Herpes Zoster Oticus who were diagnosed and treated at the Department of Acupuncture & Moxibustion in Kyung Hee Oriental Medical Hospital. Results : 1. According to the six clinical types of Herpes Zoster Oticus, the patients in this study can be grouped as 7 cases of the type II, 2 cases of the type V, and one case of the type IV and the type VI. 2. The shorter the period from onset to the first visit to a hospital, the prognosis of facial nerve palsy in Herpes Zoster Oticus was the better. 3. According to the follow up study, the H-B grades of all cases were improved. 4. According to the follow up study, the result of electrical diagnostic study was improved. Conclusions : We found in our study that the treatment of oriental medicine, such as acupuncture, herbal medicine and aroma therapy, helps the recovery of facial nerve palsy in Herpes Zoster Oticus. The further clinical study about the more cases of facial nerve palsy in Herpes Zoster Oticus and comparative study between the group of oriental medicine and that of western medicine are needed.

  • A case of a 72-year-old Korean male with herpes zoster ophthalmicus revealed. He was treated with Korean-Western cooperative treatment. We used the Numeric Rating Scale (NRS) to evaluate forehead and eye pain. Pain was improved after the administration of Korean medical treatments and disappeared after eight days. Korean-Western cooperative treatment may be effective in treating acute herpes zoster ophthalmicus.

  • Objectives: The purpose of this study is to report the effect of Korean medicine treatment in a patient with Herpes Zoster (HZ) during pregnancy. Methods: During 12 days of hospitalization, a patient was treated by Korean medicine treatment such as herbal medicine including Gami-danggui-san, acupuncture, cupping and fumigation. To evaluate the effect of treatment, we took pictures of skin lesion and symptoms were measured by Numeric Rating Scale (NRS), Short-Form McGill Pain Questionnaire (SF-MPQ) and 36 item Short Form Health Survey (SF-36). Results: After Korean medicine treatment, most of vesicles disappeared. NRS of chief complaints such as neuralgic pain and itchness changed 7-8 to 0 and therefore, quality of life of the patient was elevated sharply. Conclusions: This case shows that Korean medicine treatment is beneficial and safe for the patients who suffered from Herpes Zoster during pregnancy.

  • Zoster-associated pain (ZAP) in patients with herpes zoster (HZ) may persist for a long time, occurring even years after the rash has healed. In this case, the patient is diagnosed as having postherpetic neuralgia (PHN). Prodromal symptoms can present with constant or intermittent pain, and are often accompanied by other symptoms, resulting in misdiagnosis and/or inappropriate treatment. The aim of this study is to investigate the characteristics of the prodromal symptoms of ZAP through a multicenter study in Korea.

  • Shingles is a latent viral infection of the sensory ganglia. It can be accompanied by a variety of neurologic complications, including polyradiculitis and myelitis. A 66-year-old man with diabetes mellitus presented with progressive weakness, hypethesia and neuralgic pain in his right arm after herpes zoster infection in right C5 dermatome. He was diagnosed with zoster polyradiculomyelitis and treated with intravenous acyclovir and corticosteroid. It is a rare case of zoster neurologic complication in spite of oral acyclovir treatment.

  • Objectives : The purpose of this study is to report the effect of Korean medical treatment in patient with Herpes Zoster and hyperemesis gravidarum during pregnancy. Methods : 10 weeks pregnant patient with Herpes Zoster and hyperemesis gravidarum was treated by herbal medicine ( Bosaeng-tang ) and acupuncture. We measured visual analogue scale (VAS) and observed the change of skin lesion. Results : After treatment, vesicles and crusts of right buttock and lower leg clearly dis appeared. Patient no more suffered from nausea and vomiting. VAS (Visual analogue scale) of pain changed 8 to 0. After 7months, the patient gave birth to a healthy child. Conclusions : This case report shows that the Korean medical treatment is useful and safe in the treatment of Herpes Zoster and hyperemesis gravidarum during pregnancy.

  • Objectives : There were few reports on Erysipelas, from Secondary Skin Infection after Herpes Zoster. We treated Erysipelas, from Secondary Skin Infection after Herpes Zoster with Oriental medical approach, and got a significant results. So We are reporting this case. Methods : Herb medication, acupuncture treatment were applied for treating patient`s chief symptoms (erythema, heat sensation, swelling, pain, itching and general weakness) Results : The patient was 5-year-old boy who had the symptoms caused by Erysipelas, which was caused by secondary skin infection after herpes zoster. He had erythema, heat sensation, swelling, pain, itching and general weakness. We judged him as Soyangin(少陽人), and prescribed Hyeongbangsaback-san. After he took Hyeongbangsaback-san, the symptom and general condition were improved. In this report, we want to explain the healing process and the result of the treatment of Erysipelas. Conclusions : More study about oriental treatment on Erysipelas, from Secondary Skin Infection after Herpes Zoster is needed.

A Case Report of Herpes Zoster Patient Improved by the East-west Medical Combined Treatment (대상포진에 대한 한양방 병행치료 치험 1례)

  • Objectives : The aim of this study is to report a case of a 51-year-old patient with Herpes zoster, whose symptoms were relieved within five weeks after the East-west medical combined treatment. Methods : A Herpes zoster patient with extreme pain around the right lower ribs and lower back was admitted into the Korean medicine hospital. Bee venom acupuncture was applied directly onto the site of pain and electroacupuncture was applied on $GV_6-BL_{20}$, inferior site of spinous process of the 12th thoracic vertebra-$BL_{21}$, $GV_5-BL_{22}$, $GV_4-BL_{23}$ six times a week. Moxibustion was applied on $BL_{49}$, $BL_{50}$, $BL_{51}$, $BL_{52}$, $GB_{25}$, $LR_{13}$ six times a week, and the patient took daily doses of herbal medicine and western medicines such as anticonvulsants, analgesics, antiepileptics. The patient's symptoms were evaluated with numeric rating scale(NRS) and neuropathic pain scale(NPS). Results : The results were as follows: 1. After the East-west medical combined treatment, the NRS grade decreased from 8 to 1. 2. On the neuropathic pain scale, the patient's symptoms changed from deep sharp pain and burning sensation to superficial dull pain and coldness. Sensitivity to touch, discomfort, and itchiness also decreased. Sudden attacks of pain appeared 14 to 21 days after onset, but disappeared after final treatment. Conclusions : The results of the above patient suggests that the East-west medical combined treatment may be effective for total remission of herpes zoster. Further research is needed to confirm such effects.
