직접구 치료에 의해 호전된 척추수술후통증증후군 환자 5례 (Case Series of 5 Failed Back Surgery Syndrome Patients Who were Treated by Direct Moxibustion Therapy)

  • Objectives : To evaluate the effectiveness of direct moxibustion in patients with failed back surgery syndrome (FBSS). Methods : 5 patients (3 females and 2 males) with FBSS underwent direct moxibustion on ouch points (阿是穴) for 10 to 15 weeks (once a week). The effectiveness of the treatment was evaluated with NRS (numerical rating scale), ODI (Oswestry disability index), BDI (Beck depression inventory) score, SF-36 (short form 36 health survey) score and PSQI (Pittsburgh sleep quality index) before and after the treatment. Results : Within observation period, average value changed from 10.0 to 4.2 in NRS of pain, from 37.0 to 20.6 in ODI, from 20.6 to 12.0 in BDI score, 42.2 to 62.6 in SF-36 score, and from 8.6 to 5.2 in PSQI. Conclusions : Direct moxibustion on FBSS patients showed more or equivalent effectiveness when compared to conventional FBSS treatment modalities.

일차성 불면증의 추나 치료에 대한 문헌 연구 보고 (Chuna Manual Therapy for Primary Insomnia; A Review of Clinical Study)

  • Objectives To evaluate the evidence supporting the effectiveness of Chuna manual therapy for primary insomnia. Methods The researcher conducted search across the 3 electronic databases (Pubmed, CAJ and Oasis) to find all of randomized controlled clinical trials(RCTs) that used Chuna manual therapy as a treatment for primary insomnia. Results Four RCTs met inclusion criteria. The meta-analysis showed positive results for the use of Chuna manual therapy in terms of the PSQI when compared to medication treatments alone. Positive results were also obtained, in terms of the PSQI, when comparing Chuna manual therapy combined with acupuncture therapy to acupuncture therapy alone, but was not statistically significant. Conclusions The review found encouraging but limited evidence of Chuna manual therapy for primary insomnia. We recommend clinical trials which compare the effectiveness of Chuna manual therapy with usual care to obtain stronger evidence without the demerits of trial design.

가미귀비환(加味歸脾丸) 투여로 호전된 갱년기 증후군 환자 치험 50례 (50 Case Reports of Climacteric Syndrome Treated with Gamiguibi-hwan)

  • Objectives: The purpose of this study is to evaluate the effects of Gamiguibi-hwan on Climacteric Syndrome. Method: We treated 50 cases menopausal disorder patients with Gamiguibi-hwan for 12 weeks. We estimated the effects by Menopause Rating Scale (MRS), Pittsburgh Sleep Quality Index (PSQI) and Beck's Depression Inventory II (BDI-II). Results: After treatment, the mean of MRS, PSQI and BDI-II score were decreased. Conclusions: This case shows that Gamiguibi-hwan is effective in treating menopausal disorder patients.

  • Objective: This study reports the effects of Gyejigeojakyakgachokchilmolyeoyonggol-tang (GCMYT) in a patient with insomnia after COVID-19 vaccination. Methods: GCMYT was used to treat insomnia following vaccination, and symptoms were evaluated using the Pittsburgh Sleep Quality Index (PSQI), Insomnia Severity Index (ISI), and a visual analog scale (VAS). Results: The patient's PSQI, ISI, and VAS scores decreased from 21 to 11, from 25 to 13, and from 7 to 3, respectively. Conclusion: GCMYT is an effective treatment for insomnia following COVID-19 vaccination.

Effects of sleep-inducing juice on sleep quality and heart rate variability in adults with disturbed sleep

  • BACKGROUND/OBJECTIVES: Although some juices affect subjective sleep quality, there is a lack of information on the effect of a specific juice on objective sleep quality and heart rate variability (HRV) during sleep; thus the present study investigated whether a blended juice made from natural extracts influenced sleep quality and HRV during sleep in adults with disturbed sleep. SUBJECTS/METHODS: A randomized, crossover study was conducted on twenty-five adults (15/10, female/male) complaining of difficulty initiating or maintaining nighttime sleep (Pittsburgh sleep quality index [PSQI] ≥ 5). During feeding sessions (FS), subjects received sleep-inducing juice made of natural ingredients (250 mL/trial) twice a day for 8 weeks or non-FS (N-FS) for 8 weeks while maintaining normal activities. Sleep quality and parameters were recorded via wearable actigraph for 7 consecutive days, and PSQI scores were assessed before and after the intervention. HRV was also monitored at rest and during sleep. RESULTS: After receiving the sleep-inducing juice intervention (FS), PSQI scores were significantly decreased (P < 0.001) and correlated with a significant decline in fatigue severity scale and visual analogue scale levels (P < 0.05; both). HRV indices of vagal activity were significantly improved during FS (P < 0.05), and no significant differences in N-FS were observed. Sleep efficiency and total sleep time increased significantly (P < 0.05) and sleep latency, total counts, sleep fragmentation index, and movement index, decreased significantly (P < 0.05, all 4) during FS, with no significant differences-observed during N-FS. CONCLUSIONS: This study results demonstrated that an 8-week course of sleep-inducing juice has led to improve sleep quality, suggesting an enhanced cardiac vagal tone during sleep. Thus, it could be a well-tolerated option for adults with disturbed sleep.

노년기 요실금 여성의 하부요로증상, 신체활동, 우울이 수면의 질에 미치는 영향 (Influence of Lower Urinary Tract Symptoms, Physical Activity, and Depression on the Quality of Sleep in Elderly Women with Urinary Incontinence)

  • Purpose: This study was to identify factors influencing the quality of sleep in elderly women with urinary incontinence and examine the relationship among the factors. Methods: The study investigated 113 elderly women with urinary incontinence who were subject to the visiting care program at health centers in D-gu, D-si, South Korea. The collected data were analyzed using IBM SPSS version 19.0 along with ${\chi}^2-test$, t-test, ANOVA, Pearson's correlation, and multiple regression. Results: The total Pittsburgh Sleep Quality Index (PSQI) of the participants was found to be $11.41{\pm}4.34$. As a result of the correlation analysis, PSQI was found to have significant correlations with Female Lower Urinary Tract Symptoms (BFLUTS) (r= .32, p< .001). The Korean version of Physical Activity Scale for Elderly (K-PASE) was (r= -.34, p< .001) and the Geriatric Depression Scale Short Form (GDS-SF) was (r= .23, p= .012). Multiple regression analysis found sleeping pill use (${\beta}=-.35$, p< .001) had the strongest influence on PSQI, followed by physical activity (${\beta}=-.20$, p< .001), depression (${\beta}=.20$, p< .001), and lower urinary tract symptoms (${\beta}=.19$, p< .001). The total model explanatory power was 28.3% (F= 12.07, p< .001). Conclusion: The study findings are expected to be useful for enhanced understanding of the quality of sleep in elderly women with urinary incontinence.

The Effect of Hypnotics on Sleep Quality and Cognitive Function in Patients with Brain Tumors

  • Objective : We investigated the effect of hypnotics on sleep quality, cognitive function, and depressive mood in patients with insomnia following brain tumor resection. Methods : From patients who underwent brain tumor resection, we recruited 10 patients with insomnia who received hypnotics for more than 1 week during a 3-week follow-up period (insomnia group). We also recruited 12 control patients with brain tumors but without insomnia (control group). We evaluated sleep quality at baseline and 3 weeks later using the Insomnia Severity Index (ISI), the Pittsburgh Sleep Quality Index (PSQI), the Stanford Sleepiness Scale (SSS), and the Epworth Sleepiness Scale (ESS) and investigated cognitive function and depression using the Computerized Neuropsychological Test and the Beck Depression Inventory (BDI). Results : At baseline, SSS, ISI, PSQI, and BDI scores were significantly higher and visual continuous performance test (VCPT) and auditory continuous performance test (ACPT) scores were significantly lower in the insomnia than in the control group. Three weeks later, the patients who had received hypnotics had significantly higher ISI, PSQI, ESS, VCPT, ACPT, visual span forward and backward, and visual recognition test scores, and significantly lower BDI scores. Conclusion : Quality of sleep in patients with insomnia following brain tumor resection was initially poor but improved significantly after taking hypnotic medication. Further, the hypnotic medications appeared to contribute to the amelioration of cognitive impairments and depressive moods in patients who previously underwent brain tumor resection. We thus recommend the use of hypnotics for patients with brain tumors with insomnia.

회전근개 부분 파열환자의 조기 근력강화운동이 어깨관절의 통증, 기능 및 수면의 차이 (Difference of Early Muscle Strengthening Exercises on Pain, Function and Sleep Quality for Rotator Cuff Partial Tear Patients)

  • Purpose : The purpose of this study was to evaluate the difference of early muscle strengthening exercise(starting at 1 week) on pain, function and sleep quality for rotator cuff partial tear patients. Method : Experimental group 15 people, control group 15 people, was a total of 30 people. Group-specific arbitration method, was applied to early muscle strengthening exercise (experimental group) and general movement treatment(Starting at 6 week) (control group). Both groups were subjected to 40 minutes of basic physical therapy interventions four times a week over an six-week intervention period. In this study, Pain, function and sleep quality measured by VAS, PPT and ROM, CMS, SST and PSQI. Result : 1. The VAS and PPT in the experimental group and control group before and after treatment were significant difference(p<.01). Comparison between the two groups in the change in motion before and after treatment also showed a statistically significant(p<.01). 2. Comparison functional Assessment of the shoulder ROM, CMS and SST in the experimental group and control group before and after treatment were significant difference(p<.01). ROM was not significant differences between the groups but CMS and SST were significant differences between the groups. 3. PSQI in the experimental group and control group before and after treatment were significant difference(p<.01). Comparison between the two groups in the change in motion before and after treatment also showed a statistically significant(p<.01). Conclusion : Therefore, early muscle strengthening exercise is useful to improve the shoulder function and pain for rotator cuff tear patients.

The Characteristics of Sleep in Headache Patients

  • Purpose: To investigate the relationship between headache and sleep by evaluating sleep quality, daytime sleepiness, and specific features related to sleep-disordered breathing (SDB). Methods: One hundred one subjects with headache and 118 healthy controls were enrolled. To collect various information on headache attacks, headache group completed self-reported questionnaire about the characteristics of headache attacks and the migraine disability assessment (MIDAS) questionnaire. The subjective quality of sleep was evaluated in all of the subjects using the Pittsburgh sleep quality index (PSQI) and Epworth sleepiness scale (ESS). In addition, the following specific features of sleep were evaluated in 28 subjects selected randomly from each group: apnea-hypopnea index (AHI), prevalence of SDB, nocturnal oxygen saturation (SaO2), and oxygen desaturation index (ODI) as measured using a portable monitoring device. Results: The global PSQI and the prevalence of poor sleeping (global PSQI >5), ESS scores and the prevalence of daytime sleepiness (ESS score >10) were significantly higher in the headache group (both p<0.0001, respectively). The mean scores on the numerical rating scale and the MIDAS were significantly higher in the poor-sleeper group than in the good-sleeper group (p=0.0347 and p=0.0016, respectively). The global PQSI and prevalence of daytime sleepiness were significantly higher in the chronic-headache group than in the acute-headache group (p=0.0003 and p=0.0312, respectively). Conclusions: There is a significant association between headache and sleep. Especially, severity and chronicity of headache were significantly associated with sleep quality and daytime sleepiness.

제조업 남성 이주노동자의 직무스트레스와 수면의 질이 우울수준에 미치는 영향 (Effect of Occupational Stress and Sleep Quality on Depression among Male Immigrant Workers Manufacturing Industry, Korea)

  • Objectives: This study was conducted to assess the level of depression and its association with sociodemographic characteristics and health related characteristics, job-related characteristics, sleep quality, occupational stress among manufacturing male immigrant worker in Korea. Methods: Total number of subject were 287 workers employed in 19 manufacturing industry. A structured self-administered questionnaire was used to assess each responder's sociodemographic and health related characteristics, job-related characteristics, sleep quality (PSQI), Korean occupational stress (KOSS), Depression (CES-D). The job stress questionna ire was used according to KOSS-26 and depression symptoms were measured using CES-D, which was translated into Korean. We used multiple logistic regression analysis to assess the relationship between risk factors included job stress, sleep quality and depression. Results: The prevalence of depression in survey subjects was 63.9%and poor group (PSQI) (5 point and over at PSQI score)among all subjects were 83.28%. Adjusting for confounding factor, higher the occupational stress (KOSS), depression levels were significantly higher (${\beta}=0.5575$), worse quality of sleep, the more depressed the level was slightly high ${\beta}=0.118$) did not reach statistical significance. Conclusions: The prevalence of depressive symptoms among the study subjects was higher than that among the Korean workers. Especially, the level of depression was more influenced by occupational stress than quality of sleep. Therefore, the author recommend to prepare the effective strategy for reducing the depression and also reducing the occupational stress among immigrant male workers, and the author consider that it need to support the convenient working environment such as consultation for difficulty or health care among immigrant male workers.
