고유수용성 신경근 촉진법이 만성요통을 가진 노인환자의 체간 안정성과 균형에 미치는 영향 -등장성 수축 결합과 율동적 안정화 기법 적용- (Effects of Proprioceptive Neuromuscular Facilitation on Trunk Stability and Balance in Elderly People With Chronic Low Back Pain ; The Application of Rhythmic Stabilization and Combination of Isotonic)

  • Purpose : The purpose of this study was to examine the effects of combination of isotonic and rhythmic stabilization in proprioceptive neuromuscular facilitation on trunk stability and balance in elderly people with chronic low back pain. Subjects : Thirty-two elderly people ($73.78{\pm}8.49$ years of age) who had complaints of CLBP were randomly assigned to 2 groups: experimental group and control group. Methods : Subject trained with combination of isotonic and rhythmic stabilization in proprioceptive neuromuscular facilitation for 4weeks with aim of improving trunk stability and balance. Weight distribution, trunk muscle balance, static balance ability and dynamic balance ability were measured before, at the end of training. Results : Data were analyzed using two-way ANOVA. After the exercise programs, there were significant differences in the weight distribution, trunk muscle balance and dynamic balance ability between the experimental and control group. However there was no significant difference in the one leg stance test(p<.05). Conclusion : This study suggest that PNF programs may be appropriate for improving trunk stability and balance in elderly people with CLBP.

  • The purpose of this study was to find the effects of scapular pattern and hold-relax technique of propriocetive neuromuscular facilitation(PNF) on the range of motion(ROM) and to find the effects of pain relieve through visual analogue scale(VAS) in frozen shoulder patients. The subjects were consisted of thirty frozen shoulder patients(men ; 9, women ; 21). The measurement of the shoulder range of motion was taken by measuring the degrees of flexion, abduction, and external rotation with a goniometer and the measurement of pain was performed by VAS from pre treatment to one to four weeks The results were as follows : 1. There was the statistical significance not only in the range of motion such as shoulder flexion, abduction, and external rotation, but also in VAS during four weeks(p<.05). 2. By the result of Scheffe's multiple comparison test. there was statistical significance between the values of pre treatment and treatment after one week in the range of motion of shoulder flexion. In that of shoulder external rotation, there was statistical significance between the values of pre treatment and treatment after one week, and between those of treatment after one and two weeks. As the result of this statistical significance of shoulder external rotation, the early treatment was thought to be more effective. However, there was statistical significance in the treatment after two and three weeks in shoulder abduction and was statistical significance between the values of pre treatment and the treatment after one weeks, and between those of treatment after two and three weeks in VAS (p<.05). 3. There was not statistical significance between men and women in the range of motion of the shoulder and in VAS(p>.05). 4. There not statistical significance between th right and left frozen shoulder patients in the range of motion of shoulder and in VAS(p>.05).

  • Purpose: The purpose of this study was to determine the effects of a modified abdominal draw-in maneuver on trunk stability and functional capacity as well as pain in patients with chronic low back pain. Methods: The study included 3 patients with chronic low back pain who volunteered to participate. The modified abdominal draw-in maneuver included a posterior pelvic tilt, a traditional abdominal draw-in maneuver, and a vibration sensory feedback device. Voluntary abdominal contraction using the vibration sensory feedback device was performed by the subjects for more than 1 hour per day, 5 times per week, for 6 weeks along with common low back pain treatment. Electromyographic signals in the rectus abdominis (RA), external oblique (EO), internal oblique (IO), and erector spinae (ES) muscles were measured to compare muscle activation. The degree of pain was measured using the visual analogue scale (VAS), and functional capacity was measured using the Korea Oswestry Disability Index (K-ODI). All results were compared to the means before and after intervention. Results: After the intervention, the RA, EO, and IO showed increased muscle activation and the ES showed decreased muscle activation. The visual analogue scale decreased after intervention and the K-ODI decreased after intervention. Conclusion: Modified abdominal draw-in maneuvers in daily life combined with therapeutic exercises may be effective in relieving pain and dysfunctions in chronic low back pain patients.

  • Purpose: This study examined whether a single application of a lumbar flexion muscle energy technique (MET) could significantly influence pain, functional status, and range of motion in chronic back pain patients with restricted active trunk flexion. Methods: Twenty volunteers were randomly assigned to either a control group or an experimental group. Ten subjects in the control group received no treatment. Ten subjects in the experimental group received five sessions with the muscle energy technique, five times per week for two weeks. Outcomes were measured before and after two weeks of treatment in both groups. Pain intensity was assessed using a visual analogue scale (VAS). Range of motion was measured using the sit-and-reach test (SRT). Functional disability levels were assessed using the Korean version of the Oswestry Disability Index (KODI). Results: The experimental group showed a significant difference in VAS, KODI, and SRT between pre- and post-treatment (p < .05), while the control group showed no significant changes. The changes in VAS, ODI, and SRT were statistically significant between the groups pre- and post-treatment (p < .05). Conclusion: This muscle energy technique is clinically effective for chronic low back pain patients.

  • Purpose : The purpose of this study was to examine the effects of Rhythmic Stabilization(RS) and Combination of Isotonic(CI) of Proprioceptive Neuromuscular Facilitation on balance in patients with hemiplegia. Methods : Ten patients with hemiplegia were trained with RS and CI for six weeks. Two Standard Scale(TSS), Functional Reach Test(FRT), Time Up and Go test(TUG) and Berg Balance Scale(BBS) were used to prove their improvement of balance. Results : Body weight bearing decreased on less affected side and increased on more affected side. Distance of forward reaching for FRT increased. Time for TUG decreased. BBS score increased. They all showed significant difference. Conclusion : This study suggests that balance training with RS and CI of Proprioceptive Neuromuscular Facilitation improved balance in patients with hemiplegia.

  • Purpose : The purpose of this article is to summarize the characteristics of isotonic combination. Method : Some studies of the motor unit activation patterns during isometric, concentric, and eccentric actions, neural strategies in the control of muscle force, and concentric versus combined concentric-eccentric training were reviewed. Results & Conclusions : Eccentric torque may be relatively higher than concentric torque for two potential reasons: 1) stretch responses in the antagonist are not elicited to restrain the motion as can occur concentrically and 2) stretch responses in the agonist may augment eccentric torque production. Concentric-eccentric training has a greater influence on functional capacity than that of concentric training. Both maximal force and average force throughout the motion were significantly higher when the dynamic action was started with preactivation as compared to the mode without preactivation. The peak torques observed during the concentric phase of the eccentric-concentric muscle actions were higher than those noted in the pure concentric contraction.

수축-이완과 수동신장 기법의 단기 적용이 슬괵근의 길이 적응에 미치는 영향 (Effects of Short-term Application of Contract-Relax and Passive Stretching on the Length Adaptation in Harmstring Muscles)

  • Objectives: Few studies have reported on the flexibility of the hamstrings resulting from variable stretching method, and little studies of length adaptation at 1 day after intervention. Methods: Fifty-four healthy a woman in her twenties with no history of musculoskeletal or neurogenic disorder volunteered for this study. Subjects were randomly assigned to either a contract-relax group or a passive stretching group. Methods: Hamstring flexibility was measured with a sit and reach test(SRT) (RF-D18; SPC) before, immediatly after interventions, and 1 day after interventions. Results: Flexibility scores for participants in each of the groups significantly increased from pre-interventions to immediate and 1 day after interventions. However, the length of 1 day after interventions was shorter than immediate length after interventions. Difference in length between immediate and 1 day after interventions was some shorter in the contract-relax group versus the static group but, not significantly. Conclusions: The results of this study suggest that a contract-relax is an effective length adaptation of hamstring muscles by 1 day after intervention.

  • Purpose: The purpose of this study is to compare changes in the flexibility of hamstring muscles which are relaxed on suboccipital muscle through suboccipital muscle inhibition and combination technique. Methods: Thirty sample subjects (16 male and 14 female) were randomly divided into an experimental group (n=15) and a control group (n=15). 1. Suboccipital muscle inhibition: Suboccipital muscles of the patients were placed on a bed. The occipital region was placed on a therapist's fingers, and the posture was maintained for three minutes before remeasurement. 2. Treatment with combination technique (body bolster and wooden pillow): Wooden pillows were placed where the participants could relax the posterior arch of the atlas; additional body bolsters were also placed to avoid high pressure, and the treatment was maintained for three minutes before remeasurement. Results: There was a statistical difference in finger floor distance (FFD) and straight leg raise (SLR) results for those treated with SMI (P<0.05). There was no statistical difference in FFD and SLR results for those treated with the combination technique (P>0.05). Conclusion: Between the two intervention methods; the suboccipital muscle inhibition technique was more effective in increasing the flexibility of subjects with a shortened hamstring than was the combination technique.

수축-이완 기법 방법에 다른 최대 힘과 무릎관절 가동 범위에 미치는 즉각적인 효과 비교 -예비연구- (Immediate Effects of Various Contract-Relax Techniques on the Peck Force and Range of Motion of Knee Extension -A Pilot Study-)

  • Purpose: This study aimed to compare the immediate effects of various contract-relax (CR) techniques on the peck force and passive range of motion (ROM) of knee extension. Methods: This study recruited 17 adult males and females who obtained below $90^{\circ}$ in straight leg raising. The subjects randomly performed one of three different CR techniques (direct CR, indirect CR, and modified CR) on the right knee extension four times per day for three days. The subjects' peck force during each CR and their passive knee extension on the hip at a $90^{\circ}$ flexed position before and after each CR were measured. Results: The peck force was significantly higher in the direct and modified CR than in the indirect CR. The ROM in the passive knee extension significantly increased after direct and indirect CR. Comparing between each CR, the passive knee extension range was significantly more improved in the direct and indirect CR than in the modified CR. Conclusion: Direct and indirect CR are more effective than modified CR in the ROM of passive knee extension. The study results suggest that the modified CR did not have the same effect on the ROM of the knee extension as the direct and indirect CR and that further research is needed to recruit more subjects and to determine the effects on different muscles after modified CR.

  • Purpose: The purpose of the present study was to compare the effects of both general hold-relax techniques and hold-relax techniques using pumping on pain due to delayed muscle pain and on the range of motion (ROM) of joints. Methods: Thirty-nine young adult males and females were randomly assigned either to a hold-relax technique application group of 20 subjects or to a group of 19 subjects with hold-relax techniques applied using a pumping application. Tenderness thresholds, the ROM of joints, and pain intensities were measured on the biceps of the nondominant arm of the subjects in both groups before exercises were performed to induce delayed muscle pain. Tenderness thresholds were again measured 24hours, 48hours, and 72 hours after inducing delayed muscle pain. The relevant intervention methods were applied to the two groups after conducting the measurement at 48hours. As a statistical analysis method, repeated measure ANOVAs were conducted to examine the tenderness thresholds, ROMs of elbow joints, and pain intensities in the individual groups at the time points. Results: At 48 hours and 72 hours after application of the interventions, the general hold-relax technique application group showed greater changes in the tenderness thresholds, the ROMs of elbow joints, and the pain intensities than did the group applying hold-relax techniques using a pumping application (p>0.05). Conclusion: Based on the results of the present study, the application of hold-relax techniques is thought to have beneficial effects on pain resulting from delayed muscle pain and on limited ROMs of joints.
