PNF 결합패턴이 뇌졸중 환자의 하지 근 활성도 및 보행능력에 미치는 영향 (Effect of PNF Combination Patterns on Muscle Activity of the Lower Extremities and Gait Ability in Stroke Patients)

  • 본 연구는 PNF 결합패턴이 뇌졸중 환자의 마비측 하지 근 활성도와 보행 능력에 어떠한 영향을 미치는 지를 알아보고자 실시하였다. 뇌졸중 환자 20명을 대상으로 하여 PNF 결합패턴(주 4회, 6주간)을 적용하는 실험군(n=10)과 일반 운동치료만을 적용하는 대조군(n=10)으로 무작위 배정하였다. 하지 근 활성도 측정은 활동전위의 실효치 값으로 분석하였고, 보행능력검사는 10MWT, DGI, F8WT를 이용하여 측정하였다. 운동 적용 방법에 따른 측정 시점에 대한 대조군과 실험군의 유의성 검정은 Two-way repeated measure ANOVA를 실시하였다. 그 결과, 근 활성도를 알아보기 위해 측정한 RF(p<.05), VM, TA, LH, LG(p<.001) 근육 모두 각 측정 시기에서 통계적으로 유의한 증가가 있었으며, VM, TA, LH, LG(p<.001)에서 측정시간과 군 간의 상호작용이 나타났다. 보행능력을 알아보기 위해 실시한 10MWT, DGI, F8WT 검사에서도 각 측정 시기에서 통계적으로 유의한 향상이 있었으며(p<.001), 10MWT(p<.05), DGI(p<.001), F8WT(p<.01)에서 측정시간과 군 간의 상호작용이 나타났다. 본 연구의 결과로 보아 PNF 결합패턴은 뇌졸중 환자의 마비측 근 활성도를 증가시키는데 도움을 주고, 보행 능력 개선에 유용한 전략으로 사용될 수 있음을 증명하였다.

협응이동훈련을 위한 PNF 패턴의 결합 (The Combination of PNF Patterns for Coordinative Locomotor Training)

  • Purpose : To identify importance of coordinative locomotor training, we reviewed the coordinative locomotor training using sprinter & skater with respect to motor control theory. Methods : We reviewed literatures related with sprinter & skater and coordination.. Results : The coordinative locomotor training is useful tool to improve interlimb coordination. A problem of interlimb coordination ability is to minimize the degree of freedoms during walking and to solve with context-condition variability and how to make a interlimb coordinative structures. A way of solving method is coordinative locomotor training using sprinter & skater in PNF. The coordinative locomotor training set to fit the gait steps can be applied with gait tasks and can be extended by applying the irradiation of the PNF. Conclusion : The coordinative locomotor training using sprinter & skater in PNF is a useful way method to improve interlimb coordination during walking.

  • 본 연구는 PNF 중 스프린터 패턴 적용 시 하지의 근 활성도가 균형 능력에 미치는 영향을 알아보고자 다음과 같이 실시하였다. 편마비 환자 24명을 대조군과 실험군으로 나누어 실험군에 주 3회, 6주 동안 PNF의 상 하지 패턴의 결합인 스프린터 패턴(Sprinter pattern)을 실시하였다. 하지 근활성도는 내 외측광근, 내 외측슬괵근, 외측비복근의 활동전위의 실효치 값을 분석하였다. 균형 능력 검사는 FICSIT-4, FSST, MTD-Balance system 이용하여 측정하였다. 운동 적용 방법에 따른 측정 시점에 대한 대조군과 실험군의 유의성 검정은 Two-way repeated measure ANOVA를 실시하였고, 각 측정 항목들 간의 상관관계를 알아보기 위하여 Pearson correlation으로 분석하였다. 그 결과, 스프린터 패턴의 적용은 마비측 하지의 근 활성도를 증가시켜 정적 및 동적 균형능력을 향상시키는데 효과가 있었음을 확인할 수 있었다. 따라서 마비측 하지의 효율적 근동원을 위한 치료적 중재프로그램으로 적극적인 활용과 함께 다양한 운동 패턴들과 결합시켜 새로운 중재를 개발할 필요가 있을 것으로 생각된다.

고유수용성신경근촉진법의 결합패턴과 공 운동을 통한 요부안정화운동이 만성 요통 환자의 통증 및 정적 균형에 미치는 영향 (The Effects of Combination Patterns of Proprioceptive Neuromuscular Facilitation and Ball Exercise on Pain and Balance in Chronic Low Back Pain Patients)

  • Purpose : This study aimed to compare the effect of two modes (proprioceptive neuromuscular facilitation combination patterns and ball exercise) of low back stability for chronic low back patients. Methods : The subjects were recruited fourty patients who had low back pain. All subjects were randomly assigned to PNF combination patterns group, ball exercise group. Measurements were performed fourtimes: pre test, 2weeks, 4weeks, and 6weeks. Main outcome measures comprised the visual analogue scale(VAS), balance performance monitor(BPM). Results : The results were as follows. In the comparison of VAS score, sway area, sway path, and sway velocity according treatment period, score was significantly reduced in both PNF combination pattern group and ball exercise group. In the comparison of the both VAS and sway area between groups, there were significant. PNF combination pattern group significantly more decrease than ball exercise group at 6 weeks. However, both sway path and sway max velocity between group, there were not significantly. Conclusion : These results of this study indicated that PNF combination pattern which performed for six weeks had a significant influence than ball exercise group on low back pain.

전신진동자극훈련을 병행한 PNF 결합패턴 훈련이 뇌졸중환자의 근력, 균형 및 보행에 미치는 효과 (The Effect of the PNF Pattern Combined with Whole-Body Vibration on Muscle Strength, Balance, and Gait in Patients with Stroke Hemiplegia)

  • Purpose: The purpose of this study was to prove the effects of the PNF patterns combined with whole-body vibration (PWBV) training on muscle strength, balance, walking speed, and endurance in stroke patients. Methods: Sixteen subjects were randomly assigned to the PWBV group (n=8) and the whole-body vibration (WBV) group (n=8). The PWBV group performed PNF pattern exercises using sprinter combined with WBV, while the WBV group performed using squatting for 30 minutes. Both groups performed therapeutic interventions five days per week over a period of four weeks. The manual muscle test, timed up and go test (TUG), 10-meter walk test (10MWT), and six-minute walk test (6MWT) were used to assess the muscle strength, balance, and gait of the participants. The SPSS Ver. 19.0 statistical program was used for data processing. Statistical analysis included a pared t-test to compare the pre- and post-intervention, and an independent t-test was used to compare groups. The significance level was set as 0.05. Results: The PWBV group and WBV group showed significant improvements in the TUG, 10MWT, and 6MWT (P<0.05). Significant differences between the PWBV and WBV groups were found (P<0.05). Conclusion: The PWBV improved muscle strength, balance, gait speed, and endurance in stroke patients. Thus, PWBV may be suggested as a therapeutic intervention in patients with stroke hemiplegia.

PNF 패턴을 결합한 협응적 이동 훈련이 척수손상환자의 보행에 미치는 효과 (The Effects of Coordinative Locomotion Training Using the PNF Pattern on Walking in Patients with Spinal Cord Injury)

  • Purpose: The purpose of this study was to prove the effects of coordinative locomotion training (CLT) on walking speed, walking endurance, and balance for incomplete spinal cord injury patients. Methods: Ten subjects were randomly assigned to the CLT group (n = 5) and the treadmill (TM) group (n = 5). The CLT group performed PNF pattern exercise using the motions of the sprinter and skater for 30 minutes, while the TM group performed using a treadmill for 30 minutes. Both groups performed these therapeutic interventions for five days per week, for a period of four weeks. A 10 meter walking test, Berg Balance Scale (BBS), and 6 meter walking test were used for the assessment of gait speed, balance, and gait endurance. The SPSS Ver. 18.0 statistical program was used for data processing. A Wilcoxon signed rank test was used for the comparison of pre- and post-intervention performance and a Mann-Whitney test was used for comparison between the groups. The significance level for the statistical inspection was set at 0.05. Results: Both groups showed significant improvements in the 10 meter walking test, Berg Balance Scale, and 6 meter walking test (P < 0.05). Conclusion: CLT had an effect on the improvement of walking speed, walking endurance, and the balance of incomplete spinal cord injury patients. Thus, we suggest that CLT is a therapeutic intervention for incomplete spinal cord injury patients.

  • Purpose : The purpose of this study was to assess the effects of bridging exercise and combined lifting pattern bridging exercise on the lower extremity muscle activity. Methods : Twenty-five healthy adults volunteered to participate in this study. Subjects were required to complete following bridging exercises. Muscle activity was measured by QEMG-4 system(LXM 3204, Laxtha Korea). A paired t test was used to determine the influence of muscle activity for each exercise and descriptive statistics was used to characteristics of the subjects. Results : The biceps femoris, tibialis anterior, gastrocnemius of combined lifting pattern bridging exercise showed significance excepted vastus medialis(p<.05). In the case of men, biceps femoris showed significance in the combined lifting pattern bridging exercise(p<.05). In the case of women, biceps femoris and tibialis anterior showed significance in the combined lifting pattern bridging exercise(p<.05). Conclusion : The combined lifting pattern bridging exercise was more increased than bridging in lower extremity muscle activation. This result will be used for knee joint stabilizing exercises and biceps femoris strength training.

  • Purpose: This study aimed to examine the effects of proprioceptive neuromuscular facilitation (PNF) lower extremity patterns combined with elastic bands applied to stroke patients diagnosed with hemiplegia through self-training using the non-paralyzed side approach and the paralyzed side approach, and to investigate the differences in the effects. Methods: Nine chronic stroke patients who were being treated not more than twice a week at H, K, R, and C hospitals located in Gangwon-do, performed self-training for 16 minutes, two times per day for four weeks between August and October 2015. The subjects' balance ability was measured using the Tetrax stability index, the weight distribution index, the Berg Balance Scale (BBS), and the Timed Up and Go (TUG) test before the experiment and four weeks later. Among the statistical methods, paired t-tests were conducted for intra-group comparison of the measurements taken before and after the experiment, and independent t-tests were conducted for inter-group comparison of the ex post facto values. The statistical significance level was set to 0.05. Results: When the lower extremity patterns were applied to the non-paralyzed side group and the paralyzed side group, significant intra-group differences were observed for the Tetrax stability index, the weight distribution index, the Berg Balance Scale (BBS), and Timed Up and Go (TUG) tests (p<0.05); however, the inter-group comparisons showed no significant differences. Conclusion: The non-paralyzed side approach was found to be easy for patients to participate in and it also affected the patients' paralyzed side. Although the paralyzed side approach produced good exercise effects in a short period of time, it could result in adverse effects, such as a decrease in motivation and self-confidence. Therefore, these approaches are considered to be more effective when they are selectively applied depending on the purpose of the intervention and the degree of a patients' participation.

상지 패턴을 결합한 어깨 복합체 안정화 운동이 초등학교 야구선수들의 제구력과 구속에 미치는 영향 (The Effects of a Shoulder Complex Stabilization Exercise Combined with Upper Extremity Patterns on Ball Control and Ball Speed in Elementary School Baseball Players)

  • Purpose: The purpose of this study was to determine how a shoulder complex stabilization exercise affects ball control and ball speed in elementary school baseball players with an instable scapula. Methods: The subjects of the study were 16 baseball players attending S elementary school in Kwangju Metropolitan City. A shoulder complex stabilization exercise was conducted three times per week for one hour for four weeks. Then, the participants were divided into a scapular instability group or stability group based on their lateral scapular slide test scores. The measurement tool was measured using the target, the ball speed tester (PR1000-BC). All inspections were measured before and after the mediation period to examine the change in the subjects according to the mediation period. Results: As a result of measuring ball control, there was a statistically significant difference in the variation over time (p < 0.05), and there was no statistically significant difference in the interaction between time and group (p > 0.05). As a result of the test of effectiveness between subjects, there was no statistically significant difference found among the groups (p > 0.05). In terms of ball speed, there was no statistically significant difference found in the variation over time (p > 0.05) as well as in the interaction between time and group (p > 0.05). In terms of the effectiveness between subjects, there was no statistically significant difference found among the groups (p > 0.05). Conclusion: It was found that the shoulder complex stabilization exercise could improve ball control for elementary school baseball players. It is believed that this will help prevent and solve possible sport damages experienced during training or competitions, thereby helping athletes use training methods to improve their exercise capacity and continue their careers.
