원발성 폐암에서 정위적 체부 방사선치료의 빔 배열에 따른 선량분포의 비교 (Comparison of the Dose Distributions with Beam Arrangements in the Stereotactic Body Radiotherapy (SBRT) for Primary Lung Cancer)

  • 원발성 폐암의 정위적 체부 방사선치료(Stereotatic Body Radiation Therapy; SBRT)시에, 종양주위의 빔 배열을 균등하게 한 $360^{\circ}$회전각도(Equally angles; EA)와 종양주위의 빔 배열을 부분각도(Partially angles; PA)로 배열한 치료계획의 선량차이를 확인하기 위하여, 세기조절방사선치료(Intensity-modulated radiation therapy, IMRT)와 체적변조회전치료(Volumetric-modulated arc therapy, VMAT)의 종양의 선량, 동측 폐의 선량, 반대측 폐의 선량, 손상위험장기(Organ at risk, OAR)의 선량, 치료효율 등을 비교분석 하였다. 12명의 환자에서 각각 4종류($IMRT_{EA}$, $IMRT_{PA}$, $VMAT_{EA}$, $VMAT_{PA}$)의 치료계획을 생성하였으며, 처방선량은 총 선량 60 Gy, 4회 분할치료로 표적체적 95%에 대해 100% 선량이 포함되게 하였다. IMRT와 VMAT의 치료계획 평가에서 $360^{\circ}$회전각도의 빔 균등배열과 부분각도 빔 배열에서 변수중 선량일치지수, 균질성지수, 고선량 유출, $D_{2cm}$, $R_{50}$은 빔 배열에 따라 크게 차이가 나지 않았다. 또한 손상위험장기인 척수, 기관지, 식도의 최고선량은 각각의 방법에 따라 차이를 나타내었다. 특히 $HDS_{location}$에서 21.63%와 26.46%의 큰 차이를 나타내었다.

  • Ten, heretofore unreported, $ 5^I-methyl-5^I-[2-(5-substituted uracil-1-yl)ethyl)]-2^I-oxo-3^I$-methylenetetrahydrofurans (H, F, Cl, Br, I, $ CH_3$,$CF_3$,$CH_2CH_3$,$ CH=CH2$, SePh) (7a-j) were synthesized and evaluated against four cell lines (K-562, FM-3A, P-388 and U-937). For the preparation of ${\alpha}$-methylene-${\gamma}$-butyrolactone-linked to 5-substituted uracils (7a-j), the convenient Reformasky type reaction was employed which involves the treatment of ethyl ${\alpha}$-(bromomethyl)acrylate and zinc with the respective 1-(5-substituted uracil-1-yl)-3-butanone (6a-j). The 5-substituted uracil ketones (6a-j) were directly obtained by the respective Michael type reaction of vinyl methyl ketone with the $K_2CO_3$(or NaH)-treated 5-substituted uracils (5a-j) in the presence of acetic acid in the DMF solvent. The .alpha.-methylene-.gamma.-butyrolactone compounds showing the most significant antitumor activity are 7e, 7f, 7h and 7j (inhibitory concentration $(IC_50)$ ranging from 0.69 to $2.9 {\mu}g/ml$), while 7b, 7g and 7i have shown moderate to significant activity. The compounds 7a, 7c and 7d were found to be inactive. The synthetic intermediate compounds 6a-j were also screened and found marginal to moderate activity where compounds 6b and 6g showed significant activity $(IC_50:0.4~2.8 {\mu}g/ml)$.

토끼와 흰쥐 음경해면체 이완작용에 미치는 홍삼사포닌 분획별 효과 (Effects of Compositions of Saponin Fraction from Korean Red Ginseng in the Relaxation of Rabbit and Rat Corpus Cavernosum)

  • 사포닌 함량과 PD계 사포닌과 PT계 사포닌의 조성 비율이 상이한 사포닌 분획물 TS-1, TS-2, TS-3 즉 주종 ginsenoside의 총함량$(G-Rb_1,\;-Rb_2,\;-Rc,\;-Rd,\;G-Re,\;-Rf,\;-Rg_1)$은 각각 $41\%,40\%,62\%$이고, PD계 사포닌과 PT계 사포닌의 함유 조성비율(PD/PT)이 각각 1.55, 2.25, 2.61인 시료를 6년근 홍삼으로부터 추출 제조하여, 생체외 실험으로 음경해면체 절편에 대한 이완반응과 생체내 실험으로 사포닌분획물 투여에 따른 음경 해면체내압의 변화에 미치는 효과를 조사하였다. PHE에 의해 수축된 음경해면체 절편에 대한 이완 반응은 홍삼 사포닌은 각각 농도 의존적 이완작용을 보였으며, 음경 해면체평활근의 이완작용에 있어 각 분획물의 $ED_{50}$은 TS-1이 1.68, TS-2, 1.97, TS-3, 2.29 mg/ml서 TS-1이 가장 의의있게 낮았다. 이러한 홍삼 사포닌 분획물의 이완효과 기전은 음경 해면체평활근에 NO나 칼슘 및 칼륨 통로 등에 영향을 미쳐 복합적인 이완작용을 보이는 것으로 사료되었다. 흰쥐를 이용한 생체내 실험으로 홍삼 사포닌 투여에 따른 음경해면체내압의 변화 조사에서 각 사포닌 분획물은 농도의존적으로 해면체내압의 증가를 야기시켰다. 이들 해면체내압의 증가 정도는 TS-1이 가장 강하였고,TS-2,TS-3 순이었다. 이상의 결과로부터 홍삼의 사포닌 분획물은 음경해면체평활근의 이완작용과 내압의 증가에 영향을 미쳐 음경발기의 상승을 야기시키는 효과가 있으며 그 효과는 그 함유 조성에 따라 차별성이 있다는 것이 확인되었으므로 금후 주요 활성성분의 구명이 요망되고 있다.

배양림프구에서 Paraquat의 세포독성에 대한 Vitamin C와 Deferoxamine의 항산화 효과 (The Antioxidant Effect of Vitamin C and Deferoxamine on Paraquat-induced Cytotoxicity in Cultured Lymphocytes)

  • Purpose: As basic information of antioxidant treatments for the patient with paraquat intoxication, in human peripheral lymphocytes, the cytotoxicity of paraquat was measured, and to evaluate the antioxidant effect of vitamin C and deferoxamine against this cytotoxicity, malondialdehyde (MDA), superoxide dismutase (SOD) activity and total antioxidant status (TAS) were measured. Methods: From 10 healthy adults, after obtaining a consent, 20ml peripheral blood was collected. Experimental groups were divided to (1) control group, the group treated with an identical amount of saline, (2) P group: the group treated with paraquat only, (3) PV group: the group treated with paraquat followed by vitamin C 30 minutes later, (4) PD group: the group treated with paraquat followed by deferoxamine 30 minutes later, (5) PVD group: the group treated with paraquat followed by vitamin C 30 minutes later and subsequently deferoxamine one hour later, and (6) PDV group: the group treated with paraquat followed by deferoxamine 30 minutes later and subsequently vitamin C 1 hour later, and thus to total 6 groups. In each group, 10 samples of peripheral blood was assigned and $100{\mu}M\;paraquat,\;100{\mu}M$ vitamin C, and $100{\mu}M$ deferoxamine were used as reagent. Lymphocytes were isolated, cultured, and cytotoxicity was measured by the Microculture Tetrazolium method (MTT assay), MDA and SOD activity, and TAS concentration were measured. Results: In regard to the cytotoxicity measured in each group, their cytotoxicity was decreased in the group treated with antioxidants, in comparison with the group treated with paraquat only. In the cases that the order of the treatment of these two antioxidants was altered, viability in the PDV group $(1.077{\pm}0.121)$ was increased more that the PVD group $(0.888{\pm}0.152)$ statistically significantly (p=0.018). Concerning the amount of MDA, in comparison with the P group $(6.78{\pm}0.93{\mu}mol/L)$, after the treatment of each antioxidant, the concentration of MDA was decreased statistically significantly (p<0.05). In the group treated with two antioxidants together, in comparison with the group treated only with one antioxidant, the amount of MDA was increased statistically significantly $(PV:\;3.96{\pm}0.98{\mu}mol/L,\;PD:\;4.92{\pm}1.50{\mu}mol/L,\;PVD:\;3.22{\pm}0.83{\mu}mol/L,\;and\;PDV:\;3.42{\pm}0.95{\mu}mol/L,\;p=0.007)$. The concentration of SOD measured in the blood in each group after the administration of paraquat, in comparison with the control group, a pattern of the elevation of SOD activity and subsequent decrease was detected, however, it was not statistically significant. In the comparison of the groups treated with antioxidants, in comparison with the P group $(1419.9{\pm}265.9{\mu}mol/L)$, SOD activity was decreased statistically significantly in only the PDV group $(1176.4{\pm}238.9{\mu}mol/L)$ (p=0.017). In regard to TAS measured in each group, in comparison with the P group $(0.87{\pm}0.05{\mu}mol/L)$, in all groups treated with the antioxidants, the PV group was $1.00{\pm}0.03{\mu}mol/L$ (p=0.005), the PD group was $9.01{\pm}0.24{\mu}mol/L$ was $4.64{\pm}3.98{\mu}mol/L$ (P=0.005), and the PDV group was $9.41{\pm}0.27{\mu}mol/La$ (p=0.005), and thus total antioxidant activity was increased statistically significantly In a multiple comparison test, the PDV group showed the highest total antioxidant activity (p<0.0001). Conclusion: The result of the assessment of the antioxidant effect of vitamin C and deferoxamine on paraquat-induced cytotoxicity showed that in regard to cytotoxicity, SOD activity and TAS measurement, the best result was observed in the PDV group. Therefore, it was found that vitamin C and deferoxamine were effective antioxidants for the paraquat-induced cytotoxicity, and it suggests that the administration of deferoxamine followed by vitamin C may improve their antioxidant effect more.

소세포폐암에 대한 Etoposide와 Carboplatin 병합요법과 Topotecan 화학요법의 효과 (Clinical Response to Etoposide Plus Carboplatin and Topotecan Chemotherapy in Small Cell Lung Cancer)

  • 연구배경 : 소세포폐암에 대한 항암화학치료에 etoposide와 cisplatin(EP) 또는 etoposide와 carboplatin(EC)이 가장 흔히 사용되는 병합요법인데, 전반적인 관해율은 75-85%(제한기 65-90%), 완전관해율은 20-30%(제한기 45-75%)로 보고되고 있다. 치료후 재발되는 소세포폐암환자에 대하여 구제(salvage)화학요법제로 topotecan을 사용하였을 때 90일 이내에 재발되는 불응성 재발(refractory relapse, RR)인 경우 관해율이 6.4%인 반면에, 90일 이후에 재발되는 감수성 재발(sensitive relapse, SR)에서는 37.8%의 관해율이 보고되고 있다. 저자는 EC요법과 2차 구제화학요법으로 topotecan요법의 치료효과와 부작용을 조사하여 기존의 성적들과 비교하고자 하였다. 대상 및 방법 : 1996년부터 2002년 6월까지 전남대병원 내과에서 소세포 폐암으로 치료를 받았던 환자들 중에서 EC요법과 재발되어 topotecan구제 화학요법을 받은 환자를 대상으로 임상경과를 후향적으로 조사하였다. 일차치료로서 etoposide 100 $mg/m^2$ (day 1-3), carboplatin 300 $mg/m^2$(day 1)을 3주 이상의 간격을 두고 투여하여, 3주기 이상 화학요법을 받은 경우들을 대상으로 하였고, 이차치료로 topotecan을 투여한 경우에는 1.5 $mg/m^2$을 제 1일에서 제 5일까지 3주 간격으로 2주기 이상 받은 환자들을 대상으로 하였다. 치료에 대한 반응의 평가는 완전관해(complete remission, CR), 부분관해(partial remission, PR), 불변(stable disease, SD), 진행(progressive disease, PD)으로 구분하였고 생존기간은 치료시작일로부터 중앙생존기간(median survival time, MST)으로 표기하였다. 결 과 : 총 101예에서 1차 선택 약제로 EC요법 후 관해율은 57.4%(58예)로 CR은 15.8%(16예), PR은 41.6%(42예)이었다. 제한기(69명)에서의 관해율은 69.6%(48예)로 CR은 23.2%(16예), PR은 46.4%(32예)였으며, 확장기(32명)에서의 관해율은 31.3%(10예)로 모두 PR반응을 보였다. 수행 능력(performance status score, PS)과 동통, 호흡곤란, 기침 등 임상 증상의 변화를 평가하였는데 58예(57.4%)에서 전신상태와 증상의 호전을 보였다. 전체 관해군의 관해 유지기간(중앙값)은 10.3개월(5.8개월~57.7개월)이었고 치료에 따른 호중구 감소증(G4)은 24예(23.8%), 혈소판감소증(Grade 4)은 2예(2%)에서 관찰되었다. 1차 화학치료에 불응하여 바로 다른 약제로 바꾸어 치료한 9예와 1차 치료 후 2차구제화학요법을 받지 않았던 71예를 제외한, 21예의 환자들은 EC요법으로 SD이상 반응을 보였으나 재발하여 topotecan을 투여 받은 군이었다. 총 21예 중 관해율은 SR군(8예)의 경우 PR 25%(2예), SD 25%(2예), PD 50%(4예)를 보였고, RR군(13예)에서는 PR 15.4%(2예), SD 15.4%(2예), PD 69.2%(9예)를 나타냈으나 유의한 차이는 아니었다. 생존 기간의 비교에서도 SR군은 MST 8개월로 RR군의 MST 7개월과 비교하여 유의한 차이는 없었다. 치료에 따른 독성으로 호중구감소증(Grade 4)은 6예 (28.3%), 혈소판감소증(Grade 4)은 3예(14.3%)에서 관찰되었다. 결 론 : EC와 topotecan 치료 모두 기존의 보고들과 비슷한 관해율을 보였으며 수용 가능한 정도의 부작용이 관찰되었다.

O3/H2O2와 O3/Catalyst 고급산화공정에서 1,4-dioxane의 제거 특성 (Removal Characteristics of 1,4-dioxane with O3/H2O2 and O3/Catalyst Advanced Oxidation Process)

  • Advanced oxidation processes involving $O_3/H_2O_2$ and $O_3/catalyst$ were used to compare the degradability and the effect of pH on the oxidation of 1,4-dioxane, Oxidation processes were carried out in a bubble column reactor under different pH. Initial hydrogen peroxide concentration was 3.52 mM in $O_3/H_2O_2$ process and 115 g/L (0.65 wt.%) of activated carbon impregnated with palladium was packed in $O_3/catalyst$ column. 1,4-dioxane concentration was reduced steadily with reaction time in $O_3/H_2O_2$ oxidation process, however, in case of $O_3/catalyst$ process, about $50{\sim}75%$ of 1,4-dioxane was degraded only in 5 minutes after reaction. Overall reaction efficiency of $O_3/catalyst$ was also higher than that of $O_3/H_2O_2$ process. TOC and $COD_{cr}$ were analyzed in order to examine the oxidation characteristics with $O_3/H_2O_2\;and\;O_3/catalyst$ process. The results of $COD_{cr}$ removal efficiency and ${\Delta}TOC/{\Delta}ThOC$ ratio in $O_3/catalyst$ process gave that this process could more proceed the oxidation reaction than $O_3/H_2O_2$ oxidation process. Therefore, it was considered that $O_3/catalyst$ advanced oxidation process could be used as a effective oxidation process for removing non-degradable toxic organic materials.

A Blockade of the Central MAPK Pathway Attenuates Referred Pain in Rats with Complete Freund's Adjuvant -Induced Inflammation of the Temporomandibular Joint

  • We investigated the role of the central MAPK pathways in extra-territorial (referred) pain resulting from inflammation of the temporomandibular joint (TMJ). Experiments were carried out on male Sprague-Dawley rats weighing 220-280 g. Under anesthesia, these animals were injected with $50\;{\mu}L$ of complete Freund's adjuvant (CFA) into the TMJ using a Hamilton syringe. In the control group, saline was injected into the TMJ. To identify the extent of inflammation of the TMJ, Evans blue dye (0.1%, 5 mg/kg) was injected intravenously at 1, 3, 6, 9, 12 and 15 days after CFA injection. The concentration of Evans blue dye in the extracted TMJ tissue was found to be significantly higher in the CFA-treated animals than in the saline-treated group. Air-puff thresholds in the vibrissa pad area were evaluated 3 days before and at 3, 6, 9, 12, 15 and 18 days after CFA injection into the TMJ. Referred mechanical allodynia was established at 3 days, remained until 12 days, and recovered to preoperative levels at 18 days after CFA injection. This referred mechanical allodynia was observed in contralateral side area. To investigate the role of central MAPK pathways, MAPK inhibitors ($10\;{\mu}g$) were administrated intracisternally 9 days after CFA injection. SB203580, a p38 MAPK inhibitor, significantly attenuated referred mechanical allodynia, as compared with the vehicle group. PD98059, a MEK inhibitor, also reduced CFA-induced referred mechanical allodynia. These results suggest that TMJ inflammation produces extra-territorial mechanical allodynia, and that this is mediated by central MAPK pathways.

식이(食餌)에 의한 흰쥐 간지질(肝脂質) 및 효소활성(酵素活性) 변화(變化)에 관한 연구(硏究) (Changes in the Lipid Composition and Some Enzyme Activities in the Rat Liver as Affected by Diets)

  • Albio rats right after weaning, weighing $50{\sim}55g$, were divided into the control, high-carbohydrate-, high-lipid-, and high-protein-fed groups. and were fed for 12 weeks with the respective diets to observe the increase in body weights as well as changes in the chemical constituentes and enzyme activities in the liver tissue, with the following results. (1) There was little difference in the rate of increase in the body weights among the groups, showing normal growth, except the high-protein-fed group which showed decrease in rate of body weight increase from the 7th week after feeding. (2) The liver weight was either increased after 12 weeks of feeding with the high-carbohydrate and high-Lipid diets, or showed no difference with the high-protein diet, as compared to the control weight. (3) The liver cytosol protein content was increased when fed with the high-protein diet, but decreased when fed with the high-carbohydrate and high lipid diets, as compared to the control content. (4) The triglyceride and cholesterol contents in the liver were decreased in the high-protein-fed group, but increased markedly in the high-carbohydrate- and high-lipid-fed groups as compared to the control values. (5) The hepatic glucokinase, G6PD, LDH, and fatty acid synthetase activities were increased in the high-carbohydrate and high-lipid-fed groups, and GOT and CPT activities were increased in the high-protein-fed group. From the above results. it was known that the high-carbohydrate and high-lipid diets stimulated the hepatic lipid metabolism, giving rise to lipogenesis, but the high- protein diet could prevent the lipogeuesis leading to the body weight increase.

Optimization of Human Embryonic Stem Cells into Differentiation of Dopaminergic Neurons in Vitro: II. Genetically Modified Human Embryonic Stem Cells Treated with RA/AA or b-FGF

  • Since the establishment of embryonic stem cell, pluripotency of the cells was known to allow differentiation of the cells into various cell types consisting whole body. Several protocols have been developed to induce expression of specific genes.. However, no precise protocol that will generate a single type of the cells from stem cells has been reported. In order to produce cells suitable for transplantion into brain of PD animal model, which arouse due to a progressive degeneration of dopaminergic neurons in midbrain, human embryonic stem cell (hESC, MB03) was transfected with cDNAs cording for tyrosine hydroxylase (TH). Successful transfection was confirmed by western immunoblotting. Newly transfected cell line (TH#2/MB03) was induced to differentiate by the two neurogenic factors retinoic acid (RA) and b-FGF. Exp. I) Upon differentiation using RA/ascorbic acid (AA), embryoid bodies (EB, for 4days) derived from hES cells were exposed to RA (10$^{-6}$ M)/AA (50 mM) for 4 days, and were allowed to differentiate in N2 medium for 7, 14, 21, or 28 days. Exp. II) When bFGF was used, neuronal precursor cells were selected for 8 days in N2 medium after EB formation. After selection, cells were expanded at the presence of bFGF (20 ng/ml) for another 6 days followed by a final differentiation in N2 medium for 7, 14, 21 or 28 days. By indirect immunocytochemical studies, proportion of cells expressing NF200 increased rapidly from 20% at 7 days to 70 % at 28 days in RA/AA-treated group, while those cells expressing NF160 decreased from 80% at 7 days to 10% at 28 days upon differentiation in N2 medium. However, in differentiation by RA/AA treatment system, there was a significant increase in proportion of neuron maturity (73%) at day 14 after N2 medium. TH#2/MB03 cells expressing TH are >90% when matured at the absence of either bDNF or TGF-$\alpha$. These results suggested that TH#2/MB03 cells could be differentiated in vitro into mature neurons by RA/AA.

Mechanistic insights of metal acetylacetonate-aided dehydrocoupling of liquid-state ammonia borane NH3BH3

  • Ammonia borane $NH_3BH_3$ solubilized in organic solvent is a potential liquid-state chemical hydrogen storage material. In this study, metal acetylacetonates like $Fe(O_2C_5H_7)_3$, $Co(O_2C_5H_7)_2$, $Ni(O_2C_5H_7)_2$, $Pd(O_2C_5H_7)_2$, $Pt(O_2C_5H_7)_2$ and $Ru(O_2C_5H_7)_3$ are considered for assisting dehydrocoupling of ammonia borane in diglyme (0.135 M) at $50^{\circ}C$. The molar ratio between ammonia borane and metal acetylacetonate is fixed at 100. A protocol for the separation of the soluble and insoluble fractions present in the slurry is proposed; it consists in using acetonitrile to make the precipitation of metal-based compounds easier and to solubilize boron-based intermediates/products. The nature of the metal does not affect the dehydrocoupling mechanisms, the $^{11}B\{^1H\}$ NMR spectra showing the formation of the same reaction intermediates. The aforementioned metal acetylacetonates do mainly have effect on the kinetics of dehydrocoupling. Dehydrocoupling takes place heterogeneously and dehydrogenation of ammonia borane in these conditions leads to the formation of polyborazylene via intermediates like e.g., B-(cyclodiborazanyl) amine-borane and borazine. Our main results are reported and discussed herein.
