Removal Characteristics of Endocrine Disrupting Compounds (EDCs), Pharmaceutically Active Compounds (PhACs) and Personal Care Products (PCPs) by NF Membrane (NF막을 이용한 EDCs, PhACs, PCPs 물질의 제거 특성 평가)

  • Reports of endocrine disrupting compounds (EDCs), pharmaceutically active compounds (PhACs), and personal care products (PCPs) have raised substantial concern in important potable drinking water quality issues. Our study investigates the removal of EDCs, PhACs, and PCPs of 10 compounds having different physico-chemical properties (e.g., molecular weight, and octanol-water partition coefficient ($K_{OW}$)) by nanofiltration (NF) membranes. The rejection of micropollutants by NF membranes ranged from 93.9% to 99.9% depending on solute characteristics. A batch adsorption experiments indicated that adsorption is an important mechanism for transport/removal of relatively hydrophobic compounds, and is related to the octanol-water partition coefficient values. The transport phenomenon associated with adsorption may also depend on solution water chemistry such as pH and ionic strength influencing the pKa value of compounds. In addition, it was visually seen that the retention was somewhat higher for the larger compounds based on their molecular weight. These results suggest that the NF membrane retains many organic compounds due to both hydrophobic adsorption and size exclusion mechanisms.

  • Background: Percutaneous cardiopulmonary support (PCPS) provides passive support of gas exchange and perfusion, allowing the use of other methods of care for organ recovery, and saves lives of patients with severe cardiopulmonary failure in a wide variety of clinical settings with a minimal risk of bleeding and need for chest re~ exploration. We summarized a single center's experiences with PCPS in patients with cardiogenic shock or cardiac arrest due to the ischemic heart disease. Material and Method: Among the 20 consecutive patients with cardiogenic shock or cardiac arrest from May 1999 to June 2005, Biopump (Medtronic, Inc, Minneapolis, MN) was used in 7 patients and the self-priming, heparin-coated circuit of EBS (Terumo, Japan) was applied to remaining 13 patients. Most of cannulations were performed percutaneously via femoral arteries and veins. The long venous cannulas of DLP (Medtronic inc. Minneapolis, MN) or the RMI (Edwards's lifescience LLC, Irvine, CA) were used with the arterial cannulae from 17 Fr to 21 Fr and the venous cannula from 21 Fr to 28 Fr. Result: The 20 consecutive patients who were severely compromised and received PCPS for the purpose of resuscitation were comprised of 13 cardiac arrests and 7 cardiogenic shocks in which by-pass surgery was performed in 11 patients and 9 ongoing PCls under the cardiopulmonary support. The mean support time on the PCPS was 38$\pm$42 hours. Of the 20 patients implanted with PCPS, 11 patients ($55\%$) have had the PCPS removed successfully; overall, 8 of these patients ($40\%$) were discharged from the hospital in an average surviving time for 27$\pm$17 days after removing the PCPS and survived well with 31$\pm$30 months of follow-up after the procedure. Conclusion: The use of PCPS appears to provide the hemodynamic restoration, allowing the survival of patients in cardiac arrest or cardiogenic shock who would otherwise not survive, and patients receiving PCPS had a relatively long-term survival.

Problems of Usage of Earphones, PCPs on Hearing in Adolescents (청소년의 이어폰 및 음향기기 사용 실태와 문제점에 관한 연구)

  • Purpose: The purpose of this study was to investigate usage of earplones, PCPs, and to give the basic data for adolescent's hearing health education. The study was a descriptive research about three factors affecting hearing loss; the knowledge and attitude about noise, the perception of hearing loss. Method: For research purposes, we conducted a questionnaire survey of 1,480 high school students in Suwon, from 27 October 2007 to 31 2007. Result: Most of the subjets used PCPs for 3-5 years, the average time of using earphones per day was 1-3 hours, awake hearing acuity decrease after using is the hearing acuity became worse some, PCPs type is MP3, use place house, listening contents ballade, disorder condition experience after using if the ear it is sick. The average of concern about hearing scored the highest 3.01${\pm}$0.54. The average of perceived susceptibility scored 3.04${\pm}$0.63 and the average of knowledge about noise scored 2.76${\pm}$0.46. The average of total knowledge and attitude about noise scored 2.93${\pm}$0.52. The average of discomfort of hearing loss(2.20${\pm}$0.63) scored higher than that fear of hearing loss(1.66${\pm}$0.30). The average of total perception of hearing loss scored 2.12${\pm}$0.67. Conclusion: At least in this study are shown in the youth earphone, PCPs used as a spare because of the seriousness of the issue, so as a result of these problems related to hearing about the youth campaign, through advertising or seriousness of the young people need them. In addition, countries also need to create policies and the support for health education, especially during the regular curriculum subjects on subjects that include teenagers in the earphone, PCPs due to problems with spare on the contents of the reservoir. The severity of hearing loss young people, prevention of hearing to protect the youth.

Application of Percutaneous Cardiopulmonary Support for Cardiac Tamponade Following Blunt Chest Trauma: Two Case Reports

  • Since the advent of percutaneous cardiopulmonary support (PCPS), its application has been extended to massively injured patient. Cardiac injury following blunt chest trauma brings out high mortality and morbidity. In our cases, patients had high injury severity score by blunt trauma and presented sudden hemodynamic collapse in emergency room. We quickly detected cardiac tamponade by focused assessment with sonography for trauma and implemented PCPS. As PCPS established, their vital sign restored and then, they were transferred to the operation room (OR) securely. After all injured lesion repaired, PCPS weaned successfully in OR. They were discharged without complication on day 26 and 55, retrospectively.

A Study on the 6-Week Cleansing Effect of Placenta Cold Process Soap (자하거저온숙성비누의 6주간 세안효과에 대한 연구)

  • Objectives : Many people use natural soap instead of commercially available bar type general soap for skin care. They tend to prefer cp (cold process) type soaps that go through a sufficient maturation process in formulation. The purpose of this study was to investigate the efficacy of PCPS (placenta cp soap). Methods : The researcher selected 36 subjects, 28 as the experimental group and 8 as the control group. I asked participants to wash their face twice a day in the morning and evening, and the period was set for 6 weeks. The researcher had the people who were selected as the control group use a commercially available foam cleanser, meanwhile the experimental group use PCPS. They performed a skin test before the start of the test, and the change status was continuously investigated after 2 weeks, 4 weeks, and 6 weeks after using the soap. Results : In the case of U zone oil and pores, there was a significant decrease and in the case of skin tone, there was a significant increase in the group of using PCPS. In T zone oil change, there was no significant effect in both the experimental group and the control group. Conclusion : In the case of PCPS, it is recommended to use it in the effect of reducing the size of pores and increasing skin tone, but it can reduce u-zone oil content.

  • In the last decade, considerable efforts have been made to make a new class of solid state high power, high speed electronic device, namely, the Photo-Conductive Power Switch(PCPS), and to characterize the high-field performance of PCPS under high power, high voltage conditions. But the problem of surface flashover phenomena persist, preventing the realization of reliable and efficient high-speed, high voltage switching devices. It is essential to have a clear understanding on the physical processes behind the surface flashover problem to develop new technologies and device architectures so as to fabricate PCPS that are capable of high-field high-voltage. Also, it is imperative to identify new materials that could satisfy the requirements for high-field, high-power devices. Since surface flashover, surface breakdown phenomena is observed for all the devices that foiled at the applied field much lower than semiconductor bulk breakdown field, surface passivation is considered one of the important practical methods to improve the high field performance of the devices. Therefore, this paper was studied the main properties and mechanism of the semiconductor surface flashover before and after passivation under high electric-field.

Percutaneous Cardiopulmonary Support (PCPS) for Patients with Cardioppulmonary Bypass Weaning Failure during Open Heart Surgery (개심술 중 심폐기 이탈에 실패한 환자에게 적용한 경피적 심폐순환 보조장치)

  • Background: Recently, percutaneous cardiopulmonary support (PCPS) has been widely used to rescue patients in cardiogenic shock or cardiac arrest. However, patients with cardiopulmonary bypass (CPB) weaning failure during open heart surgery still have very poor outcomes after PCPS. We investigated clinical results and prognostic factors for patients who underwent PCPS during open heart surgery. Material and Method: From January 2005 to December 2008, 10 patients with CPB weaning failure during open heart surgery underwent PCPS using the CAPIOX emergency bypass system ($EBS^{(R)}$, Terumo Inc, Tokyo, Japan). We retrospectively reviewed the medical records of those 10 patients. Result: The average age of the patients was $60.2{\pm}16.5$ years (range, $19{\sim}77$ years). The mean supporting time was $48.7{\pm}64.7$ hours (range, $4{\sim}210$ hours). Of the 10 patients, 6(60%) were successfully weaned from the PCPS While 5 (50%) were able to be discharged from the hospital. Complications were noted in 5 patients (50%). In univariate analysis, long aortic cross clamp time during surgery, mediastinal bleeding during PCPS and high level of Troponin-I before PCPS were significant risk factors. All of the discharged patients are still surviving $34{\pm}8.6$ months (range, $23{\sim}48$ months) post-operatively. Conclusion: The use of PCPS for CPB weaning failure during open heart surgery can improve the prognosis. More experience and additional clinical studies are necessary to improve survival and decrease complications.

  • Background: The incidence of chronic obstructive pulmonary disease (COPD) is increasing and the disease is becoming an important cause of morbidity and mortality worldwide. It is important to implement evidence-based guidelines by primary care physicians (PCPs) to establish qualified management of COPD patients. The aim of this survey is to investigate the pattern of COPD management among PCPs and to apply it to the development of Korean COPD guidelines. Methods: A web-based questionnaire was prepared that consisted of 25 questions on the pattern of COPD management. A total of 217 PCPs participated in the survey from June 2006 to May 2007. Results: Many PCPs (61.8%) possessed a spirometer, but the application rate was relatively low (35.8%) and more than half of the COPD patients (57%) did not receive a diagnosis based on spirometry. Administration of oral medication was preferred than the administration of inhaled medication for both stable COPD and acutely exacerbated COPD. More than 90% of the PCPs endorsed educational measures to quit smoking and vaccinate against influenza. It was noted that 56.7% of the PCPs were aware of the GOLD guidelines, but only 7.3% tended to fully implement the recommendations of the guidelines in daily practice. Conclusion: The results of the survey indicate that despite the high awareness rate of the current COPD guidelines, deficits exist among the PCPs with respect to the diagnosis and treatment of COPD. The results of this survey should be applied for the development of new COPD guidelines in order to decrease the discrepancy between the guidelines and the daily practice of the PCPs.

Primary Care Physicians' Action Plans for Responding to Results of Screening Tests Based on the Concept of Quaternary Prevention

  • Since noncommunicable diseases (NCDs) are generally controllable rather than curable, more emphasis is placed on prevention than on treatment. For the early detection of diseases, primary care physicians (PCPs), as well as general practitioners and family physicians, should interpret screening results accurately and provide screenees with appropriate information about prevention and treatment, including potential harms. The concept of quaternary prevention (QP), which was introduced by Jamoulle and Roland in 1995, has been applied to screening results. This article summarizes situations that PCPs encounter during screening tests according to the concept of QP, and suggests measures to face such situations. It is suggested that screening tests be customized to fit individual characteristics instead of being performed based on general guidelines. Since screening tests should not be carried out in some circumstances, further studies based on the concept of prevention levels proposed by Jamoulle and Roland are required for the development of strategies to prevent NCDs, including cancers. Thus, applying the concept of QP helps PCPs gain better insights into screening tests aimed at preventing NCDs and also helps improve the doctor-patient relationship by helping screenees understand medical uncertainties.

  • We propose a new concept: that of photonic crystal phosphors (PCPs) for display and phosphor related applications. It is well known that microcavities with dimensions comparable to the emitting wavelength strongly enhance light-matter interactions, resulting in a significant increase in spontaneous emission rate, which can be directly translated into enhancement in phosphor efficiency. In recent simulations we have demonstrated that when a microcavity is formed in a nano-phosphor structure, the luminescence band is modified, and can be made spectrally sharp and tunable by engineering the geometry/material properties of the cavity and the surrounding photonic crystal lattice. New phosphor material structures based on photonic crystals are proposed. Applications to thin film EL phosphors and particle phosphors are discussed. Additionally, economic methods of synthesizing and incorporating PCPs into current display applications are proposed.
