한국 서해 만경강-동진강 하구역 및 연안역 조간대 퇴적층의 퇴적상과 미고생물학 적 연구 (Sedimentary facies and micropaleontological study of tidal sediments off the Mankyung-Dongjin River estuary, west coast of Korea.)

  • 한국 서해 동진강-만경강 하구역과 인접 연안역에 발달한 조간대의 퇴적환경과 생 층서에 대하여 연구지역에서 채취한 5개의 진동시추코아 퇴적환경과 생층서에 대하여 연구지역에서 채취한 5개의 진동시추코아 퇴적물의 퇴적상과 규조군집분석을 통해 연 구하였다. 시추공 퇴적물은 주로 니질 및 사질로 조성되어 있으며 생혼퇴적 구조와 교 호엽총리, 후라저총리, 연흔사총리등의 퇴적구조들이 여러 층들에서 관찰된다. 전체 적으로 니질퇴적층이 사질퇴적층보다 우세하며 사질층을 제외한 대부분의 지질퇴적층 에는 다수의 규조각들이 함유되어 있다. 총 61속 219종 및 변종의 규조가 동정되었으 며 산출되는 규조중 가장 다산되는 종은 Paralia sulcata 이다. 그외 Cyclotella striata Thalassionema nitzschioides Actinoptychus undulatus, Delphineis surirella, Rhaphoneis amphiceros 등이 다산된다. 산출되는 대부분의 규조는 기수 및 해수역의 연안 혹은 천해에서 서식하는 저서성 및 우연적 혹은 부분적 부유성종이다. 담수종은 1∼5%정도로 남양만 조간대에 대해 높은 산출을 보인다. 규조류의 알려진 생 태와 여러 충준에서 관찰되는 교호엽총리, 후라저총리, 연혼사총리등의 퇴적구조는 본 연구 퇴적층이 하천수의 유입을 어느 정도 받고 한수 보다는 난수의 영향을 더 강하게 받는 조간대 환경하에서 퇴적된 것으로 추정된다. 그리고 생태적 측면에서 규조군집의 수직적 변화가 인지되지 않는 것으로 보아 퇴적기간 동안 퇴적환경의 뚜렷한 변화는 없었던 것으로 생각된다. Q-mode을 이용한 규조군집 부산광역시 결과 각 시추공 별로 각각 2개의 군집으로 분류되었으며 이들 군집으로 분류되었으며 이들 군집들의 충위적 분포를 근거하여 시추공간의 생충서적 대비를 시도해 보았다.

  • 우리나라는 눈부신 경제발전을 바탕으로 아시아를 넘어 세계 곳곳에 영향력을 미치는 위상을 보유하게 되었다. 이러한 발전이 가능할 수 있었던 배경에는 우리나라가 과거 위기의 상황 속에서 국가의 존립을 지킬 수 있도록 귀중한 생명을 바치고 공헌한 분들의 존재가 있었기 때문이다. 매년 우리나라는 국가유공자, 6.25 참전용사와 유가족들과 함께 기념행사를 개최하며 대내외에 희생과 공헌에 대하여 감사의 뜻을 전하고, 경제적인 지원도 해오고 있다. 6.25 전쟁의 비극, 민주화 운동 등 지난 반세기 동안 우리나라에서 발생한 일들이 시간이 흐르면서 먼 옛날의 과거와 같은 역사로 변해 잊혀지게 될 것이다. 세대가 여러 차례 바뀌고, 지역별로 이루어지는 교류의 기회나 방법에 변화가 생겨나면서 과거에 발생한 비극적인 사태와 국가를 위해 희생한 분들을 기리는 방식도 이전과는 다른 모습이 될 것으로 보인다. 특히, 6.25 전쟁의 참전용사와 유가족들은 현재 고령의 나이로서 그 수가 점점 줄어들고 있다. 또한 코로나19와 같이 전 세계적인 전염병의 발생으로 왕래가 이전처럼 자유롭지 못하게 되면서 인적 교류나 행사 등을 기획하고 실시하는데 어려움이 발생하기도 한다. 현재 우리나라의 디지털 기술은 우리들의 삶을 크게 변화시키고 있다. 5G 통신망, 스마트 폰, 태플릿 PC, 가상현실을 체험할 수 있는 스마트 기기 등이 실생활에서 흔히 사용되고 있다. 메타버스 환경에서 비즈니스 미팅이 이루어지기도 하며, 유명 가수의 콘서트가 온라인 동영상 플랫폼 환경에서 개최되기도 한다. 인력 채용, 고객 대응 서비스 영역에도 인공지능 기술이 도입되어 기업들의 업무 효율도 향상되고 있다. 이러한 기술은 지금까지 우리가 해오던 보훈 분야에서도 활용될 수 있다. 디지털 기술 방식을 온라인 플랫폼에서 실현시켜 보훈외교 방식을 다양화한다면, 지구 반대편에 있는 보훈 대상 유공자와 가족들도 행사 참여는 더욱 편리해질 것이다. 그리고 6.25 전쟁 중의 상황을 생생히 보여줄 수 있는 메타버스 기술, 현재 생존하고 있는 참전용사들의 목소리와 표정을 디지털화하여 장기적으로 미래 세대에게 그 경험과 교훈을 공유해줄 수 있을 것이다.

자동노출제어를 적용한 흉부 방사선 검사 시 인체 이식형 의료기기가 선량과 화질에 미치는 영향 (Effect of Human Implantable Medical Devices on Dose and Image Quality during Chest Radiography using Automatic Exposure Control)

  • 본 연구는 자동노출제어 흉부 방사선 검사 시 신체 기능 일부를 대체 보조하거나 의약품 등을 주입하는 인체 이식형 의료기기가 흉부 영상의 선량과 화질에 영향을 미치는지 알아보고자 하였다. 제조회사와 모델이 다른 3대의 디지털 X선 발생장치와 인체모형 팬텀을 사용하여 흉부 검사와 동일한 방법으로 위치잡이를 선정 후 선행 연구에서 선량의 변화가 관찰된 인공심장박동기(Pacemaker), 심장 재동기화 치료기(CRT), 케모포트(Chemoport) 3개의 HIMD(Human Implantable Medical Device)를 상단 이온전리조센서에 부착한 후 Monte Carlo 방법론 기반의 프로그램 PCXMC 2.0을 사용하여 실험에서 도출된 DAP(Dose Area Product) 값을 입력하여 유효선량을 측정하였다. 또한 흉부 영상의 화질 평가를 하기 위해 가슴 부위에 관심 영역 3곳과 잡음영역 1곳을 설정하고 신호대잡음비(SNR; Signal to Noise Ratio), 대조도대잡음비(CNR; Contrast to Noise Ratio)를 측정하였다. 연구 결과는 유효선량의 유의미한 차이를 보여주었으며 AEC 적용과 미적용 그룹을 비교하였을 때 Pacemaker와 CRT는 유의한 차이가 있었다. (p<0.05) AEC 적용 시 Pacemaker에서 37%, CRT에서 52% 유효선량이 증가하였다. Chemoport는 유효선량의 10% 차이는 있었지만, 유의미한 차이를 보이지 않았다. (p>0.05) 영상 품질 평가에서는 모든 HIMD 삽입과 AEC 적용 유무에 따른 SNR, CNR의 유의미한 차이를 보이지 않았다. (p>0.05) 최종 결론은 AEC 적용 후 HIMD가 삽입된 환자의 흉부 X-ray 검사 시 유효선량이 증가하였으며 AEC 적용 유무에 따른 흉부 영상의 품질 차이는 없음을 알 수 있었다. 이는 HIMD가 삽입된 환자의 흉부 검사 시 AEC 미사용이 환자의 선량을 낮추는 타당성을 확인한 것이며 과피폭을 주의하고 피폭 저감화를 위한 다양한 방법을 모색하여야 한다.

Perceptional Change of a New Product, DMB Phone

  • Digital Convergence means integration between industry, technology, and contents, and in marketing, it usually comes with creation of new types of product and service under the base of digital technology as digitalization progress in electro-communication industries including telecommunication, home appliance, and computer industries. One can see digital convergence not only in instruments such as PC, AV appliances, cellular phone, but also in contents, network, service that are required in production, modification, distribution, re-production of information. Convergence in contents started around 1990. Convergence in network and service begins as broadcasting and telecommunication integrates and DMB(digital multimedia broadcasting), born in May, 2005 is the symbolic icon in this trend. There are some positive and negative expectations about DMB. The reason why two opposite expectations exist is that DMB does not come out from customer's need but from technology development. Therefore, customers might have hard time to interpret the real meaning of DMB. Time is quite critical to a high tech product, like DMB because another product with same function from different technology can replace the existing product within short period of time. If DMB does not positioning well to customer's mind quickly, another products like Wibro, IPTV, or HSPDA could replace it before it even spreads out. Therefore, positioning strategy is critical for success of DMB product. To make correct positioning strategy, one needs to understand how consumer interprets DMB and how consumer's interpretation can be changed via communication strategy. In this study, we try to investigate how consumer perceives a new product, like DMB and how AD strategy change consumer's perception. More specifically, the paper segment consumers into sub-groups based on their DMB perceptions and compare their characteristics in order to understand how they perceive DMB. And, expose them different printed ADs that have messages guiding consumer think DMB in specific ways, either cellular phone or personal TV. Research Question 1: Segment consumers according to perceptions about DMB and compare characteristics of segmentations. Research Question 2: Compare perceptions about DMB after AD that induces categorization of DMB in direction for each segment. If one understand and predict a direction in which consumer perceive a new product, firm can select target customers easily. We segment consumers according to their perception and analyze characteristics in order to find some variables that can influence perceptions, like prior experience, usage, or habit. And then, marketing people can use this variables to identify target customers and predict their perceptions. If one knows how customer's perception is changed via AD message, communication strategy could be constructed properly. Specially, information from segmented customers helps to develop efficient AD strategy for segment who has prior perception. Research framework consists of two measurements and one treatment, O1 X O2. First observation is for collecting information about consumer's perception and their characteristics. Based on first observation, the paper segment consumers into two groups, one group perceives DMB similar to Cellular phone and the other group perceives DMB similar to TV. And compare characteristics of two segments in order to find reason why they perceive DMB differently. Next, we expose two kinds of AD to subjects. One AD describes DMB as Cellular phone and the other Ad describes DMB as personal TV. When two ADs are exposed to subjects, consumers don't know their prior perception of DMB, in other words, which subject belongs 'similar-to-Cellular phone' segment or 'similar-to-TV' segment? However, we analyze the AD's effect differently for each segment. In research design, final observation is for investigating AD effect. Perception before AD is compared with perception after AD. Comparisons are made for each segment and for each AD. For the segment who perceives DMB similar to TV, AD that describes DMB as cellular phone could change the prior perception. And AD that describes DMB as personal TV, could enforce the prior perception. For data collection, subjects are selected from undergraduate students because they have basic knowledge about most digital equipments and have open attitude about a new product and media. Total number of subjects is 240. In order to measure perception about DMB, we use indirect measurement, comparison with other similar digital products. To select similar digital products, we pre-survey students and then finally select PDA, Car-TV, Cellular Phone, MP3 player, TV, and PSP. Quasi experiment is done at several classes under instructor's allowance. After brief introduction, prior knowledge, awareness, and usage about DMB as well as other digital instruments is asked and their similarities and perceived characteristics are measured. And then, two kinds of manipulated color-printed AD are distributed and similarities and perceived characteristics for DMB are re-measured. Finally purchase intension, AD attitude, manipulation check, and demographic variables are asked. Subjects are given small gift for participation. Stimuli are color-printed advertising. Their actual size is A4 and made after several pre-test from AD professionals and students. As results, consumers are segmented into two subgroups based on their perceptions of DMB. Similarity measure between DMB and cellular phone and similarity measure between DMB and TV are used to classify consumers. If subject whose first measure is less than the second measure, she is classified into segment A and segment A is characterized as they perceive DMB like TV. Otherwise, they are classified as segment B, who perceives DMB like cellular phone. Discriminant analysis on these groups with their characteristics of usage and attitude shows that Segment A knows much about DMB and uses a lot of digital instrument. Segment B, who thinks DMB as cellular phone doesn't know well about DMB and not familiar with other digital instruments. So, consumers with higher knowledge perceive DMB similar to TV because launching DMB advertising lead consumer think DMB as TV. Consumers with less interest on digital products don't know well about DMB AD and then think DMB as cellular phone. In order to investigate perceptions of DMB as well as other digital instruments, we apply Proxscal analysis, Multidimensional Scaling technique at SPSS statistical package. At first step, subjects are presented 21 pairs of 7 digital instruments and evaluate similarity judgments on 7 point scale. And for each segment, their similarity judgments are averaged and similarity matrix is made. Secondly, Proxscal analysis of segment A and B are done. At third stage, get similarity judgment between DMB and other digital instruments after AD exposure. Lastly, similarity judgments of group A-1, A-2, B-1, and B-2 are named as 'after DMB' and put them into matrix made at the first stage. Then apply Proxscal analysis on these matrixes and check the positional difference of DMB and after DMB. The results show that map of segment A, who perceives DMB similar as TV, shows that DMB position closer to TV than to Cellular phone as expected. Map of segment B, who perceive DMB similar as cellular phone shows that DMB position closer to Cellular phone than to TV as expected. Stress value and R-square is acceptable. And, change results after stimuli, manipulated Advertising show that AD makes DMB perception bent toward Cellular phone when Cellular phone-like AD is exposed, and that DMB positioning move towards Car-TV which is more personalized one when TV-like AD is exposed. It is true for both segment, A and B, consistently. Furthermore, the paper apply correspondence analysis to the same data and find almost the same results. The paper answers two main research questions. The first one is that perception about a new product is made mainly from prior experience. And the second one is that AD is effective in changing and enforcing perception. In addition to above, we extend perception change to purchase intention. Purchase intention is high when AD enforces original perception. AD that shows DMB like TV makes worst intention. This paper has limitations and issues to be pursed in near future. Methodologically, current methodology can't provide statistical test on the perceptual change, since classical MDS models, like Proxscal and correspondence analysis are not probability models. So, a new probability MDS model for testing hypothesis about configuration needs to be developed. Next, advertising message needs to be developed more rigorously from theoretical and managerial perspective. Also experimental procedure could be improved for more realistic data collection. For example, web-based experiment and real product stimuli and multimedia presentation could be employed. Or, one can display products together in simulated shop. In addition, demand and social desirability threats of internal validity could influence on the results. In order to handle the threats, results of the model-intended advertising and other "pseudo" advertising could be compared. Furthermore, one can try various level of innovativeness in order to check whether it make any different results (cf. Moon 2006). In addition, if one can create hypothetical product that is really innovative and new for research, it helps to make a vacant impression status and then to study how to form impression in more rigorous way.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. Our department has developed an information system for routine hematology using down-sized computer system.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The object image was the Thyroid eland cells image that was diagnosed as normal and abnormal (two types of abnormal: follicular neoplastic cell, and papillary neoplastic cell), respectively. By using the proposed region segmentation algorithm, the cells were segmented into nucleus. The 16 feature parameters were used to calculate the features of each nucleus. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. The bedside monitor consists of a main unit and module cases with various parameter modules. The main unit includes a 12.1" TFT color LCD, a main CPU board, and peripherals such as a module controller, Ethernet LAN card, video card, rotate/push button controller, etc. The main unit can connect at maximum three module cases each of which can accommodate up to 7 parameter modules. They include the modules for electrocardiograph, respiration, invasive blood pressure, noninvasive blood pressure, temperature, and SpO2 with Plethysmograph.

신생아 중환자실에 입원한 환아 어머니의 스트레스 (A Study on the Perceived Stress of Mothers in Neonatal Intensive Care Unit)

  • The parents have much expectation upon the pregnancy and child birth, and in most cases, they expect the healthy parturient child. However, we can be placed on the high-risk conditions which have the physical, social and immature infant, due to the unexpected results, among the new-born. Accordingly, these high-risk newborn and premature infants will be mostly in NICU, which the concentrated medical treatment can be given, upon their conditions. After their birth and during these periods, they will be divided from the parents, and the nurse will accomplish the bringing-up activities which they can take care of the infant, expected by the parents after their birth. The hospitalization of high-risk newborn including these premature infants is the shocking experience to the parents of family, and thus they can feel the fear and uneasiness, and these reactions of parents are troubled in the behavior at the usual days, and cause the disorder and spiritless status, and these results break the supporting ability of parents, and cause the obstruction. Also, the unavoidable division between the parents and the children as like hospitalization of children can make the parents to feel the alienation emotionally, and this causes the results which the pride on the bringing-up ability of baby gets to be lost. These problems can cause the difficulties on the bonding or the parenting in the further days, and can be related to the neglect and abuse of children. Also, it is gradually increased to study and report which the emotional division by the physical division between the mother and the baby obstructs the normal affection course between the parent and the infant. The stress caused by the birth and the hospitalization of high-risk newborn, as like this, is important in the points which it can uncertainly affect the potential energy for the relationship of parent-child who are finally healthy. Accordingly, the significance and purpose of this study are to understand the contents and degree of stress which the parents of high-risk newborn including the immature child can be experienced from the hospitalization of ICU for their new borns, and thus to offer the basic program to the nursing intervention program for these. The subject of this study is the mother of newborn in NICU of 10 General Hospitals located at the 3one of Pusan, Korea from September 1997 to October 1997, and thus makes the subject of 95 person of parents who agreed to take part in the study and it is descriptive study related to the stress of mother having the newborn in NICU. The method is based on the preceding study related to the stress of mother having the experience of child hospitalization and chronic disease child, and then acquires the advice of specialists group as like 5 nursing professors, and then is amended and supplemented. Total number of questions is 43 items and consists of 5 factors as like medical treatment &nursing procedures, disease status & prognosis, role of parents, communication & inter-personal relationships, hospital environment, and is 5 point Likert Scale. The reliability of this study method is very highly shown to be Cronbach α=0.95. The collected data is analysed as Average, Frequency, Standard Deviation, T-test, ANOVA, Pearson Correlation Coefficient, Duncan multifulrange test by use of SPSS /PC (V7.5). The results of this study is summarized as under. 1. Every characteristics of subject is which the party of mother is 28.70age(±7.48) in the average ages, 51% in the high-school graduate, 38.5% in the christianity, total monthly income is 212.55 thousand won(±1.971), 74.5% in the housewife, 72.9% in the parents and children together living and the number of children to be 1.48person(± 0.6) in average, the recognition on the prognosis of baby is 74.0% in 'Don't know', the relationship with the husband after the hospitalization of babyis 37.3% in 'More Intimate', the relationship with the family of husband to be 48% in 'No-change', and the degree which is consulted with the husband about the baby is 55% in 'very frequently' and the visiting number per week is 4.59(±1.63) in average and the accompanying person in the time of visiting is which the number of husband is 56.3% and thus is the highest. The characteristics of baby is which the age is 21.88days(±16.47) after the birth in average, the sex to be 50 person in the female 52.1% and the order of birth to be 54.2% in the first chid, and the weight in the birth to be 2770gm(±610) and the height in the birth to be 46.26cm(±7.62) in aver age. The medical diagnosis is 37.5% in the premature infant, the career of hospitalization is 96.9% in 'None', and the operation plan is 90.6% in 'None' and the execution of operation is 88% in 'None' and the nursing of incubator is 55.2% in 'Yes', and the method of feeding is 50.5% in 'Oral' and the contents of feeding is 46.9% in the 'Milk'. 2. The total stress degree of subject is almost highly shown to be as 3.36(±0.86). If it is compared upon each cause, 'stress on disease status & prognosis' is highest 3.79(±1.28), and it is in the order of 'stress on medical treatment & nursing procedures' 3.70(±0.93), 'stress on hospital environment' 3.14(±0.86), 'stress on role of parents' 3.18(±0.92) and 'stress on communication & inter personal relationship' 2.62(± 0.77) 3. As the results of checking the notworthiness of stress degree upon each variable of subject, the variable showing the noted difference was the birth weight(γ=-0.16, P=0.04), birth height(γ=-0.23, P=0.03), nursing in the incubator(F=8.93, P=0.04), feed method(F=2.94, P=0.04). That is to say, it is shown which the smaller the birth weight is, the higher the stress degree of mother is noteworthily. Also, the smaller the birth height baby is, the higher the stress of mother is. In the incubator, it os shown which the mother whose baby is nursing in the incubator is higher in the stress degree than other mothers. Upon the feeding method of baby, that is to say, TPNis the highest, and it is shown in the order of NPO, Tube feeding, and P.O. feeding. When we review the above-mentioned results, as the status is serious, it is thought which we include the supporting nursing for coping with the stress of parents in the setting-up od nursing plan for the baby in the NICU.

소규모 외식업체용 IP-USN을 활용한 HACCP 시스템 적용 및 유효성 검증 (The Implementation of a HACCP System through u-HACCP Application and the Verification of Microbial Quality Improvement in a Small Size Restaurant)

  • 대부분의 외식산업은 생산 품목, 생산 방법, 생산 기술, 대상자들이 제조가공업체와는 매우 다르고 생산량, 취급인원, 영업규모등도 학교나 병원급식과는 여러 요소의 차이점을 가지고 있다. 본 연구는 소규모 외식업체에서 HACCP 시스템 적용을 위하여 종사자 대상 위생교육과 도입 전후의 미생물 분석과 시스템 도입에 따른 유의성 평가와 평가항목별 상관관계를 분석하였다. 또한 USN를 구축하여 도입 전후의 시간-온도관리 분석을 시행하여 효율성을 평가하였다. 위생교육 전후 조리 종사자의 위생업무 지식에 관한 평가 결과 개인위생, 식품의 공급 및 저장, 식품의 취급 및 배식, 기구의 세척 및 소독에서 유의적인 차이가 있었다. 또한 위생교육 전후 조리 종사자의 수행도에 관한 평가결과 식품의 취급 및 배식, 기구의 세척 및 소독, 작업 환경관리 영역에서 유의적으로 상승한 결과를 보였다. 조리 종사자의 교육 전후의 위생지식 결과는 개인위생(p<0.05), 식품의 공급 및 저장(p<0.05), 식품의 취급 및 배식(p<0.05), 기구의 세척 및 소독 영역(p<0.05)에서 유의적인 차이를 보였다. 모든 항목에서 정답률이 상승하였고 전체 문항에 대한 정답률은 교육 전 75%에서 교육 후 90%로 향상되었다. 조리 종사자의 위생업무에 대한 수행도 결과는 식품의 취급 및 배식(p<0.05), 기구의 세척 및 소독(p<0.05), 작업 환경관리 영역(p<0.05)과 총 점수에서 유의적인 차이를 보였다. HACCP 도입 전후의 소독에 따른 양상추 샐러드 미생물 품질분석 결과는 소독전 일반세균이 양상추 원재료 4.80 logCFU/g에서 소독 후 2.41 logCFU/g으로, 라디치오는 원재료 4.15 logCFU/g에서 소독 후 2.48 logCFU/g으로, 토마토는 원재료 4.10 logCFU/g에서 소독 후 평균 1.68 logCFU/g으로 나타나 중요관리점인 세척 시 소독을 통해 미생물 수치가 감소되었다. 멸치와 꽈리고추의 미생물적 품질결과는 원재료의 일반세균수는 각각 4.52 logCFU/g, 5.72 logCFU/g으로 나타났고, 볶았을 때 멸치는 2.07 logCFU/g, 꽈리고추는 1.50 logCFU/g으로 낮아졌다. 가열 공정 후 대장균군과 장내세균은 검출되지 않았다. 하지만 멸치꽈리볶음의 생산 공정은 재료별로 따로 볶아 버무리는 공정이 있어 최종식품에는 일반세균 4.33 logCFU/g, 대장균군 0.71 logCFU/g, 장내세균 1.65 logCFU/g, 황색포도상구균이 1.27 logCFU/g으로 검출되었다. 숙주나물의 미생물 분석 결과는 조리단계 및 배식단계에서 각각 4.20 logCFU/g, 4.68 logCFU/g의 높은 수치를 나타냈으나, 적용 후의 조리단계 및 배식단계에서는 각각 3.26 logCFU/g, 3.34 logCFU/g으로 나타났다. 숙주나물 및 잡채의 후처리 작업시 HACCP 적용 전 종사자 손의 일반세균수는 평균 5.90~5.99 logCFU/hand에서 적용 후 평균 0~2.30 logCFU/hand로 나타났다. 가열조리 후처리 공정에서는 교차오염을 줄이기 위하여 종사원의 손의 위생적인 관리가 필수적인 것으로 사료된다. 도입 전과 도입 후 2개월 시점의 조리 종사자의 위생지식점수 차이, 위생수행도 차이, 양상추 샐러드와 조리 종사자 손의 미생물적 품질 개선효과 간의 상관관계를 구하였다. 위생수행도의 변화와 양상추 샐러드의 미생물적 품질 개선 효과는 유의한 양의 상관관계(p<0.01)를 보여 위생 수행도가 높아질수록 식품의 미생물적 품질이 개선될 수 있다는 것을 보여주었다. 위생지식의 변화와 조리 종사자 손의 미생물 변화가 유의한 양의 상관관계(p<0.05)가 있었다. u-HACCP 시스템 도입 전후 온도관리에서의 3가지(온도측정의 정확성, 온도관리 효율성, 온도범위의 안정성)를 평가하였는데, 도입 전에는 업무시간에 냉장고 $5^{\circ}C$ 이하, 냉동고 $-18^{\circ}C$ 이하, 온장고 $57^{\circ}C$ 이상의 기준이 지켜지지 않은 것을 발견할 수 있었다. 하지만 도입 후 냉장고, 온장고, 식기세척기마다 자동경보로 위생 사고를 예방하였고 실시간 상황모니터가 설치되어 있어 HACCP 업무가 대폭 간소화 되었으며, 무엇보다 지속적인 위생교육으로 인해 직원들의 위험온도($5^{\circ}C{\sim}57^{\circ}C$)에 대한 이해도가 높아져 냉장, 냉동, 온장고의 기준온도의 준수율이 상승되었다. 따라서 유비쿼터스 센서 온도계를 이용하여 검수, 가공, 보존, 조리, 유통 등 각 단계별로 자동 모니터링이 가능해 잠재적 위험요소를 사전에 관리자에게 통보해 줌으로써 식품의 위생안전성을 확보할 수 있고, 무선 센서 네트워크의 특징을 이용하여 식재료 안전 관리 시스템에 적용할 경우 24시간 모니터링이 가능하여 신속한 개선조치와 이력 관리 모니터링을 통한 신뢰성 있는 품질경영 시스템을 구축할 수 있을 것으로 사료된다.

Open Source를 이용한 MicroPACS의 구성과 활용 (Application of MicroPACS Using the Open Source)

  • 목적 : Small-scalled PACS, Pc-based PACS로 표현되는 MicroPACS 시스템 구축에 대한 관심도가 급격하게 증가하고 있는 추세이다. MicroPACS 시스템은 PACS를 작은 규모에서 사용할 수 있도록 구성해놓은 것이고, 이 시스템을 구성하기 위해서는 DICOM viewer나 연결프로그램 등이 필요하다. 이것은 공개소스프로그램(Open Source Program)을 통해서 어느 누구나 쉽게 무료로 다운로드를 받을 수 있게 되어있다. 본 논문은 Open source program으로 MicroPACS를 직접 구성해보았고, 저장매체로서의 활용가치를 측정하기위하여 성능, 안정성 측면에서 기존의 광 저장매체(CD, DVDRAM)와 비교 분석하였다. 실험재료 및 방법 : 1. 소형 PACS를 구축하기 위해서 먼저 다음 기준에 맞는 DICOM Server Software를 검색한다. (1) 윈도우체제에서 사용가능할 것. (2) Free ware일 것. (3) PET/CT scanner와 호환되어야 할 것. (4) 사용하기 쉬워야 할 것. (5) 저장의 한계가 없어야 할 것. 2. (1) MicroPACS의 성능을 평가하기 위해 환자 1명의 Data ($^{18}F$-FDG Torso PET/CT)를 현재 Back-up장치로 쓰이는 광 저장매체(CD, DVD-RAM)와 MicroPACS에 저장하는데 소요되는 시간(Back up time)과 workstation으로 복구되기까지의 시간(Retrieval time)을 비교해 보았다. (2) PET/CT 검사를 시행했던 환자 1명의 병록번호와 검사 시행날짜를 핵의학과 직원 7명을 대상으로 알려주고 Data를 찾는데 소요되는 시간을 MicroPACS와 광 저장매체(CD, DVD-RAM)에서 각각 측정하여 비교하였다. 3. 기존의 백업장치로 쓰였던 CD들 중에서 2004년부터 2006년까지 500장을 무작위로 뽑아서 loading을 하였고 그중에서 얼마만큼의 에러가 발생하였는지를 측정하여 MicroPACS의 안정성을 비교평가하였다. 결과 : 1. Server와 DICOM viewer 기능을 갖춘 11개의 open source software 중에서 Conquest DICOM Server를 선택하였다. 2. (1) Backup과 Retrieval 시간 비교(단위 : 분)는 다음과 같다; DVD-RAM(5.13,2.26)/Conquest DICOM Server (1.49,1.19) by GE DSTE (p<0.001), CD (6.12,3.61)/Conquest (0.82,2.23) by GE DLS (p<0.001), CD (5.88,3.25)/Conquest (1.05,2.06) by SIEMENS. (2) CD ($156{\pm}46$초), DVD-RAM ($115{\pm}21$초) and Conquest DICOM Server ($13{\pm}6$초). 3. 1년간 MicroPACS에서의 데이터손실은 없었으며(0%), 500장의 CD 중에서 14개(2.8%)가 Loading하는데 실패하였다. 결론 : 현재 많은 병원에서 도입되고 있는 Full PACS를 open source software를 통하여 소규모의 PACS로 재현해 보았고, 그 결과 가능하다는 결론이 나왔다. 데이터 저장의 유용성을 평가한 결과에서 MicroPACS를 이용하는 것이 기존의 광저장매체를 사용하는 것보다 효율적이고 작업속도가 향상 된다는 것을 확인할 수 있다.

스마트교육을 위한 오픈 디지털교과서 (Open Digital Textbook for Smart Education)

  • 스마트교육에서 디지털교과서의 역할은 학습자와 대면하는 교육미디어로써 그 중요성은 재론의 여지없다. 이러한 디지털교과서는 학습자의 편의와 더불어 교수자, 콘텐츠 제작자, 유통업자를 위하여 표준화되어야 활성화되고 산업화될 수 있다. 본 연구에서는 다음과 같은 3가지 목표를 지향하는 디지털교과서 표준화 방안을 모색한다. (1) 디지털교과서는 온-오프 수업을 모두 지원하는 혼합학습 매체의 역할을 해야 하며, 특별한 전용뷰어 없이 표준을 준수하는 모든 EPUB 뷰어에서 실행가능 해야 하며, 기존의 이러닝 학습 콘텐츠와 학습관리시스템를 활용할 수 있도록 하며, 디지털 교과서를 사용하는 학습자의 정보를 추적 관리할 수 있는 트랙킹기능이 있으면서도, 오프라인 동안의 정보를 축적하여 서버와 통신할 수 있는 기능도 필요하다. 디지털교과서의 표준으로서 EPUB을 고려하는 이유는 디지털교과서가 책의 형태를 가져야 하는데 이를 위해서 따로 표준을 정할 필요가 없으며, EPUB 표준을 채택함으로써 풍부한 콘텐츠, 유통구조, 산업기반을 활용할 수 있기 때문이다. (2) 디지털교과서는 오픈소스를 적극 활용하여 저비용으로 현재 사용가능한 서비스를 구성하여 표준과 더불어 실제 실행 가능한 프로그램으로 제시되어야 하며, 관련 학습 콘텐츠가 오픈마켓의 형태로 운영될 수 있어야 한다. (3) 디지털교과서는 학습자에게 적절한 학습 피드백을 제공하기 위하여 모든 학습활동 정보를 축적하고 관리될 수 있는 인프라를 표준에 따라 구축하여 교육 빅데이터 처리의 기반을 제공하여야 한다. 이북 표준인 EPUB 3.0을 기반으로 하는 오픈 디지털교과서는 (1) 학습활동 정보를 기록하고 (2) 이 학습활동 지원을 위한 서버와 통신하여야 한다. 현재 표준으로 정해져 있지 않은 이북의 기록과 통신 기능을 EPUB 3.0의 JavaScript로 구현하여 현재 EPUB 3.0 뷰어에서도 활용하면서 이를 차세대 이북 표준 또는 교육을 위한 이북 표준(EPUB 3.0 for education)으로 제안하여 향후 제정된 표준 이북 뷰어에서는 JavaScript없이도 처리되도록 하는 전략이 필요하다. 향후 연구는 제안한 오픈 디지털교과서 표준에 의한 오픈소스 프로그램을 개발하고, 개발된 오픈 디지털교과서의 학습활동정보를 활용한 새로운 교육서비스 방안(교육 빅데이터 활용방안 포함)을 제시하는 것이다.
