Clinic Study of Gami-Yunbueum Efficacy on the Psoriasis (건선에 대한 가미윤부음의 임상연구)

  • Psoriasis is a common inflammatory disease of the skin marked by excessive scaling associated with inflammation, affecting 1-2% of the population. Although their etiologies remain unknown, the finding of CD4+ and CD8+ lymphocytes at sites of psoriasis has led many reports to hypothesize that psoriasis is a T lymphocyte-mediated disease directed against unknown autoantigens. The oriental medicine understands the cause of psoriasis as low humidity of skin tissues and functional deteriorations of the liver and lung. Gami-Yunbueum (GY) is a mixture of two previous prescription, which were used to the protection of liver and lung. We had prescribed the GY to the three psoriasis patients for 11 weeks and examined the effectiveness of the GY by PASI (Psoriasis Area and Severity Index) score. In our experiment, GY improved the symptoms of psoriasis in all cases. These results indicated that GY can apply to the treatment of psoriasis. Furthermore, the process for treatment of psoriasis is possible to provide humidity to skin and control the functions of liver and lung.

New Interpretation on 'MyeonGu' of Baekho-tang Text of Shanghanlun through Case Reports (아토피피부염 및 건선 환자 증례를 통한 상한론 백호탕 조문의 '면구(面垢)'에 대한 새로운 해석)

  • This paper suggested MyeonGu(面垢), symptom of Baekho-tang(白虎湯) in text 219 of Shang-Han-Lun(傷寒論), could be reinterpreted. There were 4 cases, 1 case on atopic dermatitis and 3 cases on psoriasis. All of them were diagnosed by Ying-Ming-Bing(陽明病) symptom. After taking Baekho-tang, SCORAD index of atopic dermatitis in case 1 improved 62.8 to 12.8 and PASI score of psoriasis in case 2, 3, and 4 improved 13.0 to 2.4, 10.8 to 0.8, and 8.6 to 1.0 respectively. Since Baekho-tang was effective for certain skin disease with Ying-Ming-Bing(陽明病) symptom, MyeonGu could be related on skin problem. MyeonGu was mainly translated in 'someone's face seems to be covered with dirt'. However if Myeon(面) is translated in 'surface of the body' and Gu(垢) is translated in 'inflammation', it means the surface of the body suffered from papules, erythema, white scales, etc. These are features of chronic inflammatory cutaneous disease like atopic dermatitis and psoriasis. This study is meaningful in case of giving specific clinical case of MyeonGu of Baekhotang text of Shanghanlun.

The Amelioration Effect of the Ethanolic Extract of Cnidium officinale in Mice with Imiquimod-induced Psoriasis-like Skin Lesion

  • Psoriasis is an auto-immune skin disease, which is characterized by the excessive generation of plaques on the skin with typically a long-lasting red, itchy and scaly symptoms. Imiquimod, which has been used for the treatment of external genital warts, actinic keratosis, and superficial basal cell carcinoma, induced of psoriasis-like skin disorders with skin erythema and thickness in mice. In the present study, we tried to find the bioactive herbal extract against imiquimod-induced psoriasis-like skin disorder in mice. During the searching of the herbal extract with anti-psoriatic effect, the ethanolic extract of Cnidium officinale ameliorated imiquimod-induced psoriasis-like skin disorder in mice. The morphological evaluation, H&E staining and Psoriasis Area and Severity Index (PASI) score showed that ear and back thickness, and erythema induced by imiquimod were significantly reversed after the treatment of the cream of the ethanolic extract of C. officinale. The overexpressed myeloperoxidase (MPO) and keratin 6A levels were decreased by the treatment of C. officinale cream. Also, $IFN-{\gamma}$, c-fos and $I{\kappa}B-{\alpha}$ mRNA levels, which are related to the progression of psoriasis, were reduced by C. officinale cream. Thus, the ethanolic extract of C. officinale ameliorated psoriasis-like skin disorder induced by imiquimod and might be the therapeutic agent for psoriasis.

A Review of Assessment Tools in Traditional Medicine on the High Frequent Skin Diseases (다빈도 피부질환에 대한 국내외 한의학 논문에서 평가도구 사용현황에 대한 고찰)

  • Objectives : The purpose of this study is to establish the basis of the assessment tools that can be used for the diagnosis and assessment of skin diseases by reviewing the present status of the assessment tools used in domestic and overseas traditional medicine papers for the past five years. Methods : This study is based on analysis of papers on clinical trials about atopic dermatitis, psoriasis, acne, urticaria published from 1st July 2013 to 30th June 2018. The papers were searched from Oriental Medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), Pubmed, MEDLINE. Results : 40 articles on atopic dermatitis, 34 articles on psoriasis, 26 articles on acne and 10 articles on urticaria were selected. Among them, 38 articles(90%), 20 articles(85%), 22 articles(85%) and 5 articles(50%) used assessment tools in order. Conclusions : SCoring Atopic Dermatitis(SCORAD), objective SCORing of Atopic Dermatitis index(OSI), Eczema Area and Severity Index(EASI) were used in studies on atopic dermatitis. In the case of psoriasis, The Psoriasis Area and Severity Index(PASI) was used in most papers. In the study on acne, most of Korean papers used Korean Acne Grading System(KAGS), but in the overseas papers, various assessment tools were used. In the case of urticaria, the rate of use of the assessment tool was the lowest, but the assessment tool used was unified by Urticaria Activity Score(UAS). Since skin diseases can cause stress and a decrease in quality of life, the quality of life should be taken into account when evaluating treatment effects.
