A Study of the Quantitative, Qualitative Analysis on Optimizing Diagnostic Imaging Device Selection in Nasopharynx MRI (비 인두 자기공명 검사 시 최적의 진단영상 장치 선택에 관한 정량, 정성적 평가에 관한 연구)

  • The object of is this research is to find out the optimal Tesla by evaluating SNR and CNR, after testing 1.5 T and 3.0 T. The randomly selected patients tested by nasopharynx MRI transmitted in PACS were applied to the research. Two MRI units(1.5 T, 3.0 T) was used for analyzing the data. As a method of analysis, in T1W highlighting and T1 fat removal images, we set up a certain area of interest and evaluated the SNR and CNR on tongue, spinal cord, masseter muscle, fat, parotid gland, and tumor tissue. We evaluated the SNR and CNR by quantitative analysis of six tissue, measuring the quality of images for uniform fat removal, magnetic sensitivity artifact on a four-point scale by qualitative analysis. The statistical significance of this date analysis was based on independent sample verification and was accepted when the P value was less than 0.05. As a result of analysis of both devices, 3.0 T was high in the quantitative evaluation, while 1.5 T was high in the qualitative evaluation. Considering the advantages and disadvantages of each device, and if the device is selected complementarily and applied to patients, it is believed that it will provide the optimal information.

  • Purpose: Digital medical image commenced with an introduction of PACS has become more popular today in the radiation diagnosis and medical treatment and made great progress, in particular, for medical testing field, whereas it has made slow progress for radiation therapy area. In order to accommodate the current trend of digital from analog, a spherical type mechanical check device (SMCD) that is form of spherical differing from the existing form of flat or cube has been designed and tested its practicability to replace the part in mechanical check with digital image from QA operation. Materials and Methods: If the distance maintains constant between source(target) and image detector with constant distance to the center of spherical type mechanical check device(SMCD), the size will be shown as a constant image at all times regardless of its direction exposed. For the test, two accurate hemispheres are made and put together which results in a sphere of the equilateral circle. Results: It enables a variety of implementation of the existing mechanical check using digital image as follows: congruity level of radiation field and light field, size accuracy of radiation field and collimation field, gantry rotation isocenter check, collimation rotation isocenter check, room laser accuracy check, collimation rotation angle check, couch rotation angle check, and more. Conclusion: It has proved its practicability in checking isocenter congruity level as real time at the time of simultaneous rotation between gantry and couch that is applied to the non-coplanar field, which had been hard to apply as a device formed of existing flat or cube.

Radiographic Evaluation of Small Intestinal Diameter in Small Breed Dogs (소형견에서 소장 직경의 방사선학적 평가)

  • The purpose of the present study was to prove our empirical tendency of relatively high small intestinal diameter (SI) to fifth lumbar vertebral height (L5) ratio, which has been used in dogs. In this study, the ratio of SI/L5 was determined in small breed dogs weighing less than 5 kg. In addition, the effect of large volume of contrast media on the intestinal dilation was determined by performing upper gastrointestinal contrast study. Abdominal radiography and upper gastrointestinal series were performed in twelve healthy dogs weighing less than 5 kg. Small intestinal diameter (SI), fifth lumbar vertebral height (L5), and twelfth rib diameter were measured on abdominal radiographs. The range of values of SI/L5 is from 1.03 to 2.26 in plain radiography, and from 1.55 to 2.5 in contrast studies. Contrast agent significantly increased small intestinal diameter, and could be considered as mildly dilated intestinal model. Therefore, a value of 2.1 for SI/L5 is recommended as the upper limit of the normal range suggesting nonobstructive intestinal dilation.

Diagnostic Usefulness of Computed Tomography Compared to Conventional Chest X-Ray for Chest Trauma Patients (흉부 외상 환자에서 일반흉부촬영과 비교한 흉부단층촬영의 진단적 유용성)

  • Purpose: Early diagnosis and management of therapeutic interventions are very important in chest trauma. Conventional chest X-rays (CXR) and computed tomography (CT) are the diagnostic tools that can be quickly implemented for chest trauma patients in the emergency department. In this study, the usefulness of the CT as a diagnostic measurement was examined by analyzing the ability to detect thoracic injuries in trauma patients who had visited the emergency department and undergone CXR and CT. Methods: This study involved 84 patients who had visited the emergency department due to chest trauma and who had undergone both CXR and CT during their diagnostic process. The patients' characteristics and early vital signs were examined through a retrospective analysis of their medical records, and the CXR and the CT saved in the Picture Archiving Communication System (PACS) were examined by a radiologist and an emergency physician to verify whether or not a lesion was present. Results: Pneumothoraxes, hemothoraxes, pneumomediastina, pulmonary lacerations, rib fractures, vertebral fractures, chest wall contusions, and subcutaneous emphysema were prevalently found in a statistically meaningful way (p<0.05) on the CT. Even though their statistical significance couldn' be verified, other disorders, including aortic injury, were more prevalently found by CT than by CXR. Conclusion: CT implemented for chest trauma patients visiting the emergency department allowed disorders that couldn' be found on CXR to be verified, which helped us to could accurately evaluate patients.

Measurement of Kager's Triangle Area and Retrocalcaneal Surface Temperature by shoes heel height (신발 굽 높이에 따른 Kager씨 삼각의 면적과 후종족부의 표면온도 측정)

  • The aim of this study was to analyze radiological change of Kager's triangle area and retrocalcaneal surface temperature by shoes heel height. Area of Kager's triangle was measured by simple radiography study And PACS of INFINITI. Retrocalcaneal surface temperature were measured by DITI. Area of Kager's triangle and retrocalcaneal surface temperature were calculated for comparison and analysis, with flat shoes and high heel shoes. Area of Kager's triangle($0.88cm^2$) and retrocalcaneal surface temperature ($1.4^{\circ}C$)tends to decrease with high heel shoes. The highest and shortest of the Kager's triangle area and a surface temperature difference between flat shoes and high heel shoes, each $0.9cm^2$, $1.2cm^2$, $1.6^{\circ}C$, $0.5^{\circ}C$ and showed slight differences. The highest weight and the lowest weight of a surface area and the temperature difference between flat shoes and high heels, each $1.8cm^2$, $0.8cm^2$, $1.1^{\circ}C$, $0.2^{\circ}C$ and higher weight Kager's area and the surface temperature is decreased. The longest time and shortest time of a surface area and the temperature difference between high heels, each $0.8cm^2$, $1.4^{\circ}C$. In conclusion, Areas of Kager's triangle and retrocalcaneal surface temperature decrease with high heel shoes. If we wear high heel shoes for a long time, retrocalcaneal pain and blood flow disorder will occurs.

Analysis and Evaluation of Computed Tomography Dose Index (CTDI) of Pediatric Brain by Hospital Size (병원규모별 소아 두부 CT 검사 선량지표 분석 평가)

  • Even though children are exposed to the same amount of radiation, their effective dose amount is higher than those of adults. Therefore, it is very important to reduce the amount of unnecessary radiation exposure because children have a higher radiosensitivity and a smaller body size than adults. In this study, the proposal to seek ways to reduce the amount of radiation is drawn by comparing and analyzing CT Dose Index(CTDI) on the pediatric head CT which was performed at the Busan regional hospitals, to the national diagnostic reference levels. For this, the pediatric head CT scan was conducted among the CT equipments that were installed in downtown Busan. From 2,043 children 10 years old or less who were referred to the pediatric head CT scan, targeting the 28 CT equipments in the 24 hospitals that transmit dose reports to PACS, were examined retrospectively. As a result, the average value of CTDIvol, computed tomography dose index (CTDI) of infant brain, across the hospital, was 31.18 mGy, with DLP of $444.73mGy{\cdot}cm$, which exceeded the diagnostic reference level. The lower the age, the more management is needed for radiation. However, the reality is that the CT examinations are being conducted with a dose that exceeds the reference level as the age of the aged is exceeded. For this purpose, the study seeks to determine the degree of doses of doses outside the diagnostic reference level and analyze the cause of the excess dose and devise measures to reduce the dose reduction.

  • Flat-panel detector(FPD) is the driving force for realizing the next gene ration of x-ray system. In this study, we developed amorphous selenium(a-Se) based flat-panel digital X-ray detector(DXD) for radiography. The prototype detector consists of an amorphous selenium layer and a thin-film transistor(TFT) array. Comparing to other papers1)-4), optimization of amorphous selenium and progress of evaporation were similar. The pixel pitch of fabricated detector was $139{\mu}m$, fill factor was 86%, and the size was 14"${\times}$8.5". Hand and test bar pattern images were acquired. A high modulation transfer function(MTF) factor was obtained: 58% at 3.0 lp/mm.

Effect of head positioning on the vertical and horizontal magnification in panoramic radiographs: rotation along the sagittal and transverse plane (파노라마방사선사진에서 환자의 머리 위치가 하악 수직, 수평 확대율에 미치는 영향: 상하 및 좌우회전)

  • Purpose: The purpose of this study was to investigate the effect of head position changes on vertical and horizontal magnification in dental panoramic radiographs. Materials and Methods: Five 4 mm metal balls were placed above alveolar crest of dry skull considering extraction socket and dental arch. Panoramic radiographs were taken by OP-100D (Instrumentarium Imaging Co., Tuusula, Finland) at proper and rotated head position along the sagittal and transverse axis at 3 - $20^{\circ}$ upwardly, downwardly, to the left and to the right rotation. Images were stored in DICOM files and were measured by ruler equipped within INFINITT PACS software. Results: The mean horizontal magnification was $1.22{\pm}0.01-1.44{\pm}0.01$ and mean vertical magnification was $1.29{\pm}0.00-1.35{\pm}0.02$ at standard head position. There was statistical significance of horizontal magnification between the anterior ($1.24{\pm}0.02-1.31{\pm}0.03$) and the posterior area ($1.40{\pm}0.03-1.33{\pm}0.02$) (P < 0.05). Vertical magnification resulted in less variation ($1.24{\pm}0.01-1.37{\pm}0.02$) than horizontal magnification ($0.88{\pm}0.03-3.73{\pm}0.16$) according to the rotation. There was statistical significant difference on horizontal magnification (P < 0.05). Conclusion: In rotated head position, the horizontal magnification should be considered because these can cause distortion on panoramic radiographs.

Dose and Image Assessment according to Radiologic Factors Variation at Digital Humerus X-ray Examination (디지털 환경에서 Humerus 검사 시 촬영인자 변화에 따른 선량 및 화질 평가)

  • Purpose : We aim at presenting the optimum radiologic factor through the evaluation of dose variation and of image quality through the use of a grid in Humerus examination and the change of dose because of the change of radiologic factor. Materials and Methods : We divided it in 3 cases: when using a grid or not and when using IP(Image Plate) in a digital system. Also, as fixing kVp to 70kVp it changed mAs, and fixing mAs to 10 it changed kVp, we put up resolution chart and Burger rose phantom on the acrylic phantom of 7cm (the same level of Humerus) to evaluate the dose and image. We used Image J program to evaluate the quantitative resolution of the obtained image, and made the qualitative evaluation and statistical analysis of the image saved in PACS for 20 radiologic technologist with more than 10 years of experience in order of evaluate its contrast. We used SPSS10(SPSS Inc. Chicago, Illinois) for statistical analysis. Results : We observed the analytic result of resolution by the change of kVp that it was $4.539dGycm^2$ in 60kVp and $757.472dGycm^2$ in 75kVp, which increased about 64.6% of dose, while for the resolution it had the pixel value 30.7% better with 851 in 60kVp than 651 in 75kVp. Also, we analyzed the result of resolution by the change of mAs that it was $3.106dGycm^2$ in 5mAs, and $12.470dGycm^2$ in 20mAs, which increased about 400% of dose, while for the resolution DR had 678 in 5mAs, and 724 in 20mAs that increased about 6.8% of resolution. We made the qualitative evaluation of contrast by the change of kVp that DR showed the higher quality than CR, but the contrast by the change of kVp had no special different at the moment of visual evaluation, nor statistically significant difference(P>0.05). We observed the qualitative evaluation of contraste by the change of mAs that the contrast increased as DR increased mAs, and had statistically significant difference(P<0.05). On the other hand, CR had no significant difference for more than 10mAs nor statistically significant difference(P>0.05). Conclusion : In case of some patients with radiographic exposure by the repeated examination such as emergent patient or Follow up patient, they are considered to try to limit the use of a grid, to set kVp under 65kVp in fixed mode, to select less than 10mAs and to reduce the possibility of patient being bombed.
