Propionibacterium acnes (P. acnes) plays an important role in the disease pathogenesis of acne vulgaris, a disorder of pilosebaceous follicles, seen primarily in the adolescent age group. In the present study, the presence of antibodies against P. acnes (MTCC1951) were detected in acne patient (n=50) and disease free controls (n=25) using dot-ELISA and Western blot assay. The ability of P. acnes to induce pro-inflammatory cytokines by human peripheral blood mononuclear cells (PBMCs), obtained from acne patients and healthy subjects, were also analysed. The patients (n=26) who were culture positive for skin swab culture, were found to have a more advanced disease and higher antibody titres (1:4000 to >1:16000) compared to the P. acnes negative patients (n=24) and normal controls (n=25). An analysis of patients' sera by western blot assay recognized a number of antigenic components of P. acnes, rang-ing from 29 to 205 kDa. The major reactive component was an approximately 96 kDa polypeptide, which was recognised in 92% (24 of 26) of the patients sera. Further, the P. acnes culture supernatant, crude cell lysate and heat killed P. acnes whole cells, obtained from 72-h incubation culture, were observed to be able to induce significant amounts of IL-8 and tumor necrosis factor alpha (TNF-${\alpha}$) by the PBMCs in both the healthy subjects and patients, as analysed by cytokine-ELISA. The levels of cytokines were significantly higher in the patients than the healthy subjects. A major 96 kDa polypep-tide reactant was eluted from the gel and was found to cause dose dependent stimulation of the pro-ductions of IL-8 and TNF-${\alpha}$. Thus, the above results suggest that both humoral and pro-inflammatory responses play major roles in the pathogenesis of acne.
Objective: Acne scarring is the most common sequela after dermal inflammation caused by acne. Many methods are currently used to treat acne scarring; doctors using traditional Korean medicine have also attempted to treat acne scarring in a new way that was developed from traditional methods. This study examines the effectiveness of re-cell therapy.Method: Eleven patients with various types of acne scars were treated by re-cell therapy. We used the global acne scarring classification (GASC) and ECCA (from the French “echelle d’evaluation clinique des cicatrices d’acne”), both of which are generally used to measure and grade the degree of acne scarring. Results: As a result of the treatment, the conditions of all patients were conspicuously improved in both scales. The GASC scores of the patients changed from grade 19.6±10.4 to grade 4.9±3.3 (p=0.001), and the ECCA scores changed from grade 120.9±28.7 to grade 41.8±18.3.Conclusion: Re-cell therapy showed effectiveness in treating acne scarring and could be applied in clinical cases.
Acne vulgaris is a very common condition affecting up of about 80% to 90% of adolescents. The patients with acne have been shown to be adversely impacted by the effect of acne on their quality of life. Four factors are believed to play a key role in the development of acne lesions: excess sebum production, disturbed keratinization within the follicle, colonization of the pilosebaceous duct by Propionibacterium acnes, and the release of inflammatory mediators into the skin. Consequently, the target for acne therapy is these well-known pathogenic factors responsible for this disease state. Topical retinoids correct abnormal keratinization, but it should be applied cautiously because of irritation. Benzoyl peroxide is an effective bactericidal agent against P. acnes. Main topical antibiotics are erythromycin and clindamycin. Fixed combination topical products with retinoids, benzoyl peroxide and antibiotics have been introduced. Use of systemic antibiotics, including tetracyclines and macrolides rapidly improves inflammatory acne lesions. Oral isotretinoin is effective against all of the main pathogenic features of acne but is contraindicated in pregnant women and has been associated with cheilitis and dry skin. Hormonal therapy has been found to improve acne in some selective patients and should be considered for appropriate candidates. This review will present the general aspects of the pharmacological treatments for acne.

Introduction: Buthūr Labaniyya (Acne vulgaris) is a multifactorial disorder of the pilosebaceous units characterized by non-inflammatory and inflammatory clinical lesions. Several Unani medications have been used for centuries to treat acne. Objectives: Evaluation of safety and efficacy of Tila-i Muhāsā in patients with acne vulgaris Materials and Methods: This clinical study was conducted in patients with acne vulgaris. Patients applied Tila-i Muhāsā or 5% Benzoyl Peroxide (BPO) once daily for 6 weeks. Assessment of efficacy was carried out by Global Acne Grading System (GAGS) and Patient Global Assessment (PGA) scores. In addition, overall severity of acne was evaluated on a 5-point Cook's acne grading scale using photographic standards. Assessment of safety was performed through adverse drug reactions, local dermal tolerability, vital signs, and routine laboratory investigations. Results: A total of 60 patients (30 in each group) completed 6 weeks of treatment. The mean percentage reduction in GAGS score at 6 weeks from baseline in Unani group (66.97%) and BPO group (59.09%) was statistically significant (P<0.0001). After 6 weeks of therapy, the mean percentage reduction in PGA score compared to baseline in Unani group (57.44%) and BPO group (50.23%) was statistically significant (P<0.0001). No serious adverse events were reported in both groups; however, mild adverse events occurred more frequently in BPO group (30%) compared to Unani group (10%). Conclusion: Tila-i Muhāsā was found to be effective and safe in the treatment of acne vulgaris. However, further clinical studies with larger sample size and longer duration of therapy need to be conducted.
The purpose of this study is to determine the antimicrobial effect of tea tree oil a against pathogens relate to acne. Tea tree oil extracted from Australian Melaleuca alterifolia has been studied extensively for their antimicrobial properties against different type of bacteria and fungi. Tea tree oil has been reported to have antibacterial, antiinflammatory and antitumor activities, among others. By evaluating the antibacterial effect of Tea Tree Oil of Australia, the MIC value of Tea Tree oil against P. acnes was 0.05% and 0.5% against aerobic bacteria. This study showed that Tea Tree Oil has antibacterial effect against bacteria which induce acne. Tea Tree Oil had better antibacterial effect against P. acnes compared to aerobic bacteria. It is promising to develop a nature-based functional material for makeup product, which can cure and prevent acne.

Agrimonia pilosa Ledeb. is a perennial plant, which naturally habitats in whole area of Korea, and where it is popularly used for the traditional remedies. In the present study, A. pilosa Ledeb. extract was prepared to determine the anti-acne effects and application possibility as a cosmetic resource. A pilosa Ledeb. was extracted with methanol and its anti-acne effect against Propionibacterium acnes was investigated via minimum inhibitory concentration (MIC) and paper disk diffusion method. The MIC of A. pilosa Ledeb. extract and triclosan was 0.05 mg/mL and 0.04 mg/mL, respectively. This implies that A. pilosa Ledeb. extract nay be an efficient anti-acne ingredient for cosmetics, considering that it is a crude extract. The paper disk diffusion assay showed that its anti-acne effect was similar to that of triclosan. Furthermore, A. pilosa Ledeb. extract effectively inhibited the growth of several aerobic microorganisms including Staphylococcus aureus. Finally, we examine the stability of the extract to temperature and pH. The extract was very stable to high temperatures ($70{\sim}121^{\circ}C$) and to pH ($pH 2{\sim}11$), suggesting its utilization for cosmetics.

Tests of stability and toxicity, and clinical evaluation of anti-acne activity suggest that cryptotanshinone, a constituent of the roots of Salvia miltiorrhiza Bunge, is an effective active ingredient for acne vulgaris treatments. Acne vulgaris, called acne or pimples, is the most common disease of the pilosebaceous follicle unit of the skin. It affects nearly 80% of people between the ages of 11 and 30. Approximately 30% of teenagers have acne of sufficient severity to require medical treatment. Acne is a follicular disorder of the skin. It occurs in specialized pilosebaceous units on the face and body. Acne develops when these specialized follicles undergo pathologic alterations that result in the formation of non-inflammatory lesions (comedones) and inflammatory lesions (papules, pustules and nodules). An abnormality of keratinizing epithelium of these follicles, thought to be due to the action of sebum synthesized and secreted by the androgen-sensitive sebaceous glands, leads to inflammation induced by the follicular bacterium Propionibacterium acnes. Therapy involves treatments that modify these pathogenic factors and includes drugs with antikeratinizing, antibacterial and antiseborrheic actions. Acne vulgaris is a very frequent disease, seen primarily in adolescents, involving the sebaceous follicles. Acne vulgaris is characterized by a great variety of clinical inflammatory and non-inflammatory lesions: comedones, papules, pustules, nodules, cysts and scars. Acne vulgaris is a multi-factorial disease. Although its pathogenicity is unclear, extensive studies have shown that hyperseborrhea, superinfection by P. acnes and endocrinologic androgenic changes play a role in the development of acne vulgaris.

Propionibacterium acnes (P. acnes) lives in the hair follicles and pores, and it uses cell debris, sebum and metabolic byproducts of surrounding skin tissues as energy and nutrients. Increased production of sebum due to sebaceous hyperplasia or blockage of the follicle can cause growth and proliferation of P. acnes. The rapid growth of P. acnes in follicles produces cell damage, metabolic byproducts and bacterial chips, which can cause inflammation. In this study, we examined the possibility of Senecio iscoensis Hieron. (S. iscoensis) extract to regulate P. acnes-induced inflammatory signaling pathways. We observed that S. iscoensis extract effectively inhibited P. acnes-induced pro-inflammatory cytokine expressions such as IL-$1{\beta}$, TNF-${\alpha}$, and iNOS in mouse macrophage cell line Raw 264.7. The inhibitory effect of S. iscoensis in pro-inflammatory cytokine levels was accompanied by the inhibition of the transcription factors NF-${\kappa}B$ and NF-AT. However, S. iscoensis did not alter the P. acnes-induced MAPK signaling pathways. This study first suggests the potential of using S. iscoensis extract as an alternative agent for the treatment of acne.

Objectives: Although Acne vulgaris is a chronic skin disease, which its standard treatment causes therapeutic limitations and some common adverse effects, medicinal plants can be effective in treatment with low adverse effects as combination therapy. Myrtle (Myrtus Communis) has some beneficial properties, which has been administered topically and orally for some skin diseases in Persian medicine. This study aimed to compare the efficacy and safety of Myrtle formula and 1% clindamycin topical solution. Methods: This was a split-face clinical trial that was done on 55 patients with mild to moderate acne vulgaris for 16 weeks. The patients received topical Myrtle solution to the right side of the face (group 1) and clindamycin solution to the left side (group 2) twice daily for 12 weeks. All participants were examined for the acne severity index (ASI) and total acne lesions counting (TLC) at certain times during the study. Then, they stopped using them for four weeks. They also did not take the drug in the final four weeks of the study. Results: Forty-eight patients completed the study for 16 weeks; 40 (83.2%) patients were female and the rest of them were male. The mean age and standard deviation were 25.62 ± 7.62 years. After 12 weeks, the percentage changes of comedones, inflammatory lesions, ASI and TLC were significantly reduced in both groups (p < 0.001). The percentage change of inflammatory lesions and ASI decrease was significantly higher in the group 1 (p = 0.03). There was no significant difference in the incidence of side effects between the two groups. There was a more significant decrease in sebum percentage change in the group 1 (p = 0.003). Conclusion: Myrtle lotion was effective and safe for the treatment of mild to moderate acne vulgaris.
