치료적 운동프로그램이 편마비 환자의 정적·동적 균형 수행력에 미치는 영향 (The Effect of Therapeutic Exercise Program on Static·Dynamic Balance Performance in Hemiplegic Patients)

  • Purpose : The purpose of this study was to analyze the effect of closed kinematic chain exercise and proprioceptive neuromuscular facilitation exercise on the static dynamic balance performance of hemiplegic patients in order to suggest them therapeutic intervention methods. Methods : The subjects of this study were 18 hemiplegic patients grouped into 2 subgroups according to the exercise program. one group of closed kinematic chain exercise carried out sit to stand, Hooklying with pelvic lift(bridging) and stair-up & down by a hemiplegic leg. The other group of proprioceptive neuromuscular facilitation exercise carried out leg flexion-extension pattern in supine position, leg flexion pattern in standing and stabilizing reversal exercise in stating position. Each exercise was carried out over 3 sets of 10reps. Results : The results of this study were summarized as follows: 1. For both groups, there were statistically significant changes in the static balance (FICSIT-4) performance after exercise program (p<.05). 2. For both groups, there were statistically significant changes in the dynamic balance (FSST, TUG, FRT) performance after exercise program (p<.05). 3. In the comparison between both groups, there was no statistically significant difference in the static dynamic balance performance (FICSIT-4, FSST, TUGT, ER) after exercise program. Conclusion : As the results of the study shows closed kinematic chain exercise and proprioceptive neuromuscular facilitation exercise affect the improvement of hemiplegic patients'' static dynamic balance performance, it is supposed that these exercises could be therapeutic exercise program in clinical situations.

Closed-form solution for the buckling behavior of the delaminated FRP plates with a rectangular hole using super-elastic SMA stitches

  • Layer separation (delamination) is an essential threat to fiber-reinforced polymer (FRP) plates under dynamic, static, and fatigue loads. Under compressive load, the growth of delamination will lead to structural instability. The aim of this paper is to present a method using shape memory alloy (SMA) stitches to suppress the delamination growth in a FRP plate and to improve the buckling behavior of the plate with a rectangular hole. The present paper is divided into two parts. Firstly, a closed-form (CF) formulation for evaluating the buckling load of the FRP plate is presented. Secondly, the finite element method (FEM) will be employed to calculate the buckling loads of the plates which serves to validate the results obtained from the closed-form method. The novelty of this work is the development of the closed-form solution using the p-Ritz energy approach regarding the stress-dependent phase transformation of SMA to trace the equilibrium path. For the FEM, the Lagoudas constitutive model of the SMA material is implemented in FORTRAN programming language using a user material subroutines (VUMAT). The model is simulated in ABAQUS/Explicit solver due to the nature of the loading type. The cohesive zone model (CZM) is applied to simulate the delamination growth.

The clinical usefulness of closed reduction of nasal bone using only a periosteal elevator with a rubber band

  • Background: Closed reduction of nasal fracture with various instrument is performed to treat nasal fracture. Depending on the type of nasal fracture and the situation in which it is being operated, the surgeon will determine the surgical tool. The objective of this study was to investigate whether a periosteal elevator (PE) was a proper device to perform closed reduction for patients with simple nasal fractures. Methods: From March 2018 to December 2018, 50 cases of simple nasal bone fracture underwent closed reduction performed by a single surgeon. These patients were divided into two groups randomly: nasal bone reduction was performed using only PE (freer) and nasal bone reduction was performed using Walsham, Asch forcep, and Boies elevator (non-freer, non-PE). Results: The paranasal sinus computed tomography was performed on patients before and after operation to carry out an accurate measurement of reduction distance at the same level. According to the results, the interaction between instruments and fracture types had a significant influence on reduction distance (p = 0.021). To be specific, reduction distance was significantly (p= 0.004) increased by 2.157 mm when PE was used to treat patients with partial displacement compared to that when non-PEs were used. Conclusion: Closed reduction using PE and other elevator is generally an effective treatment for nasal fracture. In partial-displacement type of simple nasal fracture, closed reduction using PE can have considerable success in comparison with using classic instruments.

매복치의 교정적 견인 후 치주적 평가 (PERIODONTAL EVALUATION OF IMPACTED TEETH AFTER ORTHODONTIC TRACTION)

  • 본 연구는 1998년부터 2004년까지 조선대 소아치과에 내원한 환자 중 상악 중절치와 상악 견치의 편측성 매복으로 진단되어 closed-eruption technique을 이용한 외과적 노출 후 교정 적 견인을 시행하고 치료가 끝난 24명 (상악 중절치 10명, 상악 견치 14명)의 환자를 대상으로 하여 정상적으로 맹출한 인접 및 반대측 치아와 매복치의 치주적인 상태를 비교 분석하여 다음과 같은 결과를 얻었다. 1. 치주적인 평가에서 gingival index와 plaque index, pocket depth와 부착 치은의 비교시 견인을 시행한 상악 중절치, 견치 모두 대조군과 비교하여 유의할 만한 차이가 나타나지 않았다(P>0.05). 2. 상악 중절치의 골지지도 평가에서 근심부간과 윈심부간에 인접 중절치와 비교하여 유의할 만한 차이가 나타났다(P<0.05). 3. 상악 견치의 골지지도 평가시 견인치와 정상 맹출 치아간에 유의차를 보이지 않았다(P>0.05). 이상의 결과를 종합하여 볼 때 임상에서 closed-eruption technique을 이용한 외과적 노출 후 교정적인 견인을 이용한 매복치의 치료가 치은조직의 재생에 긍정적인 영향을 주고, 심미적으로 보다 안정적임을 알 수 있었으며, 상악 중절치 치료 시 견인의 방향과 힘 적용에 있어 정상 치조골의 양상을 유지하는지 관찰하여야 할 것으로 사료된다.

하악골 과두하부골절 정복술에서 관혈적 정복술과 비관혈적 정복술의 비교 연구 (Comparison Study of Open Reduction and Closed Reduction in Treatment of Mandibular Subcondylar Fractures)

  • Purpose: The choice of open versus closed reduction for mandibular subcondylar fracture is a debatable issue. To evaluate the advantage of open approach to closed method with IMF(intermaxillary fixation), we conducted a retrospective study to compare the outcomes of each method. Methods: From 2002 to 2006, 29 patients with mandibular subcondylar fractures were treated by open or closed reduction. 17 patients were treated by open reduction and 12 patients by closed reduction and IMF. Each group was assessed for duration of mandibular immobilization, incidences of buccal palsy, malocclusion, TMJ(temporomandibular joint) pain, and deviation of the mandible on mouth opening. Results: All cases showed accurate reduction in anatomical position, no significant displacement and no deviation on mouth opening during the follow-up period. IMF period is statistically shorter in open reduction (p<0.05). Differences in incidence of other complications were not significant statistically. Conclusion: As there are significant independent morbidities associated with IMF which requires postoperative rehabilitation, prolonged temporomandibular immobilization should not be overlooked. Some patients with poor compliances will not tolerate IMF in nonsurgical treatment. In the aspect of patient's convenience and early recovery by short IMF period, open reduction would be recommended as a better treatment method.

Buckling Strength Increment of Curved Panels Due to Rotational Stiffness of Closed-Section Ribs Under Uniaxial Compression

  • Recently, there have been studies about the increasing effect on the local plate buckling strength of flat plates when longitudinally stiffened with closed-section ribs and an approximate solution to quantitatively estimate these effects were suggested for flat plates. Since there are few studies to utilize such increasing effect on curved panels and a proper design method is not proposed, thus, this study aims to numerically evaluate such effect due to the rotational stiffness of closed-section ribs on curved panels and to propose an approximate method for estimating the buckling strength. Three-dimensional finite element models were set up using a general structural analysis program ABAQUS and a series of parametric numerical analyses were conducted in order to examine the variation of buckling stresses along with the rotational stiffness of closed-section ribs. By using a methodology that combine the strength increment factor due to the restraining effect by closed-section ribs and the buckling coefficient of the panel curvature, the approximate solutions for the estimation of buckling strength were suggested. The validity of the proposed methods was verified through a comparative study with the numerical analysis results.

닫힌 사슬 호흡 운동법이 상위 경수 손상 환자의 흉곽둘레길이와 어깨관절의 회전가동범위에 미치는 영향 (The Influence that the Closed Chain Breathing Exercise Affects on Thorax Circumference Increase and ROM of Shoulder Rotation on the Cervical Vertebral Cord Injury Patients)

  • Purpose : The purpose of this study was to investigate the effects of closed chain breathing exercise on thorax circumference increase and ROM of shoulder rotation for cervical vertebral cord injury patient's. Methods : 12 cervical vertebral cord injury patients volunteer to participate in this study. closed chain breathing exercise group of 4 subjects performed exercise three time a week. The deep breathing exercise group and general exercise group of 4 subjects performed exercise three time a week. We analyaed the descriptive statistics and $3{\times}2$ repeated measures ANOVA by SPSS 12.0 for window. Results : In comparison of rotation of shoulder ROM between pre and post value, the increase of rotation ROM of shoulder was significant in the closed chain breathing exercise group(p<.05). Conclusion : The closed chain breathing exercise helped to increased rotation ROM of shoulder.

The Effects of Closed Kinetic and Open Kinetic Chain Exercises Using Knee Reposition Sense in Chronic Stroke Patients

  • Purpose: This study aimed to determine the effects of open kinetic chain exercise (OKCE) and closed kinetic chain exercise (CKCE) using knee reposition sensing on balance, strength, and knee joint reposition sense (JPS) in chronic stroke patients. Methods: Twenty-nine hemiplegic patients participated in this study. Participants were randomly divided into 3 groups, CKCE, OKCE, and controls, with 9, 10, and 10 participants, respectively. The CKCE group completed CKCE using knee reposition sensing, whereas the OKCE group completed OKCE using knee reposition sensing. The control group completed conventional physical therapy. Results: Significant differences between the CKCE and OKCE groups were apparent for all outcomes except the functional reaching test. The CKCE group displayed significant improvements in knee JPS versus the OKCE and control groups (p<0.01). The OKCE group displayed significant improvements in knee extensor muscle strength versus the CKCE and control groups (p<0.05). The CKCE and OKCE groups displayed significantly improvements in static balance versus the control group (p<0.05). Conclusion: CKCE and OKCE improved balance, strength, and knee JPS. Additionally, CKCE might provide a more useful intervention benefit than OKCE for increasing knee JPS, a weight-bearing task. OKCE was sufficient to improve the knee extensor muscle strength.

뒤꿈치 인솔착용과 시각통제 유무가 하이힐 착용 시 균형관련 지수에 미치는 영향 (Influence of Heel Insole and Visual Control on Body Sway Index with High-heeled Shoes)

  • PURPOSE: We investigated the influence of heel insole and visual control on body sway index with high-heeled shoes. METHODS: The subjects of this study were 61 healthy students. None of the participants had any orthopedic or neurologic alterations. C90 area, C90 angle, trace length, sway average velocity were measured using a force plate by BT4. The variables were measured both with insole and without insole when wearing high-heeled shoes under the conditions of eyes open and eyes closed. The collected data were analyzed using the Kolmogorov-Smirnov test and paired t-test. RESULTS: When wearing high-heeled shoes with insole under the conditions of eyes open, trace length, C90 area, velocity were significantly more decreased than without insole (p<.01). When wearing high-heeled shoes with insole under the conditions of eyes closed, only C90 area was significantly more decreased than without insole (p<.05). When wearing high-heeled shoes with insole under the conditions of eyes open, trace length, C90 area, velocity were significantly more decreased than under the conditions of eyes closed (p<.01). CONCLUSION: The present study demonstrates that the use of high-heeled shoes with insole supported from heel to midfoot more increased static balance than without insole under the conditions of eyes open.

The Effects of Proprioceptor Training and Vestibular Organ Training on Balance Ability of Adults

  • The purpose of this study was to analysis of the effect of proprioceptor training and vestibular organ training for balance ability. The subjects was consist of two different subjects group, proprioceptor training group and vestibular organ training group. Proprioceptor training group consisted of 10 subjects and vestibular organ training group consisted of 10 subjects. Training was performed 3 times per week, 30 minutes per day, for 3 weeks. Balance ability analysis was performed using Romberg's one leg standing test and BT4 when opened eyes and closed eyes. The analysis results were as follows. There was no significant differences in balance after the training in both groups when they opened their eyes(p<.05). But there was significant differences in balance after the training in both groups when they closed their eyes(p<.05). And there was no significant difference in balance after the training between the proprioceptor training group and the vestibular organ training group when they closed their eyes(p<.05). Given the above results, proprioceptor training and vestibular organ training enhanced balance but there was no significant difference between the two methods.
