Bioeuqivalence Study of Nabumetone Tablets in Man

  • A nebumetone tablet in development $(Navuton^R)$ was tested for its bioequivalence to the erference tablet $(Uniton^R)$. Seventeen healthy Korean male subjects participated in this study. Each subject received a 1-g dose of nabumetone (2tables each) in an unbalanced, randomized, two-way crossover investigation. Serum concentrations of 6-methoxy-2-na-phthylacetic acid (6-MNA), a major metabolite of nebumetone, were measured over 120 hr interval by a high-performance liquid chromatography. The maximum serum concentration $(C_{max})$ and time to reach the maximum concentration$(T_{max})$ were read directly, but area under the serum concentration time curve from time 0 to 120 hr (AUC) and mean residence time serum curves showed multiple peaks of 6-MNA in most subjects, and the $C_{max}$ and $T_{max}$ were read from the highest serum peaks. calculated bioavailability parameters for test and reference tablets were 148.6 : 1377.9 $\mug \cdot hr/ml$ for AUC; 25.2:23.1 $\mu/ml$ for $C_{max}$; 11.8:16.4 hr for $T_{max}$, and 42.6 : 43.8 hr for MRT, respectively. The paired t-test revealed no significant differences in all the parameters between the two tablets. Analysis ofl variance (ANOVA) revealed no significant differences between groups and formulations in all the parameters ($C_{max}$ and $T_{max}$, AUC and MRT) indicating the crossover design of the experiment was properly performed. But significant differences (p<0.05) between subject/groups and periods were found for all the parameters indicating substantial intersubject and interperiodic variations for these parameters.

Preparation of Lacosamide Sustained-release Tablets and Their Pharmacokinetics in Beagles and Mini-pigs

  • The aim of the present study was to improve dosing of lacosamide, a functionalized amino acid used as an antiepileptic agent, from twice daily to once daily for the convenience of patients. A sustained-release lacosamide tablet was developed and dissolution testing was employed to determine in vitro release behavior using water or buffer solutions at pH 1.2, 4.0, or 6.8. Lacosamide was released for 12 h from the sustainedrelease (SR) tablet, as compared to complete release within 1 h from an immediate-release $Vimpat^{(R)}$ tablet. Each formulation (100 mg) was orally administered to six beagle dogs and six mini-pigs under fasted conditions, and pharmacokinetic parameters such as the area under the concentration time curve ($AUC_t$), the maximum plasma concentration ($C_{max}$), and the time at which this occurred ($T_{max}$) were calculated. These results showed similar values for $AUC_t$, $C_{max}$, and $T_{max}$ following oral administration of immediate-release ($Vimpat^{(R)}$) and SR lacosamide tablets.

Cardiometabolic Index, Triglyceride-glucose Index를 이용한 대사증후군 진단 예측지수에 대한 고찰: 부산지역 건강검진대상자 중심으로 (Consideration of Predictive Indices for Metabolic Syndrome Diagnosis Using Cardiometabolic Index and Triglyceride-glucose Index: Focusing on Those Subject to Health Checkups in the Busan Area)

  • This study investigates the utility of the Triglyceride-glucose(TyG) index and Cardiometabolic Index(CMI) as predictors for diagnosing metabolic syndrome. The study involved 1970 males, 1459 females, totaling 3429 participants who underwent health checkups at P Hospital in Busan between January 2023 and June 2023. Metabolic syndrome diagnosis was based on the presence of 3 or more risk factors out of the 5 criteria outlined by the American Heart Association/National Heart, Lung, and Blood Institute(AHA/NHLBI), and participants with 2 or fewer risk factors were categorized as normal. Statistical analyses included independent sample t-tests, chi-square tests, Pearson's correlation analysis, Receiver Operating Characteristic(ROC) curve analysis, and logistic regression analysis, using the Statistical Package for the Social Sciences(SPSS) program. Significance was established at p<0.05. The comparison revealed that the metabolic syndrome group exhibited attributes such as advanced age, male gender, elevated systolic and diastolic blood pressures, high blood sugar, elevated triglycerides, reduced LDL-C, elevated HDL-C, higher Cardiometabolic Index, Triglyceride-glucose index, and components linked to abdominal obesity. Pearson correlation analysis showed strong positive correlations between waist circumference/height ratio, waist circumference, Cardiometabolic Index, and triglycerides. Weak positive correlations were observed between LDL-C, body mass index, and Cardiometabolic index, while a strong negative correlation was found between Cardiometabolic Index and HDL-C. ROC analysis indicated that the Cardiometabolic Index(CMI), Triglyceride-glucose(TyG) index, and waist circumference demonstrated the highest Area Under the Curve(AUC) values, indicating their efficacy in diagnosing metabolic syndrome. Optimal cut-off values were determined as >1.34, >8.86, and >84.5 for the Cardiometabolic Index, Triglyceride-glucose index, and waist circumference, respectively. Logistic regression analysis revealed significant differences for age(p=0.037), waist circumference(p<0.001), systolic blood pressure(p<0.001), triglycerides(p<0.001), LDL-C(p=0.028), fasting blood sugar(p<0.001), Cardiometabolic Index(p<0.001), and Triglyceride-glucose index (p<0.001). The odds ratios for these variables were 1.015, 1.179, 1.090, 3.03, and 69.16, respectively. In conclusion, the Cardiometabolic Index and Triglyceride-glucose index are robust predictive indicators closely associated with metabolic syndrome diagnosis, and waist circumference is identified as an excellent predictor. Integrating these variables into clinical practice holds the potential for enhancing early diagnosis and prevention of metabolic syndrome.

흰쥐에서 글리피지드가 로살탄의 약물동태에 미치는 영향 (Effects of Glipizide on the Pharmacokinetics of Losartan after Oral and Intravenous Administration in Rats)

  • The present study was to investigate the effect of glipizide on the pharmacokinetics of losartan in rats. Losartan was administered intravenously (3 mg/kg) and orally (9 mg/kg) in the presence and absence of glipizide (0.3 and 1 mg/kg) to rats. The pharmacokinetic parameters of losartan were significantly altered by the presence of glipizide compared with the control group (given losartan alone). Presence of glipizide significantly (p<0.05, 0.3 mg/kg) increased the area under the plasma concentration-time curve (AUC) of losartan by 48.2% and peak plasma concentration ($C_{max}$) of losartan by 47.4%. Consequently, the absolute bioavailability (AB%) of losartan in the presence of glipizide was 38%, which was enhanced significantly (p<0.05) compared to that in the oral control group (25%). The relative bioavailability (RB%) of losartan increased by 1.18- to 1.48-fold in the presence of glipizide. However, there was no significant change in the peak plasma concentration ($T_{max}$) and terminal half-life ($T_{1/2}$) of losartan in the presence of glipizide. In contrast, glipizide did not affect the pharmacokinetics of intravenous losartan. In conclusion, the presence of glipizide significantly enhanced the oral bioavailability of losartan, implying that glipizide might be mainly to inhibit the cytochrome P450 (CYP) 2C9-mediated metabolism, resulting in reducing gastrointestinal and/or hepatic first-pass metabilism of losartan rather than in reducing P-glycoprotein-mediated efflux and renal elimination of losartan. Concurrent use of glipizide with losartan should require close monitoring for potential drug interactions.

현실임분 생장특성을 반영한 삼나무 지위지수 추정 모델 개발 (Development of Site Index Model for Cryptomeria japonica Stands by the Current Growth Characteristics in South Korea)

  • This study was carried out to provide basic data for logical forest management by developing a site index curve reflecting the current growth characteristics of Cryptomeria japonica stands in Korea. The height growth model was developed using the Chapman-Richards, Schumacher, Gompertz, and Weibull algebraic difference equations, which are widely used in growth estimation, for data collected from 119 plots through the 7th National Forest Inventory and stand survey. The Chapman-Richards equation, with the highest model fit, was selected as the best equation for the height growth model, and a site index curve was developed using the guide curve method. To compare the developed site index curve with that on the yield table, paired T-tests with a significance level of 5% were performed. The results indicated that there were no significant differences between the site index curve values at all ages, and the p-value was smaller after the reference age than before. Therefore, the site index curve developed through this study reflects the characteristics of the changing growth environment of C. japonica stands and can be used in accordance with the site index curve on the current yield table. Thus, this information can be considered valuable as basic data for reasonable forest management.

Effect of Clarithromycin on the Pharmacokinetics of Ambroxol in Rats

  • This study investigated the effect of clarithromycin on the pharmacokinetics of ambroxol in rats. The pharmacokinetic parameters of ambroxol in rats were determined after the oral administration of ambroxol (12 mg/kg) in the presence or absence of clarithromycin (5 or 10 mg/kg). Compared with the control (given ambroxol alone), coadministration of clarithromycin significantly (p<0.05 at 5 mg/kg; p<0.01 at 10 mg/kg) increased the area under the plasma concentration-time curve (AUC), peak plasma concentrations $(C_{max})$ and absorption rate constant $(K_a)$ of ambroxol. Clarithromycin increased the AUC of ambroxol in a dose dependent manner within the dose range of 5 to 10 mg/kg. The absolute bioavailability (AB%) of ambroxol in the presence of clarithromycin was significantly higher than that of the control (p<0.05 at 5 mg/kg; p<0.01 at 10 mg/kg), and the relative bioavailability (RB%) of ambroxol with clarithromycin was increased by 1.32-to 1.71-fold. However, there were no significant changes in time to reach peak concentration $(T_{max})$ and terminal half-life $(T_{1/2})$ of ambroxol in the presence of clarithromycin. Coadministration of clarithromycin enhanced the bioavailability of ambroxol, which may be due to the inhibition of intestinal and hepatic metabolism of ambroxol by CYP 3A4. Further studies for the potential drug interaction are necessary since ambroxol is often administrated concomitantly with clarithromycin in humans.

  • Purpose: The purpose of this study is to define the cut off values of cold hypersensitivity of hands by using digital infrared thermographic imaging(DITI). Methods: Thermographic images of 130 patients with cold hypersensitivity of hands(CHHG, n=65) and non-cold hypersensitivity of hands(NCHHG, n=65) were retrospectively reviewed. We used the temperature difference the palm(PC8) and the upper arm(LU4) for diagnosing cold hypersensitivity of hands. The temperature differences of between two groups were analysed using independent samples t-tests. The cut off values were calculated by ROC curve analysis. Analyses were undertaken using SPSS version 17.0. P value of < 0.05 was considered significant. Results: The temperature difference the palm(PC8) and the upper arm(LU4) were significantly different between groups(p < 0.001). Using receiver operating characteristic curve analysis, the sensitivity, specificity, and area under the curve were 70.8%, 73.8%, respectively both hands. The AUC was 0.822 on right hand and 0.818 on left hand. The optimum cut-off value was defined as $-0.05^{\circ}C$. Conclusions: These results suggest that DITI is a reliable instrument for estimating the cold hypersensitivity of hands.

  • Objectives: The purpose of this study is to find diagnostic points and define the cut off values of hot flashes by using digital infrared thermographic imaging. Methods: Thermographic images of 75 patients with hot flashes (HF, n=35) and non-hot flashes (NHF, n=40) were retrospectively reviewed. We used the temperature difference between Ex-HN3 and CV17, LU4, CV12, CV4 for diagnosing hot flashes. The temperature differences of between two groups were analysed using independent samples t-tests. The cut off values were calculated by received operating characteristic curve analysis. Analyses were undertaken using SPSS version 17.0. and p-value of <0.05 was considered significant. Results: The temperature difference Ex-HN3 and LU4 were the most significantly different between groups (p<0.001). Using receiver operating characteristic curve analysis, the sensitivity, specificity, and area under the curve were 65.7%, 72.5%, 0.729, respectively. The optimum cut off value was defined as $1.00^{\circ}C$. Conclusions: These results suggest that the digital infrared thermographic imaging is a reliable instrument for estimating hot flashes.

Electra-Optic and Ionic Properties of Twisted Nematic Cells With Different Chiral Pitch

  • We investigated electro-optic and ionic properties of twisted nematic cells by using control of chiral pitch. These properties are observed in practical experiment and simulations. C-V and V-T curve characteristics were obtained from three types of cells with d/p. It is shown that d/p ratio of short cells exhibit faster response time improved by 20% than normal cell. Also, inter-gray response time is improved each rise time and decay time. And, the increase of saturation voltage is happened because of the small twist angel change from initial state at high voltage near 5V. To compensate for longer black level tail, gamma curve index was varied from g = 2.2 to g = 2.7 in module status. Additionally, adding chiral dopant into TN cells improved ionic characteristics such as increasing VHR, Ion density and DC Hysteresis were decreased..

신경계중환자의 사망예측모델(Mortality Probability Model II)에 대한 타당도 검증 (Verification of Validity of MPM II for Neurological Patients in Intensive Care Units)

  • Purpose: Mortality Provability Model (MPM) II is a model for predicting mortality probability of patients admitted to ICU. This study was done to test the validity of MPM II for critically ill neurological patients and to determine applicability of MPM II in predicting mortality of neurological ICU patients. Methods: Data were collected from medical records of 187 neurological patients over 18 yr of age who were admitted to the ICU of C University Hospital during the period from January 2008 to May 2009. Collected data were analyzed through $X^2$ test, t-test, Mann-Whiteny test, goodness of fit test, and ROC curve. Results: As to mortality according to patients' general and clinically related characteristics, mortality was statistically significantly different for ICU stay, hospital stay, APACHE III score, APACHE predicted death rate, GCS, endotracheal intubation, and central venous catheter. Results of Hosmer-Lemeshow goodness-of-fit test were MPM $II_0$ ($X^2$=0.02, p=.989), MPM $II_24$ ($X^2$=0.99 p=.805), MPM $II_48$ ($X^2$=0.91, p=.822), and MPM $II_72$ ($X^2$=1.57, p=.457), and results of the discrimination test using the ROC curve were MPM $II_0$, .726 (p<.001), MPM $II_24$, .764 (p<.001), MPM $II_48$, .762 (p<.001), and MPM $II_72$, .809 (p<.001). Conclusion: MPM II was found to be a valid mortality prediction model for neurological ICU patients.
