Basics of particle therapy II: relative biological effectiveness

  • In the previous review, the physical aspect of heavy particles, with a focus on the carbon beam was introduced. Particle beam therapy has many potential advantages for cancer treatment without increasing severe side effects in normal tissue, these kinds of radiation have different biologic characteristics and have advantages over using conventional photon beam radiation during treatment. The relative biological effectiveness (RBE) is used for many biological, clinical endpoints among different radiation types and is the only convenient way to transfer the clinical experience in radiotherapy with photons to another type of radiation therapy. However, the RBE varies dependent on the energy of the beam, the fractionation, cell types, oxygenation status, and the biological endpoint studied. Thus this review describes the concerns about RBE related to particle beam to increase interests of the Korean radiation oncologists' society.

  • Bronchogenic carcinoma involving the carina has remained a challenging problem for thoracic surgeons. Carinal resection and reconstruction is limitedly indicated because this aggressive surgical approach has been reported to be associated with significant morbidity and mortality while long-term outcome has not been determined. Wesuccessfully performed carinal reconstruction and sleeve right upper lobectomy assisted with ECMO for a 60-year-old male with squamous cell carcinoma in the right upper lobe extending to the carina.

  • Surface modification of polypropylene hollow fiber membranes was peformed in order to develop blood-compatible biomaterials for use in the blood contacting and oxygenation membranes of a lung-assist device(LAD). Blood compatibility was determined by using anticoagulation blood and evaluating formation of blood clots on their surfaces as well as activation of plasma coagulation cascade, platelet adhesion, and aggregation. It was verified that the number of platelets on the silicone coated fibers was significantly lower than those on polypropylene. It was also found that the polypropylene hollow fiber membranes using plasma treatment exhibited suppression of complement activation in blood compatibility test.

  • Bioventing is commonly used for petroleum hydrocarbon (PHC) spills. This process provides better subsurface oxygenation, thus stimulating degradation by indigenous microorganisms. Therefore soil vapor monitoring points (VMPs) are extremely important in determining the potential effectiveness of bioventing and in long-term monitoring of bioventing progress. In this study in-situ soil gas monitoring well (GMW) was developed and presented the pilot test results which recover the contaminated site by bioventing method. The result of application was successful and it was expected that GMW developed could be applied to the evaluation procedure of bioventing effectiveness and long-term remediation potential.

  • We measured hemodynamic responses of seizure in the mouse brain using frequencydomain near infrared spectroscopy (NIRS) and electroencephalogram (EEG). We adapted microfabricated optical holder for consistent contact of the optical fiber to the mouse brain. Our results show that the cerebral oxygenation and hemodynamics of mice can be stably monitored with EEG in the mouse brain.

  • Some coumarins were sythesized for the screening of their cytotoxic activities against L1210 cell. Of the conmarins sythesized, 6, 7-dihydroxycoumarin (esculetin) and 7, 8-dihydroxycoumain (dephnetin) as coumarins with dioxygenated A-ring, and 6-acetoxy-5, 7-dimethoxycoumarin and5, 7-dimethoxy-6-hydroxycoumarin as trioxygenated ones, show considerable cytotoxic activities, ED 50 being 4. 3, 8. 8, 17.2 and 5.5 $\mu$g/ml in the same other as the substances. THe extent oxygenation of the A-ring and the positions of the oxygen functions eventually play an important role for the cytotoxic activity.

  • This paper proposes a method of noninvasive reflectance light to measure the blood fractional volume (Vb) and oxygen saturation ($SO_2$) of biological tissue. We chose the red light of 660nm and infrared light of 880nm. In Vivo reflectance data were obtained by the physiological changes front the surface of the skin over the calf in human subject. The reflected light intensity from different layers within a biological tissue was measured by specially designed reflectometer to apply photon path diffusion model. The collected data represent the changes of blood (ractional volume and oxygen saturation at each reflected light wavelengths. The data evaluation was assessed by examining the slopes of the plotted indices for the changes in oxygen saturation and blood (ractional volume. The results presented in this paper claim that light reflectance can separately discriminate the change of blood volume and that of oxygenation in muscle and also in skin.

  • We report the successful use of Nifedipine in the treatment of acute pulmonary hypertension in an young child after a cardiac operation. This patient had undergone patch closure of large ventricular septal defect. She had signs of severe pulmonary artery hypertension unresponsive to hyperventilation, oxygenation, sedation, and a myriad of vas-oactive drugs. Nifedipine, 0.3mg /kg every 4 hours, effectively treated her pulmonary artery hypertension and allowed for a smooth postoperative course and positive outcome. The drug lowered systemic diastolic pressure, but not systolic pressure.

Catalytic Oxygenation of Alkenes and Alkanes by Oxygen Donors Catalyzed by Cobalt-Substituted Polyoxotungstate

  • The cobalt-substituted polyoxotungstate [(CoPW11O39)5-] has been used as a catalyst in olefin epoxidation and alkane hydroxylation reactions. The epoxidation of olefins by iodosylbenzene in CH3CN yielded epoxides predominantly with trace amounts of allylic oxidation products. cis-Stilbene was streoselectively oxidized to cis-stilbene oxide with small amounts of trans-stilbene oxide and benzaldehyde formation. The epoxidation of carbamazepine (CBZ) by potassium monopersulfate in aqueous solution gave the corresponding CBZ 10,11-oxide product. Other transition metal-substituted polyoxotungstates (M=Mn2+, Fe2+, Ni2+, and Cu2+) were inactive in the CBZ epoxidation reaction. The cobalt-substituted polyoxotungstate also catalyzed the oxidation of alkanes with m-chloroperbenzoic acid to give the corresponding alcohols and ketones. The presence of CH2Br2 in the hydroxylation of cyclohexane afforded the formation of bromocyclohexane, suggesting the participation of cyclohexyl radical. In the 18O-labeled water experiment, there was no incorporation of 18O into the cyclohexanol product when the hydroxylation of cyclohexane by MCPBA was carried out in the presence of H218O. Some mechanistic aspects are discussed as well.

Leaflet Fracture and Embolization of a CarboMedics Prosthetic Mitral Valve: Case Report

  • Fracture of prosthetic valve leaflets in the absence of traumatic injury is very rare. Leaflet fracture can cause acute pulmonary edema and cardiogenic shock and is potentially life-threatening, requiring emergency surgery. Thus, a leaflet fracture must be diagnosed quickly and accurately. We present the case of a 46-year-old man with CarboMedics prosthetic aortic and mitral valve replacements implanted 24 years previously. The patient presented at our emergency department with abrupt dyspnea and fever. We diagnosed severe mitral valve regurgitation with anterior leaflet fracture. The patient underwent venoarterial extracorporeal membrane oxygenation and delayed mitral valve replacement. The foreign body was removed step by step because the diagnosis was missed. Two pieces of broken leaflets were found in the left common iliac artery and left external iliac artery. The patient was treated successfully and remains asymptomatic 1 year following surgery.
